1.EFFECTS OF DIFFERENT DIETARY FATTY ACID COMPOSITION ON T LYMPHOCYTE FUNCTIONS AND LIPID PEROXIDATION IN MICE

Objective: To study the effects of dietary n-6/n-3 PUFA ratio on T lymphocyte functions in mice. Methods: 96 male BALb/c mice were randomly divided into 8 groups with dietary S:M:P 1:1.5:3.7 or 1:1.5:1, respectively. The n-6/n-3 PUFA ratio was 1, 7.5, 15, 30 respectively in each S:M:P diet. Each diet contained 70.0g/kg lipid,and different n-6/n-3 PUFA ratios by mixing different oils. At the end of 12w, all mice were killed and blood was collected. The spleen T lymphocyte proliferation, IL-2 production and serum MDA were measured. Results: When dietary S:M:P was 1:1.5:3.7, the proliferation of spleen lymphocytes and IL-2 production from mice with dietary n-6/n-3 ratio 1 were suppressed (P

Objective To analyze optimal amount of drinking water,ways of drinking water,timelimit of drinking water on the effect of bowel preparation for intravenous pyelography (IVP) examination after oral administration of 50% magnesium sulfate 60 ml. Methods 200 patients who gave their consentfrom June 2009 to February 2010 drank 50% magnesium sulfate 60 ml for IVP check were divided into four groups. Group A: a large amount of graded oral administration of warm water 1200 ml within 1h; Group B: a small amount of oral administration of warm water 1200 ml within 1h;Group C: a large amount of graded oral administration of warm water 2000 ml within 1h; Group D: a medium amount of graded oral administration of warm water 1200 ml within 2 h. And then the bowel preparation effect of the four gro ups were analyzed. Results Through the comparison, group A was much better than the other groups on the aspects of cathartic satisfaction, the effective degree of reading films and flatulence. Conclusions To take a large amount of graded oral administration of warm water 1200 ml within 1h can improve cathartic satisfaction,the effective degree of reading films and flatulence.
3.Clinical application of high-frequency electric snare in treatment of airway neoplasm

Objective To investigate the clinical efficacy and safety of high-frequency electric snare for the treatment of main airway neoplasm through bronchoscopy. Methods The clinical datas of 18 patients from July 2013 to April 2016, who had main airway neoplasm and were treated with high-frequency electric snare through bronchoscopy, were retrospectively analyzed. Meanwhile, the interventional bronchoscopic techniques partly included argon plasma coagulation and cryoablation. After operarion, the relief of symptoms and complications were observed. Results After operarion, the clinical symptoms, shortness of breath, dyspnea index score and the degree of airway stenosis were improved significantly. No serious complications such as massive hemoptysis occurred in all patients. Conclusion High-frequency electric snare is an effective and safe approach for the treatment of main airway neoplasm.
4.Effect of eletroacupuncture for Z syndrome without organic lesion and serum leptin.

OBJECTIVETo explore the effect and mechanism of electroacupuncture (EA) for Z syndrome without organic lesion (metabolic syndrome combined with obstructive sleep apnea hypopnea syndrome).
METHODSFifty-eight patients with Z syndrome were divided into three groups according to mild,moderate and severe degree. Acupuncture and EA were used at Daimai (GB 26), Zhongwan (CV 12), Xiawan (CV 10), Zusanli ST 36), Qihai (CV 6) and Huaroumen (ST 24), etc., once a day and five times a week. The treatment of ten times was a course, and two courses were acquired continuously. Sleep respiration monitoring (PSG) was done before EA and in one week after treatment respectively. Triacylglycerol (TG) fasting blood glucose (FBG) fasting insulin (INS) and serum leptin (Lep) were tested before and after treatment in the three groups.
RESULTSAfter treatment apnea hypopnea index (AHI) and the percentage of the time of arterial oxygen saturation (SaO₂) less than 90% taken in the total sleep time (SLT 90%) were improved apparently than those before treatment in the three groups (all P < 0.05). The levels of TG, FPG, INS and Lep were decreased after treatment in all groups (all P < 0.05).
CONCLUSIONEA can improve AHI and nocturnal hypoxia of Z syndrome, and the mechanism may be related to decreasing the indices of metabolism syndrome and leptin.

5.Effect analysis of setting up full-time visiting nurses in emergency transfusion room

Objective To explore the effects analysis of setting up full-time visiting nurses in emergency transfusion room. Methods The transfusion adverse events, patient satisfaction and quality control score of patients before and after setting up full-time visiting nurses were analyzed through setting up the post of full-time visiting nurses in emergency transfusion room. Results Compared with the posts without full-time visiting nurses, the adverse events of infusion decreased from 0.61% (200/32728) to 0.18%(48/26470),χ2=63.762,P<0.05,and the patient satisfaction score increased from 91.33 ± 1.21 to 95.67 ± 1.37, t=10.277, P<0.05, and the quality control score was increased from 91.00 ± 1.55 to 96.50 ± 1.38, t=12.845, P<0.05. Conclusions Setting up full- time visiting nurses can strengthen the management of transfusion and satisfy the needs of patients' life and treatment. It is an important guarantee to reduce the risk of transfusion, ensure the safety of infusion and improve the satisfaction of patients.
6. Influencing factors of occupational stress in female workers of labor-intensive enterprises

OBJECTIVE: To investigate the current status and influencing factors of occupational stress in female workers of labor-intensive enterprises. METHODS: Totally 910 female workers from 5 labor-intensive enterprises in Guangdong Province was selected as the research subjects by random cluster sampling method. A questionnaire survey on occupational stress was conducted using the Occupational Stress Inventory-Revised Edition. RESULTS: Among the 910 female workers,14. 9%( 136/910) showed medium or higher scale on occupational role level,20. 1%( 183/910) showed medium or higher scale on occupational stress level,and 21. 4%( 195/910) showed medium or higher scale on personal resources lack. Multivariate logistic regression analysis indicated that the risk of occupational stress was high in single female workers and/or those exposed to occupational hazards( P < 0. 01). The higher the occupational role level,the higher risk of occupational stress( P < 0. 01). The less personal resources,the higher risk of occupational stress( P < 0. 01).CONCLUSION: Some female workers in labor-intensive enterprises showed medium or higher occupational stress level. The main influencing factors of occupational stress are marital status,exposure to occupational hazards,occupational role and personal resources.
7.FLUKA-based simulation analysis of induced radioactivity in proton therapy site

Objective To simulate and analyze the dose distribution from external exposure and its influencing factors of induced radioactivity in proton therapy site. Methods Referencing a domestically under-construction proton therapy facility, a geometric model of the proton therapy site was constructed, and the FLUKA program was used to simulate the distribution of the induced radioactive dose of the proton therapy site under the conditions of different energies, beam angles, irradiation time, cooling time and medium of the treatment site. Results For a 230 MeV proton beam with a current of 3.0 nA, directed along the negative Z-axis and irradiating a phantom for two minutes, at the shutdown moment, the ambient dose equivalent rates in air and vacuum 5, 30, and 50 cm away from the phantom surface were (1 039.02±5.82)-(127.86±1.20) and (1 037.96±4.38)~(127.35±0.93) μSv/h, respectively. The mean difference was 0.51~1.06 μSv/h, and the air-immersed external irradiation accounted for <1% of the total irradiation, which rapidly decreased to 1/15 of the shutdown moment value after cooling for 10 minutes. Under the condition of 130~250 MeV, the ambient dose equivalent rates at the shutdown moments 5, 30 and 50 cm away from the surface of the phantom were (427.49±3.12)-(1 058.41±4.66), (100.36±0.92)-(259.70±1.69) and (50.15±0.68)-(131.93±1.11) μSv/h, respectively. Irradiation for one-five minutes, and at the moment of shutdown at 5, 30, and 50 cm from the surface of the phantom were (688.19±3.33)-(1 594.04±8.08), (167.60±1.35)-(388.24±2.96) and (84.73±0.69)-(195.94±1.56) μSv/h. The peripheral dose-equivalent rate of the sensed radioactivity decreases with the irradiation time, the energy of the beam, and the distance from the model. The peak dose equivalent rate around the induced radioactivity exists in the beam direction, which is significantly larger than that in the non-beam direction. Conclusion Proton therapy sites are characterized by relatively large levels of induced peripheral radioactive dose equivalent rates, mainly originating from patients. In actual practice, a suitable working position can be chosen according to the direction of the beam current, especially the direction of the final irradiation field beam current, in the non-beam current direction and as far away from the patient as possible. Within 10 minutes after the end of treatment, staff should try to avoid close contact with the patients.
8.Effect of casting back the whole life cycle of medication orders on the nurse work

Objective To evaluate the effect of casting back the whole life cycle of medication orders on the nurse work .Methods Sixty-two nurses were surveyed by the self-designed questionnaire .The standard of casting back medication orders was carried out through the design of the module for casting back medication orders based on the information of nurse workstation and the check of Itouch bar code .The time of carrying out the medication orders and the medication safety were compared before and after the implementation . Results After the implementation , the time of carrying out the doctor ’ s advice for every bed on daily decreased from (4.36 ±1.28)min to (0.97 ±0.34)min, and the difference was statistically significant (t=20.181,P<0.01).The error rate of carrying out the doctor ’ s advice for every thousand beds on daily decreased from 0.74‰to 0.15‰, and the error rate of medication decreased from 1.33‰ to 0.35‰, and leakage rate of medication decreased from 1.24‰to 0.06‰, and the differences were statistically significant (χ2 =13.548, 19.073, 36.347, respectively; P<0.05).Conclusions Implementation of the management of casting back the whole life cycle of medication orders can improve the work efficiency , and decrease the occurrence of nursing defects, and ensure the safety of patients .
9.Distribution and drug resistance analysis of carbapenems-resistant Klebsiella pneumoniae

Objective To investigate the clinical distribution and drug resistance of carbapenemsresistant Klebsiella pneumoniae (CRKP).Methods A total of 134 clinical strains of CRKP were collected from inpatients in our hospital from January 2014 to December 2016.VITEK-2 compact automatic microbiological analyzer was used to identify the bacteria and the supporting gram-negative bacterial drug susceptibility card was used for susceptibility testing.The sensitivity of other clinical commonly used antimicrobial agents was measured by K-B method.And the clinical distribution of CRKP and its resistance to antimicrobial agents were investigated and analyzed.Results The clinical strains of CRKP were isolated mainly from urine (n=70,52.2%) and sputum (n=38,28.4%).Klebsiella pneumoniae was identified in samples from the department of neurosurgery,including neurosurgery ICU (47.8%,64/134),ICU (23.9%,32/134),department of hepatobiliary surgery (8.2%,11/134) and department of urology (6.0%,8/134).Among all 22 antimicrobial agents tested,the resistant rates of CRKP to 16 antimicrobial agents were >90%,especially for ampicillin it was 100%,and those to 19 antibiotics were >80%,only for tigecycline it was 23.1%.There were 16 strains of CRKP with positive extended spectrum β-lactamases (ESBLs) (11.9%).All the CRKP strains were resistant to more than three kinds of antimicrobial agents,except one strain that was resistant to two kinds.Conclusion There is a wide range of clinical distribution of CRKP,which is resistant to most of antimicrobial agents,while tigecycline still has a strong antibacterial activity to CRKP.
