1.Induction chemotherapy with low-dose homoharringtonine, cytarabine, and granulocyte colony-stimulating factor priming in patients with advanced myelodysplastic syndrome or post-myelody splastic syndrome acute myeloid leukemia

This regimen was given for one course for induction, and was followed by conventional chemotherapy as maintenance or consolidation when complete remission(CR) achieved, or succeeding with other treatment when no response could be observed. Results Six patients achieved CR (54.5%) and one achieved partial remission (PR)(9.1%) with one course of treatment. Among 6 of 11 patients with CR, 5 relapsed at 2,3,6,8 and 16 months respectively. Three relapsed patients were retreated with the same protocol but achieved only one partial responses. Nine of the 11 patients had been died and their mean survival (since induction chemotherapy) was 9.2 months. Infectious complications during cytopenia were less serious than conventional chemotherapy withno treatment-related.Conclusion This moderate intensity protocol with G-CSF priming is effective and safe but remissions are of short duration.
2.Establishment of the system of nursing sensitive quality indicators in stroke unit

Objective? To establish the the system of nursing sensitive quality indicators in the stroke unit and to provide a reference for nursing quality control in the stroke unit. Methods? Totally 23 experts on clinical nursing, nursing management and hospital quality management from the field of Neurology across China were selected for enquiry between July and August 2018. With the "structure-process-outcome" theory from total quality management as the framework, the item pool for the the system of nursing sensitive quality indicators in the stroke unit was established for expert consultation by literature analysis and group discussion. Results? The expert authority score (Cr) of two rounds was 0.80 and 0.81. Kendall's W of the first and second round of expert opinions was 0.316 and 0.412 (P< 0.05). Finally, a the system of nursing sensitive quality indicators in the stroke unit was established, which included 3 primary indicators, 10 secondary indicators and 10 tertiary indicators. Conclusions? Since the expert authority score is high and the results are reliable, the the system of nursing sensitive quality in the stroke unit may provide a reference for quality control in the stroke unit.
3.Improvement of safety compliance in stoke patients through multiple-care safety management

Objective To investigate the effects of multiple safety management model on improving the safety compliance of stroke patients. Methods This study used prospective control study. A total of 853 stroke patients collected since May 2013 to February 2014 were divided into control group with 438 cases and observation group with 415 cases according to admission time. The control group used the conventional model, the observation group accepted the multiple safety management model, factors were recorded including pathogenic condition, medicine assessment, safety education and functional training. Comparison of safety compliance, satisfaction and rate of adverse nursing events were made in two groups using prospective case-control study. Results The safety compliance was 49. 75% and 63. 68% in control and observation group respectively(χ2 =21. 19,P=0. 01). Rates of fall, bed-falling, pressure sore and unplanned extubation were 1. 99%, 1. 24%, 3. 23% and 5. 22% in control group and they were 0. 25%, 0. 00%, 0. 25% and 0. 75% in observation group (P<0. 01). In terms of satisfaction, the rates were 77. 63% and 92. 47% in control and observation group (χ2 =30. 06,P<0. 01). Conclusions Multiple safety management model contributes to the improvement of safety compliance, decreases adverse nursing events and increases satisfaction in stroke patients. Considering the advantages and operability, multiple safety managements are worth popularization in clinical nursing.
4.Application effect of workshop nursing ward rounds on specialist training for nurse of Neurology Department

Objective To explore effect of workshop nursing ward rounds on specialist training for nurse of Neurology Department.Methods Totals of 134 nurses of Neurology Department were randomly divided in two groups, 71 nurses in the intervention group and 63 nurses in the control group. The nurses of intervention group received specialist training based on workshop nursing ward rounds, while the nurse of control group received specialist training of traditional mode. We compared two groups of nurses about disease-related knowledge and patient satisfaction with nursing work.Results The nurses of intervention group have higher scores than the nurses of control group in following aspects: patient's condition (80.8±9.2), implementation of nursing measures (85.9±8.4), and implementation of health education (86.3±8.1) (t=4.015, 7.075, 9.319;P<0.01). The patient's satisfaction in the intervention group was higher than that in the control group (P<0.05).Conclusions Workshop nursing ward rounds in neurology specialist training can make the responsibility system nursing effectively implemented and improve the patient satisfaction.
5.Zhongfeng Xinglou Tongfu Capsule Improves Inflammatory Damage in Mice with Cerebral Hemorrhage by Mediating CCK-8 to Regulate the IL-10/STAT3 Signaling Pathway

Objective To explore the regulatory effect and mechanism of Zhongfeng Xinglou Tongfu Capsule on the brain inflammatory response of mice with cerebral hemorrhage through cholecystokinin octapeptide(CCK-8)mediated the interleukin 10(IL-10)/signal transduction and transcriptional activator 3(STAT3)signaling pathway.Methods A total of 110 C57BL/6 mice were divided into sham operation group,cerebral hemorrhage group,Zhongfeng Xinglou Tongfu Capsule low-,medium-,and high-dose groups according to the random number table method,with 22 mice in each group.The intracerebral hemorrhage model was established by collagenase method.At 2 hours after the establishment of the model,Zhongfeng Xinglou Tongfu Capsule suspension was intragastrically administered at a dose of 10 mL·kg-1.The sham operation group and the cerebral hemorrhage group were intragastrically administered with the same dose of normal saline.The neurofunctional score and left-turn rate of the mice were recorded.The dry and wet weight method was used to detect brain water content on the 72 h after surgery,and the Western Blot method was used to detect tumor necrosis factor-α(TNF-α),interleukin 1β(IL-1β),interleukin 6(IL-6),interleukin 10(IL-10),and the relative expression levels of JAK-1,STAT3,and p-STAT3 pathway proteins around the hematoma of mice at 72 h after surgery.On the 3rd and 5th day after surgery,Elisa method was used to detect the relative expression level of CCK-8,qPCR method was used to detect the expression levels of IL-10 and STAT3 mRNA.Results Compared with the cerebral hemorrhage group,the NDS scores of mice in the low-,medium-,and high-dose groups of Zhongfeng Xinglou Tongfu Capsules were significantly reduced on the 3rd and 5th days after intervention treatment(P<0.01).And the left-turn rate in the low-dose group on the 3rd and 5th days after intervention treatment was reduced(P<0.05),the left-turn rate in the medium-dose group was reduced on the 3rd day(P<0.05)and the 5th day(P<0.01).Furthermore,the left-turn rate was obviously reduced on the 3rd and 5th day in the high-dose group(P<0.01).The brain water content of mice in the low-dose group was reduced(P<0.05),and the brain water content of mice in the medium-and high-dose groups was significantly reduced(P<0.01).In the low-dose group,the expressions of pro-inflammatory factors TNF-α,IL-6 and JAK-1,STAT3,and p-STAT3 pathway proteins were significantly reduced(P<0.01),while the expression of IL-1β was reduced(P<0.05),the expression of IL-10 increased significantly(P<0.01).The expression of pro-inflammatory factors TNF-α,IL-1β,IL-6,as well as JAK-1,STAT3,and p-STAT3 pathway proteins in the medium-and high-dose groups was significantly reduced(P<0.01),the expression of anti-inflammatory factor IL-10 increased significantly(P<0.01).The content of CCK-8 in the low-dose group was increased on the 3rd day after treatment(P<0.05),and the content increased significantly on the 5th day(P<0.01).The content of CCK-8 in the medium-and high-dose groups obviously increased on the 3rd and the 5th day after treatment(P<0.01).The relative expression level of IL-10 mRNA was significantly increased after treatment(P<0.01),but the relative expression level of STAT3 mRNA was significantly decreased after treatment(P<0.01).Conclusion Zhongfeng Xinglou Tongfu Capsule can target the content of brain-gut peptide CCK-8 and regulate the IL-10/STAT3 signaling pathway to reduce the secondary inflammatory reaction and cytotoxic brain edema after cerebral hemorrhage.
