1.The current situation investigation on oral care of the tube intubation in the emergency department of the 3A hospital in Shanxi province

Objective To investigate the status quo of oral nursing in the emergency department of top three hospitals in Shanxi province, and to analyze the existing problems so as to provide the basis and reference for improving the clinical operation standard. Methods In the form of WeChat questionnaire, a questionnaire survey was conducted to investigate 190 nurses who met the requirements in emergency departments of 6 top three hospitals in Shanxi province. Results All the 6 hospitals surveyed did not incorporate oral care into the nurse examination program; 28.42%(73/190) of the subjects had oral assessments of endotracheal intubation according to their clinical experience;chlorhexidine (70.00%,133/190) and physiological saline (65.79%,125/190) were the most commonly used oral care solutions in clinic;48.42%(92/190) of nurses performed this operation for 6-10 min;91.58%(114/190) of nurses believed that oral care played an important role in the recovery of these patients. Conclusions The oral care and oral care of the patients in the emergency department are mostly clinical experience, so it is urgent to establish a unified standard of implementation on the basis of evidence. The professional knowledge of oral nursing of the patients undergoing oral trachea intubation is low, so it is suggested that the training of emergency nurses be strengthened and the operation should be included in the assessment of nursing operations.
2.The qualitative study of effectiveness of expressive writing intervention on patients with laryngeal cancer post total laryngectomy

Objective:The aim of this study was to explore the perception and cognition of patients with laryngeal cancer post total laryngectomy accepted expressive writing intervention, analyze the intervention effect from the perspective of patients, and provide a basis for psychological intervention of these patients.Methods:From July to August 2021, the descriptive research method was applied to 13 patients with laryngeal cancer post total laryngectomy who had participated in the expressive writing intervention hosted in the General Hospital of Ningxia Medical University, in order to understand the patients′ perception and cognition of the intervention, and the data were analyzed by using content analysis method.Results:A total of 2 themes and 9 sub themes were extracted. Theme 1: the positive feedback of expressive writing intervention: relieve related physical symptoms and improve sleep; reduce anxiety and regulate emotions effectively; reduce psychological stress and promote positive coping; reduce stigma and improve social withdrawal; make up for lost pronunciation and clear mind. Theme 2: Difficulties and challenges in the application of expressive writing intervention: physical discomfort affects the compliance of patients, poor economic status affects the enthusiasm of patients to participate, poor social and family support affects the willingness of patients to participate, and literacy and writing habits affect the completion of participation.Conclusions:The application of expressive writing intervention promotes the psychological and social functional rehabilitation of patients with laryngeal cancer post total laryngectomy and has satisfactory effects and certain feasibility. The researchers should fully consider the influencing factors of intervention and optimize the plan in the future.
3.Effects of personnel training and management based on cognitive conflict in Interventional Therapy Department

Objective:To explore the effect of training and management based on cognitive conflict on nursing ability and adverse events of nurses in Department of Interventional Therapy.Methods:Using the convenient sampling method, 90 nurses in the Department of Interventional Therapy of three Class Ⅲ Grade A hospitals in Shanxi Province from January to October 2021 and 170 patients with interventional therapy admitted to three Class Ⅲ Grade A hospitals in Shanxi Province from February 2020 to October 2021 were selected as the research object. The patients admitted before the implementation of personnel training and management based on cognitive conflict from February to December 2020 were set as the control group, and the patients admitted after the implementation of personnel training and management based on cognitive conflict from January to October 2021 were set as the research group, with 85 cases each. We compared the theoretical knowledge, nursing skills, clinical strain ability and critical thinking ability of nurses before and after the implementation of personnel training and management based on cognitive conflict, as well as the occurrence of adverse events and nursing satisfaction of patients.Results:Three and six months after the implementation of personnel training and management based on cognitive conflict, the scores of intervention theoretical knowledge, nursing skills, clinical strain ability and critical thinking ability of nurses were higher than those before the implementation, and the scores in six months after the implementation were higher than those in three months after the implementation, with statistically significant differences ( P<0.05) . The total incidence of adverse events in the research group was 11.76% (10/85) , which was lower than 23.53% (20/85) in the control group with a statistical difference ( P<0.05) . The patient satisfaction of the research group was 95.29% (81/85) , which was superior to 85.88% (73/85) of the control group, with a statistically significant difference ( P<0.05) . Conclusions:The implementation of personnel training and management based on cognitive conflict in the Interventional Therapy Department can improve the quality of nursing service of nurses, reduce the risk of adverse events, and increase patient satisfaction, which is worthy of clinical application.
4.Construction and practice of OEC nursing management model for emergency critically ill patients

Objective To construct and explore the clinical application effect of OEC nursing management model for emergency critically ill patients.Methods Haier's OEC management concept is integrated with the nursing management of emergency critically ill patients,and the OEC nursing management model for emergency critically ill patients is constructed from three aspects:goal system,daily cleaning system and incentive mechanism.Before and after the implementation of OEC nursing management model,the core abilities of nurses in critical cases,the results of nursing quality control for critically ill patients,the enthusiasm of nurses and the satisfaction of patients and doctors on nurses'work were compared.Results After the implementation of the project,the scores of all dimensions and total scores of the questionnaire of core abilities of nurses in critical cases were significantly higher than those before the implementation(P<0.05).In addition to the implementation dimension of graded nursing,the scores of all dimensions and total scores of nursing quality control for critical ill patients were significantly higher than before implementation(P<0.05).The working enthusiasm of nurses was significantly higher than before implementation(P<0.05).Both patients and doctors'satisfaction with nurses was significantly higher than before implementation(P<0.05).Conclusion OEC nursing management model for emergency critically ill patients can improve the core abilities of nurses in critical care,improve the quality of nursing for critically ill patients,stimulate nurses'work enthusiasm,and enhance patients'and doctors'satisfaction with nurses'work.
