1.Palliative resection of advanced primary hepatocellular carcinoma

Objective To evaluate the effect of palliative resection of advanced primary hepatocellular carcinoma (PHCC). Methods 98 patients with advanced PHCC were divided randomly into two groups in our hospital from March 1996 to Jan. 2000:(1) Therapy group (49 cases), dealt with palliative resection of liver cancer and implanted with a drug delivery system (DDS). (2) Control group (49 cases), only implanted with DDS. Results In therapy group and control group, The decline rate of AFP was 60.0% and 31.7% respectively (P<0.05); and the survival rates of 0.5, 1, 3 years after operation were 85.7% (42/49), 60.5%(23/38), 45.4%(10/22) and 67.3%(33/49), 32.5%(13/40), 10%(2/20) (P<0.05). Conclusions Palliative resection of liver cancer can improve survival duration and life quality of patients with advanced HCC.
2.A review of family and parenting interventions in children with attention-deficit/hyperactivity disorder

Family environment plays an important part of the occurrence,development and outcome of attention-deficit/hyperactivity disorder (ADHD).Family and parenting interventions focus on the inter environment,aiming at forming an effective behavioral management,a better communication strategy,which will be conductive for child's symptoms as well as functions.Parents'emotion and self-efficacy as well as family functioning will be affected also.Common family and parenting skills include parent training and systematic family therapy,which respectively focus on behavior management and the whole family in view of system.The Multimodal Treatment Study of Children with ADHD (MTA) may have potential effects compared with single medical treatment and behavioral treatment.In China,parent training and structural family therapy have been proved to be effective.However,further consideration and experiments are necessary,especially on the long-time ending and individual management.
3.The expression of autophagy and related genes in patients with active systemic lupus erythematosus

Objective To investigate the autophagy and the expression of its related genes in the peripheral blood mononuclear cells (PBMCs) of active systemic lupus erythematosus (SLE) patients.Methods Patients with newly onset or recently-diagnosed SLE (n=20) were enrolled.RA patients (n=10) and healthy blood donors (n=10) were used as controls.PBMCs from all subjects were immediately isolated by Ficoll-Hypaque density gradient centrifugation.And then monocytes were removed by wall sticking method.The morphology was observed using transmission electron microscopy (TEM).Messenger RNA (mRNA) expression of Beclin 1 and microtubule-associated protein 1-light chain 3 (MAPLC3) were determined by reverse transcription polymerase chain reaction (RT-PCR) and real-time quantitative RT-PCR respectively.Results TEM showed autophagic phenomenon in PBMCs from active SLE.On the mRNA level,expression of Beclin 1 and LC3 was significantly increased in fresh isolated SLE cells as compared with RA or healthy donor's PBMCs.Conclusion Based on these results,we can conclude that autophagy occurs in active SLE and the expression of its related genes is significantly higher in active SLE than in RA or normal controls.The enhanced autophagy may indicate its role in the pathogenesis of SLE.

Objective To evaluate the feasibility and safety of vaginal enlarged amputation of cervix to treat patients with cervical cancer of stage Ⅰ a1 and cervical intraepithelial neoplasia grade Ⅲ(CIN Ⅲ)who were unfit for conization surgery.Methods From July 2002 to May 2007,patients with cervical cancer at stage Ⅰ a1,diagnosed by pathology after loop electrosurgical excision procedure(LEEP),large area CIN Ⅲ(the area of lesion≥3/4 on colposcopy),CIN Ⅲ coexisted with vaginal intraepithelial neoplasia (VAIN)in the superior segment of vagina,CIN Ⅱ-Ⅲ recurrence or with residual lesion,positive margin after conization of cervix,who wanted to preserve fertility and(or)corpus uteri were selected to receive vaginal enlarged amputation of cervix.Results Forty-eight eases including 5 with cervical cancer in stage Ⅰ a1,38 with large area CIN Ⅲ(9 with gland involvement),2 with residual lesion and 2 with positive margin after LEEP,1 recurrence after cold knife conization,received the procedure successfully.The median age was 34 years(range 27-40),median operation time was 60 minutes(range 30-100),median blood loss was 40 ml(range 5-300),and median hospital stay was 10 days(range 7-17).After follow-up 1-39 months,no patient had postoperative complications and recurrence,and all patients resumed normal menstrual cycle and sexual life.Condusion Vaginal enlarged amputation of cervix appears to be a safe and feasible procedure for patients with cervical cancer at stage Ⅰ a1 and CIN Ⅲ who are unfit for conization surgery.
6.Advances in research on therapeutic strategies of targeting pancreatic cancer stroma

Pancreatic cancer stroma plays a critical role in tumor progression, invasion, metastasis and resistance.Targeting tumor cell alone could not meet the demand for prolonging patients'' survival.Growing studies have laid emphasis on developing combined regimens between targeting pancreatic cancer stroma and chemotherapy, radiotherapy and immunotherapy.We are faced with some new opportunities in spite of the great challenges brought to the research and development of targeting drugs owing to the complicated stroma components, crosstalking signal pathways and abnormal angiogenesis of pancreatic cancer.In this article, recent advances in therapeutic strategies of targeting pancreatic cancer stroma are reviewed and analyzed from the aspects of extracellular matrix (ECM), cancer associated fibroblasts (CAFs) and vessels, in the hope of providing some novel ideas for targeting therapy against pancreatic cancer.
7.Executive function performances in adults with attention deficit hyperactivity disorder

Objective To explore the executive function profiles in adults with attention deficit hyperactivity disorder(ADHD)in a Chinese sample population. Methods We identified 68 outpatients fulfilling the diagnostic criteria for ADHD according to the Diagnostic and Statistical Manual of Mental Disorders-IV(DSM-IV)and 74 normal adults.All subjects completed clinical interview, Behavior Rating Inventory of Executive Function-Adult Version (BRIEF-A) and Cambridge Neuropsychological Test Automated Battery (CANTAB). Results ADHD group had higher scores in all factors of BRIEF-A questionnaire compared with normal control (P<0.01). ADHD group had a higher score in SWM strategy score [(30.84±5.58)vs.(28.80±5.85), t=2.056,P=0.042]compared with control group. Compared with control group, ADHD group had a lower score in RVP total hits [37(33,41.75) vs.3(37,48), Z=-3.184,P<0.01]. Total correct rejections[(508(497.5,518)vs. 516(503,529.5),Z=-2.229,P=0.0260)]were significantly lower,while RVP total misses were significantly higher [(17 (12.25,21)vs.12(7,20),Z=2.293,P=0.022)].ADHD-RS-IV attention deficit score was positively correlated with SWM strategy while RVP total misses were negatively correlated with RVP total hits and RVP correct rejections.Working memory score in BRIEF-A was significantly correlated with the SWM strategy,RVP total hits, RVP total correct rejections and RVP probable of hit as well as RVP total misses. Conclusion Adults with ADHD have executive functioning deficits which is correlated with ADHD core symptoms.
8.The mediating role of illness perception between fear of disease progression and sleep quality in patients with ovarian cancer during chemotherapy

Objective:To explore the mediating role of illness perception in the relationship between fear of disease progression and sleep quality in patients with ovarian cancer during chemotherapy, and to provide a theoretical basis for improving sleep quality in patients with ovarian cancer during chemotherapy.Methods:From January to August 2022, 300 patients with ovarian cancer undergoing chemotherapy in Shandong First Medical University Affiliated Provincial Hospital, Qilu Hospital of Shandong University, Shandong Cancer Hospital were included by convenient sampling. A cross-sectional questionnaire survey included the Chinese version of the Fear of Progression Questionnaire Short Form for cancer patients, the Brief Illness Perception Questionnaire, and the Pittsburgh Sleep Quality Index. Bivariate factor analysis, Spearman correlation analysis, and the Bootstrap confidence interval evaluation method were used.Results:A total of 287 valid questionnaires were collected. The scores of fear disease progression, illness perception, and sleep quality were 30.00 (22.00, 36.00), 37.00 (32.00, 44.00), and 6.00 (3.00, 11.00), respectively. Sleep quality was positively correlated with fear disease progression ( r=0.250, P<0.001) and illness perception ( r=0.326, P<0.001). Illness perception played a partial mediating role in the relationship between fear of disease progression and sleep quality, accounting for 41.4% of the total effect. Conclusions:Ovarian cancer patients during chemotherapy reported poor sleep quality. In clinical practice, health care providers including nurses can take interventions aimed at reducing fear of disease and improving illness perception level to improve the sleep quality of ovarian cancer patients during chemotherapy.

Bioinformatics methods were used to annotate the suspicious pathogenic genes of congenital microtia in detail, and construct the protein-protein interaction (PPI) networks to clarify the function and interaction of pathogenic genes, so as to predict the potential pathogenic genes.

Objective:
To identify a pig model with bilateral external ear defects accompanied by aural atresia and investigate its application in plastic surgery.
Methods:
Erhualian×Shaziling F2 pig inbreeding population was introduced, and examination of external ear morphology was conducted in all individuals. Temporal computed tomography scanning and mutational detection of HOXA1 gene were conducted in one affected and one normal individuals.
Results:
In Erhualian×Shaziling F2 pig inbreeding population, there were 57 normal and 18 affected individuals among the 75 pigs. Affected subjects presented bilateral external ear defects accompanied by aural atresia; temporal computed tomography scanning showed bilateral aural atresia and dysplasiaof middle ear; and gene detection identified homozygous mutation of HOXA1 gene.
Conclusions
Pig model with HOXA1 gene homozygous mutation resembles human microtia at different levels. Our findings provide the theoretical basis for its application to study further pathological mechanism for human microtia.
