1.Screening analysis of deafness gene in neonatus

Objective To analyze the molecular epidemiology characteristic of neonatal deafness susceptibility genes .Methods Hearing screening and deafness susceptibility genes screening were performed in 1 674 cases of newborn to analyze the epidemiolog‐ical characteristics .Results Among 1 674 cases of neonatus ,37 cases were with deafness susceptibility gene abnormalities ,inclu‐ding 2 cases of 176 del 16 mutations ,5 cases of 299 del AT heterozygous mutation ,16 cases of 235 del C mutation ,9 cases of IVS7‐2A>G heterozygous mutations ,1 case of 2168A> G mutation ,2 cases of 538C> T heterozygous mutation ,2 cases of 1494C> T mutation ,and the positive rate was 2 .21% .Conclusion Hearing screening combined with deafness susceptibility gene screening could detect possible hearing loss children from molecular level ,providing favorable reference for the early detection ,predict and in‐terventions .
2.Comparison of efficacy between veno-venous extracorporeal membrane oxygenation (VV-ECMO) and VV-ECMO combined with prone position ventilation for the treatment of acute respiratory distress syndrome

Objective:To observe the effects of veno-venous extracorporeal membrane oxygenation (VV-ECMO)combined with prone position ventilation (PPV) on oxygenation index (PaO 2/FiO 2), respiratory compliance (Crs) and vasoactive inotropic score (VIS) in severe acute respiratory distress syndrome (ARDS) patients. Methods:Eighteen patients with severe ARDS requiring VV-ECMO support in Liuzhou People's Hospital from June 2018 to April 2020 were selected for retrospective analysis, and 8 patients among of these cases received PPV after VV-ECMO. The differences in PaO 2/FiO 2, VIS and Crs before and 1, 2 or 3 days after treatment were compared between VV-ECMO group and VV-ECMO combined with PPV group, as well as the differences in these indices before PPV and 2 hours after PPV daily in VV-ECMO combined with PPV group. The incidence of adverse events in two groups were also observed. Results:Before treatment, there was no significant difference in PaO 2/FiO 2, Crs between two groups. Over time, PaO 2/FiO 2 and Crs increased and VIS decreased in both groups. Compared with before treatment, there were statistically significant differences in PaO 2/FiO 2 and VIS from 1 day after treatment [PaO 2/FiO 2 (mmHg, 1 mmHg = 0.133 kPa): VV-ECMO group was 197.75±39.80 vs. 75.57±7.44, VV-ECMO combined with PPV group was 255.20±31.92 vs. 68.24±11.64; VIS: VV-ECMO group was 5.51±3.72 vs. 10.20±7.10, VV-ECMO combined with PPV group was 6.73±3.32 vs. 14.50±2.48, all P < 0.05], up to 3 days after treatment [PaO 2/FiO 2 (mmHg): VV-ECMO group was 231.96±32.76 vs. 75.57±7.44, VV-ECMO combined with PPV group was 285.61±19.40 vs. 68.24±11.64; VIS: VV-ECMO group was 2.26±1.90 vs. 10.20±7.10, VV-ECMO combined with PPV group was 2.13±1.55 vs. 14.50±2.48, all P < 0.05], and the PaO 2/FiO 2 1 day and 3 days after treatment in VV-ECMO combined with PPV group were significantly higher than those in VV-ECMO group (mmHg: after 1 day of treatment was 255.20±31.92 vs. 197.75±39.80, after 3 days of treatment was 285.61±19.40 vs. 231.96±32.76, both P < 0.05). Before treatment, Crs of VV-ECMO combined with PPV group was significantly lower than that of VV-ECMO group (mL/cmH 2O: 17.91±0.82 vs. 20.54±1.26, P < 0.05). From 1 day after treatment, the Crs in VV-ECMO combined with PPV group was significantly higher than that before treatment (mL/cmH 2O: 21.20±1.50 vs. 17.91±0.82), the peak value was (24.93±2.18) mL/cmH 2O on 3 days after treatment, however, there was no significant difference between the two groups (all P > 0.05). In VV-ECMO combined with PPV group, compared with before PPV treatment, the PaO 2/FiO 2 and Crs of 2 hours after PPV treatment in 1, 2 and 3 days were significantly rose, and it reached the highest level in 3 days [PaO 2/FiO 2(mmHg): 285.61±19.40 vs. 189.91±28.34, Crs (mL/cmH 2O): 24.93±2.18 vs. 23.35±1.45, both P < 0.05]; the VIS was only increased in 2 hours after PPV treatment on the first day than before (6.73±3.32 vs. 6.38±3.22, P < 0.05). There were no related serious adverse events happened after PPV treatment. Conclusions:The combination of PPV during VV-ECMO could further increase PaO 2/FiO 2, improve hypoxemia and implement further protective lung ventilation to reduce the potential hazards during mechanical ventilation. In addition, no serious adverse events were observed in this study, suggesting PPV is safe during VV-ECMO.
3.Relationship between chronotype and mental health problems among middle school students in Taiyuan

Objective:
To examine mental health problems among adolescents in Taiyuan City and its relationship to chronotype, so as to give a scientific basis for supporting mental health of middle school students.
Methods:
From October to November 2021, a total of 2 621 pupils were enrolled from three junior high schools and two senior high schools in Taiyuan by using a multi stage stratified cluster random sampling method to investigate demographic characteristics, mental health status, and chronotype. Chi squared test, Spearman correlation analysis and Logistic regression were used to analyze the relationship between sleep patterns and mental health problems in middle school students.
Results:
The proportion of morning, intermediate and evening types of middle school students sleep were 17.9%, 65.6%, and 16.4%, respectively. The overall detection rate of mental health problems was 13.8 %, which was statistically significant among middle school students in different grades, self perceived family economic status, the number of friends and sleep patterns ( χ 2=42.69, 29.15, 46.02, 93.99, P <0.05). After adjusting for grade, self perceived family economic status and the number of friends, Logistic regression analysis revealed that mental health problems were positively associated with evening type ( OR =2.84) and negatively associated with morning type ( OR =0.61)( P <0.05).
Conclusion
Chronotype is associated with mental health problems among middle school students, with a higher risk for evening type and a lower risk for morning type. Mental health of middle school students can be enhanced by changing their chronotype.
4.Qualitative study on post-discharge coping difficulty of delivery women based on social ecosystem theory

Objective:To analyze post-discharge coping difficulty of delivery women based on social ecosystem theory.Methods:From March to May 2022, 23 delivery women from Affiliated Hospital of Guilin Medical University were selected as research objects by the purpose sampling method. The phenomenological research method was used to conduct face-to-face semi-structured interview. The content analysis method was used to analyze the data and extract the theme.Results:A total of 3 themes and 11 sub-themes were extracted. The micro level included 1 theme, namely care stress and self recovery problems coexist, including difficulty in breastfeeding, feeling powerless to raise children, anxiety, disturbance in body recovery of delivery women, prominent sense of sleep deprivation, insufficient knowledge of infant care and body recovery. The mesoscopic level included 1 theme, namely changes in life atmosphere, including caregiver relationship conflict, lack of family support and sense of connection. The macro level included 1 theme, namely interaction of social environment, including living environment not meeting expectations, sense of economic burden, expectation of instant professional guidance.Conclusions:The post-discharge coping difficulty of delivery women is complex and varied. Medical staff should use information technology, family synchronous empowerment, social worker intervention and other methods to meet the needs of delivery women, reduce their post-discharge coping difficulties.
5.Effects of responsibility management combined with nursing intervention based on Rosenthal effect in patients with multiple myeloma

Objective:To explore the effect of responsibility management combined with nursing intervention based on Rosenthal effect on the quality of life and adverse reactions of patients with multiple myeloma.Methods:Using convenience sampling method, 75 patients with multiple myeloma admitted to the Hematology Department of Henan Provincial People's Hospital from March 2016 to July 2018 were selected as the control group conducting responsibility management. A total of 75 patients with multiple myeloma admitted from December 2018 to March 2021 were selected as the observation group, and the nursing intervention based on Rosenthal effect was applied on the basis of the control group. The quality of life of the two groups of patients before and after the intervention was compared, and the incidence of adverse reactions during the admission of the patients was counted.Results:Before the nursing intervention, there was no statistically significant difference in the scores of various indicators of the quality of life between the two groups of patients ( P>0.05) . After the nursing intervention, three functional indicators of physical function, cognitive function, emotional function, as well as scores of the fatigue, nausea and vomiting, loss of appetite, insomnia symptoms and general health of patients in the observation group were better than those in control group, and the differences were statistically significant ( P<0.05) . The incidence of adverse reactions in the observation group was 9.3% (7/75) , which was lower than 24.0% (18/75) in the control group, and the difference between the two groups was statistically significant ( P<0.05) . Conclusions:Responsibility management combined with nursing intervention based on Rosenthal effect can improve the quality of life of patients with multiple myeloma and reduce the incidence of adverse reactions, which is worthy of clinical application.
6.Study on Protective Effects of the Ethanol Extract of Garcinia oblongifolia on LPS-induced RAW 264.7 Cell Inflammatory Injury

OBJECTIVE:To investigate the anti-inflammatory activity of 70% ethanol extracts from Garcinia oblongifolia (GOEE)on LPS-induced RAW 264.7 cells and its potential molecular mechanism. METHODS :GOEE was obtained after the fresh G. oblongifolia epicarp refluxed with 70% ethanol. The contents of total phenol and total flavonoids were determined by Folin-Ciocalteau assay and UV spectrophotometer. MTT assay was used to detect the cytotoxicity of different doses of GOEE. The inflammatory model was induced in RAW 264.7 cells by lipopolysa- ccharide (LPS). Using dexamethasone and N-acetyl-L-cysteine as positive control ,Griess assay and 2′,7′-dichloro- fluorescein assay were used to detect the contents of NO in cell culture medium and ROS in cells. The levels of TNF-α,IL-6,and IL- 1β in cell culture medium were measured by ELISA. The protein expression of p 65,p-p65,IκBα,p-IκBα,HO-1 in cells and NRF 2 in nucleus were determined by using Western blotting assay. RESULTS:The contents of total phenol and flavonoids in GOEE were (20.191±1.264)and(12.571±0.020)mg/g,respectively. At the concentration below 500 μ g/mL, GOEE had no significantly effect on survival rate of RAW 264.7 cells(P> 294043)0.05). Compared with control group ,the contents of NO and ROS,the levels of TNF-α,IL-6 and IL- 1β,ratio of p-p 65 top65,ratio of p-IκBα to IκBα,protein expression of NRF 2 were increased significantly in LPS model group (P<0.05 or P<0.01). Compared with LPS model group ,the contents of NO(except for GOEE 50 μg/mL group)and ROS ,the levels of TNF-α,IL-6 and IL- 1β,ratio of p-p 65 to p 65 and ratio of p-IκBα to IκBα were decreased significantly in GOEE groups and positive control groups ,while protein expression of HO- 1 and NRF 2 were increased significantly (P<0.05 or P<0.01). CONCLUSIONS:GOEE attenuates LPS-induced macrophages inflammation injury by inhibiting the inflammatory response and the phosphorylation of NF-κB pathway,promoting NRF 2 protein transportation to the nucleus.
