Objective To study epicanthus anatomy in cadavers in order to explore a more ideal technique for the correction of epicanthus. Methods On the basis of epicanthus anatomic dissection in 6 adult cadavers, canthoplasty and a new fixation technique for correcting epicanthus was performed clinically in the following procedures: incising the epicanthal fold and partial inner canthus ligament, pulling the new canthus medially and fixing it on the lateral nasal aponeurosis. Results Anatomic dissection showed that the epicanthus was caused by malposition of the orbicularis oculi muscule. More than 200 patients with epicanthus had been operated on by this technique and 165 of them were available for postoperative follow up for 6 months to 4 years with better cosmetic results. Conclusions This technique has less tissue damage and more favorable postoperative scar. It is a simple and reliable surgical approach for correction of epicanthus.

Objective: To investigate the characters of the ageing changes in TMJ of New Zealand rabbit from 7 to 10 months old. Metho ds: 12 New Zealand rabbits aged 7,8,9 and 10 months old and 3 in eac h age group, were killed under anesthesia. Bilateral TMJs were histologically examined with HE stainnig. Results: Quite similar histolo gical structures were found in the TMJs in different age groups. Very few change s occurred in the fibre layer of osteo-articular surface. The thickness of the proliferate layer was decreased slightly in anterior and middle part saggitally, resulting in comparable increase of the hypertrophy layer with age. No hypertro phy layer was found in the cartilage over tempora articular surface. The thickne ss of the cartilage-like osteo layer was uneven, the thickest part was in the area towards condyle anterior slope and without age change. The thickness of d isc increased a little bit with age. Conclusion: The thick ness of the cartilage over TMJ articular surface of New Zealand rabbit at the a ge from 7 to 10 months old changes a little. The layer structures keep considera bly constant.

Objective To explore a new method for lacrimal passage irrigation after laser dacryocystoplasty surgery. Method One-hundred patients (104 eyes), which underwent laser dacryocystoplasty surgery combined with lacrimal drainage tube indwelling, were divided into two groups. In Group A (50 patients, 52 eyes), lacrimal passage irrigation was performed by traditional No.5 needle. While in Group B (50 patients, 52 eyes), it was done by No.7 blunt round needle designed by the researchers. Lacrimal passage irrigation was performed three times, each at the 3rd day, 1 week and 1 month after surgery. The silicone tube was removed 3 months after surgery and the treatment was evaluated. Results The total effective rate was 64% (32/50) in Group A and 92%(46/50) in Group B. The difference was statistically significant (χ2=18.537, P < 0.01). Conclusions The No.7 blunt round needle showed better effect when used for lacrimal passage irrigation after laser dacryocystoplasty surgery. It could improve the efficiency of lacrimal passage irrigation, thus decreasing irrigation times and reducing psychological pressure for the patients, which is worthy of clinical application.

Objective To explore the role of problem-based learning(PBL) nursing ward round on critical thinking capacity of nurses from surgical departments. Methods All nurses from surgical depart-ments were randomly divided into the experimental group(86 nurses) and the control group(85 nurses) by drawing lots. Objects from the experimental group received PBL nursing ward round training twice a week for 8 months, while those from the control group used the traditional nursing ward round training. The influ-ence of the different teaching methods on the critical thinking capacity of the nurses was assessed before and after the training, using critical thinking disposition inventory-Chinese version (CTDI-CV). Results Total critical thinking capacity was improved significantly in the experimental group after the training. All sorts of critical thinking characteristics were strengthened in different degree. Conclusions PBL nursing ward round training can effectively facilitate the critical thinking capacity of surgical nurses.

Objective To investigate the change of adiponectin level with menopause status in women aged 40 to 65, and its relationship with androgen. Methods A cross-sectional study included woman (aged from 40 to 65) who were in hospital for routine check-up at the Sun Yat-sen Memorial Hospital from August to October in 2013. All subjects underwent laboratory examinations of adiponectin, sex hormone binding globulin (SHBG), dehydroepiandrosterone-sulfate (DHEA-S), total testosterone (TT), collected anthropometric measurements and then calculated free androgen index (FAI) and body mass index (BMI). According to their menstrual status, the subjects were divided into 4 groups: premenopausal group with 119 subjects, perimenopausal group with 60 subjects, early postmenopausal group with 62 subjects, late postmenopausal group with 64 subjects. Results (1) Adiponectin levels declined to its lowest level in menopausal transition and gradually becoming higher after menopause, which showed a U-shaped trajectory. When compared adiponectin levels in late postmenopausal group [(13 ± 5) mg/L] with those in perimenopausal [(8 ± 6) mg/L] or early postmenopausal group [(9 ± 6) mg/L], it all showed significantly difference (P<0.05). (2) Both the adiponectin levels were negatively correlated with waistline in the 4 groups (premenopausal group, r=-0.276;perimenopausal group, r=-0.334;early postmenopausal group, r=-0.211;late postmenopausal group, r=-0.218; all P<0.05). Levels of adiponectin were positively correlated with SHBG (r=0.536, P<0.05) and negatively with FAI (r=-0.363, P<0.05) in menopausal transition, while in late postmenopausal group, negatively correlated with level of DHEA-S (r=-0.450, P<0.05). When adjusted for age, BMI and waistline, the above correlations still exist. Conclusions Adiponectin levels declined to its lowest level in menopausal transition and gradually becoming higher after menopause, which showed a U-shaped trajectory during the sequential menopause status transition in middle aged women. Low level of adiponectin in menopausal transition is closely associated with the relative excess androgen occurred during this stage.

BACKGROUND:In allogene hematopoietic stem cell transplantation, the choice of preconditioning scheme is an important link of the success of hematopoietic stem cell transplantation, and a major research direction of stem cell transplantation. The myeloablative pretreatment scheme has great toxicity, and pretreatment related death rate is high. Thus,it is necessary to explore an ideal pretreatment scheme to expect a decrease in side effects and relapse.OBJECTIVE:To observe the effect of modified Bu/CY pretreatment regimen for treating hematologic malignancies.METHODS:The 8 patients were selected at the Department of Hematology,Haikou Municipal People's Hospital Affiliated to Xiangya School of Medicine,Central South University from November 2003 to March 2008,and received modified Bu/CY pretreatment:cytarabine 2.0-3.0 g/(m2·d)×2 d,intravenous drip, for 24 consecutive hours;myleran 4 mg/(kg·d)×3 d;cyclophosphamide 50 mg/(kg·d)×2 d;methyl-cyclohexyl nitrosourea 25 mg/(m2·d)×1 d;antithymocyte globulin 25 mg/(kg·d)×4 d. We have increased the arabinosylcytosin dose twice, and changed to a 24-hour infusion via intravenous drip,so that pretreatment strength was increased and promote lasting implantation of hematopoietic stem cells, based on the modified program.Graft-versus-host disease(GVHD)prevention:cyclosporin A and mycophenolate mofetil would be used advanced to minus 7 days (one day before stem cell transfusion is minus 1)based on the classic methotrexate regimen. The ABO blood group changes and DNA were tested in patients before and after ransplantation.RESULTS AND CONCLUSION:①The detection of hematopoietic reconstitution after transplantation:All patients have received hematopoietic reconstitution, with no pretreatment-related death. The white blood cells reduced to 0 after-3- +7 days of hematopoietic stem cell transplantation, and continues to(3-22)days,+10- +21 days white blood cells > 1.0×109/L,+11- +51 days, platelets > 20x109/L.②Incidence of GVHD:of 8 patients, there were GVHD Ⅳ grade(intestinal)in 1 case, acute graft-versus-host disease grade Ⅰ-Ⅱ in 3 cases.Above-mentioned results indicated that the further modification of BU/CTX2 regimen may be an effective pretreatment program, with a few side effects, which is better than the classic total-body irradiation/CY regimen. What's more,it is simple accurate, reliable rote of anti-leukemia and will be a safe and effective method for treating hematologic malignancies.
