1.Clinical study of psychological intervention combined with bisoprolol and trimetazidine in the treatment of heart failure with left ventricular ejection fraction

Objective To study the clinical effect of psychological intervention combined with trimetazidine and bisoprolol in the treatment of left ventricular ejection fraction with heart failure. Methods 40 patients with heart failure were enrolled in this study. 40 patients were divided into control group and study group by digital randomization. There were 20 patients in each group and the patients in the control group.The patients were treated with trimetazidine and bisoprolol. The study group was treated with psychological intervention on the basis of the treatment. The clinical efficacy, adverse reactions, left ventricular ejection fraction and anxiety self-rating were analyzed (SAS), self-rating depression scale (SDS) score. Results The total effective rate of the clinical treatment group was 90.00% higher than that of the control group (70.00%)P<0.05. There was no significant difference between the two groups in the adverse reaction. The mean left ventricular ejection fraction of the study group was (40.25 ± 5.24), which was significantly lower than that of the control group (43.92 ± 5.30). The scores of the study group were (50.26 ± 1.35) and (51.38 ± 2.61) respectively in SAS and SDS scores (54.36 ± 2.60), (56.98 ± 3.25) in the control group, (P<0.05). Conclusion The patients with left ventricular ejection fraction retention psychological failure in the application of trimetazidine, bisoprolol treatment based on the necessary psychological intervention, can effectively improve the patient's negative emotions, improve patient compliance, and then improve clinical efficacy, clinical significant application value.

3.The correlation of serum proinflammatory cytokines levels with the severity of acute pancreatitis

Objective To explore the value of serum level of interleukin 6 (IL-6),tumor necrosis factor alpha (TNF-α) and high mobility group box-1 protein( HMGB1) in early assessment the severity and prognosis of acute pancreatitis (AP). Methods Thirty-three severe acute pancreatitis (SAP) patients and 38 mild acute pancreatitis (MAP) patients were selected as study objects;and 51healthy individuals were set as control group. Serum IL-6,TNF-α and HMGB1 concentrations were determined by enzyme-linked immunosorbent assay (ELISA),the association of them and the scores of Ranson,APACHE Ⅱ . Balthazar CT,serum biochemical parameters and prognosis was analyzed.Results The serum IL-6 levels of SAP group,MAP group and healthy control group were (553. 72±175.76) pg/ml,(265. 73±179. 95) pg/ml and (16. 43±3. 32) pg/ml;and there were statistical significance of these three groups (all P<0. 01). There was no significant difference of TNF-α in the three groups (all P>0. 05). The serum HMGB1 levels of SAP group,MAP group and healthy control group were (11. 48±6. 94)μg/L,(6. 13±5. 80)μg/L and (1. 82±0. 64)μg/L respectively,and there were statistical significant of these three groups (all P<0. 05). The correlation coefficient of serum HMGB1 with IL-6 and TNF-α were 0. 896 and 0. 724 (P<0. 01) respectively. The IL-6 level was positively correlated with the scores of Ranson,APACHE Ⅱ and Balthazar CT. The TNF-αconcentration was positively correlated with APACHE Ⅱ score,and the HMGB1 concentration positively correlated with scores of Ranson and Balthazar CT. The levels of IL-6,TNF-α and HMGB1were all positively correlated with the serum creatinine concentration. The IL-6 levels of patients with local and/or systemic complications were significantly higher than those without complications.Conclusion The serum levels of IL-6,TNF-α and HMGB1 are significantly correlated with the severity of pancreatitis,all of them take part in the development of acute renal insufficiency. The high level of serum IL-6 significantly correlated with complications.
4.Association between the nutrition statuses of thiamine and riboflavin and the antioxidant capacity in the middle-aged and elderly

Objective To investigate the nutrition statuses of thiamine and riboflavin and their relationship with the antioxidant capacity and blood glucose and lipids levels in the middle-aged and elderly. Methods Totally 314 subjects aged above 40 years old from communities in Shanghai were enrolled. The 3-day survey of 24-hour's diet recall was made to evaluate the daily nutrient intakes using a nutrition assessment software provided by CDC China. Thiamine and riboflavin levels were detected by urine load test. Fasting blood glucose ( FBG), total choles-terol (TC), triglyceride (TG), superoxide dismutase (SOD) activity and malondialdehyde (MDA) were deter-mined by commercial kits. Results The average daily intakes of thiamine and riboflavin were (0. 82 + 0. 36) and (0. 91 +0.48) rag, respectively. The middle-aged and elderly whose intakes of thiamine and riboflavin reached recommended nutrient intake (RNI) level accounted for 11.8% and 17.2%, while those lower than 60% RNI ac-counted for 51.3% and 49. 4%, respectively. Urine load test showed the deficiency rates of thiamine and ribofla-vin were 65.0% and 58.6%, respectively. Compared with the normal group, plasma MDA, FBG, TC, and TG levels progressively increased and SOD activity decreased in insufficient and deficient groups; the plasma MDA, FBG, and TC levels were significantly different between normal group and thiamine deficient group ( P < 0. 05 ).The FBG and TC levels were negatively correlated with the status of thiamine ( r = - 0. 246, r = - 0. 154, P <0. 05). The MDA and TG levels were negatively correlated with the status of riboflavin ( r = - 0. 136, r =-0. 297, P <0. 05). Conclusions The nutrition statuses of thiamine and riboflavin are poor in the middle-aged and elderly in Shanghai. The nutritional statuses of thiamine and riboflavin are positively correlated with the antioxi- dant capacities and are negatively correlated with the level of FBG, TC, and TG.
5.A survey on the relationship between trace elements and blood lipids and glucose in Shanghai adults

Objective To study the relationship between trace elements and blood lipids and glucose in Shanghai adults. Methods Totally 245 subjects aged above 18 years from two communities in Shanghai were en-rolled into this study. Blood trace elements (Fe, Zn, Cu, Mg, Pb, Mn, and Cd) were examined with atom ab-serption spectrophotometry. Body mass index (BMI), waist hip rate (WHR), and blood pressure (SBP, DBP)were also determined. Results Compared with the normal control group, plasma Fe and Mn levels were signifi-cantly lower while Zn and Zn/Cu levels were significantly higher in hyperlipemia group (P <0.05 or P <0. 01 ) ;plasma Zn, Zn/Cu, and Mn levels were significantly lower while Cu and Pb levels were significantly higher in dia- betes mellitus group (P <0. 05 or P <0. 01 ) . A tendency of lower Mg and higher Cd levels were also found in both disease groups, but there were no statistical significance ( P >0. 05 ). Conclusions The metabolism of trace elements is abnormal in patients with hyperlipemia and diabetes mellitus. Intervention of these trace elements disor-der may be valuable in the prevention and treatment of these diseases.
7.PRODUCTION AND CHARACTERIZATION OF A MONOCLONAL ANTI-HUMAN LEUCOCYTE ANTIBODY(1C34-5)

A monoclonal antibody 1C34-5 raised against human peripheral mononuclear cells was produced. This antibody, a monoclonal IgG1. showed positive reaction with about 90% of T and B lymphocytes, monocytes. granulocytes and bone marrow cells, but negative reaction with red blood cells and platelets by indirect immunoflurorescent technique. 1C34-5 also reacted with leucocytic cell lines except a non-T non-B lymphoid line Reh but not with erythroid line K562, HeLa cells and fibroblast cells so far tested.This antibody was also assayed histologically by an indirect immunoperoxidase technique against a variety of human normal tissue frozen sections. It was found that 1C34-5 bound to all lymphoid tissues including lymph nodes, tonsil, thymus. Peyer's patches and leucocytes scattered in other tissues, but not to all non-hematopoietic tissues as well as erythropoietic foci in fetal liver. Thus. 1C34-5 appears to recognize a human leucocyte antigen specifically.
8.Comparison of clinical characteristics between diastolic heart failure and systolic heart failure

Objective: To retrospectively analyze clinical data of patients with heart failure (HF) for understand clinical characteristics, hospitalization costs and curative effects constituent ratio of diastolic heart failure (DHF) and systolic heart failure (SHF) in order to elevate recognition for two HF. Methods: According level of left ventricular ejection fraction (LVEF), a total of 807 HF patients in Affiliated Union Hospital of Fujian Medical University were divided into to DHF group (LVEF>45%, n=656) and SHF group (LVEF≤45%, n=151). According to NYHA classification, two HF group were respectively further divided into class Ⅱ, Ⅲ, Ⅳ subgroups respectively. Clinical data of two HF group were compared. Results: Structure ratio of DHF and SHF occupied 81.3% and 18.7% respectively. Heart function: Compared with DHF group, there were significant increase in NYHA Ⅲ,Ⅳ class(23.5%, 9.6% vs.47.7%, 21.9%), E/A rate[(0.74±0.31) vs. (1.26±0.56)] in SHF group . Clinical characteristics: Female ratio of DHF group was higher than that of SHF group; Compared with DHF group, there were significant increase in percentages of patients with coronary heart disease, pneumonia, chronic renal dysfunction, arrhythmia and cardiac enlargement, significant decrease in percentage of patients with hypertension, P<0.001 all in above indexes; and significant increase in mean duration of hospital stay [(12.9±8.1)d vs. (14.5±11.6)d, P <0.05], hospitalization cost [12323(8530~33815) yuan vs. 19554(9186~49927) yuan, P <0.05] and mortality rate during hospitalization (0.5% vs. 3.3%, P <0.01) in SHF group. Conclusion: Diastolic heart failure is of high proportion among heart failure. Compared with DHF patients, the heart failure, cardiac damage were more severe, the admission duration, hospitalization costs and mortality rate during hospitalization were more in systolic heart failure.

Objective To investigate the timing and role of systemic retroperitoneal lymphadenectomy in patients with ovarian cancer. Methods From Jan. 1987 to Feb. 1994, 50 patients with ovarian cancer, who underwent retroperitoneal lymphadenectomy on second-look laparotomy (SLL), were retrospectively studied. Results The median age was 49 years. Overall survival at 3 and 5 years were 72% and 62%, respectively. [Abstract ] Objective To investigate the molecular mechanism how genistein increased the sensitivity of colon cancer cell to 5-fluorouracil(FU)and promoted colon cancer cell apoptosis.Methods SW480 colon cancer cell line was chosen as experimental object.Genistein 80 μmol/L and 5-FU 30 μg/mL used separated or combined for 48 hours were set as drug experiment group.There were four experiment groups in this study:control group (without any drug),5-FU group,genistein group,5-FU and genistein combined group.The expression of survivin,Bcl-2,p21 ,caspase3 and caspase 9 in the tumor cells of each group at mRNA and protein level was deteced by real-time quantitative polymerase chain reaction (PCR) and Western blotting.The relative expression quantity of genes was determined by comparative threshold method (2 -ΔΔCT )andβ-actin was taken as an internal reference.Semi quantitative analysis was performed for protein relative expression quantity.The DNA combination activity of nuclear factor(NF)-κB of each group was measured by electrophoretic mobility shift assay (EMSA).Single factor analysis of variance was used for mean comparison among multiple groups and LSD test was for mean comparison between two groups.Results The expression of caspase3,caspase 9 and p21 of 5-FU and genistein combination group at mRNA (1 .903±0.122,2.726±0.050 and 2.541 ±0.393)and protein level (0.534±0.077,1 .161 ± 0.172 and 0.463±0.016)were all higher than those of control group (1 .001 ±0.052,1 .000±0.014 and 1 .001 ±0.037;0.080 ±0.043,0.248±0.059 and 0.139 ±0.021 ),genistein group (1 .559 ±0.038, 2.394±0.095 and 1 .686 ±0.061 ;0.335 ±0.052,0.478 ±0.059 and 0.304 ±0.018)and 5-FU group (1 .198±0.063,1 .051 ±0.043 and 1 .399±0.055 ;0.194±0.015 ,0.337 ±0.036 and 0.231 ±0.011 );the expression of survivin of 5-FU and genistein combined group at mRNA and protein level (0.165 ± 0.018 and 0.216±0.014)were all lower than those of control group,genistein group and 5-FU group (1 .001 ±0.033,0.775 ±0.044 and 0.395 ±0.030;0.594±0.079,0.375 ±0.014 and 0.295 ±0.014), and all the differences were statistically significant (gene,F = 802.865 ,52.760,39.992,187.288, 37.435 ;protein,F =10.466,44.483,19.490,200.011 ,45 .238;all P <0.01);the difference was also statistically significant in the expression of caspase3 and p21 of 5-FU group and genistein group compared with those of control group (LSD test,P <0.05 ).The results of EMSA assay showed that the DNA binding activity of NF-κB protein of genistein group (461.64 ±15.41 )and combined group (585.28 ±7.82) significantly decreased compared with that of control group (1 067.97 ±36.01)and 5-FU group (718.83± 23.18,LSD-test,P < 0.05).Conclusions Genistein combined with 5-FU seemed to have synergistic effects on apoptosis of colon cancer cell.The mechanism was that genistein inhibited the DNA binding activity of NF-κB mediated by genistein,further up-regulated caspase 3,caspase 9,p21 and down regulated survivin.Therefore,genistein may provide assistance in colon cancer chemotherapy.
