1.Relationship between heart diastolic function and pulse wave velocity in patients with diabetes mellitus

Objective To evaluate the relationship between the heart diastolic function and pulse wave velocity in patients with diabetes mellitus.Methods Fifty-five patients with diabetes mellitus underwent echocardiogram exam,PWV,and BNP determination.Patients were divided into two groups according to the results:the group with normal heart diastolic function(n =33)and the group with abnormal heart diastolic function( n =22).Another 30 healthy volunteers served as control group.The values of PWV were compared.between the three groups.The relationship between E/E' and PWV,BNP and PWV were evaluated with correlation analysis.Results The PWV value of abnormal heart diastolic function group was significantly higher than that of other two groups.PWV was positively correlated with E/E'and BNP ( r =0.58,P < 0.05 ; r =0.63,P < 0.05).Conclusion PWV is associated with the heart diastolic function.PWV can serve as an important factor to predict abnormal heart diastolic function in patients with diabetes mellitus.
2.Analysis of expenditure in the medical insurant inpatients

Objectives: To investigate the affecting factors on expenditure in Medical insurant inpatients. Methods: 593 medical insurant inpatients, who were from Feb 1, 2001 to Feb 31, 2002 in Nanjing Jinling hospital, were enrolled in this study. The inpatient expenditure was analyzed from the aspect of operation or not, age, and payment mode, used the descriptive analysis and multi factor analysis on affecting factors of the total expense, self bearing expense, and self affording expense. Results: The hospitalized days, operation or not, severity of disease were important affecting factors. The self bearing expense of medical insurant inpatients was associated with hospitalized days, operation or not, and total expense. While the self affording expense of them was associated with expense of drug, hospital bunk, radiation and others. Conclusions: We can control these associated affected factors to lighten the economic weight of patients, and to reduce the ratio of individual payment.
3.Clinical observation of tegafur pins cisplatin combined with three-dimensional conformal radiotherapy in the treatment of non-operation esophageal carcinoma

Objective To evaluate the therapeutic effetive,toxicity and survial rate of non-operation esophageal carcinoma patients treated by tegafur plus cisplatin combined with three-dimensional conformal radiotherapy (3D-CRT).Methods 184 cases of non-operation patients with esophageal carcinoma were divided into two groups.The cases of treatment group were treated by cisplatin and tegafur.The cases of control group were treated by cisplatin and 5-Fu.All patients received 3D-CRT,and the total radiation dose was 50-70 Gy.Each group had 14 cases treated with primary chemotherapy for 1 course,28 cases for 2 courses,and 50 cases without any chemotherapy.Results The recent effect rate was 69.57 % (64/92) in treatment group and 67.39 % (62/92) in control group,there was no statistical difference (x2 =0.101,P =0.874).Radioactive esophagitis and leukopenia were the main side effect of treatment.The treatment group was significantly lower than that of the control group,the difference had statistical significance.The 1-,2-and 3-year survival rates were 74.9 %,61.6 %,52.8 %,respectively in treatment group and 75.8 %,56.9 %,48.0 %,respectively in control group.The control group survival rates were significantly higher than the treatment group,the difference had statistical significance (x2 =7.325,P =0.007).Conclusions Cisplatin plus tegafur or 5-Fu combined with 3D-CRT is effective way of treatment for non-operation of esophageal carcinoma.The toxicity of cisplatin plus tegafur group is significantly lower than cisplatin plus 5-Fu group,therefor it can improve the quality of life of the patients obviously.

6.Basic skills level of health literacy and its influencing factors in permanent residents in Fujian province in 2020

Objective:To investigate and analyze on the basic skills level of health literacy and its influencing factors in permanent residents in Fujian province in 2020.Methods:In this cross-sectional study, from April to November in 2020, 3 304 urban and rural permanent residents aged 15-69 years in 13 counties (cities or districts) in Fujian province were selected by stratified multistage cluster sampling and random sampling for household questionnaire survey. Based on “Health literacy of Chinese citizens—basic knowledge and skills”, the questionnaire included three aspects: basic knowledge and concept, healthy lifestyle and behavior and basic skills. There were 50 questions in total, including 12 questions about basic skills with a total score of 16 points. A score of 13 points and above was considered as having basic skills. A total of 3 304 questionnaires were issued and reclaimed, and among which 3 250 questionnaires were valid. According to the data of the Sixth National Census in 2010 in Fujian province and the basic data collected in the monitoring process, the basic weight, no response and post-stratification adjustment of the samples were made to obtain the representative data of the province. The χ2 test and multivariate logistic regression were applied to analyze the influencing factors of basic skill level of health literacy in permanent residents in Fujian province in 2020. Results:According to the weighted data, the basic skill level of health literacy in permanent residents in Fujian province in 2020 was 26.02%, and it was higher in urban area than that in rural area (35.00% vs 20.15%). It was higher in people aged 25-34 years (35.36%), people with high education level (college/undergraduate and above 47.20%), civil servants (47.49%), medical personnel (44.00%), people with chronic diseases (27.04%) and people with good health condition (31.31%), respectively (all P<0.05). Among the 12 questions on basic skills, the correct rate in 6 questions was ≥50%. The question with the highest correct rate was “The correct practices after the occurrence of severe infectious diseases in the local area” (88.2%), and the question with the lowest correct rate was “Correctly identify drugs with‘OTC’ printed on the label”(28.7%). Education level (illiterate/less literate, OR=0.036, 95% CI: 0.018-0.073; primary school, OR=0.091, 95% CI: 0.053-0.155; junior high school, OR=0.230, 95% CI: 0.148-0.357; senior high school/vocational high school/technical secondary school, OR=0.351, 95% CI: 0.232-0.532; college/undergraduate and above, OR=1.000) was positively correlated with the basic skill level of residents′ health literacy ( P<0.001). Conclusions:The overall level of basic health skills in permanent residents in Fujian province in 2020 is relatively high, educational level is a positive correlation factor.
7.Predictive value of serum ferritin in early gestation,pre-pregnancy body mass index and gestational age for gestational diabetes mellitus

Objective To investigate the predictive value of serum ferritin(SF)in early gestation,pre-pregnancy body mass index(BMI)and gestational age for gestational diabetes mellitus(GDM).Methods A total of 260 pregnant women who had a routine prenatal examination in Wenzhou People's Hospital were collected from January 2021 to December 2022.Fasting venous blood was collected at the first prenatal examination(within 14 weeks'gestation)for SF.According to the results of 75 grams oral glucose tolerance test(OGTT)between 24 weeks and 28 weeks,they were divided into GDM group(n=62)and normal glucose tolerance(NGT)group(n=198).The general clinical data of the two groups were compared;Multivariate Logistic regression analysis was conducted to determine the independent risk factors of GDM.Receiver operating characteristic(ROC)curve was used to further analyze the predictive value of SF in early gestation,pre-pregnancy BMI and gestational age for GDM.Results Pregnant women in GDM group had higher SF,BMI and gestational age in early pregnancy than those of NGT group(P<0.05).Pre-pregnancy BMI,gestational age and SF increased in early pregnancy were independent risk factors for GDM(P<0.05).The area under the curve(AUC)of SF in early pregnancy as well as pre-pregnancy BMI and gestational age for predicting GDM were 0.612,0.691 and 0.664.The best tangent point values were 94.045ng/ml,21.125kg/m2 and 28.500 years old,respectively.The total AUC of ROC predicted GDM by SF in early gestation,pre-pregnancy BMI and gestational age was 0.750,the sensitivity and specificity were 66.1%and 79.3%,respectively.Conclusion SF in early gestation,pre-pregnancy BMI and gestational age are important factors in the development of GDM,the combined detection of SF in early pregnancy,pre-pregnancy BMI and gestational age has a certain predictive value for GDM.
8.The current status and influencing factors of compassion fatigue in clinical nurses

Objective To investigate the current status and influencing factors of compassion fatigue in clinical nurses,and to provide theoretical references for prevention of nurse burnout and promotion of nurse retention.Methods Totally 992 clinical nurses from ten tertiary hospitals were enrolled through random cluster sampling to complete general information questionnaire,the Professional Quality of Life Scale (Pro-QOL-V),the Resilience Scale for Adults(RSA) and the Jefferson Scale of Empathy·Health Professionals(JSE-HP).Results The prevalence of moderate and high risk of secondary stress and burnout were 71.1%,73.2%,respectively;clinical nurses with work experience of 6 to 10 years presented the highest level of burnout(P<0.01);lower empathy and resilience,as well as adverse work environment were risk factors of compassion fatigue.Multiple regression analysis showed that the influencing factors of nurse burnout were compassion satisfaction,secondary stress,resilience,workload,recognition,empathy,and payment,which explained 70.5% of the total variance.Conclusion Secondary stress,burnout and compassion satisfaction in clinical nurses are influenced by nurses' empathy,resilience and work environment,and nursing administrators should pay attention to receiving recognition as positive predictors of burnout,except for workload and payment.
9.Characterization and immunoprotective effect of SjIrV1, a 66 kDa calcium-binding protein from Schistosoma japonicum.

Calcium-binding protein is an indispensable protein which performs extensive and important functions in the growth of Schistosoma japonicum. Based on our primary study on tegument surface proteins of S. japonicun, a cDNA encoding a 66 kDa calcium-binding protein of S. japonicum (Chinese strain) was cloned, sequence analysis revealed that it was identical with that of SjIrV1 of Philippines strains S. japonicum. The expression of SjIrV1 were detected by Real-time PCR, using cDNA templates isolated from 7, 14, 21, 28, 35 and 42 days worms and the results revealed that the gene was expressed in all investigated stages, and the mRNA level of SjIrV1 is much higher in 42 d female worms than that in 42 d male worms. The cDNA containing the open reading frame of IrV1 was subcloned into a pET28a (+) vector and transformed into competent Escherichia coli BL21 for expression. The recombinant protein was purified using a Ni-NTA purification system, and confirmed by high performance liquid chromatography (RP-HPLC) and tandem mass spectrometry (MS/MS). Western blotting analysis showed that recombinant SjIrV1 (rSjIrV1) could be recognized by the S. japonicum infected mouse serum and the mouse serum specific to rSjIrV1, respectively. Immunofluorescence observation exhibited that SjIrV1 was mainly distributed on the tegument of the 35-day adult worms. ELISA test revealed that IgG, IgG1 and IgG2a antibodies are significantly increased in the serum of rSjIrV1 vaccinated mice. The study suggested that rSjIrV1 might play an important role in the development of S. japonicum.

10.A Meta-synthesis of qualitative studies of treatment experiences in cancer patients receiving immune checkpoint inhibitors therapy

Objective:To synthesize the findings of qualitative research on the treatment experience of cancer patients receiving immune checkpoint inhibitors therapy to provide a basis for developing relevant health education strategies and other supportive care interventions.Methods:Qualitative research on the treatment experience of cancer patients receiving immune checkpoint inhibitors therapy, published by May 2022, were searched in English and Chinese databases, including Cochrane Library, PubMed, Embase, PsycINFO, CINAHL, Web of Science, CNKI, Wanfang, VIP, and CBM. The quality of the included studies was evaluated by Joanna Briggs Institute Critical Appraisal Tool for qualitative studies. The research results were summarized and Meta-synthesis using the pooled integration method.Results:A total of 13 articles were included and 62 findings were grouped into 12 new categories according to their similarities. These 12 categories resulted in 4 synthesized findings: life was affected, facing multiple decision scenarios, presence of some unmet needs and gaining positive experiences and personal growth.Conclusions:Although patients experience some positive experiences, a large proportion of them experience significant health distress. Healthcare professionals should develop targeted health education with supportive care strategies on the basis of a comprehensive assessment.
