Chinese Journal of cardiovascular Rehabilitation Medicine 2012;21(3):221-225
Objective: To explore safety and effects of exercise rehabilitation in aged patients with chronic heart failure (CHF). Methods: A total of 83 aged CHF patients were randomly divided into exercise rehabilitation group (n=42, received exercise training based on usual care) and usual care group (n=41, received usual care). Period of treatment was eight weeks and patients were followed up for 12 months. New York heart association (NYHA) classification was used to represent cardiac function. Left ventricular ejection fraction (LVEF), left ventricular end-diastolic dimension (LVEDd) were determined by ultrasound cardiography, 6 min walking distance (6MWD) and oxygen metabolic equivalent (METs) also were determined , plasma level of brain natriuretic peptide (BNP) was examined. Minnesota living with heart failure questionnaire (MLHFQ) was used to represent quality of life. Rehospitalization rate and mortality rate within 12 months were recorded in all patients. Results: On 8 th weeks after treatment, the LVEF, LVEDd and NYHA class of two groups all significantly improved(P<0.05 all),compared with usual care group, there were significant improvement in LVEF [(54.7±6.2)% vs. 65.4±8.7)%], LVEDd [(49.6±8.3)mm vs.(40.2±9.3)mm] and NYHA class [(2.7±0.8)classes vs.(1.9±0.9)classes], P<0.05 all; 6MWD [(122.7±9.2) m vs. (175.6±8.7) m] and METs [(5.8±1.8) vs. (8.4±2.4)] also significantly increased (P<0.01), and plasma level of BNP [(43.4±9.8) pg/ml vs. (31.7±8.9) pg/ml, P<0.05] significantly decreased in exercise rehabilitation group. No severe adverse events occurred in exercise rehabilitation group. After 12 months, compared with usual care group, there were significant increase in MLHFQ score [(45.6±8.2) scores vs. (68.9±7.9) scores], significant decrease in rehospitalization rate caused by heart failure (24.4% vs. 9.5%) , P<0.05 all in exercise rehabilitation group. Conclusion: Exercise rehabilitation is safe and effective in aged patients with chronic heart failure, which can significantly improve cardiac function, enhance exercise capacity and increase quality of life.

Objective To examine the expression of angiotensin Ⅱ (Ang Ⅱ) receptor subtypes in human left and right atrial tissue in atrial fibrillation underlying rheumatic heart disease. Methods Atrial tissue samples were obtained from 39 patients with rheumatic heart disease, 25 with atrial fibrillation(AF) and 14 with sinus rhythm(SR) during open heart surgery. AT1 and AT2 mRNA levels were measured with semi-quantitative reverse transcription polymerase chain reaction techniques. AT1 and AT2 protein levels were measured with immunohistochemical techniques. Results Compared with that of the SR group, left atrial inner diameter was significantly increased in the patients of the AF group. The AT1 mRNA and protein levels in the LA significantly increased in patients with AF compared with those in patients with SR (P < 0. 05), whereas AT2 mRNA and protein were not significantly altered. Investigations of Ang Ⅱ receptor subtypes' mRNA and protein levels in the RA did not exhibit any significant changes either in AT1 or AT2 in patients with AF and SR. Conclusions AF is associated with an up-regulation of AT1 in LA, but does not appear to influence the AT2 expression. This may indicate a possible pathophysiologie role for renin-angiotensin system in the development of AF. The series of effects mediated by ATI activation may be one of the molecular mechanisms involved in the process of atrial remodeling.

In order to obtain high yield of the HrpNEcc protein with a lower total cost, fermentation and lactose induction conditions for recombinant E.coli BL21(DE3)/pET30a(+)hrpN Ecc were optimized in flasks and the recombinant E.coli was fermentated in 7L fermenter. The optimized incoulum concentration was 5% and the optimized nutrient medium was TB medium. The HrpNEcc protein yield reached 417.60mg/L by adding 3g/L exogenous inducer lactose in the growth prophase of log-phase for the recombinant E.coli.The HrpNEcc protein yield was higher 36.73% than that of the case of no any inducer, and was higher 16.85% than that of the case of adding IPTG.The wet weight of cell pellet of the recombinant E.coli reached 57.24g/L after fermentation in 7L fermenter, the HrpNEcc protein reached 3.29g/L,about 50.2% of total cellular protein.

Objective To investigate the application of Critical Care Chest Ultrasonic Examination (CCUE)-plus (CCUE-plus) in the etiological diagnosis in patieuts with dyspnea and/or hemodynamic instability caused by abdominal abnormalities. Methods Patients who suffered from dyspnea and/or hemodynamic instability in the Department of Critical Care Medicine,Xiangya Hospital,Central South University from September 2013 to September 2016 were recruited in this study.A total of 255 consecutive patients completed CCUE within 2hrs of admission. If the diaphragm could not be seen in the routine phrenic points according to Bedside Lung Ultrasound Evaluation (BLUE) protocol,it would be found along midaxillary line and defined m-point.The 59 patients with altered diaphragmatic position (m-point was more than 2 cm higher than phrenic point) received sequential abdominal ultrasonography. The latter ultrasonographic findings were compared with CT results. Results There were 42 (71.19%) cases with positive findings of abdominal ultrasonography, including 18 cases of seroperitoneum,16 cases of intestinal obstruction and 8 cases combined.Compared with 56 patients who applied with CT exam, the abdominal ultrasonography revealed a sensitivity of 76.7% and a specificity of 100.0% to diagnose seroperitoneum (AUCRoc 0.917);whereas the sensitivity was 75.0% and the specificity was 90.9% (AUCRoc 0.778) to diagnose intestinal obstruction. Moreover, there were 44 (74.58%) patients with normal left ventricular systolic function;more than three quarters (46/59,77.97%) patients had pulmonary consolidation. Conclusion In patients with dyspnea and/or hemodynamic instability caused by abdominal abnormalities and altered diaphragmatic position in BLUE protocol,CCUE-plus protocol has a high positive predictive value of more than 90% in abdominal abnormality. The findings of abdominal ultrasonography may change therapeutic target from cardio-pulmonary optimization to relief of intestinal obstruction or drainage of seroperitoneum.
