1.Establishment and evaluation of isoprenaline-induced myocardial fibrosis mouse model

Aim To establish and evaluate a mouse model of my-ocardial fibrosis by hypodermic injection of isoprenaline. Meth-ods Kunming mice were randomly divided into 2 groups, namely, the myocardial fibrosis model group and the control group, 10 mice in each group. The mice in the model group were given isoprenaline 5 mg · kg-1 by hypodermic injection. From the following day, the dose of isoprenaline was reduced to 2. 5 mg·kg-1 , and lasted for 30 days. The mice in the control group were treated with the physiological saline in the same way. Final-ly, heart weight was then weighed and the cardiac weight index (CWI) was calculated. Hydroxyproline (Hydro) level in myo-cardium was determined by a colorimetric method. HE and Mas-son’s trichrome staining were used to estimate the extent of myo-cardial fibrosis and calculate the collagen volume fraction ( CVF) . RT-PCR was used to measure the myocardial Collagen I mRNA expression. Results Compared with the control group, the CWI and Hydro content in the myocardial tissues in the model group were increased(P<0.01). The content of col-lagen in the myocardial tissues and the CVF were increased obvi-ously(P<0.01). The RT-PCR results showed that the left ven-tricle Collagen I mRNA expression in the model group increased obviously( P<0.01 ) . Conclusion Isoprenaline-induced myo-cardial fibrosis model has been established and the method is very simple, economic and reliable.

Literatures available was reviewed on the thrombosis and mechanism of new antithrombotic agents.The pathophysiology of the thrombosis involves dysfunction of endothelium and cell adhesion.The new antithrombotic drugs include factor Ⅹa inhibitors, GPⅡb/Ⅲa receptor blocking agents and the drugs modulating the function of endothelium.
3.Effect of thymosin ?1 for advanced non-small-cell lung cancer in chemotherapy patients

Purpose:To evaluate the effects of thymosin ?1 (T?1) in chemotherapy of NSCLC. Methods:We have conducted a prospective randomized controlled clinical trial.40 non-treated cases of stage Ⅲ-Ⅳ NSCLC were randomized into T?1 group ( chemotherapy combined with thymosin ?1) and control group (chemotherapy alone). The number of T cell subgroups, the activity of NK cell and phagocytic index of neutrophilic granulocyte in peripheral blood were detected and quality of life and changes of cancer lesions were evaluated before and after treatment. Results:After treatment,the levels of CD4 in the T?1 group was significantly higher than before treatment(P0.05). There was no significant difference of myelosuppression between the two groups. No infections or severe toxicity occurred in the T?1 group. 4.Experimental study of osthole on inhibition of thrombosis and platelet aggregation

Aim To investigate the effect of osthole against thrombosis and platelet aggregation.Methods Rat model of artery-vein bypass thrombosis and mouse model of thrombosis by injecting collagen-adrenaline to the vein of the tail were used to measure the thrombus weight and to observe the number of dead mice in 5 min and the recovery mice from hemiplegia in 15 min. ADP,thrombin and arachidonic acid were used to induce platelet aggregation on human and the rate of platelet aggregation in 1,3 and 5 min and the maximum rate of platelet aggregation were detected.Results Osthole inhibited artery-vein bypass thrombosis and reduced thrombus weight in rats to defferent degrees,inhibited the thrombosis induced by collagen-adrenaline and reduced the death rate in 5 min and increased the recovery rate in 15 min. Osthole also inhibited human platelet aggregation induced by ADP,thrombin and arachidonic acid in vitro.The values of IC_(50 )(dose of the drug giving 50% inhibition) were 0.444 g?L~(-1) for ADP,0.186 g?L~(-1) for thrombin, 0.421 g?L~(-1) for arachidonic acid, respectively.Conclusion Osthole exert remarkable effects against thrombosis and platelet aggregation.
5.Olmsted syndrome in a baby girl and her mother

A 2-month-old baby girl developed universal keratotic plaques soon after birth. Physical examination revealed well-defined, dark erythematous, keratotic plaques with thick scales and mild infiltration at the periorbital, perioral, perianal and vulvar regions, as well as deep fissures of both hands and feet covered with thick yellowish crusts. Another case was a 24-year-old female, the mother of the baby, who presented with hyperkeratotic plaques at perioral and perianal regions, congenital alopecia universalis, mutilation of fingers and toes with massive thick keratotic yellow crusts and scales. Histopathology of skin lesions from the gluteal region of the baby showed psoriasiform hyperplasia of the epidermis, slight inflammatory infiltration of dermal papillae and superficial dermal perivascular regions. Immunohistochemistry demonstrated the positive staining for acidic keratin (AE1) in the prickle cell layer and granular layer and for CK10 in the upper prickle cell layer and granular layer. Electron microscopy showed increased cell space and decreased tonofilament. Both the baby girl and her mother were diagnosed with Olmsted syndrome.
7.Correlation of vitamin D with essential hypertension and its target organ damages in the elderly

Objective To investigate the correlation of vitamin D with essential hypertension and its target organ damages in the elderly.Methods In the retrospective study,365 elderly participants aged ≥ 60 years hospitalized in our department during January 2013 to February 2014 were involved,including 247 patients with confirmed essential hypertension(HT)and 118 patients without HT.Serum 25-hydroxy vitamin D[25(OH)D]levels were measured and compared between HT and non-HT group,and binary regression analysis was used to discuss potential influencing factors of HT.The damage index of vitamin D target organ was compared between vitamin D deficiency group(n=198)and vitamin D non deficiency group(n=49).Results The median serum 25(OH)D level of participants is 35.3 nmol/L,77.5%(283 cases)were in the insufficient vitamin D status.The level of 25(OH)D was significantly lower in HT group than in non-HT group(34.0 vs.38.7 nmol/L,P<0.05).There were not statistically significant differences between vitamin D insufficient group and sufficient group(P>0.05)in albumin/creatinine ratio,serum creatinine,estimated glomerular filtration rate,ankle-brachial index,pulse wave velocity,intima media thickness of carotid artery,Sokolow-Lyon index,cornell index and left ventricular mass index.The serum 25(OH)D level was significantly lower in CHD plus HT group than non-CHD(P<0.05).The 25(OH)D level was negatively correlated with coronary heart disease(B=-0.021,P=0.019).The serum 25(OH)D level was significantly lower in groups with double-and triple-vessel coronary artery disease than in non-CHD group(P<0.01).Conclusions Vitamin D insufficiency is common in the elderly.The serum 25(OH)D level is significantly lower in old patients with HT especially with stage 3 HT than in non-HT ones.The 25(OH)D level is lower in HT patients with comorbid CHD,and more obvious in HT patients with multi-vessel coronary artery disease.An insufficient vitamin D status is not correlated with hypertensive target organ damages including kidney,peripheral vascular and left ventricular cardiac structure.
8.Effects of ondansetron on morphine physical dependence in mice

AIM To investigate the effects of ondansetron, a selective 5 Hydroxytryptamine3 (5 HT 3) receptor antagonist, on morphine physical dependence. METHODS The morphine dependent models in mice and in isolated Guinea pig ileum were used. RESULTS Pretreatment of ondansetron for 12 days significantly reduced morphine withdrawal symptoms in mice ,such as body weight loss(Groups 2~100 ?g?kg -1 ?d -1 ) or reduced both body weight loss and jumping times (Group 100 ?g?kg -1 ?d -1 ). In addition, concomitant treatment with ondansetron(1~20 ?mol?L -1 ) dose dependently suppressed the contraction induced by naloxone in Guinea pig ileum. CONCLUSION The chronic pretreatment of ondansetron can prevent morphine physical dependence to some extent.
9.Construction of a comprehensive quality management system for PIVAS by feed forward control theory

Objective The objective of this study was to explore the construction of a comprehensive and systematic quality management system for pharmacy intravenous admixture service (PIVAS) and form quality management method and model reflecting distinct gradations,specific responsibility and all staff participation,so that we can provide safe and effective intravenous solutions for the patients and guarantee the quality and safety of intravenous infusion and improve management quality.Methods According to Quality Management Standards of PIVAS published by Ministry of Public Health and JCI Standard,with the theory and methods of feed forward control and delicacy management,by establishing and completing three-grade-quality management organization,defining the quality control indicators,selecting and demonstrating with the application of expert meeting method and Delphi method,a scientific quality control system was formed.Statistics on unreasonable prescriptions,adverse drug reaction of intravenous infusion,defect of pharmacy intravenous admixture and satisfaction of medical staff and patients were compiled and evaluated throughout the implementation of the practice changes in 38 wards.Results Significant differences were observed in various indicators,comparing statistics collected during 6 months of the same season throughout the implementation of the practice changes.x2 value was 160.54,21.39 and 10.29 by comparison of unreasonable prescriptions,adverse drug reaction and number of drug preparation errors.The satisfaction degree of medical staff and patients improved evidently,x2 value was 14.34 and 26.76.Conclusions The construction and implementation of three-grade-quality management system adapt to high-risk,high strength,high sense of responsibility,process-oriented and intensive management of PIVAS.The impact on the continuous improvement PIVAS quality and assurance in safety of intravenous infusion is remarkable.
10.Observation on the therapeutic efficacy of thread hanging combined with cotton plug in the treatment of stage Ⅲ paronychia

Abstract: Objective To observe the curative effect of thread-hanging combined with cotton plug on stage Ⅲ paronychia. Methods Sixty-one patients with stage Ⅲ paronychia were selected and randomly divided into a treatment group and a control group. The treatment group (n=31) was treated with thread-hanging and tampon under local infiltration anesthesia, and changed dressing and tampon every day after operation. After the wound healed, the patient soaked his feet in warm water every day and changed the tampon himself until the symptoms subsided, and the knot did not receive special treatment, and the nail plate would naturally shed as it outgrew the paronychia. The control group (n=30) was treated with thread-hanging and nail groove reconstruction under nerve block anesthesia, and the dressing was changed every day after operation. After thread removal, the patients soaked their feet in warm water every day until the symptoms subsided, and the knot was not specially treated, and it naturally fell off with the growth of the deck beyond the nail groove. The postoperative Visual Analog Scale (VAS) pain score, pain duration, wound healing time, cure rate, effective rate and recurrence rate of paronychia, and patients' satisfaction with the operation were compared between the two groups. Results Compared with the control group, the treatment group had lower VAS pain scores on the first and third postoperative days (2.1±0.3) and (0.2±0.1) vs. (6.3±0.1) and (3.2±0.2), respectively, shorter duration of pain and wound healing time (3.3±0.3) days and (10.1±0.5) days vs. (5.2±0.3) days and (15.2±0.3) days, respectively, higher cure rate (87.1% vs. 66.7%), lower failure rate (12.9% vs. 33.3%), lower recurrence rate (7.4% vs. 20.0%), and higher patient satisfaction (97.0% vs.75.3%). The treatment group showed significant superiority over the control group in all outcomes. Conclusion For patients with stage Ⅲ paronychia, thread-hanging combined with cotton tampon without nail groove reconstruction is advantageous as it avoids additional skin trauma, and does not affect the nail appearance and normal periungual barrier after healing, , reduces patient discomfort, and shortens the time off work, resulting in a higher cure rate. This treatment approach is therefore worth promoting in clinical practice.
