1.Combined measurement of H-FABP and MPO in the prognosis of non-ST elevation acute cornary syndromes

Objective To evaluate the prognosis value of heart‐type fatty‐acid binding protein(H‐FABP)and myeloperoxidase (MPO) in non‐ST elevation acute coronary syndromes(NSTE ACS) .Methods 181 NSTE ACS patients were divided into 4 groups according to the level of H‐FABP and MPO ,and the baselines of 4 groups such as gender ,age ,hypertension ,smoking ,body mass in‐dex ,diabetes mellitus ,hyperlipidemia were compared .The incidences of adverse cardiac events in 4 groups were compared after a 2 years′follow‐up .Results By multivariate COX regression adjustment for other risk factors ,the relative risk(RR) of H‐FABP for adverse cardiac events was 2 .023(95% CI:1 .029 -3 .987 ,P=0 .002) ,and MPO was (95% CI:2 .196 -5 .325 ,P< 0 .05) .The 2 years′follow‐up showed the incidence of adverse cardiac events in NSTE ACS patients with higher H‐FABP and MPO levels was higher than the NSTE ACS patients with one or two indicators of normal levels .Conclusion The combined application of H‐FABP and MPO could has clinical significance for the prognosis of patients with NSTE ACS .
2.Clinical Application of Myeloperoxidase in Diagnosis and Prognosis of Non-ST Elevation Acute Cornary Syndromes

Objective To evaluate the diagnostic value of myeloperoxidase (MPO)in diagnosis of non-ST elevation acute cor-onary syndromes (NSTE ACS).Methods The MPO was determined in 61 unstable angina pectoris (UAP)patients and 54 non-ST-elevation myocardial infarction (NSTE MI)patients,then analyzed the data with receiver operationg characteristic curve (ROC curve)statistical software.The major adverse cardiac events (MACE)were obseved in 106 NSTE ACS patients through 1 year of follow-up.Results The MPO values of NSTE MI group and UAP group were 672±418 ng/ml and 459± 328 ng/ml,the difference was statistically significant (t=3.928,P <0.001).The cTnI values were 558±261 pg/ml and 405 ±278 pg/ml of respectively.The difference was statistically significant (t = 3.089,P = 0.003).The area under the ROC curve of MPO was 0.726 when the optimal cutoff value was 382 ng/ml in the diagnosis of NSTE MI.The sensitivity and specificity was 92.6% and 39.8%.The area under the ROC curve of cTnI was 0.799 when the optimal cutoff value was 484 ng/ml in the diagnosis of NSTE MI.The sensitivity and specificity was 65.5% and 92.3%.High MPO maintained a strong association with the risk of major adverse cardiac events through 1 year of follow-up.Conclussion MPO is ahigh sensitivity marker of the NSTE MI and of clinical value for prognosis of NSTE ACS.
3.Application effect of situational experience health education with psychological counseling for maintenance hemodialysis patients

Objective To explore the application effect situational experiencing health education with psychological counseling in maintenance hemodialysis patients. Methods A total of 80 patients, who had the final stage of renal failure and treated by maintenance hemodialysis in Tongde Hospital of Zhejiang Province in January 2013 to February 2014, were selected and randomly divided into observation group and control group on average. The patients of control group received conventional health education, while the patients of observation group were on the basis of conventional health education, received health education market environment and simultaneously experiencing psychological counseling. We compared the patients′general information such as the age, and utilized concise health SF-36 quality of life questionnaire and self-rating anxiety scale ( SAS ) and self-rating depression scale ( SDS) to questionnaire. Results The difference between the patient′s age and other general information were not statistically significant (P>0. 05). Scores of observation group were higher than these of the control group in vitality, physical function, social function, bodily pain, mental health, physical function, role emotional and general health aspects of eight dimensions ( t =6. 102, 4. 484, 5. 311, 5. 268, 5. 339, 3. 939, 7. 885, 6. 952;P <0. 01). After nursing intervention, the score of SAS acquired (53. 7 ± 9.1), higher than (58. 9 ± 11. 0) of control group (t =4. 621,P <0. 01). The score of SDS improved significantly compared the score of control group (45. 4 ± 6. 8) vs. (54. 6 ± 7. 6), which had statistical significance between two group (t =5. 623,P <0. 01). The complication incidences of observation group patients were apparently lower than those the control group including high blood pressure, low blood pressure, low blood glucose, and fistula occlusion (χ2 =6. 023, 7. 352, 5. 364, 5. 964;P<0. 05). Conclusions The situational experience health education with psychological counseling can improve maintenance hemodialysis patients′quality of life and mental health.
4.Application of "three-in-one" nursing management model on patients with maintained hemodialysis

Objective To explore the effects of the "three-in-one" nursing management model when applied in the patients with maintained hemodialysis.Methods Totally 102 patients with maintained hemodialysis who were treated in Tongde Hospital of Zhejiang Province from July 2015 to August 2017 were selected by convenient sampling, and equally and randomly divided into a control group and an observation group. Patients in the control group received routine nursing and management, while patients in the observation group received the "three-in-one" nursing and management of hospitals, communities and families. Their personal information was filed when they were admitted into hospital, and the nursing work was transferred to the nurses in the community health service center in the vicinity of the patients' residence which was chosen one week before the patients were discharged from hospital. These nurses provided family rehabilitation guidance to the patients. The changes in blood pressure, degree of thirst, taste acuity and the resting salivary flow rate were compared between the patients in the two groups before and after the intervention. The Self-Management Scale and the MOS Item Short Form Health Survey (SF-36) were used to compare the self-management behavior and quality of life between the patients in the two groups.Results After three months' intervention, the systolic pressure of the patients in the observation group and the patients in the control group was (132.67±12.54)mmHg and (141.57±12.09) mmHg respectively (t=3.370,P<0.05); the taste acuity score of the patients in the observation group and the patients in the control group was (78.23±8.72) and (66.31±9.65), respectively (t=6.545, P<0.05); the scores for self-management behavior and overall health of the patients in the observation were (70.52±10.05) and (87.65±6.92), both higher than those of the patients in the control group (t=4.656, 2.660;P<0.05).Conclusions The "three-in-one" nursing and management model applied in the patients with maintained hemodialysis can help to improve taste acuity, self-management behavior and quality of life in these patients, thus worthy of clinical application.
