2.A Case-Control Study on the Relationship Between Environmental Risk Factors Exposed in Early Pregnancy and Congenital Heart Disease

Objective To understand the effect of environmental risk factors exposed in the first trimester of pregnancy on congenital heart disease, then provide scientific evidence for congenital heart disease prevention. Methods A hospital-based 1:1 matched case-control study was conducted. The risk factors were obtained by field investigation with standardized questionnaires. The data was dealt with single factor analysis and conditional Logistic regression using SPSS version 11.5. Results Folic acid(OR=0.340, 95%CI: 0.178-0.649), milk(OR=0.660, 95%CI: 0.460-0.947), meat(OR=0.771, 95%CI: 0.583-0.867) and nausea and vomiting of pregnancy(OR=0.457, 95%CI: 0.271-0.770)were significantly associated with congenital heart disease. Maternal infection(OR=2.736, 95%CI: 1.462-5.121), taking medicine(OR=2.735, 95%CI: 1.483-5.044), poisonous chemicals(OR=2.764, 95%CI: 1.065-7.177) and mental stress(OR=2.211, 95%CI: 1.022-4.782) were risk factors of congenital heart disease. Conclusion To prevent congenital heart diseases, pregnant women should take more nutriment, keep healthy state and avoid infecting, taking medicine and exposing chemical toxicants in the first trimester of pregnancy.
3.Protective effects of resveratrol on oxidized low-density lipoprotein-induced injury on endothelial cells

Objective To study the protective effect of resveratrol on endothelial cells injury induced by oxidized low-density lipoprotein (oxLDL) and its protective mechanism. Methods The injured model was established by endothelial cell treated with oxLDL. In the experiment, endothelial cell were cultured in vitro and divided into seven groups which were: control group; solvent (DMSO) group, oxLDL group, quercetin plus oxLDL group, Reseveratrol-L (0.5 mol/L), M(5 mol/L), H(50 mol/L) plus oxLDL group respectively. Endothelial cells were incubated with 200?g/ml oxLDL for 18 hours in the absence or presence of various concentrations of resveratrol and quercetin. The apoptotic index was detected by flow cytometric. Meanwhile, the content of Malondialdehyde (MDA), and the activity of SOD, GSHPX in the culture medium and homogenate of endothelial cells were measured. Results Incubation of endothelial cells with oxLDL for 18 h elicited a significant increase in the percent of apoptotic endothelial cells, in control group, the percent of apoptotic cells is 7.6?0.6, however in oxLDL group, it increased to 41.4?2.3, furthermore, oxLDL induced the increase of the contents of MDA, and accompanied by the decrease in the activity of SOD, GSHPX in the endothelial cells, the difference of was significant as compared with control group. Pretreatment of resveratrol with 0.5~50 mol/L decreased these oxLDL-induced changes, in Reseveratrol with 50 mol/L group, the percent of apoptotic cells decreased to 13.0?1.5, the difference of was significant as compared with oxLDL group (P
4.Effect of application of health education model on patients undergoing extracorporeal shock wave lithotripsy of urinary calculi

ObjectiveTo observe the effect of application of health education model on patients un-dergoing extracorpereal shock wave lithotripsy of urinary calculi. MethodsWe randomly divided 278 patients undergoing extracorporeal shock wave lithotripsy of urinary calculi into the experimental group(136 cases) and the control group(142 cases).The experimental group received health education according to the model of health education, while the control group adopted routine health education. The treatment effect of lithotripsy after 1 week,2 weeks and 1 month was evaluated. ResultsAfter health education,the cure rate at I week after lithotripsy was 50.7%, which was higher than that of the control group,28.2%, the cure rates at 2 weeks and 1 month were 58.2% and 86.0%, which were better than those of the control group, 49.0% and 70.4%. ConclusionsApplication of health education model can improve the treatment effect of extracorporeal shock wave lithotripsy of urinary calculi and alleviate pain of patients as soon as possible.
5.The relationship between obesity phenotypes and the changes of bone mineral density and vitamin D receptor in type 2 diabetes mellitus patients

Objective To investigate the correlation between vitamin D receptor gene and bone mass and obesity phenotypes in patients with type 2 diabetes mellitus.Methods 318 patients with type 2 diabetes were chosen as diabetes group,and 50 healthy people were selected as healthy control group.Vitamin D receptor gene Apa Ⅰ type was detected in the two groups.Height,weight and body mass index(BMI)biochemical index,fat content(FM),lean tissue content(LM)and bone mineral density were detected in patients with type 2 diabetes mellitus.The relationship between vitamin D receptor gene(Apa Ⅰ)polymorphism and BMD and obesity phenotypes in type 2 diabetes was analyzed.Results The VDR gene distribution between the diabetes group and healthy control group showed no signif-icant difference(Z =0.561,P >0.05).The vitamin D receptor genotype in the diabetes group included AA 31 cases (9.7%),Aa type 108 cases(34.0%),aa type 179 cases(56.3%),while the vitamin D receptor genotype in the healthy control group comprised AA 7 cases(9.3%),Aa type 29 cases(38.7%),aa type 39 cases(52.0%).The percentage of AA in both groups was significantly less than that of Aa and aa(χ2 diabetic group =4.127,3.976,all P <0.05;χ2 healthy control group =5.129,4.213,all P <0.05).Proportion of normal bone mass and average bone density in AA,Aa,aa type decreased(χ2 =15.552,P <0.05;F =5.127,P <0.05),the genotype AA was not detec-ted in osteoporosis group.BMI and FM were the highest in AA,which were significantly higher than those of Aa,aa (F =4.319,4.263,all P <0.05).Conclusion Vitamin D receptor gene Apa Ⅰ type polymorphism is related with BMD and obesity in type 2 diabetes mellitus,and it has predictive value on bone mass changes.The increase of BMI and FMmay be beneficial to bone mineral density.
6.Expression and correlation of cell cycle protein-D1 and Ki-67 in laryngeal cancer tissues

Objective To investigate the expression and correlation of cell cycle protein-D1 and Ki-67 in laryngeal cancer tis-sues.Methods Immunohistochemical streptavidin-perosidase ( SP) staining method was used to detect the expression of Cyclin D 1 and Ki-67 proteins in laryngeal squamous cell carcinoma tissues and their correlation between Cyclin D 1 and Ki-67 was analyzed .Re-sults Among 56 cases of Cyclin D1 positive over-expressed laryngeal squamous cell carcinoma tissues , 41 cases of Ki-67 had positive over-expression.Among 51 cases of Ki-67 low-expressed tissues, 35 cases of Cyclin D1 had low-expression.Spearman rank correlation test showed that expressions of Cyclin D 1 and Ki-67 in laryngeal squamous cell carcinoma tissues had a significant positive correlation ( rs =0.620, P <0.01).Conclusions Cyclin D1 and Ki-67 expressions in laryngeal squamous cell carcinoma have a significantly positive correlation .
7.Analysis on influence factors of medication compliance in elderly patients with hypertension

Objective To know the medication compliance in elderly patients with hypertension,to analyze the influence factors of medication compliance,and to propose forward intervention countermeasures.Methods The treatment of hypertension in elderly patients was investigated by questionnaire survey.Medication compliance and influence factors were analyzed.Results 166 cases with poor medication compliance,and the rate was 45.0%; 203 cases with good medication compliance,and the rate was 55.0%.It's showed significantly difference on degree of education between the observation group and control group (x2 =12.071,P=0.002).The proportion of peasant and worker was higher in the observation group than control group (78.9% vs.62.1%,x2 =12.817,P=0.002).The smoker (48.8%) and drinker (59.0%) in observation group more than smoker (27.1%) and drinker (31.0%) in control group(x2=18.481,P<0.001; x2=29.115,P< 0.001).The ratio of payout of-pocket for healthcare was higher in the observation group (21.1%) than control group (9.9%) (x2=9.084,P=0.003).The duration of hypertension was longer in the observation group (8.8±4.6) than control group (6.0±4.1) (t=1.806,P=0.036).The income per month was less in the observation group (1850±150) than control group (2150±200) (t=5.799,P< 0.001).There were regression relationship between medication compliance and degree of education,medical security and duration of hypertension.Conclusions According to the influence factors of medication compliance in elderly hypertensive patients,it's useful to improve medication compliance by enhancing health education and designing a personal treatment.
8.Distribution and drug resistance of major pathogens for urinary tract infections

Objective To investigate the distribution and drug resistance of major pathogens for urinary tract infections in the First Affiliated Hospital of Nanjing Medical University.Methods Strains from midstream urine culture of patients with urinary tract infections were collected during January 1 and December 31,2011.All strains were identified by API system,and disk diffusion method was used for drug sensitivity test.Results Totally 1129 strains were isolated,in which 667 (59.1% ) were Gram-negative strains,266 (23.5%) were Gram-positive strains,and 196 (17.4) were Candida.Among Gram-negative strains,Escherichia coli,Klebsiella pneumoniae and Proteus mirabilis were highly sensitive to carbapenem antibiotics; while Acinetobacter baumannii and Pseudomonas aeruginosa were highly resistant to most antibiotics including cephalosporins and penicillinase inhibitor,and the resistance rates were over 50%.Among Gram-positive strains,the major strains Enterococcus avium and Enterococcusfaecalis were completely sensitive to vancomycin and teicoplanin,and highly sensitive to linezolid (resistance rate below 10% ).Candida albicans and Candida glabrata were highly resistant to voriconazole and fluconazole (with the resistance rates of 47.2% - 60.0% ), but were completely sensitive to amphotericin and nystatin.Conclusion Gram-negative strains account for most urinary tract infections in the First Affiliated Hospital of Nanjing Medical University with high drug resistance rates.
9.Analysis of real-time review and intervention on irrational outpatient infusion prescriptions

Objective To analyze type of irrational outpatient infusion prescriptions returned by clinical pharmacists'pre -prescription review and intervention in our hospital.Methods Outpatient infusion prescriptions reviewed by clinical pharmacists in 2014 were statistically analyzed in terms of type,constitution and rate of qualified prescriptions.Results Type of unstandard writing or input errors prescriptions accounted for 35.19%,which was the great majority of the irrational outpatient infusion prescriptions.The rate of qualified prescriptions was 99.15%,which was higher than 91.53% in 2011 when pre -prescription review and intervention had not implemented.Conclusion Pre -prescription review and intervention could improve the quality of outpatient infusion prescriptions.Finally,safety and effectiveness of outpatient infusion could be guaranteed.
10.Drug therapy for advanced hepatocellular carcinoma: approach, controversy and progress

For unresectable advanced hepatocellular carcinoma (HCC),besides sorafenib,alternative drugs and treatment modalities are required.Clinical studies of hepatic arterial infusion chemotherapy (HAIC),transcatheter arterial chemoembolization (TACE),and system chemotherapy have shown favorable efficacy and tolerance in advanced HCC patients.In addition,the potential efficacy of sorafenib combined with focal treatment is also an interesting issue.As more therapies become available,decision-making for treating advanced HCC becomes increasingly complex.In our opinion,diverse treatment modalities should be utilized for the best interest of patients.Based on predictive biomarkers,we should develop a precise patient stratification system to select suitable candidates for each treatment modality in future studies,as is useful for improving prognosis of patients with advanced HCC.
