1.Optimal immunohistochemical diagnostic index for supraclavicular lymph node metastasis in lung cancer

Objective To explore the optimal immunohistochemical (IHC)diagnostic index for supraclavicular lymph node me-tastasis in lung cancer.Methods In the premise of conforming to the evidence-based medicine,the biopsy of lymph node in 135 pa-tients with supraclavicular lymph node metastasis was performed and 7 antibodies were selected to be detected.135 cases were di-vided into two classes:class A (110 cases)and B (25 cases).The class A included 14 cases of primary lesion inside the lung by the histological analysis,43 cases of primary lesion inside the lung by the cytological detection and 53.cases of primary lesion inside the lung by the radiographic analysis.The class B included 25 cases of lesion outside the lung by the histological analysis.The detecting antibodies included CK7,CK20,EMA,CEA,TTF-1,SPB and vimentin.Results The sensitivity and specificity of single antibody for deducing the primary lesion inside the lung were 90.0% and 56.0% for CK7,98.0% and 40.0% for CK20,90.9% and 4.0%for EMA,80.9% and 36.0% for CEA,62.7% and 100.0% for TTF-1,65.6% and 100.0% for SPB,and 60.9% and 60.0% for vimentin.The quadruple antibodies (CK7,CK20,TTF-1 and SPB)showed the highest sensitivity(85.0%)and highest specificity (100.0%),which made the relatively high detection rate (80.0%)of lung cancer in clinic.Conclusion Analyzing and researching the IMC results is of great significance to guide the surgical pathological practice based on the evidence-based medicine.

Src family kinases (SFKs) are the largest non-receptor tyrosine kinase family and play key roles in the regulation of cell morphology,proliferation,growth,adhesion,and motility.The activation of src kinases couples with many signals input on cell surface,including growth factor,cytokine,immune cell receptor,G protein-coupled receptor,integrin,and other cell adhesion molecules.In addition,as an important molecular switch connecting many extracellular and intracellular important signaling pathways,src kinases also play an important role in the occurrence of cerebrovascular diseases.
3.Study on application of heparinzed plasma in the emergency biochemical test

Objective:To detect differences in results of routine biochemical indexesand serum anticoagulant heparin, explore the feasibility of plasma heparin in the emergency biochemical test in.Methods: Two hundred cases of patients in the emergency department of our hospital as the research object, peripheral venous blood samples, were installed on the ordinary drying tube and a heparin plasma in vitro, determination of 10 emergency patients in serum and plasma biochemical items using biochemical analyzer, comparative analysis of the effect of heparin plasmabiochemical test results, and to compare the two groups report time.Results: The difference of serum group and Mg2+ group, Cl-, plasma Ca2+, Cr, BUN, UA, ALT, GLU and other data is not obvious, no statistical significance, The plasma concentration of K+ group was significantly lower than serum group, serum concentration of Na+ was higher than group difference was significant, with statistical significance(t=8.167,t=4.361;P<0.05). Plasma group reported significantly less than the serum group(t=22.94,P<0.05).Conclusion: heparin sodium does not dissolve in the blood, with a strong ability for anticoagulation, emergency biochemical test can avoid the effect of blood coagulation test results, and shorten the time for patients toreport, to win the precious time, attention is needed to correct potassium ionconcentration difference test.
4.DIFFERENTIATION AND BIOLOGICAL CHARACTERISTICS OF THE LYMPHATIC ENDOTHELIAL PROGENITOR CELLS ISOLATED FROM UMBILICAL CORD BLOOD

Objective To study the changes of biological characteristics of CD34~+/CD133~+/VEGFR-3~+ lymphatic endothelial progenitor cells(EPCs) isolated from umbilical cord blood after induction with VEGF-C and investigate the mechanisms of EPCs differentiation toward lymphatic endothelial cells. Methods Mononuclear cells were isolated from umbilical cord blood using density centrifugation with Percoll solution.CD34~+/CD133~+/VEGFR-3~+ cells were sorted with FACS.Differentiation of the cells was induced with VEGF-C.The ultrastructural changes of the cells were viewed under scanning and transmission electron microscopes.Changes in expression of markers of the cells were viewed under confocal laser scanning microscope. Results Lymphatic EPCs isolated from umbilical cord blood expressed CD34,CD133 and VEGFR-3.At 7 day after induction with VEGF-C,CD34~+/CD133~+/VEGFR-3~+ cells were shuttle-like,there were a lamellipodium,numerous filopodia and many short microvilli.Caveolae were observed.Mitochondrion and rough endoplasmic reticulum were rich in cytoplasm.At 14 day after induction,the cells demonstrated appearance of the endothelial cell and expressed lymphatic endothelial specific makers LYVE-1 and 5'-nucleotidase,the expression of CD133 disappeared.Weibel-Palade body was observed in the cytoplasm of the cell.Conclusion There are CD34~+/CD133~+/VEGFR-3~+ lymphatic EPCs in human umbilical cord blood.The cells may differentiate into lymphatic endothelial cells through VEGF-C/VEGFR-3 signaling pathway after induction with VEGF-C.
5.Adverse Drug Reactions Induced by Gatifloxacin:Analysis of 866 Cases During 2003~2006

OBJECTIVE:To study the characteristics of adverse drug reactions(ADR) induced by gatifloxacin.METHODS:A total of 866 ADR cases induced by gatifloxacin collected in Zhejiang ADR Monitoring Center between Oct.30,2003 and Oct.30,2006 were analyzed with Excel.RESULTS:The common ADR induced by gatifloxacin were lesions of skin and appendages,gastro-intestinal system,systemic and nervous system.Most of the cases were slight in symptoms,but gatifloxacin could also induce severe ADR such as dysglycemia etc that resulting in potential risks to vital human organs.CONCLUSION:ADR monitoring should be emphasized when using gatifloxacin so as to decrease or avoid the occurrence of ADR.
6.A prospective study of relationship between abnormal glucose metabolism during pregnancy and family history of diabetes

Objective To study the relationship of gestational diabetes mellitus(GDM) and ~family history of diabetes (FHD), clinical characteristics and maternal and infant outcomes of GDM and gestational impaired glucose tolerance (GIGT). Methods A prospective case-control study was performed in 244 women with GDM/ GIGT, including 93 patients with family history of diabetes and 151 without FHD, and 244 normal OGTT cases as control from Feb. 2004 to Aug. 2004 in Dept of OB &GYN of Peking University First Hospital. Univariate analysis was used to estimate the odds ~ratios with 95% CI. Results (1)FHD was a risk factor of GDM/GIGT, OR=2.9.The co-effect of parents DM on transmission was excessive. (2)Maternal≥30 years old with FHD were greater than those without FHD (75.3% vs 62.9%). More intakes of fruits and carbohydrate per day and more weight gain during pregnancy in pregnant women with FHD were lower than those in patients without FHD. (3) The prevalence of GDM was indifferent ~between patients with and without FHD. 50 g OGCT plasma glucose (PG), fasting PG and 3 h PG in 75 g OGTT of patients with FHD were higher than those of patients without FHD. The number of pregnant women treated by insulin was significantly different between patients with and without FHD. (4) No difference was found between family and non-family history of diabetes in the prevalences of pre-eclampsia, FGR, polyhydramnios, preterm labor, LGA and SGA. Conclusions (1) FHD is a risk of GDM/GIGT, and exposure to a diabetic intrauterine environment is a mechanism responsible for this excessive maternal inheritance. (2)~Besides family history of diabetes, more intake of fruits or carbohydrate and larger gain of body weigh during pregnancy may result in GDM/GIGT.

Objective To investigate the characteristics of insulin requirements during pregnancy in women with pre-gestational diabetes mellitus. Methods A retrospective study was conducted on 116 pregnant women with pre-gestational diabetes admitted to the Department of Obstetrics & Gynaecology, First Hospital, Peking University from 1981 to 2003. Ninty-five among the 116 cases required insulin during pregnancy among which 82 cases were enrolled in the observational group. Results Eighteen women in the observational group had OGTT performed during pregnancy whose values were above the diagnostic criteria of non-pregnant diabetes. The insulin requirements increased with the gestational age with the peak dose at 32~36 weeks followed by slight decrease till delivery. 9.Evaluation of anti anxiety treatment in residual dizziness after successful canalith repositioning procedure in benign paroxysmal positional vertigo

Objective To study the effect of anti anxiety treatment in residual dizziness after successful canalith repositioning procedure in benign paroxysmal positional vertigo.Methods 68 cases of benign paroxysmal position vertigo patients with residual dizziness after successful canalith repositioning procedure were collected,who visit department of Neurology..They were randomly assigned to anti anxiety group and the control group according to randomized controlled method.Deanxit (0.5mg/10mg,1times/day)combined with betahistine mesilate tablets (6mg, 3 times/day)were taken by patients in the anti anxiety group;take betahistine mesilate tablets (6mg,3 times/day) were taken in the control group.Follow up 3 months,the duration of residual dizziness and the changes of symptoms between the two groups were compared.The symptoms of all patients were evaluated before and after treatment by dizziness handicap inventory.Results The duration of residual dizziness in the control group was (16.38 ±8.83)days, and that in the anti anxiety group was (10.13 ±5.10)days.The duration of residual dizziness in the anti anxiety group was significantly shorter than that in the control group (t =3.440,P <0.05).In the control group,the scores of first week,s econd week and third week were (26.60 ±13.54)points,(20.80 ±10.97)points and (6.27 ±5.88)points.In the anti anxiety group,the scores of first week,second week and third week were (23.10 ±11.02)points,(13.33 ± 5.83)points and (0.67 ±2.50)points.There was not significant difference between the two groups (t =1.122,P >0.05)in the treatment of first week.In second week and third week,the scores of the anti anxiety group were lower than those of the control group,the differences were statistically significant (t =3.378,4.935,all P <0.05).After 3 weeks of treatment,the scores of the anti anxiety group were significantly decreased in the functional,emotional and physical scores (t =5.297,4.537,4.451,all P <0.01 ).Conclusion Deanxit can shorten residual dizziness duration of benign paroxysmal position vertigo patients after successful canalith repositioning procedure,and can reduce the degree of residual dizziness.
10.Advances in toxicological study and rapid detection of common mycotoxins in medicinal herbs

Mycotoxins,secondary metabolites produced by certain fungi,have become one of the most harmful factors that affect the clinical safety of medicinal herbs that probably can be contaminated by harmful toxins generated from fungi in the whole process from planting to clinical use. Therefore,more toxicological research of mycotoxins,a better knowledge of the pathogenesis and quick detection with sensitivity and accuracy will play an important role in targeted therapy of poisoning by mycotoxins and early warning . In this paper,the current status of mycotoxin contamination in medicinal herbs was ana?lyzed,and the progress of toxicological study on common contaminants was reviewed. In view of the high toxicity of toxins,the strategy of ″Prevention First″ is highly desirable. Hence,the development of rapid detection of typical mycotoxins was systematically discussed. The review was intended to provide ref?erence for ensuring clinical safe administration of medicinal herbs and for reducing the risk of mycotoxin poisoning.
