OBJECTIVE To investigate the restrained factors in the disposal of medical dispute caused by nosocomial infection and try to find out the method to solve the problem. METHODS The several restrained factors in the disposal of medical dispute about nosocomial infection were enumerated and analyzed. RESULTS There are several restrained factors in the disposal of medical dispute about nosocomial infection according to law. First, nosocomial infection can not be died out, and medical dispute about nosocomial infection will exist for ever as a result of the specialty of supply of medical service. Secondly, current legal system construction of nosocomial infection management in our country is in the stage of being established and consummated step by step. Determinant criteria of medical negligence behavior are not perfect. Feasibility of part of the actual rules and standard in nosocomial infection is not good. Thirdly, collecting evidence is very difficult in some medical dispute caused by nosocomial infection . It is restricted by hospital condition, medical cost and medical documents. Because of the restrained factors mentioned above, when nosocomial infection occurs, medical workers concerned can not find or confirm the reason of infection in most cases. Then hospital has to compensate patients for loss in order to make concessions to avoid trouble. CONCLUSIONS We make following suggestions for the status. First, we should strengthen legal system construction about nosocomial infection, and safeguard both of hospital and patients′ interests . Secondly, insurance system of medical risk should be perfected. Thirdly, medical workers should abide by rules of operation. Fourthly, we can not hide the truth when nosocomial infection occurs. Fifthly, we must fulfil the "attention duty" in order to reduce medical dispute.

Objective: To explore the effects of ranitidine on pharmacokinetics of rhein in rats after oral administration of Dachengqi Decoction (DCQD), a compound traditional Chinese herbal medicine. Methods: Twelve male Sprague-Dawley rats were divided into DCQD group and DCQD plus ranitidine group, and were orally administered with DCQD at a dose of 10 g/kg or DCQD (10 g/kg) combined with ranitidine (150 mg/kg), respectively. Blood samples were gathered after a series of time intervals. Metabolism of rhein was determined with a reversed-phase high-performance liquid chromatography with internal standard of 1, 8-dihydroxyanthraquinone and the data were analyzed with DAS 2.1 program. The pharmacokinetic parameters were compared between the two groups. Results: The pharmacokinetic parameters of rhein in the DCQD group, including peak concentration (C(max)), area under the plasma concentration-time curve (AUC), distribution phase half-life (t(1/2alpha)), elimination rate constant (K(10)) and central to peripheral transfer rate constant (K(12)), were significantly different to those in the DCQD plus ranitidine group (P<0.05, P<0.01). There were no significant differences in the other parameters between the two groups. Conclusion: Ranitidine can influence the pharmacokinetics of rhein in rats after oral administration of DCQD.

ObjectiveTo detect the mutations of GJB3 and GJB4 genes in two sporadic cases of erythrokeratodermia variabilis(EKV).MethodsGenomic DNA was extracted from two sporadic patients with EKV,their family members, and 100 normal human controls. All the exons and adjacent splice sites of GJB3 and GJB4 genes were amplified by PCR.Mutation scanning was carried out via direct bidirectional DNA sequencing.ResultsA G134C mutation was found at the GJB3 gene in patient 1,which caused a substitution of glycine by alanine at codon 45 (G45A).No mutation was found in the GJB4 gene in case 1 or GJB3 and GJB4 genes in case 2.ConclusionA missence mutation G45A in GJB3 gene is found in a patient with EKV.

Objective To study the change of immune factor in COPD rats after intravenous injection of regulatory T cells. Methods Twenty-one SPF rats was divided into three groups at random, rat COPD model was built by smoking. We used magnetic bead isolation technic to separate CD4+CD25+ regulatory T cells.Regulatory T cells were cultured and injected into rats though rats' caudal vein according to different dose,5 × 104/mL,5 × 105/mL,5 × 106/mL respectively.Flow cytometry was used to analyze cell factors.ELISA was used to analyze IL-6 and CRP.Restlts Adding JJ316 or IL-2 into medium benefited the proliferation of CD4+CD25+ regulatory T cells. On the 20 th day, regulatory T cells CD4+CD25+ proliferation stopped by adding JJ316 or IL-2 respectively.Regulatory T cells were cultured and injected into rats though rat caudal vein according to different dose. The levels of CRP and IL-6 were decreased when rats were injected by CD4+CD25+regulatory T cell after one week. Conclusions Injection of regulatory T cells is helpful to control inflammation progression of COPD,so the increase of regulatory T cells of patients with COPD may decrease inflammation progression of COPD.

9. Practice and experience of preoperative conversation as involved by the office of doctor-patient relationship
Chinese Journal of Hospital Administration 2019;35(12):1010-1012
Effective communication between doctors and patients can reduce the occurrence of medical complaints and disputes. The office of doctor-patient relationship acts as the complaints management department of the medical institution and undertakes the management of medical complaints. Since 2017, our hospital has introduced the staff of the doctor-patient relationship office to participate in major preoperative conversations. The mode establishes a communication bridge between clinicians and surgical patients and their families, so that patients′ informed consent is more complete and more realistic, and the sufficiency and effectiveness of doctor-patient communication is enhanced. It has a positive effect on medical complaints and dispute risk prevention.

In recent years, public emergencies, especially public health emergencies, have occurred frequently. In the process of emergency rescue, there is often a shortage of emergency medical supplies. Public donation is one of the effective ways to alleviate the shortage of emergency medical supplies. In this paper, the authors analyzed the problems of public hospitals accepting social donations in public health emergencies, discussed the legal risks, and put forward countermeasures and suggestions for relevant departments.
