Objective To explore efficacy and safety of simulated artificial pancreas in modulating stress hyperglycemia in critically ill patients. Methods A prospective randomized controlled study was performed. Seventy-two critically ill patients with stress hyperglycemia, aged 18-85 years, acute physiology and chronic health evaluationⅡ(APACHEⅡ) score over 15, two consecutive random blood glucose 11.1 mmol/L or higher, glycated hemoglobin (HbA1C) below 0.065, unable to eat food for 3 days after inclusion, or only accepting parenteral nutrition, admitted to intensive care unit (ICU) in Shanghai Punan Hospital of Pudong New District from January 1st, 2015 to June 30th, 2017 were enrolled. The patients were divided into three groups according to the random number table method, high-intensity group and low-intensity group were injected Novolin R (high-intensity group 2/3 dosage, low-intensity group 1/3 dosage) to modulate stress hyperglycemia by simulated artificial pancreas. Simulated artificial pancreas consisted of Guardian real time glucose monitoring system (GRT system), close-circle control algorithm and micro-pump;subcutaneous injection of Humulin 70/30 was applied to modulate stress hyperglycemia in humulin group. Real-time glucose levels of interstitial fluid in abdominal wall, equivalent to blood glucose levels, 10 minutes each time, were monitored by using of GRT system for all patients in three groups. Fasting serum levels of stress hormones including epinephrine and cortisol and insulin resistance index (IRI) were recorded within 24 hours after inclusion. Mean blood glucose, blood glucose variation coefficient, blood glucose target-reaching rate, blood glucose target-reaching time, hypoglycemia rate and 6-month mortality were measured. Twenty healthy adults from health administration department of the hospital were recruited as healthy control group. Results A total of 60 eligible critically ill patients were included in this study, each group with 20 patients. There was no significant difference in gender, age, APACHE Ⅱ scores among three groups. The levels of serum epinephrine, cortisol and IRI within 24 hours after inclusion in the three groups were significantly higher than those in healthy control group. The mean blood glucose levels of humulin group, low-intensity group, high-intensity group were decreased (mmol/L: 10.2±3.2, 8.4±2.6, 8.1±2.2), the blood glucose target-reaching rate were increased [40.2% (3 295/8 196), 71.1% (5 393/7 585), 80.4% (6 286/7 818)], the blood glucose target-reaching time were shortened (hours: 49.1±5.8, 24.6±4.6, 17.5±4.2), the hypoglycemia rates were increased respectively [1.3% (108/8 196), 2.8% (211/7 585), 4.0% (313/7 818)], with statistically significant differences (all 1 = 0.000). There was no significant difference in blood glucose variation coefficient and 6-month mortality among three groups [blood glucose variation coefficient: (29.4±3.7)%, (28.5±5.3)%, (26.1±4.6)%, 6-month mortality: 55.0%, 45.0%, 40.0%, all 1 > 0.05]. Conclusions Simulated artificial pancreas could effectively and safely modulate stress hyperglycemia in critically ill patients, high-intensity modulation could bring about better efficacy in the regulation of hyperglycemia. High-frequency blood glucose monitoring by using GRT system could promptly identify hypoglycemia and help it to be corrected.

Background/Aims:
Some inflammatory bowel disease (IBD) patients in remission suffer from irritable bowel syndrome (IBS)-like symptoms (IBD-IBS). The pathogenesis has not yet been elucidated. The study aim is to evaluate relationships among quality of life (QOL), psychological status, and visceral sensitivity, and explore the formation mechanism of IBD-IBS.
Methods:
Forty-seven patients with Crohn’s disease in remission, 24 ulcerative colitis in remission, 26 IBS, and 20 healthy controls were included in the study. The abdominal pain, QOL, anxiety, and depression were evaluated through questionnaires. Visceral sensitivity was measured by rectal balloon distension. The serum levels of 5-hydroxytryptamine (5-HT) and nerve growth factor (NGF) were measured by enzyme-linked immunosorbent assay. The expressions of tryptase, 5-HT, NGF, and related receptors in colonic tissues were detected by immunohistochemistry and western blot.
Results:
Prevalence of IBS-like symptoms in Crohn’s disease and ulcerative colitis patients in clinical remission was 29.8% and 50.0%, respectively. The QOL was lower, the anxiety/depression scores were higher in IBD-IBS patients than those without IBS-like symptoms. Additionally, patients with IBD-IBS existed visceral hypersensitivity. Besides, abdominal pain was associated with poor QOL, visceral hypersensitivity, anxiety, and depression in IBD-IBS patients. The number of mast cells (MCs) and expressions of 5-HT, NGF, and related receptors were higher in IBD-IBS patients than those with no such symptoms. The serum levels of 5-HT and NGF positively correlated with abdominal pain and visceral hypersensitivity.
Conclusion
IBD-IBS patients may have low QOL and psychological abnormalities, as wells as visceral hypersensitivity which may be related to increased 5-HT and NGF levels released from activated mast cells.

Objective:To study the effects of compound Xiongdanyinchen granules(CXG)on intestines, brain and hypothalamic-pituitary-adrenal(HPA)axis in rats with metabolism-associated fatty liver disease(MAFLD).Methods:50 SD rats were randomly divided into normal control group(normal rats fed with basic feed), model control group(MAFLD model rats fed with normal saline replacing CXG), intragastricly CXG-treated groups with doses of(4.73 g·kg -1·d -1)for low dose group, (18.92 g·kg -1·d -1)for medium dose group, and(9.46 g·kg -1·d -1)for high dose group for 4 weeks. All groups rats were sampled, and rat liver, colon and hypothalamic tissues were stained by hematoxylin-eosin(HE)for observing pathological changes.Fluorescence-based real-time quantitative polymerase chain reaction(PCR)and immunohistochemical detection were used for detecting the colon and hypothalamus to see levels of adreno-corticotropic hormone releasing hormone(CRH), adrenocorticotropic hormone(ACTH)and cortisol(CORT)expression. Results:HE staining showed that the fat vacuoles of liver cells were reduced in CXG group. As compared with the model control group, CXG group showed the hepatic cords were arranged neatly and the hepatic lobules were clearly visible, with significantly improved signs. As compared with the model control group, the CXG group showed that the structure of each layer of the colon wall was basically orderly, and the infiltration of inflammatory cells in the submucosa was reduced, which showed significant improvement. The hypothalamic nerve cell edema and solid shrinkage were reduced in CXG group. The results of quantitative PCR showed that as compared with the model control group, the proteins expression of CRH, ACTH and CORT in colon tissues were decreased in medium-dose and high-dose CXG group(all P<0.05), and the proteins expression of CRH, CORT and ACTH in hypothalamus tissues were decreased(all P<0.05)in high-dose CXG group.Immunohistochemical results showed that CXG could reduce the proteins expression of CRH, ACTH and CORT in colon tissue and hypothalamus tissue(all P<0.05). Conclusions:Compound Xiongdanyinchen granules can reduce liver tissue injury and intestinal inflammatory response, improve intestinal mucosal barrier, reduce hypothalamic nerve cell swelling and necrosis, and reduce the secretion of CRH, ACTH and CORT in colon and hypothalamus of MAFLD rats. These results suggest that the mechanism of action of compound Xiongdanyinchen granules in treating MAFLD may be related to the reduce dysfunction of hypothalamus-pituitary-adrenal(HPA)axis.

Objective To investigate the effect and mechanism of Jiawei Ditan Decoction on cardiomyocyte ferroptosis in rats with chronic intermittent hypoxia based on the phosphoinositide 3-kinase(PI3K)/protein kinase B(Akt)signaling pathway. Methods Thirty 8-week-old male Sprague-Dawley rats were randomly divided into normal control,low-oxygen intervention, and Jiawei Ditan Decoction intervention groups using the random number table method, with 10 rats per group. The low-oxygen and Jiawei Ditan Decoction intervention groups were treated with intermittent hypoxia chamber for modeling, with 8 h of intervention daily.Gavage administration was performed before and after daily intervention. The intervention group received a dosage of 16.38 g/kg Jiawei Ditan Decoction, whereas the other two groups received normal saline. The experimental intervention period was 12 weeks.Hematoxylin and eosin staining was used to observe the morphology of myocardial tissue.Transmission electron microscope was used to observe the mitochondrial ultrastructure in cardiomyocytes.Biochemical detection was used to measure the Fe2+and malondialdehyde(MDA)content in myocardial tissue.Phosphorylated-phosphoinositide 3-kinase(p-PI3K),PI3K,phosphorylated-protein kinase B(p-Akt),Akt,solute carrier family 7 member 11(SLC7A11),glutathione peroxidase 4(GPX4),nuclear factor erythroid 2-related factor 2(Nrf2),and acyl-coenzyme A synthetase long-chain family member 4(ACSL4)protein expression in myocardial tissue were measured using western blotting. The rat cardiomyocyte cell line,H9c2,was cultured in high glucose Dulbecco's Modified Eagle Medium, and the ultra-high-performance liquid chromatography was used to detect drug components in Jiawei Ditan Decoction. The CCK-8 assay was used to detect the cell survival rate of cells treated with different concentrations of Jiawei Ditan Decoction. The cells were divided into the normoxic, erastin, and erastin+Jiawei Ditan Decoction intervention groups, and p-Akt and GPX4 protein expression was measured using western blotting.p-PI3K and GPX4 protein expression were detected in the normoxic,low-oxygen intervention 12 h,and low-oxygen intervention 12 h+740Y-P groups. The p-PI3K,PI3K,p-Akt,Akt,and GPX4 protein expression was detected in the normoxic,low-oxygen intervention 6 h,low-oxygen intervention 12 h,low-oxygen intervention 12 h+Jiawei Ditan Decoction low-dose,low-oxygen intervention 12 h+Jiawei Ditan Decoction medium-dose,and low-oxygen intervention 12 h+Jiawei Ditan Decoction high-dose groups(drug concentrations of 5,10,and 20 g/L,respectively).Results The myocardial cells in the low-oxygen intervention group were disordered and swollen and the mitochondrial arrangement of myocardial cells was disordered, with increased mitochondrial membrane density, ruptured cristae, and vacudar degeneration compared with those in the normal control group. The myocardial cells in the Jiawei Ditan Decoction intervention group were arranged neater, and the morphology of mitochondria was relatively regular than those in the low-oxygen intervention group, and no apparent swelling was observed.Compared with the normal control group, the low-oxygen intervention group showed an increase in the MDA and Fe2+content,a decrease in the p-PI3K/PI3K and p-Akt/Akt values, decreased SLC7A11,GPX4,and Nrf2 expression, and increased ACSL4 expression(P<0.05).Compared with the low-oxygen intervention group, the Jiawei Ditan Decoction intervention group showed a decrease in MDA and Fe2+content,an increase in the p-PI3K/PI3K and p-Akt/Akt values, increased SLC7A11,GPX4,and Nrf2 expression, and decreased ACSL4 expression(P<0.05).Jiawei Ditan Decoction contained DL-stachydrine,D-(+)-malicacid,adenosine,etc.The cell survival rate of the 10.00 g/L group increased(P<0.05)compared to that of the Jiawei Ditan Decoction 0,1.25,2.50,5.00,and 20.00 g/L groups.Compared with the normoxic group,p-Akt and GPX4 expression decreased in the erastin group(P<0.05);compared with the erastin group, the erastin+Jiawei Ditan Decoction intervention group showed increased p-Akt and GPX4 expression(P<0.05).Compared with the normoxic group,p-PI3K and GPX4 expression decreased in the low-oxygen intervention 12 h group(P<0.01);compared with the low-oxygen intervention 12 h group, the low-oxygen intervention 12 h+740Y-P group showed increased p-PI3K and GPX4 expression(P<0.05).Compared with the normoxic group,GPX4 expression and the p-Akt/Akt and p-PI3K/PI3K were reduced in the low-oxygen intervention 6 and 12 h groups(P<0.05);compared with the low-oxygen intervention 6 h and 12 h groups, the p-PI3K/PI3K and p-Akt/Akt values increased in the low-oxygen intervention 12 h+Jiawei Ditan Decoction low, medium, and high-dose groups(P<0.05).GPX4 expression was also increased(P<0.05).Conclusion Jiawei Ditan Decoction may improve cardiomyocyte ferroptosis in rats by regulating the PI3K/Akt signaling pathway, thereby playing a protective role against chronic intermittent hypoxia-induced cardiac injury in rats.
