1.Diagnosis and treatment of metastatic pancreatic tumor with a case series of 10 patients

Objective To describe the diagnosis and treatment of metastatic pancreatic cancer. Methods The clinical data of 10 cases of metastatic pancreatic tumor in the first affiliated hospital of China Medical University from July 1997 to July 2007 were analyzed retrospectively. Results The etiologies of primary tumors were lung cancer(n=3), colonic carcinoma(n=2), stomach cancer(n=2), renal cell carcinoma(n=2), nasopharyngeal carcinoma(n=1). The median interval between the diagnosis of primary tumor and pancreatic metastases was 40 months (range:0~192 months). All the metastases were located in the pancreatic heed and neck, and solitary metastasis was detected in one ease, while other 9 cases were multiple metastases. The mean maximum tumor size was 3.03 cm. The main clinical manifestations were abdominal pain, bloating, anorexia and jaundice. 2 cases underwent pancreaticoduodeneetomy, 1 case underwent arterial pancreatic perfusion chemotherapy, 1 case underwent percutaneous biliary stenting and 2 cases received systematic chemotherapy, 1 case received radiotherapy, 3 patients did not accept any therapy. 7 patients were followed-up, the median survival was 10.6 months (range:2~44 months). Conclusions Metastatic pancreatic cancer was rare and the clinical manifestation was non-specific, lndividuaized treatment should be selected on a case-by-case basis. Aggressive surgical resection should be offered to some selected patients.
2.Expression of TMPRSS4 in pancreatic cancer and clinical significance

Objective To investigate the expression of TMPRSS4 mRNA,protein in human pancreatic cancer tissues and to explore the relationship between the expression of TMPRSS4 protein and the clinicopathologic parameters.Methods Real-time PCR and Western blotting were used to detect the expressions of TMPRSS4 mRNA and protein in 16 samples of pancreatic cancer tissues and adjacent normal pancreatic tissues.The expression of TMPRSS4 protein in 61 samples of pancreatic cancer tissues and 26 samples of adjacent pancreatic tissues and 4 samples of normal pancreatic tissues was detected by using immunohistochemistry and its relationship with clinicopathological features was analyzed.Results The expression of TMPRSS4 mRNA and protein of pancreatic cancer tissues were significantly higher than those in adjacent pancreatic tissues (9.09 ± 7.01 vs.1.27 ± 0.72; 1.223 ± 0.125 vs.0.667 ± 0.106,P < 0.01 ) ;the expression rate of TMPRSS4 protein of pancreatic cancer tissues was 67.2% (41/61),which were significantly higher than that in adjacent pancreatic tissues[3.8% (1/26),P < 0.01].There was no TMPRSS4 protein expression in normal pancreatic tissues.There was no significant correlation between the expression of TMPRSS4 protein and the age,gender,tumor location or tumor size was found.There was significant correlation between the expression of TMPRSS4 protein and the degree of differentiation,lymph node metastasis,and clinical staging (P < 0.05 ).Conclusions TMPRSS4 protein is highly expressed in pancreatic cancer tissues,and the expression of TMPRSS4 is associated with the degree of malignancy of pancreatic cancer.
3.The study of external factors affecting auditory ability of infants and toddlers after cochlear implantation.

OBJECTIVE:
To analyze external factors affecting auditory ability of infants and toddlers after cochlear implantation in the first year of switch-on.
METHOD:
Seventy-five infants and toddlers after cochlear implantation were selected as subjects, using LittlEARS Auditory Questionnaire to assess and analyze the correlations with auditory ability and external factors (including gender, cochlear implanted age, pre-implant hearing aid fitting, caregivers' education background, household income and rehabilitation modes) in different stages (before switch-on, and 3 months, 6 months, 9 months, 12 months after switch-on).
RESULT:
The mean scores of LittlEARS were significantly different in cochlear implanted age group, pre-implant hearing aid fitting group and rehabilitation modes group (P < 0.05), and there was no significant difference in other groups such as external factor gender, caregivers' education background and household income (P > 0.05). The correlations with the mean scores of LittlEARS and cochlear implantation age or pre-implant hearing aid fitting were significant at 3 months or 6 months after switch-on(/r/ ≥ 0. 3, P < 0.01). However, the correlation with the mean scores of LittlEARS and rehabilitation modes was significant at 12 months after switch-on(/r/ > ≥ 0.3, P < 0.01).
CONCLUSION
Cochlear implanted age and pre-implant hearing aid fitting were the important factors affecting auditory ability of infants and toddlers after cochlear implantation in the first year of switch-on. The effect of rehabilitation modes on auditory ability of infants and toddlers after cochlear implantation was slow.

4.Serous cystadenoma of the pancreas: a report of 18 cases

Objective To investigate the diagnosis and treatment of serous cystadenoma of the pancreas. Methods The clinical data of 18 patients with serous cystadenoma of the pancreas which were admitted into the First Affiliated Hospital of China Medical University from October 1999 to October 2010 were retrospectively analyzed. Results There were 15 females(83.3%) and 3 males (16.7%).Tumors were present in the pancreatic body and tail in 12 cases ( 66. 7% ), in the pancreatic head in 3 cases ( 16. 7% ) and in the pancreatic neck in 3 cases( 16. 7% ). The mean maximum diameter of the tumor was 6. 5 cm. No specific clinical features were indentified. The size of the tumor was significantly correlated with clinical symptoms. CT was main examination with correct diagnosing rate of 61.1%. All 18 patients received surgical resection. Pancreaticoduodenectomy was performed in 3 patients, distal pancreatectomy in 5 cases,spleen-preserving distal pancreatectomy in 5 cases, middle pancreatectomy in 3 cases, and tumor enucleation in 2 cases. Postoperative pancreatic fistula developed in 10 cases (55.6%);Fistula was healed by conservative therapy in all these 10 cases. Postoperative followed up from 6 to 125 months (mean,48. 3months) found no recurrence or metastasis. Conclusions CT was main imaging examination for serous cystadenoma of the pancreas. Surgical resection should be adopted for serous cystadenoma of the pancreas with clinical symptoms but uncertain malignancy.
5.MRI diagnosis of abdominal pregnancy

Objective To observe the application value of MRI in diagnosis and clinical management of abdominal pregnancy.Methods MRI and clinical data of 5 patients with abdominal pregnancy proved surgically were analyzed retrospectively.MRI performances were analyzed and compared with findings in operation.Results Prenatal MRI correctly diagnosed abdominal pregnancy in all 5 patients,including 1 case of primary and 4 cases of secondary abdominal pregnancy.The position of uterine rupture was revealed in all 4 cases of secondary abdominal pregnancy.MRI performances of placenta conformed to operative findings in all 5 patients.A lot of fresh intra-abdominal hemorrhage was found in 1 case,oligohydramnion in 2 cases,and abnormalities of placenta and fetus in 2 patients,which were consistent with findings of operation.Conclusion Prenatal MRI offers a high accuracy rate in diagnosis of abdominal pregnancy.MRI can show fetus,placenta and the relationship with surrounding tissue,therefore being helpful to clinical management of abdominal pregnancy.
6.Study on the imbalance of T helper 17 cells/regulatory T cells in children with Henoch-Schonlein purpura complicated with Helicobacter pylori infection

Objective:To investigate the roles of T helper 17(Th17)cells and regulatory T(Treg)cells, interleukin(IL)-17 levels, and IL-6 levels in the pathogenesis of Henoch-Schonlein purpura(HSP)complicated with Helicobacter pylori(HP)infection in children. Methods:Children diagnosed with acute HSP and/or HP infection by the Department of Pediatrics, Xiamen Maternal and Child Health Care Hospital from January 2017 to December 2018 were enrolled in the study.There were 15 children with HSP complicated with HP infection(HSP-HP group), 26 children with HSP(HSP group), and 21 children with HP infection(HP group). Twenty healthy children of the same age over the same period were included in the healthy control group.The proportions of Th17 cells and Treg cells were tes-ted by flow cytometry(FCM), and concentrations of IL-17 and IL-6 in plasma were measured by enzyme linked immunosorbent assay(ELISA).Results:(1)Compared with that of the healthy control group [(1.42±0.63)%], the proportion of Th17 cells in the HSP-HP group, HSP group and HP group [(2.79±0.78)%, (2.04±0.77)%, (2.08±0.86)%] was significantly increased( F=5.271, P<0.05). The levels of IL-17 and IL-6 in the HSP-HP group [(552.16±121.13) ng/L and (12.36±6.55) ng/L], HSP group [(506.39±113.62) ng/L and(11.82±5.01) ng/L] and HP group [(483.14±121.77) ng/L and (10.19±3.87) ng/L] were significantly higher than those in the healthy control group [(302.96±82.83) ng/L and (7.81±2.04) ng/L]( F=13.113, 6.692, all P<0.01). The proportion of Treg cells in the HSP-HP group [(1.35±0.49)%], HSP group [(1.13±0.86)%] and HP group [(1.09±0.65)%] was significantly lower than that in the healthy control group [(2.31±0.83)%]( F=6.983, P<0.05). (2)The proportion of Th17 cells and the level of IL-17 in the HSP-HP group were significantly higher than those in the HSP and HP groups(all P<0.05). There was no significant difference in the proportion of Th17 cells and the level of IL-17 between the HSP group and HP group(all P>0.05). There was no significant difference in the proportion of Treg cells and the level of IL-6 among the HSP-HP group, HSP group, and HP group(all P>0.05). Conclusions:The imbalance of Th17/Treg cells in peripheral blood may contribute to the development of HSP-HP, HSP and HP infection in children.High expression of Th17 cells and their related IL-17 is more prominent in children with HSP-HP.
7.Clinical Diagnosis and Treatment Recommendations for Ocular Toxicities of Target Therapy and Immune Checkpoint Inhibitor Therapy.

The increasing use of target therapy and immunocheckpoint inhibitors in cancers has brought new hope of survival to patients with advanced tumors. However, more and more adverse side-effects and toxicities of these medications had been reported, affect almost all human organs including the eye. These adverse effects may affect the entire ocular tissues, like eyelid, eye lashes, conjunctiva, cornea, uvea, retina, optic nerve and so on, which are always been ignored by patients and doctors. In this paper we will summarize the characteristics of the related ocular diseases and give our advice on how to diagnose and manage these diseases.
