3.Comparative study of pressure-control ventilation and volume-control ventilation in treating traumatic acute respiratory distress syndrome.

OBJECTIVETo observe the clinical therapeutic effect and side effect of pressure-control ventilation (PCV) on traumatic acute respiratory distress syndrome (ARDS) compared with volume-control ventilation (VCV).
METHODSForty patients with traumatic ARDS were hospitalized in our department from June 1996 to December 2002. Twenty were treated with PCV (PCV group) and 20 with VCV (VCV group). The changes of the peak inflating pressure and the mean pressure of the airway were observed at the very beginning of the mechanical ventilation and the following 12 and 24 hours, respectively. The transcutaneous saturation of oxygen pressure, the pressure of oxygen in artery, the mean blood pressure, the central venous pressure, the heart rate and the incidence of the pressure injury were also monitored before ventilation and 12 hours after ventilation.
RESULTSThe pressure of oxygen in artery, the transcutaneous saturation of oxygen pressure, the heart rate and the respiratory rate in the PCV group were obviously improved after ventilation treatment. The peak inflating pressure, the mean pressure of the airway and the central venous pressure in the PCV group were lower than in the VCV group. The incidence of pressure injury was 0 in the PCV group while 10% in the VCV group.
CONCLUSIONSThe clinical effect of PCV on traumatic ARDS is better and the incidence rate of pressure injury is lower than that of VCV. PCV has minimal effects on the hemodynamics.

4.Effect of thymosin alpha 1 on cellular immune function in elderly patients with malignant tumor.

OBJECTIVETo investigate the effect of thymosin alpha 1 on cellular immune function in the elderly patients with malignant tumor.
METHODSThirty patients with malignant tumor were injected with thymosin alpha 1 subcutaneously at the dose of 1.6 mg q.d. for the first month and q.o.d. for the following month. The number of T cell subgroups and the activity of NK cell in peripheral blood were detected and the quality of life of the patients were evaluated before treatment and at the end of treatment.
RESULTTreatment of thymosin alpha 1 increased the number of CD4 cells and improved the NK activity, and also improved the quality of life of the elderly patients with malignant tumor. There were no side effects found.
CONCLUSIONThymosin alpha 1 can enhance the cellular immune function of the elderly patients with malignant tumor.

5.Vinorelbine induces apotosis and decreases telomerase activity in human epithelial ovarian cancer cells line SKOV 3

Objective To investigate the effect of different concentrations of vinorelbine on apoptosis ,telomerase ac-tivity and expression of human telomerase-reverse transcriptase gene ( hTERT ) in human epithelial ovarian cancer cells SKOV3.Methods Ovarian cancer cells SKOV 3 were treated with vinorelbine under different concentrations . The cell proliferation was measured by cell counting kit-8 ( CCK-8) assay , and the cell apoptosis was detected by flow cytometry.The telomerase activity of SKOV3 cells was determined by TRAP-PAGE-silver staining;The mRNA expression of hTERT was performed by RT-PCR assay .Results Vinorelbine could significantly inhibited the prolifer-ation of SKOV3 cells,induce cell apoptosis (P<0.01),reduce the telomerase activity and expression of hTERT mRNA (P<0.01), in dependent of a concentration-time manner.Conclusions The detection of telomerase activity and the mRNA expression of hTERT might be vital for predicting the prognosis of patients with epithelial ovarian cancer .
6.Application and curative effect of plasma exchange in myasthenia gravis patients with thymectomy.

OBJECTIVE:
To explore the application of plasma exchange in myasthenia gravis (MG) patients with thymectomy, and to evaluate its curative effects.
METHODS:
The clinical data of 50 patients with thymectomy were analyzed retrospectively, and the patients were divided into Group A (23 cases, undergoing thymectomy and plasma exchange) and Group B (27 cases, undergoing thymectomy). The difference of serum concentration of AChRab was examined by ELISA, and the time of auxiliary breath, crisis of myasthenia and clinical evaluation in the 2 groups were compared.
RESULTS:
Serum concentration of AChRab in Group A descended obviously after the plasma exchange. One day and one week after the operation, the AChRab concentration in Group A was obviously lower than that in Group B, but there was no difference one month after the operation. One week after the operation, the time of auxiliary breath,crisis of myasthenia and clinical evaluation in Group A were less than those in Group B.
CONCLUSION
Plasma exchange together with thymectomy can improve the temporary curative effects in patients with MG. It is safe and effective for MG.

7.Radiation-induced hypopituitarism in patients with nasopharyngeal carcinoma

Objective To investigate the clinical characteristics and imaging features of radiation-induced hypopituitarism (RIH) in nasopharyngeal carcinoma (NPC) patients after radiotherapy.Methods Retrospective study was performed on the clinical data of 9 NPC patients (7 males and 2 females), diagnosed as RIH after radiotherapy in our hospital from 2000 to 2009; their clinical manifestations, anterior-pituitary hormone levels, cranial MRI features and follow-up results were analyzed. Results The interval between complications after radiotherapy and diagnoses of RIH varied from 10 months to 15 years.Multiple hormone deficits appeared in 5 patients; thyrotropin deficiency and hyperprolactinemia were the most common complications, followed by adrenocorticotropic hormone (ACTH) insufficiency.Typical radiological imaging of the pituitary gland displayed partially empty sella and atrophy of pituitaly gland,which was demonstrated in 2 patients.Five patients got improvement after hormone replacement therapy. Conclusion Radiation-induced hypopituitarism may occur in NPC patients; most patients are subclinical without typical radiological alterations in MRI for pituitary.Routine evaluation of adenohypophysis function is necessary for early detection of hormone deficiency and early hormone replacement treatment.
8.Analysis of the differences in the expression of HSP27 and c-kit between benign prostatic hyperplasia and prostatic cancer tissues.

OBJECTIVETo examine the differences in the expression of HSP27 and c-kit between benign prostatic hyperplasia (BPH) and prostatic cancer (PCa) tissues and to analyse the relationship between their expression and BPH and PCa, especially the relationship with the occurrence, development, prognosis and treatment of PCa.
METHODSAn immunohistochemical staining (SP method) for HSP27 and c-kit was undertaken on 40 BPH and 40 PCa tissues samples.
RESULTSConsistent patterns of cytoplasmic staining for HSP27 were seen in all sections of tissue from BPH. The glandular epithelium stained very strongly positively and the stroma stained positively. The staining for HSP27 in PCa tissues was located in the cytoplasm of glandular epithelia, but the expression of HSP27 in PCa was higher than BPH (P < 0.05). The staining for c-kit in BPH tissues was located in the cytoplasm of smooth muscle cells, and in PCa tissues was located in epithelial cells. The expression of c-kit in PCa tissues was lower than BPH (P < 0.05). The expression level of both HSP27 and c-kit were decreased with the development of grade of PCa (P < 0.05); HSP27 was increased with the development of clinical stage of PCa (P < 0.05 ); c-kit was decreased with the development of clinical stage of PCa (P < 0.05).
CONCLUSIONThe expression level of HSP27 and c-kit was highly correlated with the process of the development from BPH to PCa, and also correlated with tumor grades and stages. The expression of HSP27 and c-kit may be used as an important pathological index and may be helpful for the treatment of PCa.

9.Effect of Jinguo Weikang Capsule on proto-oncogene expression of gastric mucosa in rats with gastric precancerous lesions.

OBJECTIVETo study the effect of Jinguo Weikang Capsule [see text] on the gene expression of H-ras, epidermal growth factor receptor (EGFR), P53 and C-myc of the gastric mucosa in rats with gastric precancerous lesions, and to investigate the action mechanism of JWC on gastric precancerous lesions.
METHODSA rat model with paratypical proliferation of the gastric epithelium mucosa was established by using 60Co irradiation. Rats were divided into the normal group, model group, high-, medium-, low-dose JWC treatment groups, and the vitacoenzyme control group, and were treated for 30 days. The expression of H-ras, EGFR, P53 and C-myc genes of the gastric mucosa was detected by using immunohistochemical methods.
RESULTSThe expression and over-expression rates of H-ras, EGFR, P53 and C-myc gene in the high-and medium-dose JWC treatment groups were significantly lower (P<0.05) as compared with those of the model group.
CONCLUSIONJWC can inhibit the expression of the H-ras, EGFR, P53 and C-myc genes expression of the gastric mucosa in rats, which may be one of mechanisms involved in suppressing or reversing gastric carcinogenesis.

10.Noninvasive ventilation for obstructive sleep apnea hypopnea syndrome in children.

OBJECTIVETo study the efficacy and safety of noninvasive ventilation (NV) for treating children with obstructive sleep apnea.
METHODSTwenty-one children confirmed obstructive sleep apnea hypopnea syndrome (OSAHS) with full-night polysomnography (PSG) or ambulatory screening device were enrolled in the study. The NV treatment was carried out successfully for all cases. Nasal continuous positive airway pressure (nCPAP) compliance data were gathered via clinical follow-up examination, telephone interview, or mailed questionnaire. The statistical analysis was performed with SPSS 11.0 statistical software. Pre- and on-nCPAP parameters were compared with paired t-test. Twenty-one OSAHS children (17 boys, 4 girls) were enrolled into the study. The mean age of the children was 4. 5 years; ranging from 40 days to 11 years.
RESULTSnCPAP pressure was increased from 4 cm H2O by (1 cm H2O = 0.098 kPa) 0.2 cm H2O each time to the treating pressure which was between 4.8 and 16 cm H2O. Before nCPAP treatment, apnea hypopnea index (AHI) was (80.8 +/- 45.1)/h, the lowest pulse oxygen saturation (SPO2) 0.557 +/- 0.135 and SPO2 <0.90 time during sleep (42.9 +/- 31.9) %, which were much worse compared to that with nCPAP treatment, the above parameters decreased to (6.7 +/- 12.4)/h, 0.862 +/- 0.082, (1.1 +/- 2.5) % respectively(P <0.01).
CONCLUSIONSNoninvasive ventilation is a safe and effective treatment for OSAHS children. It is possible to use nCPAP as a short-term treatment or as a long-term treatment at home.
