Objective To explore the effect of diversified and teamwork health education mode on disease-related knowledge of coronary artery disease patients.Methods 147 cases of patients with coronary artery disease hospitalized in the general hospital of Tianjin medical university were divided into the treatment group which was given diversified health education of teamwork mode and the control group which was given traditional health education.The self-designed questionnaire was used to evaluate the effect of teamwork health education mode.Results Level of knowledge in the treatment group was significantly improved when compared with the control group,the increase amplitude of knowledge level in the treatment group was higher than the control group,and the scores of the treatment group were higher than the control group in the dimensions of diet,prevention,clinical manifestation,treatment,activity,the differences were statistically significant.Conclusions With diversified,multidisciplinary,and teamwork health education mode the patients' level of knowledge was significantly improved,the correct and positive awareness about disease were increased,and the patients were urged to take healthy behavior,so that their physical and mental state will maintain the best.

Objective To study the clinical significance of molecular markers and routine coagulation tests in patients with cerebral infarction and to set up a serial programs of laboratory diagnosis,monitoring and treatment of cerebral infarction.Methods Prothrombin fragment 1+2(F1+2),thrombin antithrombin III complex(TAT), D-dimer(D-D),Von willebrand Factor(vWF),antithrombin(AT), protein C(PC),prothrombin time(PT),activated partial thromboplastin time(APTT) and thrombin time (TT) were determined in 90 patients with cerebral infarction and 60 normal control subjects.Results The levels of F1+2,TAT,D-D,vWF were significantly higher in patients with cerebral infarction than that in control subjects. But the levels of PC,AT,PT,APTT,TT in patients with cerebral infarction were no different from that in control subjects.Conclusion There were hypercoagulable states in patients with cerebral infarction.The activity of prothrombin is higher, thrombin is generated more, the activity of fibrinolysis is higher too, but anticoagulation system is not sufficiently activated .Meanwhile, perhaps endothelial lesion would be the main role of coagulation system activating and pathogenesis.Molecular markers such as F1+2,TAT,D-D,VWF can be as diagnositic signs but routine coagulable tests can not display the hypercoagulable states in patients with cerebral infarction.

Objective:To investigate the effects of exenatide on podocyte in diabetic nephropathy mice.Methods:Diabetic nephropathy mice models were induced by using streptozocin-treated C57BL/6J mice on high fat diets, which were randomized by random number table to diabetic nephropathy control group (DN group, n=8) and exenatide treatment diabetic nephropathy group (DN+ Ex group, n=8). The C57BL/6J mice on normal chow diet were used as normal control group (NC group, n=8). After the intervention, blood glucose, renal function, and urine albumin/creatinine ratio were measured. Pathological glomerular changes were observed by pexiodic acid-Schiff stain (PAS) staining. The mRNA expression of profibrotic molecules Collagen Ⅳ, transforming growth factor-β (TGF-β), and Fibronectin in glomerular lysates were measured by realtime quantitative PCR (RT-PCR). Podocyte injury and apoptosis were evaluated by immunofluorescent staining and transmission electron microscopy. The expression of Nephrin, Cleaved caspase-3, protein kinase B (Akt), and phosphorylated Akt (p-Akt) in glomerular lysates were examined by western blotting. Results:Compared with the DN group, urine albumin/creatinine ratio was significantly decreased in the DN+ Ex group ( P<0.01). PAS staining and analysis found that exenatide administration ameliorated mesangial matrix expansion and glomerular hypertrophyin in DN group ( P<0.05). RT-PCR analyses showed that the glomerular expression for Fibronectin, TGF-β, and Collagen Ⅳ were significantly decreased in the DN+ Ex group compared with the DN group ( P<0.01). Immunofluorescent staining and transmission electron microscopy revealed that exenatide treatment improved podocyte injury and apoptosis in DN group. Western blotting analyses showed that exenatide increased the Nephrin expression, decreased the Cleaved caspase-3 expression, increased the p-Akt expression in glomerular lysatesin diabetic nephropathy mice ( P<0.01). Conclusion:Exenatide attenuates podocyte injury and apoptosis and proteinuria, and prevents the progression of diabetic nephropathy mice. Phosphatidylinositol 3-kinase (PI3K)/Akt pathway in glomerular lysates may be related to the protective effects of exenatide.

Objective:To investigate the effects of nitric oxide(NO) and inducible nitric oxide synthase (iNOS) in the experimental abdominal aortic aneurysm (AAA) rat model.Methods:An intra-aortic elastase infusion model was used.Control rats received intra-aortic saline infusion.In the remaining groups,intra-aortic elastase infusion was used to induce aneurysm formation.These rats were treated with intraperitoneal injections of saline postoperatively(experimental group),aminoguanidine postoperatively(medicine group).Serum NO and aortic diameter were measured,Changes of histology,iNOS and MMP-9 were observed in the aortic wall.Results:Experimental group produced AAAs with significant production of iNOS,MMPs and serum NO compared with controls.In medicine group reduced aneurysm size and displayed suppression of MMPs expression,inflammatory infiltrates and serum NO production were detected.Conclusion:Expression of iNOS and MMP-9 are induced and serum NO levels are increased in experimental AAA,iNOS and NO production by iNOS play an important role with detrimental effects during experimental aneurysm development.

BACKGROUND: Bushen Huogu decoction can effectively prevent and treat osteoporosis, but the concrete mechanism of pharmacology is still not clear. 25-hydroxy vitamin D3 and 1, 25-dihydroxyvitamin D3 are important coupling factors, which can regulate bone resorption and formation.OBJECTIVE: To investigate the curative effects of Bushen Huogu decoction on the bone mineral density, bone biomechanics, and level of 25-hydroxy vitamin D3 and 1, 25-dihydroxyvitamin D3 in blood serum, liver and kidney in the ovariectomized osteoporotic rats.METHODS: Totally 108 healthy female Sprague-Dawley rats were randomly divided into model group, sham-operated group, andtreatment group. All rats had been ovariectomized to induce estrogen absence and further establish osteoporotic models, except those in sham-operated group. Treatment group of rats were intragastrically administrated with 2 mL Bushen Huogu decoction,twice a day.RESULTS AND CONCLUSION: Compared with model group, the bone mineral density in the rat femur was significantly increased in the treatment group (P < 0.05), the index of maximal stress and maximal loading of the femoral head were also increased (P <0.05). The concentration of 25-hydroxy vitamin D3 and 1, 25-dihydroxyvitamin D3 in blood serum, liver and kidney were significant higher in the treatment group than those in the model group, and the levels were similar with those in sham-operated group (P >0.05). In the early period of estrogenic hormone absence, the Chinese kidney-tonifying drugs could activate bone metabolism, raise bone mineral density and reinforce quality of bone through up-regulating expression of 25-hydroxy vitamin D3 and 1,25-dihydroxyvitamin D3.

Objective To explore the ultrasound contrast agent (SonoVue) effects upon rabbit testis which were injured after the acute unilateral complete torsion-detorsion and its related mechanisms,on the condition of the diagnostic ultrasound radiation with different doses and different imaging modes.Methods 35 healthy New Zealand white adult rabbits were randomly divided into 5 groups(S,A,B,C,and D),with 7 in each group.Left testis needed ischemia reperfusion after complete 3 hours.Group S:reperfusion without SonoVue group;group A:small-dose (0.1 ml/kg) SonoVue conventional imaging group;group B:high-dose (1 ml/kg) SonoVue conventional imaging group ; group C:small-dose (0.1 ml/kg) SonoVue blasting group; group D:high dose(1 ml/kg) SonoVue blasting group.Groups needed ultrasonic imaging before operation and 24 hour reperfusion.The changes for each group's contrast parameters were analyzed.The contents of malondialdehyde(MDA) and superoxide dismutase (SOD) of testicular tissue were detected,and the expressions of Caspase 3 were examined after 24 hours to make a comparison study among these groups.Results In group D,the time to peak(TTP) and half of declining time(DT/2) were significantly prolonged (P ＜ 0.05),peak-base indensity (PBD) was significantly descended (P ＜ 0.05),and the time-intensity curve manifested that the peak was lower and delayed.SOD in C was lower slightly than in group S.Compared with other groups,SOD in group D decreased,and MDA increased (P ＜ 0.05).Under light microscope,the Johnen's scores in groups A,B,C showed no difference with group S2 (P ＞0.05),while were lower compared with group D (P ＜0.05).Compared with others groups,apoptosis index(AI) and Caspase-3 in group D increased with significance (P ＜0.05).Conclusions Under the diagnostic ultrasound radiation,when conventional imaging and small-does imaging blasted,they did not aggravate the ischemia testicular,while for high-dose,it aggravated.

Objective To establish normal range of components of cord blood cells in healthy fetuses from 19 to 37 weeks' gestation and to provide proof for diagnosis of hematological disorders in prenatal fetuses and premature infants.Methods Twelve hematological parameters were determined in 182 fetuses using Coulter GENS system 2 full automated blood cell counter,and the blood cells were classified by microscope.Results The number of white blood cell(WBC) was increased gradually from 3.58?10~9/L to 5.76?10~9/L with the gestational weeks increasing from 19 to 37 weeks.The differential counts indicated that the lymphocytes represented the main population.The number of lymphocytes and normoblast was decreased gradually and that of neutrophils was increased gradually.The numbers of monocytes,eosinophils and basophils remained stable as the increasing of gestational weeks.The red blood cell(RBC),hemoglobin(HGB) and hematocrit(HCT) were increased gradually but mean corpuscular volume(MCV) was decreased.The differences between mean corpuscular hemoglobin(MCH),platelet volume distribution width(PDW),mean platelet volume(MPV),plateletcrit(PCT) and mean corpuscular hemoglobin concentration(MCHC) were not significant among different fetuses' ages.Conclusion The components of cord blood cells in healthy fetuses are dynamic and the establishment normal range of components of cord blood cells in healthy fetuses is helpful to diagnose the disorders in prenatal fetuses and premature infants.

Objective To investigate Helicobacter pyloric (Hp) infection in children who underwent gastroscopy in Nanjing area.Methods From Jan.2001 to Dec.2010,2 990 pediatric patients with gastrointestinal symptoms who underwent endoscopy and Hp rapid urease test of gastric mucosa specimens.Results There were 2 990 patients who underwent gastroscopy during the 10 years including 1 718 cases of male and 1 272 cases of female.The rate of Hp infection was 53.8％ (924/1 718 cases) and 52.4％ (667/1 272 cases) in male and female,respectively.There was no significant difference (x2 ＝ 0.532,P ＞ 0.05).From Jan.2001 to Dec.2005,611 patients were tested and the rate of Hp infection was 66.9％ (409/611 cases).From Jan.2006 to Dec.2010,2 379 patients were tested and the rate of Hp infection was 49.7％.There was significant difference(x2 =58.13,P =0.01) between the 2 groups.The rate of Hp infection were 49.6％,48.1％,55.2％,60.0％ in 0-6 years group,＞6-9 years group,＞9-12 years group,and ＞ 12 years group,respectively.There was significant difference among the 4 different age groups (x2 =23.66,P ＜ 0.01).The rate of Hp infection in nodular gastritis(61.4％) was higher than that in chronic superficial gastritis (50.6％).There was significant difference between the 2 groups (x2 ＝ 7.42,P ＜ 0.01).The rate of Hp infection in allergic purpura (46.2％) was higher compared with the children with normal gastroscopy results (15.9％).There was significant difference between the 2 groups (x2 =10.19,P ＜ 0.01).Conclusions There is gradually downward trend by year in the rate of Hp infection of children who underwent gastroscopy in Nanjing area by year.The rate of Hp infection is increasing with the age after the age of 6 years.Hp infection may play a role in the nodular gastritis and allergic purpura.

Objective To evaluate the diagnostic value of the aldosterone-renin ratio (ARR) for primary aldosteronism (PA). Methods Serum aldosteronos ( ALD ) and plasma renin activities (PRA)among 44 subjects with primary aldosteronism, 9 subjects with phecchromocytoma, 8 subjects with nonfunctional adrenal tumors, 12 subjects with Cushing syndrome, 4 subjects with stenosis of renal artery and 13 subjects with primary hypertension were retrospectively reviewed. ARR was calculated. The receiver operating characteristics (ROC) curves for every index were used to evaluate diagnostic value. Results The area under the curve(AUC) in the ROC curve of ALD in a supine position was 0. 947, the cut-off value of diagnosis of PA. The AUC for the ROC curve of ALD in upright position was 0. 889, the cut-off value of ALD diagnosis of PA. The AUC for the ROC curve of ARR in a supine position was 0. 978, the cut-off value of diagnosis of PA. The AUC for the ROC curve of ARR in upright position was 0. 981, the cut-off value of specificity. If ARR was combined with ALD in upright position was used, the diagnostic value was better than either index. When ALD > 275 ng/L and the AUC for the ROC curve in upright position was 0. 989,specificity. Conclusions The diagnostic value of ARR in diagnosis of primary aldosteronism is higher than ALD and PRA. ARR in upright position is better than that in supine position, especially when combined with ALD > 275 ng/L in upright position.
