1.Study of testicular ultrastructure in congenital bilateral absence of vas deferens

Objective To study the testicular ultrastructure in CBAVD patients. Methods Ultrastructure of testicular tissue was studied in 13 CBAVD patients with azospermia whereas the testicular morphology under light microscopy was normal. Results The presence of thickening,multilayered,finger shaped protuberent to seminiferous tubules in variable degrees in basement membrane was confirmed at ultrastructural level in 13 CBAVD patients.Collagen fibres revealed an increase in number,proliferation,disturbance in variable degrees and no polarity in all the 13.Malformed heads,metachromatic granules nuclei with abundant bulla,full or partial absence of middle mitochondria sheath were seen in most of spermatozoa.Of 5 cases,invagination and low electronic density layered materials were presented in acrosome and acrosomal cyst respectively.Most of sertoli cells presented abundant lipid droplets and degradative bodies in their cytoplasm in all cases.In 2 cases,tight junctions between sertoli cells were not found. Conclusions Microstructural alteration of testicular tissue,especially conspicuous in spermatozoa and in the limiting lamina of testis,might be a contributory cause of azospermia in addition to the clinical absence of vas deferens.
2.Analysis on TORCH tests in 6 027 cases of pregnant women,puerperae and neonates

Objective To analyze TORCH infection situation in 6 027 cases of pregnant women,puerperae and neonates.Methods The TORCH IgM and IgG was detected by using ELISA for 6 027 cases of pregnant women,puerperae and neonates,and the positive rates of the antibodies to variant TORCH pathogens were analyzed.Results The positive rates of HSVⅠ-IgM,HSVⅡ-IgM,RV-IgM,TOXO-IgM and CMV-IgM were 0.02%,0.02%,0.12%,0.12% and 0.20% respectively.The positive rates of HSVⅠ-IgG,HSVⅡ-IgM,RV-IgM,TOXO-IgM,and CMV-IgM were 63.32%,13.31%,52.83%,12.68% and 58.57% respective-ly.Positive rate of CMV-IgM in neonates was higher than that in pregnant women,the difference was statistically significant(P <0.05).There were totally 28 cases of neonataes detected with acute CMV infection.Conclusion TORCH screening in pregnant women,puerperae and neonates helps the detection of infection disease in perinatal period.
3.Clinical analysis of patients with Wegener's granulomatosis complicated with pulmonary infection

ObjectiveTo investigate the clinical features and risk factors of patient with Wegener's granulomatosis complicated with pulmonary infection.Methods Patients with Wegener's granulomatosis admitted to our hospital in the past 11 years were retrospectively analyzed.Comparisons between groups were performed by t tests or Fisher test.ResultsPulmonary infection occurred in 27 cases with an incidence rate of 29%.Twenty-six percent of pulmonary infections occurred at the initial diagnosis,and 44% occurred within 6 months,while 30% occurred later than 6 months.The clinical manifestations of pulmonary infection were productive cough (89%),hemoptysis (63%),fever and fatigue (56%),chest pain and pactoralgia (33%).The most common causative pathogen were bacteria(59% ),fungi(37% ),and tubercle bacillus(37% ).Sinus infection(P=0.01),hypoproteinemia(P=0.03),hypoimmunoglobulinemia (P=0.007),and methylprednisolone pulse therapy(P=0.002) were the risk factors for pulmonary infection.ConclusionThe occurrence of Wegener's granulomatosis complicated with pulmonary infection is high within 6 months.The most common clinical manifestation is productive cough.The most common causative pathogens are bacteria,tubercle bacillus and fungi.Sinus infection,hypoproteinemia,hypoimmunoglobulinemia,and methylprednisolone pulse therapy are risk factors of pulmonary infection.
4.Effect of comprehensive nursing on small area of diabetic foot ulcer

Objective To study comprehensive nursing meassures for small area of diabetic foot ulcer. Methods A total of 58 patients with small area of diabetic foot ulcer in our department were divided into two groups.The comprehensive nursing group (CN group,30 cases) adopted comprehensive nursing and treatment as follows, surgical dcbridement, infrared radiation of wound surface,sprinkling of growth factors(bFGF and rhEGF),covered by hydrocolloid dressing(Algoplaque).The control group(28cases) received hydropathic compress with 0.5% povidone iodine or 5% furacillin solution.Wound heal-ing progreasion,average healing time and the positive rate of wound germiculture of the two groups were observed. Results The average healing time of the CN group was (29.6±9.4)d,which was shorter than that of the control group,( 39.2±13.5 )d, P < 0.05. Statistical difference existed in the positive rate of wound germiculture at the 7th day and the 14th day between the two groups (P < 0.05). Conclusions Routine medical treatment and general nursing measures plus surgical debridement,infrared radiation of wound sur-face,sprinkling of growth factors and hydrocolloid dressing possess facilitative healing effect for small area of diabetic foot ulcer.
6.APPLICATION OF TRICKLING FILTERS IN TREATMENT OF WASTEWATER FROM GAS-GENERATING WITH HEAVY OIL

A research combined trickling filter system and active sludge aeration system was applied in the treatment of industrial wastewater from gas-generating with heavy oil. The wastewater contained both high contents of NH+4-N and mixed hydrocarbons including various PAHs. Its BOD5/COD ratio was less than 0.3 and belongs to recalcitrant, toxic wastewater. The results showed a touch-growth biofilms system was formed on the porous packing material and it played a key role in the decrease of toxicity of the influent. It could also improve the biodegradability of the wastewater.
7.Study on Treatment of Dyeing Wastewater in the Combination Process of ABR and SBR

A laboratory scale combination process of anaerobic baffled reactor(ABR) with sequencing batch reactor(SBR) for treatment of real dyeing wastewater was studied.The effects of operational conditions were investigated.The results demonstrated that removal rates of COD,colour and aniline were 32%~95%,89%~99% and 50%~98%,respectively,the effluents of COD were 30.0 ~97.1mg/L,colour were 8 ~40 times dilution ratio,concentration of aniline were 0.20 ~0.95 mg/L,which could meet the National Discharge Criteria(GradeⅠ) under the operational conditions of HRTs of 24~36 h,organic loading rates of 0.43 ~2.46 kg COD/(m~(3)?d),the influent pH values of 6.5~8.0,ambient temperatures of 20℃~35℃ at the ABR stage and DOs of 2 mg/L,reaction times of 3~10h,settle times of 2 h at the SBR stage.
8.Relative bioavailablity of cefaclor effervescent tabletsin human volunteers

Aim To study relative bioavailablity of cefaclor effervescent tablets in healthy volunteers. Methods According to the crossover design, A volunteers were each orally given a single does of the 0.75 g cefaclor effervescent tablets and cefaclor capsules with an interval of 5 days between the two formulations.The plasma concentrations of the drug were determined by RP-HPLC.Pharmacokinetic parameters were obtained by ATPK programe,and calculated on the basis of open single compartment model.Results After a single oral dose, the peak levels in plasma averaged Cmax(31.27?5.81)?g?ml-1 and(30.56?5.25) ?g?ml-1 at (0.58?0.12)h and(0.73?0.17)h and AUC0~4(35.48?4.65) ?g?h?ml-1 and (35.89?2.90) ?g?h?ml-1 for tablet and capsule,respectively. Conclusion The result shows that two formulations are bioequivalence.
9.Investigation of information about the parents' knowledge and inquiry to the nursing participating of the consultant

Objective To learn the information about the parents' knowledge and inquiry to the nursing participating of the consultant. Methods One hundred and twenty cases of parents were investigated with the self-designed questionnaire. The rates of grade data was accomplished with statistical analysis. Results Knowledge of consultant: 82.5 percent eases parents knew their fight of consultant provided by the hospital. 70.32 percent of parents were satisfied with the nursing consultant for them. Parents of different ages had different ideas about the consultant contents and ranges provided by the doctors and nurses (P < 0.01). Styles of consulting: 67.5 percent of parents wanted to be consulted in prior. 53.33 percent wanted to be consulted at the suitable situation and be informed part of the contents. 78.53 percent wanted to be informed by the nurse in charge of the children. Lines of the consulting contents : the former three ones are as followings, staff of the department, environment, rules and discipline;kinds of the diet;function of the medication, purposes of the tests, items to be care about. Conclusions Parents agreed with the nursing consulting in the practice of medical affairs. More sufficient nursing consultants were needed. Consultant was the basic insure of their informed consents.
10.Surgical management of male infertility beyond assisted reproductive technology.

OBJECTIVETo investigate the role of urological surgery for male infertility which is beyond the help of assisted reproductive technology (ART).
METHODSWe retrospectively reviewed the records of 7 male infertility patients who, having failed to respond to ART, underwent urological surgery from 1999 to 2003. Of the 7 cases, 4 were varicocele, 2 were post-bilateral vasectomy and 1 was bilateral chronic epididymitis. For the 7 patients, 6 times we performed in vitro fertilization-embryo transfer (IVF-ET) and 5 times we attempted intracytoplasmic sperm injection (ICSI) , but all failed. After that, we resorted to surgical approaches, ligation of internal spermatic veins in 4 cases, bilateral vasovasostomy in 2, and unilateral epididymovasostomy in 1.
RESULTSAfter surgical intervention, 2 cases fathered children, 1 achieved pregnancy and 3 cases improved in the quality and quantity of seminal fluid; only 1 failed to respond to the therapy.
CONCLUSIONIn the treatment of male infertility, priority should be given to surgical intervention and, in case of failure, assisted reproductive technology could be considered.
