1.The influence of the value of the echocardiography in the detection of cancer therapeutics-related cardiotoxicity by World Alliance Societies of Echocardiography(WASE) study

Objective:To explore the influence of the value of the echocardiography in the detection of cancer therapeutics-related cardiotoxicity by World Alliance Societies of Echocardiography(WASE) study.Methods:Tirty-seven breast cancer patients underwent echocardiography at baseline before chemotherapy (T0) and after the second (T2), fourth (T4), sixth (T6) chemotheraphy cycles in Qilu Hospital of Shandong University from March 2017 to March 2018. According to different guidelines[ASE lower limit of normal left ventricular ejection fraction(LVEF-LLN) 53%, ESC LVEF-LLN 50% and the WASE study (Women LVEF-LLN 58%, Asian Women LVEF-LLN 59%)], the number of patients with myocardial injury during chemotherapy was evaluated, and compared with the global longitudinal strain(GLS) standard, the percentages of patients with myocardial injury in different standards were analyzed.Results:With the up-regulation of LVEF-LLN, more patients with early myocardial damage were found. During chemotherapy, 1 patient showed left ventricular ejection fraction (LVEF) fell by >10% from baseline to LVEF<50% (2016 ESC Guidelines), 3 patients showed LVEF fell by >10% from baseline to LVEF<53% (2014 ASE Guidelines), 7 patients for LVEF fell by >10% from baseline to LCVEF<58%, and 9 patients for LVEF fell by >10% from baseline to LVEF<59%, which was still less than the number of patients detected by GLS criteria (18 patients), but the sensitivity of LVEF fell of >10% from baseline to LVEF<59% was not statistically different from GLS standard ( P>0.05). Conclusions:With the up-regulation of LVEF-LLN, the sensitivity of LVEF in evaluating early myocardial damage was increased, but GLS was still the most sensitive index to monitor the cancer therapeutics-related myocardial damage.

5.Analysis of Marketing Conception in State-Owned and Private-Funded Hospital Management

Objective To compare the differences between state-owned hospitals and private-funded hospitals in Shanghai on the introduction and application of hospital marketing conception. Methods Questionnaire and interviews were conducted to reflect understandings in marketing conception,advertisements,hospital brand,customers' sense of satisfaction,etc. Results State-owned hospitals were less concerned in marketing conception compared with their private counterparts,even less sufficient in marketing input or management. Conclusion The state-owned hospitals should update their conception and enhance their ability of marketing management.While the private hospitals need to maintain the moderation of marketing and deal with the relationship among marketing,profit and development properly.

In this study, we investigated the expression of CXCL12 (SDF-1)/CXCR4 in trophoblasts and the role they play in the gestation. Immunochemistry was used to detect the expression of CXCR4 and CXCL12 in human villi and placenta. Highly purified extra-villous trophoblasts (EVTs) ere detected for CXCR4 and CXCL12 in vitro by immunocytochemistry. The chemotaxis of CXCL12 was tested in transwell and the chemotactic activity was quantitatively examined. It was suggested that both CXCR4 and CXCL12 were expressed in trophoblasts and were decreased with the gestation time P < 0.05). In a certain coverage, CXCL12 exhibited chemotactic activity which was positively correlated with its concentration [(r) = 0.68, P < 0.01], the maximum chemotactic index (CI) was 1.62 +/- 0.12. Our results suggest that interaction between CXCR4 and CXCL12 is involved in materno-fetal immunological tolerance in all three trimesters of gestation and contributes to the invasion of EVTs during pregnancy.

8.Observation on scavenging free radical function of Artemisia burning products.

OBJECTIVETo study the pharmacologic action of Artemisia burning products.
METHODSThe extractions of Artemisia burning products were determined by spectrophotometry. The scavenging ability of Artemisia burning products on DPPH was evaluated. The chemical components and structures of Artemisia burning products were analyzed by Gas Chromatography and Mass Spectrometry (GC-MS).
RESULTSThe scavenging ability of extractions from Artemisia burning products was the strongest. Thirty-six chemical components were detected, and the 5-tert-Butylpyrogallol among them had a stronger anti-oxygen capacity, its scavenging free radical ability was 1.55 times and 1.21 times as strong as VitC and BHT, respectively.
CONCLUSIONThe scavenging free radical ability of 5-tert-Butylpyrogallol extracted from Artemisia burning products is stronger than the natural antioxidant of VitC and artificial synthetic of BHT.

9.Alterations of myocardial regional motion time variance caused by different extent of coronary artery stenosis

Objective To access the alterations of ischemic myocardial regional motion time caused by different extent of coronary artery stenosis by 2D-strain technique. Methods Two-dimensional images were acquired of the standard parasternal and apical (4-chamber,2-chamber and long-axis) views and short-axis views (mitral annulus, papillary muscle and apex) in 140 patients with coronary artery disease. All left ventricular segments were divided into 5 groups according to coronary stenosis seen on angiography:normal,25% -49% ,50% -74% ,75% -99% and 100%. The time to segmental systolic peak strain, systolic peak strain rate, end systolic strain, early diastolic peak strain rate and late diastolic peak strain rate in longitudinal, radial, circumferential directions and the systolic peak rotation, systolic peak rotation rate, early diastolic rotation rate,late diastolic rotation rate were analyzed with EchoPAC offline software. Results In longitudinal direction,compared with normal group,the time to early diastolic peak strain rate of segments with 75% - 99% coronary stenosis increased significantly, the time to systolic peak strain rate, early diastolic peak strain rate of segments with occluded artery were significantly delayed compared with the other 4 groups (P <0.05). In radial, circumferential directions, the time to systolic peak strain, systolic peak strain rate,late diastolic peak strain rate of segments with 25% - 49% was shorter than that of normal segments. Those indices for segments with 50% - 74% coronary stenosis were delayed compared with segments with 25% - 49% stenosis, however, those for segments with 75% - 99% coronary stenosis showed some improvement in the comparison with segments with 50% - 74% stenosis. All time indices for segments with occluded arteries were delayed ( P <0.05). About rotation, the time to the systolic peak rotation,systolic peak rotation rate,early diastolic peak rotation rate with 25% -49% stenosis was shorter than those of normal segments, the time to systolic peak rotation and early diastolic peak rotation rate for segments with 75% - 99% stenosis improved compared with segments with 50% - 74% stenosis.Conclusions 25% -49% coronary stenosis could lead to alterations of peak myocardial systole and diastole motion time in radial,circumferential and rotation directions,and ≥75% coronary stenosis caused the all 4 directions motion delays. Also the time indices for segments with 75% - 99% coronary stenosis showed some improvement in the comparison with segments with 50% -74% coronary stenosis.

Objective To study the protective effects of the coronary collateral circulation to ischemic myocardial regional function with coronary artery stenosis≥75%using 2D-strain technique.Methods Two-dimensional images of apical four-,two-chambers,long-axis views and LV short-axis views were obtained in 121 patients.According to the results of coronary angiogram,all the 121 patients were divided into 3 groups:group 1,with normal coronary artery;group 2,with stenosis≥75%in≥1 main coronary artery involved right coronary artery,left anterior descending branch and left circumflex branch and with the presence of coronary collateral circulation(CCC);group 3,with stenosis≥75% in≥1 main coronary artery but with no presence of CCC.The segments' systolic peak strain(Sps),strain rate (SRs),end systolic strain (Yes),early diastolic strain rate (SRe) and late diastolic strain rate (SRa) in longitudinal (L),radial (R),circumficial (C) directions and the rotation (Rot),rotation rate were analyzed with EchoPAC offline software.Results Compared with 1 group,LSps,LSes,LSRs,LSRa,RSRe,RSRa,CSRe,Rotsre,Rotsra of 2 group,LSps,LSes,LSRs,LSRa,LSRe,RSes,RSRs,RSRe,RSRa,CSps,CSes,CSRs,CSRe,CSRs,Rotps,Rotsrs,Rotsre,Rotsra of 3 group decreased significantly (P<0.05) ;compared with 2 group,LSps,LSes,LSRs,RSes,RSRs,CSps,CSes,CSRs,CSRa,Rotps,Rotsrs,Rotsre of 3 group decreased markedly (P<0.05).Conclusions Coronary collateral circulation provides a protection effection to ischemic myocardial regional function.2D-strain technique may serve as an efficient method to assess ischemic myocardial regional function.
