Objective To explore the value of intravascular ultrasound (IVUS) in diagnosing plaque disruption and arterial thrombosis. Methods Balloon-induced arterial wall injury and then feeding on a diet of 1% cholesterol for 8 weeks, 17 New Zealand white rabbits were established as atherosis(AS) models. At the end of the eighth week, the resulting plaques were incubated transluminally with recombinant adenovirus carrying a p53 transgene. After two weeks, the rabbits underwent pharmacological triggering with Russell viper venom(RVV) and histamine. IVUS was used before and after the rabbits were triggered pharmacologically. The value of IVUS in diagnosing plaque disruption and arterial thrombosis was derived by comparing the results of IVUS with pathologic findings. Results IVUS found that plaques disruption and thrombosis occurred in 12 of the 14 rabbits, totally 18 thrombi. A histologically confirmed plaque disruption and intraluminal thrombus were visualized in 12 of the 14 rabbits, totally 20 thrombi. By comparing the results of IVUS with histopathology, the true positive rate was 85% and false negative rate was 15%, the sensitivity and specificity of IVUS were respectively 85% and 67%. IVUS data correlated well with the histopathology regarding thrombus length(r=(0.73),P(0.05)). Conclusions IVUS can detect plaque disruption and arterial thrombosis.

OBJECTIVETo compare the differences in the clinical efficacy between Guan's quadruple therapy for kidney disease and the conventional western medication in the treatment of chronic renal failure (CRF).
METHODSEighty patients were randomized into an observation group and a control group, 40 cases in each one. In the observation group, besides the physician instruction of low protein and low phosphorus diet in the two groups, Guan's quadruple therapy for kidney disease was applied, including (1) the isolated herbal moxibustion at Shenshu (BL 23) and Pishu (BL 21), or Ganshu (BL 18), Zusanli (ST 36) and Guanyuan (CV 4) alternatively; (2) acupuncture at the auricular points such as shen (CO10), jiaogan (AHR6), Shenshangxian (TG2), fei (CO14) and pizhixia (AT4); (3) injection of mixture of astragalus injection and lidocaine injection at Feishu (BL 13), Pishu (BL 21), Shenshu (BL 23) and the others; (4) modified Shenshuai Yihao decoction according to syndrome differentiation. In the control group, the conventional western medication was used. After 6 months of treatment, the differences were observed between the two groups in the clinical efficacy, serum creatinine (Scr), blood urea nitrogen (BUN), endogenous creatinine (Ccr) and 24 h urine protein quantitation (UPQ).
RESULTSIn the observation group, after treatment, the remarkably effective rate was 50.0% (20/40) and the total effective rate was 82.5% (33/40), which were superior to 25.0% (10/40) and 45.0% (18/40) in the control group (both P < 0.05). After treatment, the levels of Scr, BUN and 24 h UPQ were reduced apparently (all P < 0.05), and the level of Ccr was increased apparently in the two groups (both P < 0.05). After treatment, the levels of Scr, BUN and 24 h UPQ were reduced much more apparently in the observation group as compared with those in the control group (all P < 0.05).
CONCLUSIONGuan's quadruple therapy for kidney disease achieves a better efficacy on CRF compared with the conventional western medication. This therapy improves renal functions, relieves clinical symptoms and physical signs and benefits the life quality of patients.

OBJECTIVETo study the therapeutic effect of Shenle capsule (SLC) in treating mesangial proliferating glomerulonephritis (MsPGN) and to explore its therapeutic mechanism and clinical indication.
METHODSAdopting case control method, taking angiotensin-converting enzyme inhibitor (benazepril) as control agent, the 142 cases of MsPGN were randomly divided into 3 groups, treated with SLC (Group A, n = 36), SLC plus benazepril (Group B, n = 68) and benazepril alone (Group C, n = 38) respectively. Changes of fibrinogen, lipids, renal function and urinary protein were observed.
RESULTSThe total effective rate in Group A was higher than that in Group C, but with insignificant difference. The total effective rate in Group B after 3 courses of treatment was 89.74%, which was higher than that in Group C and Group A (P < 0.05). Levels of cholesterol (CH), triglyceride (TG), serum creatinine, fibrinogen and urinary protein (UP) were significantly lowered in Group A after treatment, with the levels of CH, TG and UP lower than those in Group C, while CH, TG and fibrinogen were unchanged in Group C after treatment.
CONCLUSIONSLC is superior in higher efficacy and less side-effects in treating MsPGN, its effect is related with the degree of kidney pathological changes, it is more effective in treating patients with mild pathological change than in those with severe change. The outcome of combined use of SLC and angiotensin-converting enzyme inhibitor would be better.

Objective Neonatal lupus erythematosus (NLE) is an uncommon" passive autoimmune disease, which is associated with transplacental passage of maternal antibodies. It is often misdiagnosed as intrauterine infection or sepsis. The main purpose of this retrospective study was to summarize its clinical manifestations related with pathogenesis. Methods Data of all the NLE neonates, including clinical manifestations, immunochemical evidence of serum antinuclear antibodies (ANA), antibody to Ro/Sjogren's syndrome A ( anti-Ro/SSA), antibody to La/Sjogren' s syndrome B (anti-La/SSB) and anti-dsDNA antibodies in both infants and mothers, and images from head ultrasound and CT scans were analyzed. Follow-up was performed until one and half years of age or when all the clinical abnormalities had been resolved. Results Totally 8 cases (3 males and 5 females ) seen between September 2003 and February 2006 met the diagnostic criteria of NLE, in whom 4 were small for gestational age and one was born prematurely. Mean gestational age was (38.1 ± 1.9 ) weeks, mean birth weight (2605 ± 420) g, mean admission age (22.4 ± 27.7 ) days (2 hours-72 days) and mean age of onset (9.4 ± 12. 1)days (0-28 days). The common clinical manifestations included cutaneous lupus lesions (8 infants ), neural system abnormalities (2 infants ) and congenital heart block (2 infants). Annular, erythematous or desquamative lesions were seen in skin and all disappeared before 6 months of age. One patient presented with third degree atrio-ventricular block and was delivered by cesarean section because of " fetal distress" He did not recover by the end of one and half years follow-up. One infant was hypotonic with delayed neuro-motor development initially and during follow-up with both abnormal neonatal behavioral neurological assessment (NBNA) and imaging findings. Brain CT scan showed generalized low density involving periventricular and deep white matter at one week of age. At the age of one and a half years, he presented with normal mental development index determined by Child Development Center of China (CDCC) infant intelligence mensuration. Other abnormal clinical findings such as hepatosplenomegaly, anemia, thrombocytopenia, cholestasis and elevated liver enzyme activities were all resolved before 6 months of age. Only 3 mothers of the NLE infants were diagnosed as systemic lupus erythematosus (SLE) before parturition and only one received partial therapy. At least anti-Ro/SSA antibody or anti-La/SSB antibody or ANA was found in the affected patients. Seven cases had circulating anti-Ro and/or anti-La antibodies in the mothers and in the newborns, while ANA was positive in seven newborns and in all mothers. All the clinical symptoms disappeared before 18 months ot age except for congenital heart block. No special intervention was applied. Conclusions Serum auto-antibodies should be investigated to rule out NLE when a newborn infant has congenital heart block or rashes or thrombocytopenia, although there is no maternal history of SLE. Central nervous system abnormalities in NLE are likely to be a transient phenomenon and whether it will cause long-term sequelae is uncertain.

Squalene synthase (SQS) is a key enzyme in plant terpenoid biosynthetic pathway. This study focused on cloning and analysis of Huperzia serrata SQS (HsSQS1) gene. After searching the transcriptome dataset of H serrata, one unique sequence encoding SQS was discovered. The primers were designed according to the transcript sequence of HsSQS1 from the H. serrata transcriptome dataset. The open reading frame of HsSQS1 was cloned using RT-PCR strategy. The bioinformatic analysis of this gene and its corresponding protein were performed. The cDNA (named as HsSQS1) contains a 1263 bp open reading frame and encodes a predicted protein of 420 amino acids. The GenBank accession number for this gene is JQ004938. HsSQS1 contains two transmembrane regions, without signal peptide. The conserved domain of squalene synthase was presented in HsSQS1. HsSQS1 was more abundant in H. serrata root than in leaf and stem. This study cloned and analyzed squalene synthase gene from H. serrata for the first time. The result will provide a foundation for exploring the mechanism ofterpenoid biosynthesis in H. serrata plants.
