1.Clinical and Pathological Manifestation of Henoch-Schonlein Purpura Nephritis in Children

Objective To study the relationship of clinical manifestation and pathological changes and prognosis in Henoch-Schonlein purpura nephritis(HSPN)in children.Methods Clinical and pathological characteristics of 42 children with HSPN were analysed.Among them,40 children were detected of angiotensin-convertion enzyme(ACE)gene and had been followed up.Results Among them,there were 9 cases of level Ⅰof pathological types,21 cases of level Ⅱ,12 cases of level Ⅲ,but no cases of level Ⅳ.Ⅰand Ⅱ level were found in those cases of clinical manifestation with solitary hematuria and albuminuria.Pathological grades were Ⅰ,Ⅱ and Ⅲ levels in the cases of hematuria and albuminuria.Pathological types of nephrotic syndrome(NS)were Ⅱ and Ⅲ level,which were of more gross hematuria than those of other grades.ACE gene DD had serious pathological damnification.Conclusions Change of pathology cannot only be anticipated by clinical manifestation of HSPN.But if pathological damnification gets more serious,the albuminuria gets more serious.Gross hematuria and albuminuria can serve as indicators of biopsy.NS of ACE DD type have serious pathological damnification.Children with HSPN has favourable prognosis in the future.

3.Effect of H2O2,extracellular matrix and out segment of photoreceptor on expression of focal adhesion kinase in RPE cell

Background The underlying mechanism of choroidal neovascularization(CNV) is multifactorial and complex.Focal adhesion kinase(FAK) plays a crucial role in controlling essential cellular processes and influencing distinct steps of the angiogenic response.But to our knowledge,seldom study on the effect of FAK on CNV formation has been reported previously.Objective In this study,the effect of several CNV risk factors on the expression of FAK in cultured retinal pigment epithelium(RPE) cells was investigated to illuminate effect of FAK on CNV.Methods Human RPE cells were isolated from donor eyes and exposed to H2O2,swallow of outer segment of photoreceptors(POS) and extracellular matrix(ECM) separately with the treating as follows:RPE cells were co-cultured with 10,20,50 and 100μmol/L H2O2 for 20 days;POS(1×106/ml) were co-cultivated with RPE cells for 20 days(setting control group,POS group,hypoxia group with 200μmol/L CoCl2,and POS+hyoxia group);RPE cells were cultured on the plates coated with 100mg/L fibronectin(FN),laminin(LN) or collagen typeⅠfor 30minutes or 1 hour.The expression of FAK and pFAK in RPE cells were examined by Western blot analysis.Results FAK was highly expressed in the 20μmol/L and 50μmol/L H2O2 groups compared with control group(P<0.01);while he expression level of pFAK was reduced after treated with H2O2 in comparison with the control group(P<0.01).After cultured with POS for 20 days,the undigested lysosome could be observed in RPE cells.The expressions of FAK and pFAK in RPE cells were not significantly changed between control group and POS groups(P>0.05),but those in hypoxia group were significantly up-regulated in comparison with control group(P<0.01).Compared with the hypoxia group,the expression amount of pFAK was elevated in POS+hyoxia group(P<0.01).In comparison with control group,the increased pFAK expression was seen in FN,LN and collagen typeⅠtreating for 1-hour groups(P<0.05,P<0.01).Conclusion FAK pathway participates in several CNV-initiated signaling,such as H2O2,POS and ECM,in cultured RPE cells.It is reasonable to believe that FAK potentially plays an important role in CNV-dependent disorder.
4.Association between autism spectrum disorder and epilepsy in children.

OBJECTIVETo examine the association between autism spectrum disorder (ASD) and epilepsy in children.
METHODSA total of 190 children with ASD were enrolled. A self-designed questionnaire, Childhood Autism Rating Scale, and Autism Behavior Checklist were used to determine the association between ASD and epilepsy.
RESULTSAmong the 190 children with ASD, 20 (10.5%) had epileptic seizures and 12 (6.3%) were diagnosed with epilepsy. The rates of abnormal physical development and hearing disorders before the age of one year were significantly higher in ASD children with epileptic seizures than in those without epileptic seizures (P<0.05). The ASD children diagnosed with epilepsy and those receiving epilepsy treatment had a significantly increased rate of abnormal physical development before the age of one year (P<0.05). The ASD children with epileptic seizures had poorer sensory responses and behavioral competencies than those without epileptic seizures (P<0.05). Epilepsy treatment have a positive effect on behavioral competencies in ASD children (P<0.05).
CONCLUSIONSThere is a significant association between ASD and epilepsy in children. The possibility of the comorbidity between ASD and epilepsy may be assessed according to the status of growth and development before the age of one year, sensory responses and behavioral competencies, and the presence or absence of epileptic seizures.

5.A rare pulmonary benign bi-phasic tumor: a case report of pulmonary adenofibroma and literature review

Pulmonary adenofibroma is an extremely rare benign primary tumor of the lung,with characteristic bi-phasic differential pattern.They are usually sub-pleural solid pulmonary nodules with clear margins.The tumor is composed of glands and peri-gland stroma.The glands are often quite simple,forming long and narrow tubules,with uniformly monolayer lining cells.Combined papillary or phyllodes structures were reported in some cases.The stromal cells are spindle-shaped and look mild,mixed with the collagen bands.Up till now,only a few cases of pulmonary adenofibroma have been reported all over the world.And because of the limited recognition,this tumor is easily misinterpreted as malignancy in frozen section or biopsy specimens.We reported a new case of pulmonary adenofibroma.The mass peripherally located in the left lobe of the lung,found by chance in a 74-year-old woman.The patient underwent a wedge resection of the left lung by the thoracoscope,because of the slowly gradual enlargement of the mass annually.An oval grayish-white nodule was sub-pleural located in the specimen,with solid and rubbery texture,but without a distinct capsule.Two distinct components of simple glands and mild spindle cell stroma were found to mix together uniformly under the microscope.Collagen bands of various widths evenly surrounded each stromal cell.A few small coarse papillae or phyllodes structures were randomly distributed in some area.The immunohistochemical staining pattern of the glandular cells was accordant with type Ⅱ alveolar epithelium.Stromal cells were positive with CD34,B-cell lymphoma-2 (Bcl-2),CD99and estrogen receptor (ER),while S-100,smooth muscle actin (SMA) and all the mesothelium markers were negative.The patient was disease free after the surgery,although the follow-up time was only one year.Besides the new case above,we also reviewed all the reported cases,and tentatively discussed the probable histological origin of pulmonary adenofibroma.
6.Advancement of colloidal gold chromatographic technique in screening of ochratoxin A.

Ochratoxin A (OTA) is a toxic secondary metabolite mainly produced by Aspergillus and Penicillium species, existing in a variety of foodstuffs and Chinese medicines. OTA is difficult to be detected in practice because of the characteristics such as trace amounts, toxicity, existing in complex matrices. In the numerous detection technologies, colloidal gold chromatographic techniques are highly sensitive, specific, cost-effective and user-friendly, and are being used increasingly for OTA screening. Recently, with the development of aptamer technology and its application in chromatographic technique, a newly colloidal gold aptamer chromatographic technique has been developed. This review elaborates the structures and principles of both traditional and newly colloidal gold chromatographic techniques, focuses on newly colloidal gold aptamer chromatographic technique, summarizes and compares their use in rapid detection of OTA. Finally, in order to provide a reference for better research of related work, the development trends of this novel technique are prospected.

7.Clinical significance and expression of the inflammatory cytokines (IL-1, IL-2, IL-6 and IL-10) in blood serum of the patients after total hip replacement.

OBJECTIVETo evaluate the changes in the expressions of the inflammatory cytokines (IL-1, IL-2, IL-6 and IL-10) in blood serum of the patients after total hip replacement (THR).
METHODSFrom February 2010 to June 2010, thirty patients undergone primary total hip arthroplasty in one side including 18 male and 12 female with the mean age of (58.4 +/- 6.6) years (ranged from 52 to 70 years). Twenty patients were Ficat II type,and 10 patients were Ficat III type. The contents of IL-1, IL-2, IL-6 and IL-10 in blood serum were detected on the day before operation and 1 to 3 days after operation respectively by ELISA. At the same time, the content of C reactive protein was also detected, and the results were statistically analyzed.
RESULTSThe contents of IL-1, IL-6 and IL-10 in the serum of patients at the first day after THR were significantly higher than those before operation (t = 2.62, 3.51, 2.21, P < 0.05), and reached the peak at 1 to 3 days after operations, then subsequently decreased. However, the content of IL-2 in the serum of patients after operation was obvious lower (t = 2.16, P < 0.05),and recovered to normal level gradually.
CONCLUSIONIt is the key time of treatment at 1 to 3 days after THR. Monitoring IL-1, IL-2, IL-6 and IL-10 after operation in time, and monitoring life sign at the same time could sensitively observe the effect of treatment and forecast the trend of diseases in patients.

8.Mechanism of Wenwei Tiaochang Granule on Rats with Diarrhea-Predominant Irritable Bowel Syndrome

OBJECTIVE To explore the effects and mechanisms of Wenwei Tiaochang Granule on diarrhea-predominant irri-table bowel syndrome rats.METHODS 60 SD rats,were randomly divided into control group,model group,positive drug group,and low,middle and high dose groups of Wenwei Tiaochang Granule.In order to make the rats get diarrhea-predomi-nant irritable bowel syndrome,the rats were administrated with senna,bound limbs and clipped tails for 14 d.After 14 d of ad-ministration with positive drug and Wenwei Tiaochang Granule,the blood,jejunum,colon,thymus and spleen were collected. Motilin assay kit was used to detect motilin (MTL) content in plasma and jejunum.Use the weight of thymus and spleen to calculate their organ index.Calcitonin gene-related peptide(CGRP) and substance P (SP) mRNA level and protein expression level was detected by RT-PCR and Western-blot in colonic tissues.RESULTS Compared with the control group,the growth rate and food intake increased slower in model group,at the same time,the thymus index,spleen index and MTL content in plasma and jejunum decreased significantly,while the CGRP and SP mRNA and protein expression level of colon increased sig-nificantly (P <0.05~0.01).Compared with the model group ,the middle and high dose groups of Wenwei Tiaochang Granule could significantly increase the thymus index,spleen index and MTL content in plasma and jejunum,and decrease CGRP and SP mRNA and protein expression level of colon(P <0.05~0.01).CONCLUSION By regulating the content of MTL and the expression level of CGRP and SP mRNA and protein,Wenwei Tiaochang Granule can be used to treat diarrhea-predominant ir-ritable bowel syndrome.
9.Comparison of different pathways of early enteral nutrition after liver transplantation recipients

Objective To explore the choice of pathway of early enteral nutrition after orthotopic liver transplantation.Methods 96 liver transplantation recipients were separated into nasogastric tube group(42 cases) and gastrostomy-jejunal tube group (54 cases) in our center. The preoperative general condition, nutritional status and basic diseases in two groups were collected. The complications and treatment results of two groups were compared.Results Compared with nasogastric tube group, the start time of enteral nutrition was significantly advanced[(2.33±0.25) vs (0.52±0.12), t=46.770,P<0.01]; the effective use of time of the feeding tube was markedly extended [(8.7±3.9) vs (28.6±17.9),t=7.071, P<0.01]; the supplementary from the third to the 7th day were observably increased in the gastrostomy-jejunal tube group(P<0.01). In Meld score above 15, incidence of gastric retention[5.1%(2/39) vs 25%(8/32), χ2=5.736,P=0.035] and the feeding tube accidently pulled out[0 vs 12.5%(4/32),χ2=5.166,P=0.037], postoperative hospital stay and hospital costs in gastrostomy-jejunal tube group were significantly lower than that of nasogastric tube group(P<0.05). The incidence of diarrhea and tube blockage was no significant difference(P>0.05). Meld score in different circumstances, between the two groups of pulmonary infection, abdominal infection, acute renal failure and acute rejection were not significantly different(P>0.05).Conclusions The use of percutaneous gastrostomy-jejunal tube has the advantages of early postoperative enteral nutrition in patients with Meld score above 15 ,it is worth to apply.
10.Bibliometric analysis of the adverse drug reactions of rivaroxaban

Objective To analyze the general law of adverse drug reac-tions (ADRs) in rivaroxaban.Methods Based on the domestic litera-ture retrieved from China hospital knowledge database ( CNKD), VIP network,VIP, Wanfang database ,and PubMed from 1 January 2008 to 11 January 2018, ADRs caused by rivaroxaban were summarized and statis-tically analyzed.Results The study collected 78 literature and 40589 cases of the ADRs of rivaroxaban.Rivaroxaban caused various complicat-ed ADRs and clinical manifestations , which on the top of lists were re-spiratory system disorders and gastrointestinal disorders .Conclusion Adverse drug reactions monitoring of rivaroxaban should be strengthened to ensure its safety and medication.
