Background Refractive lens exchange is one of corrective surgeries for high myopic eyes and is concerned in clinic recently. Its clinical value is worthy of consideration. Objective This study was to investigate the efficacy and safety of refractive lens exchange for high myopic eyes. Methods Phacoemulsification and intraocular lens implantation was performed on 124 eyes of 65 patients with high myopia. The mean age of these patients was 51. 4±8. 57 years old,and the preoperative corrected visual acuity was 4. 11±0. 51. The mean spherical equivalent was ( -20. 17±5. 34) D. The mean axial length was (31. 33±2. 08) mm and intraocular lens power 2. 88 D. The follow-up time was 31 months. The uncorrective visual acuity, best corrective visual acuity, the spherical equivalent lens and complications were observed after operation. Written informed consent was obtained prior to the surgery. Results The uncorrective visual acuity improved after the operation in all the eyes. The uncorrective visual acuity was ≥0.5 in 15 eyes(12% ). The best corrected visual acuity improved in 114 eyes (92% ) following the surgery and that of 64 eyes (51. 6% ) was 2s 0. 5. The mean postoperative spherical equivalent was ( -2. 57 ± 1. 76 ) D in the entire follow-up duration. Posterior capsular opacification was found in 58 eyes (46. 7% ) and received laser capsulotomy. Retinal detachment occurred in 4 eyes throughout the follow-up period. Conclusion Refractive lens exchange is an effective and safe method for high myopic eyes. But preoperative fundus examination and long-term postoperative follow-up should be carried out to prevent the complications.

OBJECTIVETo systematically optimize the cultivation conditions of adventitious roots of Pseudostellaria heterophylla.

METHODTissue cultivation technology and ultraviolet spectrophotometry were adopted to observe the effect of inoculum volume, sucrose concentration, inorganic salt concentration, number of cultivation days, gradual scale-up cultivation and bubble different angles of bioreactor on the growth of adventitious roots of P. heterophylla, and determine the content of constituents such as saponin, polysaccharide and amino acid.

RESULTThe propagation multiple of adventitious roots reached the maximum when the inoculum was 6 g in a 1 L culture shake flask. With the increase in sucrose concentration, the dry weight propagation multiples of adventitious roots followed an up and down trend. The inorganic salt concentration in a cultivation dish had a greater effect on the growth of adventitious roots, particularly 3/4 MS was the most favorable for the growth of adventitious roots. The growth curve of P. heterophylla was "S", with the biomass reaching the maximum at the 28th day.

CONCLUSIONThe inoculum volume, sucrose concentration, inorganic salt concentration, gradual scale-up cultivation and angles of bubble bioreactor had a significant effect on the growth of adventitious roots of P. heterophylla. The contents of saponin and amino acid in adventitious roots were higher than that in cultivated P. heterophylla, whereas the polysaccharide content were lower than that in cultivated P. heterophylla.

Phytantriol (PT), ethanol (ET) and water were used to prepare in situ cubic liquid crystal (ISV2). The pseudo-ternary phase diagram of PT-ET-water was constructed and isotropic solution formulations were chosen for further optimization. The physicochemical properties of isotropic solution formulations were evaluated to optimize the composition of ISV2. In situ hexagonal liquid crystals (ISH2) were prepared based on the composition of ISV2 with the addition of vitamin E acetate (VitEA) and the amount of VitEA was optimized by in vitro release behavior. The phase structures of liquid crystalline gels formed by ISV2 and ISH2 in excess water were confirmed by crossed polarized light microscopy and small angle X-ray scattering, respectively. Rheological properties of ISV2 and ISH2 were studied by a DHR-2 rheometer. In vitro drug release studies were conducted by using a dialysis membrane diffusion method. Pharmacokinetics was investigated by determination of sinomenine hydrochloride (SMH) concentration in synovial membrane after intra-articular injection of SMH-loaded ISH2 in adjuvant-induced arthritis rats. The optimal ISV2 (PT/ET/water, 64 : 16 : 20, w/w/w) loaded with 6 mg x g(-1) of SMH showed a suitable pH, injectable and formed a cubic liquid crystalline gel in situ with minimum water absorption in the shortest time. The optimal ISV2 was able to sustain the drug release for 144 h. The optimal ISH2 system was prepared by addition of 5% VitEA into PT in the optimal ISV2 system. This ISH2 (PT/VitEA/ET/water, 60.8 : 3.2 : 16 : 20, w/w/w/w) was an injectable isotropic solution with suitable pH. The new ISH2 was able to sustain the drug release for more than 240 h. Local pharmacokinetics study indicated that the retention time and AUC(0-∞) of ISH2 group were increased significantly compared with that of SMH solution group and the AUC(0-∞) of ISH2 group was 6.01 times higher than that of SMH solution group. The developed ISH2 was suitable for intra-articular injection that may apply to patients in the treatment of rheumatoid arthritis.

Background The aspheric intraocular lenses(IOLs)can reduce ocular spherical aberration to some degree.However,the clinical effect depends more on the IOL proper alignment.It becomes more important to study the IOL position in eye,Objective This study was to analyze the position alteration of IOL after phacoemulsification combined with implantation of one-piece soft IOLs.Methods In this prospective control study,80 eyes of 40 patients with age-related cataract were enrolled.The phacoemulsification with IOL implantation was performed in all the eyes.Decentration and tilt of IOL in the nasal superior,superior temporal,inferior temporal and nasal inferior quadrants(the intersection point of the system optical axis and the IOL maximum cross plane were regarded as the ordinate origin)were measured by rotating Scheimpflug camera(Pentacam Oculus)in 3 months postoperatively under the mydriasis condition.Written informed consent was obtained from each subject prior to this trial.Results In the right eye group,the IOL decentered toward temporal in 26 eyes(65％)and infratemporal in 16 eyes(40％).IOLs tilted temporally in the horizontal plane in 37 eyes(92.5％)and tilted inferiorly in the vertical plane in 34 eyes(85.0％).In the left eye group,IOLs decentered temporally 33 eyes(82.5％)and 20 IOLs (50％)infratemporally,IOLs tilted temporally in the horizontal plane in 37 eye(92.5％)and 36 IOLs(90％)tilted inferiorly in the vertical plane.There was no statistical difference for the intercomparsion of horizontal/vertical decentration in various quadrant in the right eye(F =0.221,0.792,P＞0.05).The obvious elevated horizontal decentration was found in the supertemporal and infratemporal quadrants compared with supernasal quadrant in the left eyes but there was no significant difference in the vertical decentration among 3 quadrants(F=0.576,P＞0.05).Decentrations were positively correlated with the tilt in both horizontal and vertical plane(right eye horizontal plane:r=0.374,P=0.002;right eye vertical plane:r=0.402,P=0.001 ;left eye horizontal plane:r=0.377,P=0.002；left eye vertical plane:r=0.347,P=0.002).Conclusions The one-piece soft IOLs(Adapt AO)decenter toward temporal mostly in 3 months after surgery,especially infratemporally in the eye.And the optical axis of the IOL tilt toward infratemperol mostly in both right and left eyes.The decentration and tilt are consisted in the corresponding direction between the right and left eyes.The position of the IOLs showed mirror symmetry between right and left eyes.The IOLs decentration show the positively correlation to tilt whatever in horizontal and vertical plane.

Objective To explore the diagnosis value of 320-row detector dynamic volume CT in complex congenital heart disease (CCHD)with double outlet right ventricle(DORV).Methods Seventy-eight patients who proveed DORV by surgery in Xinqiao Hospital of Third Military Medical University were reviewed.Thirty-six patients of group A performed 320-row detector dynamic volume CT by using segmental analysis,and were compared with the group B(42 Cases) performed conventional 64-slice CT respectively.Results In 36 cases of DORV confirmed by surgery in group A,MSCT provided accurate qualitative diagnosis in all cases.The accuracy rate of diagnosis of the group B was 90.7％.There was no significant differences compared with the group A (P＞0.05).There were ventricular septal defect in all the 78 cases,pulmonary stenosis in 56 cases,atrial septal defect in 34 cases,pulmonary hypertension in 21 cases,patent ductus arteriosus in 16 cases,coarctation of aorta in 9 cases.Conclusion The 320-row detector dynamic volume CT has important diagnostic value for DORV of the anatomical diagnosis.

Objective To evaluate the effect of progressive muscle relaxation combined with health education on the psychological state of the maintained hemodialysis patients and provide basis for the psychological intervention for them. Methods Forty-two MHD patients were randomly assigned to the intervention group and control group. Patients in the control group received the conventional nursing, and patients in the intervention group received the conventional therapy combined with nursing intervention for two times. Results After the first intervention, the various psychological testing indicators of intervention group were obviously lower than those of control group and there was statistically significant difference between two groups(P ＜ 0. 05 ). The differences of index of physiological chemistry between two groups had no statistical significance (P ＞ 0. 05 ). After the second intervention, the various psychological testing indicators of intervention group were further lower than those of control group and there was statistical significant difference ( P ＜ 0. 01 or P ＜ 0. 05 ). The Hb and Ca2 + of the intervention group were remarkably higher than control group,while PO43+ and K+ of intervention group were significantly lower than control group. Both differences had statistical significance(P ＜ 0. 05 ). Conclusions Progressive muscle relaxation combined with health education can greatly improve the psychological and physiological status of MHD patients.

OBJECTIVETo clone SABATH methyltransferase (rLjSABATHMT) gene in Lonicera japonica var. chinensis, and compare the gene expression and intron sequence of SABATH methyltransferase orthologous in L. japonica with L. japonica var. chinensis. It provide a basis for gene regulate the formation of L. japonica floral scents.

METHODThe cDNA and genome sequences of LjSABATHMT from L. japonica var. chinensis were cloned according to the gene fragments in cDNA library. The LjSABATHMT protein was characterized by bioinformatics analysis. SABATH family phylogenetic tree were built by MEGA 5.0. The transcripted level of SABATHMT orthologous were analyzed in different organs and different flower periods of L. japonica and L. japonica var. chinensis using RT-PCR analysis. Intron sequences of SABATHMT orthologous were also analyzied.

RESULTThe cDNA of LjSABATHMT was 1 251 bp, had a complete coding frame with 365 amino acids. The protein had the conservative SABATHMT domain, and phylogenetic tree showed that it may be a salicylic acid/benzoic acid methyltransferase. Higher expression of SABATH methyltransferase orthologous was found in flower. The intron sequence of L. japonica and L. japonica var. chinensis had rich polymorphism, and two SNP are unique genotype of L. japonica var. chinensis. The motif elements in two orthologous genes were significant differences.

CONCLUSIONThe intron difference of SABATH methyltransferase orthologous could be inducing to difference of gene expression between L. japonica and L. japonica var. chinensis. These results will provide important base on regulating active compounds of L. japonica.

Objective To explore the differences of TCM constitutions and lifestyles among the subjects with positive tumor marker screening and those with normal tumor marker screening. Methods Totally 245 subjects with positive tumor marker screening from September 2014 to December 2017 in Weihai area were selected as the observation group and 300 subjects with normal tumor marker screening were selected as the control group. Field questionnaire was used to investigate TCM constitutions and lifestyles. Results In the observation group, the ratio of constitution of yin-yang harmony was significantly lower than the control group, with statistical significance (χ2=207.483, P＜0.001). The ratios of constitution of phlegm-dampness, constitution of blood stasis, constitution of qi deficiency and constitution of qi stagnation were significantly higher than the control group, with statistical significance (P＜0.05). There was statistical significance between the 2 groups in irregular rest, heavy drinking, regular smoking, lack of exercise, irregular diet and partial eclipse (P＜0.05). CEA, a positive indicator of tumor markers in the observation group, was commonly found in constitution of phlegm-dampness (40.43％) and constitution of qi deficiency (22.70％); CA199 was commonly found in constitution of qi stagnation (38.71％) and constitution of qi deficiency (24.19％); AFP was commonly found in constitution of blood stasis (38.46％) and constitution of qi deficiency (23.08％); CA125 was commonly found in constitution of blood stasis (33.33％) and constitution of qi stagnation (19.05％); CYFRA21-1 was commonly found in constitution of phlegm-dampness (54.55％) and constitution of qi deficiency (22.73％); PSA was commonly found in constitution of phlegm-dampness (61.52％) and constitution of damp-heat (23.08％). Conclusion Effectively controling the distribution of TCM constitutions of subjects with positive tumor marker screening in Weihai area and understanding the influence of lifestyles on tumor markers can provide guidance for clinical improvement of patients' biased constitutions and reduction of positive indicators of tumor markers.

Objective To study knowledge of healthy volunteers in-volved in Phase ⅠClinical Studies to clinical researches and related fac-tors.Methods The public awareness of research for therapeutic ad-vancements through knowledge and empowerment (PARTAKE) question-naires, designed by Duke University , were administered to 100 partici-pants in phaseⅠclinical study in Peking university third hospital.Re-sults The following perceptions were reported: research benefited our society (96.0%) , human experiment was necessary for developing new therapeutic method (91.0%), confidentiality and voluntary participating were adequately approved ( 90.0%) , all the undesirable effects were caused by the research ( 18.0%) , and volunteers would be paid for any undesirable adverse reactions completely ( 81.0%).Conclusion Re-sults suggest that the healthy volunteers have a good command of clinical research generally.They are aware of protections to participants , but they do not have a full understanding of compensation for damages .

OBJECTIVETo investigate the effect of Shuganlipi decoction on Th1/Th2 cytokines, liver function and HBV replication in patients with chronic hepatitis B (CHB).

METHODSEighty-six confirmed CHB cases were randomly divided into control group (n=42) and experimental group (n=44) for treatment with routine western medication and additional treatment with Shuganlipi decoction, respectively. The production of IFN-γ, IL-2, IL-6, IL-10 and liver function, HBV DNA, and HBeAg were detected in all the patients.

RESULTSThe total response rate to the treatment was significantly higher in the experimental group than in the control group (78.13% vs 57.14%, P<0.01). ALT, AST, TBIL and ALB were all improved obviously in the two groups after the treatments (P<0.01). In terms of ALT and ALB, the experimental group showed more obvious improvement than the control group(P<0.05). The treatments also resulted in significant increases of IFN-γ and IL-2 levels and reductions of IL-6 and IL-10 levels in the two groups (P<0.01).

CONCLUSIONShuganlipi decoction can improve the liver function and activity of Th1/Th2 cytokines to promote the clearance of liver cell HBV infection.
