1.Obstructive sleep apnea as an independent risk factor for stroke: possible mechanislms

Obstructive sleep apnea (OSA),the most common type of sleep disordered breathing was closely associated with the occurrence of ischemic stroke.It has been demonstrated that OSA is an independent risk factor for ischemic stroke.Its pathogenic mechanism includes atherosclerosis,coagulation abnormalities and cerebral hemodynamic changes caused by long-term periodic hypoxia hypoxemia and hypercapnia.The prognosis of ischemic stroke patients with OSA is poor.Therefore,it needs to be treated early to avoid further damage to brain tissue and reduce the mortality.
2.Uniform requirements for reporting Meta-analysis of randomized controlled trials: the QUOROM statement

The QUOROM statement means a set of quality standards of Meta analysis report. It comprises a checklist and a flowchart for reporting a Meta analysis. They are primarily used in writing, reviewing or evaluating reports of Meta analysis for simple two group, parallel RCTs. The statement is a continually evolving document. A few international medical journals have endorsed the QUOROM statement and require authors submitting manuscripts for publication to include all the information on the checklist and flowchart of QUOROM statement. Based on introducing the background, processes of forming QUOROM statement and reviewing the contents and requirements of QUOROM, we prepare this paper, hoping to improve the quality of Meta analysis report for randomized controlled clinical trials in our country. Some comments and suggestions are given at the same time.

The epidermal growth factor receptor (EGFR) is overexpressed in many epithelial cancers,and is commonly caused by EGFR gene amplification and gene mutations. The most frequently occurring variant,the type III mutation (EGFRvIII) ,is characterized by an inframe deletion of exons 2-7 of the coding sequence. It is expressed only in tumors and not found in normal tissues, and therefore represents an attractive therapeutic target. The tumor therapy methods targeted for EGFRvIII include immu-notherapy, ribozyme, RNA interference, etc.

OBJECTIVETo investigate the mechanism that the formulas for activating blood and resolving stasis can regulate hemopoietic stem cell to produce new blood.
METHODRats were established animal model of acute cerebral infarction by referencing Olivette' method. They were randomly divided into model group, the group of the high, middle, low dose of the formulas for activating blood and resolving stasis. Each group and then wasrandomly divided into subgroups by 1, 3, 7, 14, 28 d. Xuesaitong capsule was formulated into 20, 40, 60 g x L(-1) with normal saline. The rats were given gavage drugs once a day until the experient ended, and the model group was administrated by intragastrical perfusion of normal saline. ELISA was used to detect the expression of SCF in peripheral blood and bone marrow among different groups at different time points. Flow cytometry was used to observe the changes of CD117 in blood and bone marrow.
RESULTThe CD117+ HSC and SCF concentration in peripheral blood and bone marrow of model group were increasing during 1-14 d,there was a peak on the 14th day, then the expression was reducing. CD117+ HSC and SCF concentration rising trend in the group of the high, middle dose of the formulas for activating blood and resolving stasis was preceded model group (P < 0.05).
CONCLUSIONActivating blood and resolving stasis can regulate hemopoietic stem cell to produce new blood, and it is through the regulation of CD117+ HSC number to achieve the purpose.

5.Osteoporosis-related knowledge, attitudes,and behaviors of residents from a community in Shnnohai

Objective To understand and raise the level of osteopomsis-relied knowledge, attitudes and behaviors for prevention and treatment of osteoperosis(OP)in residents of a community. Methods A survey was performed with a questionnaires method targeting at the elderly residents in Changfeng community.The survey was conducted with a specifically designed questionnaire,followed by a second survey.during which health edueafion was stressed.Data were analyzed with SPSS 11.0 software. Results In 3524 residents.3367 responded positively.The ratio of male to female was 1:2.10.The average age of the respondents was 61.34 years.They demonstrated a low level of OP KAP.76.72% of them heard the term of OP,60.11% had a correct judgment of their own bone heahh,42.26% knew the cause of OP.and 37.75% understood the diagnostic method of OP.The level of OP KAP varied in different groups of targeted residents.Of all the questions.the understanding of the diagnosis ranked the lowest.2281 people participated in the second survey,which showed improvement in their OP KAP.The difference was statistically significant.1439 people auended the consultation.99.86% of them considered the activities necessary,96.92% expressed their satisfaction,and 98.54% planed tO take measures to improve their bone health.Conclusion The poor understanding of bone heaith status and the knowledge about prevention and treatment of OP is why it is widely spread in the modem society,which however Call be improved by education.
6.Biomechanical effects of osteoporosis vertebral augmentation on the adjacent intervertebral levels using a three-dimensional finite element analysis

Objective To investigate effects of volume,distribution and leakage of bone cement in ver- tebroplasty on the adjacent intervertebral bodies using an experimentally calibrated and anatomically accurate fi- nite-element model of elderly L4-L5 vertebral bodies.Methods Computed tomography(CT)scanning was done,at 1 mm intervals,on L4-L5 vertebral bodies of the lumbar spine of an old man cadaver that had no abnormal findings on rnentgenograms.The L4-L5 motion segment data were obtained from the CT scans to establish in computer a three-dimensional finite element model of L4-L5 functional spinal unit(FSU)with the software Ansys 7.0.Compressive load after virtual vertebroplasty on the damaged model,unipedicular and bipedieolar injections and leakage of cement into the intervertebral space,and the resulting endplate and disc stresses of the adjacent vertebral bodies were analyzed in various spatial distributions of the filling material and different loading conditions. An anatomically accurate finite-element model of elderly L4-L5 vertebral bodies was developed.Results The FSU study suggested that changes in stress and strain at adjacent levels were minimal.Furthermore,endplale and disc stresses of the adjacent vertebral bodies were not influenced by bone cement filling volumes or distributions except under bending,whereas asymmetric distribution and leakage of bone cement to the disk increased the stress of adjacent endplates.Conclusion Since asymmetric distribution and significant leakage of cement into the intervertebral space can increase the stresses of endplates of adjacent vertebral bodies and may lead to a fracture, symmetric placement of cement in operation should be achieved and leakage of cement avoided.
7.Retrospective Analysis of ADR Induced by Shuxuening Injection in Beijing Area during 2003-2013

OBJECTIVE:To investigate the occurrence of ADR induced by Shuxuening injection in Beijing area,and to pro-vide reference for rational drug use in clinic. METHODS:ADR induced by Shuxuening injection in Beijing area during 2003-2013 was classified statistically in respects of population,organs/systems involved in ADR,clinical manifestations,occurrence time, drug combination and prognosis. RESULTS:Of 1648 ADR cases,the incidence of ADR in the elderly patients was higher than young patients,and that of female patients (60.92%) was higher than that of male patients (39.14%). Multiple organs/systems were involved in Shuxuening injection-induced ADR,and top 3 were nervous system (422 cases,21.33%),skin and its appen-dants (411 cases,20.78%) and others (305 cases,15.42%). Clinical manifestations were skin rash,itching,dizziness and ner-vousness,palpitation,arrhythmia,etc. ADR mostly occurred within 5-30 min after medication. In ADR reports,there were a vari-ety of drugs combined with Shuxuening injection. Shuxuening injection combined with Alprostadil injection was most common(61 cases,accounting for 23.46% of drug combination). Among 1648 ADR cases,there were 622 cured cases and 1039 cases im-proved,4 cases were not improved,3 cases had sequelae and 1 case died. CONCLUSIONS:Clinical application of Shuxuening in-jection should strictly follow the indications of drug package inserts;differentiation of symptoms and signs is based on specific dos-age,solvent,route of administration,etc. It is necessary to strengthen the prescription check,and strengthen the observation after medication especially for the elderly and other special groups so as to reduce and avoid ADR.

Objective To construct and identify the RNAi eukaryotic vector of Skp2 gene and to observe its interfering effect on the growth of SPC-A-1 lung cancer cells.Methods The specific shRNA sequence was designed and synthesized according to the Skp2 cDNA sequence in GenBank.The sequence was cloned into plasmid pGenesil-1.Then recombinant vector was transfected into SPC-A-1 lung cancer cells by Lipofectamine 2000.The expressions of Skp2 mRNA were analyzed by RT-PCR and the levels of Skp2 protein were detected by Western blot.The cell growth suppression was analyzed by MTT assay.Distribution of cell cycle was assessed by flow cytometry.Results The sequence of template and specific siRNA was correct by sequence analysis.Obvious decrease was observed in the levels of Skp2 mRNA and Skp2 protein after Skp2 shRNA transfection(P
9.Analysis of patients with spinal cord injurythe following lower limb vein thrombus

ObjectiveTo study the features of lower limb vein thrombus patients after spinal cord injury and the effects of thrombolysis therapy.MethodsWe practiced ultrasonography on 207 patients with lower limb swelling after spinal cord injury. ResultsAmong the 207 patients, 66 had thrombus, the positive rate was 31%: acute thrombus 58 cases, subacute thrombus 1 cases and chronic thrombus 7 cases. Conclusions Ultrasonography provides an important basis for the qualitative diagnoses of lower limb vein thrombus. It has certain clinical guiding effects in choosing methods of treatment.
10.Transcatheter arterial chemoembolization combined with CT-guided percutaneous thermal ablation versus hepatectomy in the treatment of hepatocellular carcinoma

Introduction:Transcatheter arterial chemoembolization (TACE) plus thermal ablation has been widely used recently in the treatment of hepatocellular carcinoma (HCC). In this study, we aimed to compare results of the combination of TACE and percutaneous thermal ablation with those of hepatectomy in patients with HCC. Methods:The clinical data of 137 HCC patients who sequentially received TACE and computed tomography (CT)-guided percutaneous thermal ablation as an initial curative treatment (combination group) and 148 matched HCC patients who received hepatectomy (surgery group) between 2004 and 2011 were collected and analyzed. After TACE, multiphase contrast-enhanced CT was performed to identify the total number of tumors as well as lipiodol deposition in the liver. Survival was calculated by using the Kaplan-Meier method and compared by using the log-rank test. The prognostic factors were assessed with multivariate Cox proportional hazards regression analysis. Results:Of all 285 patients, 225 (79.0%) had cancerous lesions≤5 cm in diameter. In preoperative contrast-enhanced CT or magnetic resonance imaging, the number of tumors was 1–4 for each patient. The 1-, 3-, and 5-year overal survival rates were 95, 74%, and 67%in the combination group and 88, 66, and 47%in the surgery group, respectively (P=0.004);the corresponding recurrence-free survival rates for the two groups were 92, 69, and 61%and 75, 58, and 44%, respectively (P=0.001). In the multivariate analysis, treatment al ocation was an independent prognostic factor for survival. Only 60 patients in the combination group had sufficient imaging data, and 135 new lesions with lipiodol deposition were diagnosed as malignancies in 22 of 60 patients, whereas 20 new lesions were found in 11 of 148 patients in the surgery group. Conclusion:The combination of TACE and CT-guided percutaneous thermal ablation for HCC improves survival of HCC patients compared with hepatectomy.
