Objective To investigate the clinical significance of B-type natriuretic peptide(BNP) in the process of fluid resuscita-tion with different crystalloid-colloid ratio in the patients with severe acute pancreatitis (SAP) .Methods Clinical data of 85 SAP patients were analyzed retrospectively in the Emergency Center of Chongqing during January of 2010 to December of 2012 .Early goal-directed therapy(EGDT) was confirmed the end criterion of the end point of resuscitation .Low crystalloid-colloid ratio group (n=45) and high crystalloid-colloid ratio group(n=40) were divided according to crystalloid-colloid ratio (3∶1) as the borderline , BNP were observed at the time point of before fluid resuscitation ,and 1 ,2 ,3 days after resuscitation and the day before discharge . Results Different fluid resuscitation of crystalloid-colloid ratio could improve hemodynamics in patients ,BNP increased with the to-tal amount of resuscitation liquid ,were positively correlated with liquid volume .The total amount of fluid of the high crystalloid-colloid ratio group was significantly higher than that of the low crystalloid-colloid ratio group .Conclusion BNP could guide early fluid resuscitation and low crystalloid-colloid ratio should be adopted for SAP patients .

Abstract: Objective To investigate the prevalence and pathogenic characteristics of Yersinia enterocolitica infection in children with diarrhea under 5 years of age in western Yunnan, and to provide a basis for the prevention and treatment of infectious diarrhea in children. Methods Feces were collected from under five-year-old children with diarrhea in the First Affiliated Hospital of Dali University from 2020 to 2021. Clinical information of the cases was also collected. Yersinia enterocolitica was isolated from the samples after cold enrichment on selective culture plates, and the pathogenic characteristics of Yersinia enterocolitica were analyzed by biological type and serotype and virulence gene detection. Results A total of 397 feces were collected. Seven strains of Yersinia enterocolitica were isolated in three samples, and the prevalence of Yersinia enterocolitica infection was 0.76% (3/397). Among the three positive samples, two Yersinia frederiksenii or Yersinia intermedia were isolated in specimen No. 212 , and five Yersinia enterocolitica were detected in specimens No. 24 and 226. Two Yersinia enterocolitica isolated from one sample were biological type 1A, and the virulence gene test results were ail-/ystA-/ ystB+ /yadA-/virF-, which were non-pathogenic Yersinia enterocolitica. Three Yersinia enterocolitica isolated from the other sample were biological type 3, serotype O∶3 (rfbc+), and virulence gene detection results were ail+/ystA+/ystB-/yadA+ /virF+, which were pathogenic Yersinia enterocolitica. While pathogenic Yersinia enterocolitica was detected from feces of children with diarrhea at 11 months of age with a infection rate of 0.50%(2/397). Conclusion Sporadic infection of pathogenic Yersinia enterocolitica was found in under five-year-old children in western Yunnan Province. It is necessary to strengthen the monitoring and research of Yersinia enterocolitica.

OBJECTIVETo observe the effect of magnesium sulfate, Nifedipine Tablet (NT) combined Salvia Injection (SI) on endothelin-1 (ET-1), nitric oxide (NO), thromboxane A2(TXA2), prostacyclin I2(PG2), and hemorheology of preeclampsia patients.

METHODSTotally 704 preeclampsia patients were randomly assigned to the treatment group and the control group, 352 cases in each group. All patients were treated with magnesium sulfate combined NT (on the first day: slow intravenous injection of magnesium sulfate 5 g + intravenous dripping of magnesium sulfate injection 10 g + oral administration of NT 30 mg; on the second and third day, intravenous dripping of magnesium sulfate injection 10 g + oral administration of NT 30 mg), while those in the treatment group were dripped with SI additionally at 20 mL per day for 3 consecutive days. Before and after treatment plasma levels of endothelin-1 (ET-1), nitric oxide (NO), TXA2, PGi2, and hemorheology indicators [such as high blood viscosity (HBV), low blood viscosity (LBV), plasma viscosity (PV), erythrocyte rigidity index (ERI), fibrinogen (FIB)] of two groups were detected.

RESULTSCompared with the same group before treatment, serum levels of ET-1, TXA2, HBV, LBV, PV, ERI, and FIB decreased in the two groups after treatment (P <0. 05), but levels of NO and PG2 increased (P <0. 05). Compared with the control group in the same period, levels of ET-1, TXA2, HBV, LBV, PV, ERI, and FIB decreased in the treatment group after treatment (P <0. 05), but levels of NO and PGI2 increased (P <0. 05).

CONCLUSIONMagnesium sulfate, NT combined SI could effectively regulate the balance of ET-1/NO and TXA2/PGI2, and improve hemorheology of preeclampsia patients.

Objective To investigate expression of Human papillomavirus (HPV) 11 type E7 protein antigen in prokaryotic cells and its potential use for the serodiagnosis of condyloma acuminatum (CA).Methods The full-length gene encoding for HPV11 E7 protein was amplified by PCR,and cloned into vector pET32a(+) to form recombinant pET32a(+)/HPVll E7 plasmid.The fusion His-E7 protein was expressed and analyzed by using SDS-PAGE and Western blotting.Using ELISA assay,HPV11 E7 fusion protein were also used to screen human serum IgG antibody from 93 patients with CA,43 patients with cervix cancer and 58 healthy control subjects.Results Highly expressed fusion His-E7 protein was obtained,and purified protein served as a special diagnostic antigen to screen human serum antibody for CA serodiagnosis.It showed that CA group,cervix cancer group and healthy control human serum IgG antibody average value were 1.545?0.131,0.586?0.155 and 0.674?0.150 respectively,positive rate were 76.3% (71/93),11.6% (5/43) and 5.2% (3/58).There was significantly difference between the CA group to compare cervix cancer group and healthy control (P

Objective:To explore value of vector flow mapping (VFM) technique in quantitative determination of left ventricular flow energy loss (LVFEL) in different phases and segments in hypertensive patients.Methods:A total of 130 hypertensive patients treated in our hospital from Jan 2014 to Jan 2017 were enrolled as hypertension group.Another 130 healthy subjects undergoing physical examination simultaneously were enrolled as healthy control group.According to clinical pattern,hypertension group was further divided into normal structure group (NS group,n＝40),concentric remodeling group (CR group,n＝ 56) and concentric hypertrophy group (CH group,n＝34).LVFEL of different phases and segments were measured and compared by VFM technique among all groups.Re-sults:LVFELs of different phases and segments in hypertension group were significantly higher than those of health-y control group,P＝0.001 all.Compared with NS group,there were significant rise in [ (15.10 ± 1.22) N·m-1· s-1vs.(17.94 ± 1.28) N·m-1·s-1vs.(16.76 ± 1.24) N·m-1·s-1] in CR group and CH group,and that of CR group was significantly higher than that of CH group,P＝0.001 all;compared with NS group and CH group,there was significant rise in middle segment flow EL [ (8.10 ± 1.20) N· m-1·s-1,(8.22 ± 1.18) N· m-1· s-1vs.(8.94 ± 1.16) N· m-1·s-1] in CR group,P＜ 0.01 all;apical segment flow EL of CH group was significantly higher than that of NS group [ (4.59 ± 1.07) N·m-1·s-1vs.(3.91 ± 1.09) N·m-1·s-1],P＝0.006;mid-di-astolic flow EL of CR group was significantly higher than that of NS group [ (8.87 ± 1.03) N·m-1·s-1vs.(8.25 ± 1.05) N·m-1·s-1],P＝0.006.Conclusion:Vector flow mapping technique can accurately and quantitatively determine LVFEL in different phases and segments in hypertensive patients,which provides reliable evidence for early revealing hemodynamic changes.

In order to investigate the sleep quality and influencing factors in patients with Parkinson's disease (PD),201 PD patients were enrolled and underwent extensive clinical evaluations.Subjective sleep evaluation was assessed using the Pittsburgh Sleep Quality Index (PSQI),and the Epworth Sleepiness Scale (ESS).It was found that poor sleep quality (77.11％) and excessive daytime sleepiness (32.34％) were commonly seen in PD patients and positively correlated with disease severity.Then 70 out of the 201 PD patients and 70 age-and sex-matched controls underwent a polysomnographic recording.The parameters were compared between PD group and control group and the influencing factors of sleep in PD patients were analyzed.The results showed that sleep efficiency (SE) was significantly decreased (P＜0.01),and sleep latency (SL) and the arousal index (AI) were increased (P＜0.05) in the PD group as compared with those in the control group.SE and total sleep time (TST) were positively correlated with the Hoehn and Yahr (H&Y) stage.There was significant difference in the extent of hypopnea and hypoxemia between the PD group and the control group (P＜0.05).Our results indicate that PD patients have an overall poor sleep quality and a high prevalence of sleep disorder,which may be correlated with the disease severity.Respiratory function and oxygen supply are also affected to a certain degree in PD patients.

Objective To esplore the relationship between sexual concept and psychological health among college famale students majored in nursing and provide scientific evidence for adolescent sex education. Methods 1 093 college famale students majored in nursing were investigated with sexual concept questionnnaire and SCL-90. Results 35.1% subjects were in love, 38.2% agreed with sexual behavior before marriage, 9.7% agreed with extramarital sex. The score of all factors in Scl-90 were higher than the norm. The results showed the positive relationship between paranoid ideation and loving behavior, between hostility and paranoid ideation and premarital sex, between paranoid ideation and psychosis and extramarital sex, and the negtive relationship between interpersonal relationship and extramarital sex. Conclusions We should not ignore the effects of psychological factors on college famale students majored in nursing when carrying out the sexual education, especially for the students with higher paranoid ideation, hostility and psychosis who will be needed other education.

This study using maltodextrin as raw material, 1%-5% polyvinylpyrrolidone K30 as template agent, 1%-5% ammonium bicarbonate as pore-forming agent, curcumin and ibuprofen as model drugs. Porous maltodextrin was prepared by template and pore-forming agent methods, respectively. The structure and drug delivery behavior of porous maltodextrin prepared by different technologies were comprehensively characterized. The results showed that the porous maltodextrin prepared by pore-forming agent method had larger specific surface area (6.449 4 m²·g^-1) and pore size (32.804 2 nm), which was significantly better than that by template agent method (3.670 2 m²·g^-1, 15.278 5 nm). The adsorption kinetics between porous maltodextrin prepared by pore-forming agent method and curcumin were suitable for quasi-first order adsorption kinetic model, and that between porous maltodextrin and ibuprofen were suitable for quasi-second order adsorption kinetic model. While the adsorption kinetics between porous maltodextrin prepared by template agent method and two model drugs were both suitable for the quasi-first order adsorption kinetic model. In addition, the dissolution behavior analysis showed that the porous maltodextrin prepared by the two technologies can significantly improve the dissolution behavior of insoluble drugs, and the drug release was both carried out by diffusion mechanism, which suitable for the Peppas kinetic release model, but the porous maltodextrin prepared by template agent method had a faster release rate. The change of nozzle diameter had no significant effect on the adsorption process and drug release behavior of porous maltodextrin. In conclusion, the porous maltodextrins prepared by two different technologies were both beneficial to the delivery of insoluble drugs, and the template agent method was the best for delivery of insoluble drugs. This study can provide theoretical basis for the preparation of porous particles, promote the application of porous particles in insoluble drugs, and improve the bioavailability of insoluble drugs.

Objective To compare the safety and efficacy of the different doses of sufentanil combined with propofol target controlled infusion for modified electronic convulsive therapy ( MECT ) .Methods A total of 100 patients with mental disorders were randomly divided into four groups:control group (group C), sufentanil 1 group (S1), sufentanil 2 group ( S2 ) and sufentanil 3 group ( S3 ) , 25 cases in each group.In sufentanil groups, patients were respectively intravenously injected with 0.1, 0.2 and 0.3 μg? kg -1 sufentanil 10 mL, and those in control group were given 0.9%NaCl solution.Propofol was given through target controlled infusion, and the initial target density of 3.0 μg? mL-1 was set, and increased by 0.5μg? mL-1.When eyelash reflex disappeared, succinylcholine 1.0 mg? kg -1 was injected by intravenous injection.The vital signs of patients before anesthesia ( T1 ) , target control infusion stop immediately ( T2 ) , immediately after MECT finished ( T3 ) and consciousness recovery ( T4 ) , clinical effects,the seizure energy index, the seizure duration, awakening time, assisted ventilation time and adverse reactions were recorded.Results The difference had no statistical significance in vital signs, the seizure energy index, the seizure duration of all patients ( P >0.05 ) .Compared with group C, assisted ventilation in group S2 and S3 was prolonged, and the waking time was extended in group S3, total effective rate was increased in group S1 to S3 ( P<0.05 ) .Compared with group S1 , assisted ventilation time and waking time were extended, myalgia was decreased, and the total effective rate was increased in group S3(P<0.05).Compared with group S2 , the assisted ventilation was prolonged in group S3 ( P<0.05 ) .The incidences of nausea and vomiting and sleepiness were increased, while muscle pain and headache incidence were decreased as sufentanil dose increased. Conclusion Sufentanil combined with anesthesia of propofol target controlled infusion can be safe and effective for MECT, and 0.2 μg? kg-1 sufentanil is preferred.

OBJECTIVETo explore the effect of Tanreqing Injection (TRQI) on the treatment of acute exacerbation of chronic obstructive pulmonary disease (AECOPD) with Chinese medicine syndrome of retention of phlegm and heat in Fei (RPHF).

METHODSIn a prospective randomized controlled clinical trial, 90 patients with AECOPD of RPHF syndrome were randomly assigned to 3 groups, TRQI and controls A and B, each with 30 cases. The TRQI group was administered with the intravenous injections of 20 mL TRQI once a day and conventional Western medicine treatment. Control group A was administered with the intravenous injection of 15 mg ambroxol hydrochloride twice a day and conventional Western medicine treatment, and control group B was administered with conventional Western medicine treatment only. The treatments were administered for 10 days. Chinese medical symptoms and signs were scored, and plasma concentrations of interleukin (IL)-8 and neutrophil elastase (NE) were recorded.

RESULTS(1) The Chinese medical symptoms (cough, sputum amount, expectoration, dyspnea and fever) and signs (tongue and pulse) improved significantly in the TRQI group (P<0.05 or P<0.01), and improvements in cough, sputum amount and expectoration were better in the TRQI group than control group B (P<0.05); there was no significant difference between the TRQI group and control group A (P>0.05). The sign of tongue was also improved significantly in the TRQI group (P<0.05). (2) The overall effects in the TRQI group and control group A were significantly better than in control group B (P<0.05), with no significant differences between the TRQI group and control group A (P>0.05). There was no significant difference in the total effective rate among the three groups (P>0.05). (3) After treatment, the plasma concentrations of IL-8 and NE decreased in the TRQI group and control group A (P<0.05), and the concentration of IL-8 in control group B decreased (P<0.05). The difference in IL-8 was greater in the TRQI group than in control group A and B before and after treatment, and the change in NE was greater in control group A than in the TRQI group and control group B, but there was no statistical significance among the three groups with regards to the change in IL-8 or NE (P>0.05).

CONCLUSIONTRQI could improved the Chinese medical signs and symptoms in the patients with AECOPD, possibly because of the decreasing plasma levels of IL-8 and NE which could improve response to airway inflammation and mucus hypersecretion.
