1.Clinical observation of sitagliptin combined with insulin aspart 30 with in the treatment of secondary failure of sulphonylurea in type 2 diabetes mellitus

Objective To observe the effect of sitagliptin combined with insulin aspart 30 in the treatment of secondary failure of sulphonylurea in type 2 diabetes mellitus. Methods Fifty-six cases were divided into group A and group B in random block design, with 28 cases of each group. The patients in group A was treated with sitagliptin combined with insulin aspart 30, while the patients in group B was given subcutaneous injection of insulin aspart 30R. All patients were treated for 12 weeks. Fasting plasma glucose(FPG), 2-hour postprandial plasma glucose(2 hPG), glycosylated hemeglobin(HbA1c), insulin secretion index (HOMA-β), body mass index (BMI), and incidence of low blood glucose before and after treatment were compared. Results Compared with that in group B, FPG [(5.61 ± 1.14) mmol/L vs. (7.8 ± 1.22) mmol/L], 2 hPG [(7.62 ± 1.35) mmol/L vs(9.72 ± 1.41) mmol/L] and HbA1c [(7.11 ± 0.83)%vs.(8.32 ± 1.04)%] in group A had a significant decrease;HOMA-β[(50.31 ± 5.12) vs. (41.86 ± 4.53)] of group A was higher than that of group B (P

Objective:To study the analgesic effect of etomidate infusion for radical mastectomy in breast cancer and its effect on hemodynamics and inflammatory mediators.Methods:From August 2015 to July 2016,84 patients with radical mastectomy were divided into the observation group and the control group according to the order of admission.The observation group was given continued infusion of etomidate,the control group was given continued infusion of propofol.The visual analogue (VAS) at different time points after operation,diastolic blood pressure (DBP),systolic blood pressure (SBP) and heart rate (HR) at the time ofextubation and serum IL-2,IL-10,IL-12 levels at 72 hours after operation were compared between two groups.Results:At the time of extubation,the DBP,SBP and HR were significantly lower in the observation group than in the control group (P<0.05).The VAS scores of observation group were significantly lower than those of the control group at 1,5,10,24 and 48 hours after operation (P <0.05).The serum levels ofIL-2 in the observation group was significantly higher than that of the control group (P<0.05),IL-10 and IL-12 levels were lower than control group (P<0.05).Conclusion:Sustained infusion of etomidate had good analgesic effect in the treatment of breast cancer patients underwent radical mastectomy with little effect on the hemodynamics and inflammatory mediators.
3.Application of tunnel and penetrating ureteroneocystomy in kidney transplantation

Objective To investigate the new method of ureter bladder anastomosis in renal transplantation.Methods The ureter was pulled into bladder for 1.2 cm through tunnel at the lateral- top of bladder wall,and the ureter fixed on the bladder wall by 2-3 acus with catgut suture.Results Forty of 42 patients had no complications,and recovered very well,except for 1 cases of necrosis caused by acute rejection and 1 case on urine leakage caused by catheter obstruction from blood clot.Conclu- sion This method is simple,easy to operate,safe and reliable with less complications.

5.Clinical and endoscopic features of pediatric eosinophilic gastroenteritis in 49 cases

Objective To explore the clinical features and endoscopic manifestations of eosinophilic gastroente-ritis(EG)in children.Methods A retrospective analysis was conducted of the clinical manifestation,laboratory examination,endoscopy(upper and/or colonoscopy)performance,diagnosis and treatment of 49 cases of patients who were diagnosed as EG in Children's Hospital of Nanjing Medical University from July 2013 to July 2015.Results The common clinical manifestations of EG in children were hematochezia(23 cases),diarrhea(20 cases),vomiting(18 cases)and abdominal pain(15 cases).The ages of children admitted to hospital for the first time ranged from 1 month and 7 days old to 13 years and 7 months old,and the mean age was 59.4 months old,in which most patients were younger than 1 year old,accounting for 38.77%(19 cases)of all.Peripheral blood eosinophilia was present in 20 cases(40.82%)of the patients,and only 6/18 cases(33.33%)of the patients had elevated serum IgE.Upon endoscopic analysis,the lesions involved esophagus(4 cases),stomach(14 cases),duodenum(20 cases),small intestine(15 cases),colon(28 cases)and rectum(6 cases),and the most common manifestation under gastroscopy was mucosal hyperemia edema(27 cases)and erosion(9 cases),while the most common manifestation under colonoscopy was mucosal hyperemia edema(25 cases)and nodular hyperplasia(24 cases).All patients improved with food restriction,in which 8 cases were treated with glucocorticoid while 9 cases with oral Singulair and 9 cases with oral Loratadine.All children with symptoms were alleviated somewhat,but 5 cases of them relapsed after drug withdrawal.Conclusions The clinical manifestations of EG in children varied and were mainly hematochezia,vomiting,diarrhea and abdominal pain.Some patients had the elevated peripheral blood eosinophilia and serum IgE.The most common manifestations under gastroscopy were mucosal hyperemia edema and erosion while the most common manifestations under colonoscopy were mucosal hyperemia edema and nodular hyperplasia.
7.Effect of integrinβ1 on multidrug resistance in gastric cancer SGC-7901 cells

AIM:To study the effect of integrin β1 on multidrug resistance in gastric cancer and its possible mechanisms .METHODS:The expression of integrin β1 at mRNA and protein levels in the SGC-7901 cells and SGC-7901/DDP cells was determined by qPCR and Western blot .The expression of integrin β1 in the SGC-7901/DDP cells was silenced by antisense oligodeoxynucleotide .The cell viability was detected by the CCK-8 assay, the cell apoptosis were ana-lyzed by flow cytometry, and the protein levels of integrin β1, Bcl-2/Bax, cleaved caspase-3/caspase-3, cytochrome C ( Cyt-C) and p-AKT/AKT were determined by Western blot .RESULTS:The expression of integrin β1 at both mRNA and protein levels was significantly upregulated in SGC-7901/DDP cells.The expression of integrin β1 was increased in SGC-7901 cells treated with chemotherapeutic agents such as cisplatin , paclitaxel and 5-fluorouracil .Knockdown of integrin β1 induced apoptosis of SGC-7901/DDP cells with an increased sensitivity to the chemotherapeutic agents .Meanwhile , knock-down of integrin β1 downregulated the protein levels of Bcl-2/Bax, p-AKTSer473 and p-AKTThr308 , while promoted the release of Cyt-C and upregulated the protein level of cleaved caspase-3.CONCLUSION:Knockdown of integrin β1 increases the sensitivity of SGC-7901/DDP cells to the chemotherapeutic agents , and promotes the cell apoptosis via mitochondrial apop-tosis pathway .The mechanism may be related to the attenuation of AKT pathway by inhibiting phosphorylations of AKT at Ser473 and Thr308.
8.Effects of Sevoflurane Combined with Remifentanil on Intraoperative Related Indicators and Quality of Post-operative Recovery in Patients with General Anesthesia

OBJECTIVE:To investigate the effects of sevoflurane combined with remifentanil on intraoperative related indica-tors and quality of postoperative recovery in patients with general anesthesia. METHODS:52 patients with elective abdominal sur-gery were randomly divided into observation group and control group. All patients were treated with etomidate 0.3 mg/kg+atracuri-um 0.5 mg/kg+remifentanil 1μg/kg for anesthesia induction;then observation group was received sevoflurane by inhalation and con-trol group was propofol by infusion. The SBP,DBP,HR and BIS in 2 groups were recorded before anesthesia induction(T0),after anesthesia induction(T1),time of intubation(T2),skin incision(T3),10 minutes after establishing pneumoperitoneum(T4),end of pneumoperitoneum(T5) and extubation(T6),respectively. Operation and anesthesia duration,recovery time and extubation time, MMSE score and incidence of adverse reactions were recorded. RESULTS:SBP and HR in T6 and DBP in T4-6 in 2 groups were sig-nificantly higher than T0 of same group,BIS in T1-5 was significantly lower than T0,the differences were statistically significant(P<0.05),however,there was no significant difference between 2 groups (P>0.05). Recovery time and extubation time in observa-tion group were significantly lower than control group(P<0.05),there was no significant difference in the operation and anesthesia duration between 2 groups(P>0.05). MMSE scores in 2 groups were significantly lower than before after 0.5 h and 1 h of extuba-tion,and control group was lower than observation group(P<0.05 or P<0.01);MMSE scores in 2 groups were significantly high-er than 0.5 h and 1 h after extubation(P<0.05),however,there was no significant difference between 2 groups and before(P>0.05). There was no significant difference in the incidence of adverse reactions between 2 groups(P>0.05). CONCLUSIONS:Sevoflurane combined with remifentanil has satisfactory anesthesia,sevoflurane has better controllability,with good safety.
9.Study on Inhibitory Effect of Aqueous Extract of Taxus Chinensis var. mairei Combined Erlotnib on A549 Xenograft in Nude Mice and Its Mechanism.

OBJECTIVETo study the effect of aqueous extract of Taxus chinensis var. mairei (AETC) combined Erlotnib on the growth of A549 xenograft in nude mice and its mechanism.
METHODSThe xenograft model in nude mice was established by inoculating A549 cells subcutaneously. BALB/c nude mice bearing A549 xenograft were randomly divided into six groups, i.e., the low dose Erlotinib group (A) , the standard dose Erlotnib group (B) , the low dose Erlotinib combined AETC group (C), the standard dose Erlotnib combined AETC group (D), the AETC group (E), the control group (F), 12 in each group. Different medication was performed for 7 successive weeks after 24 h. One mL blood was withdrawn and tumor tissues taken. The tumor inhibition rate was calculated. The combined effect was analyzed by Jin's Formula [Q = Ea + b/(Ea + Eb-Ea x Eb) ]. mRNA and protein expression levels of epidermal growth factor receptor (EGFR), cyclooxygenase-2 (COX-2), and B cell lymphoma-2 (Bcl-2) in xenografts were detected using real-time RT-PCR and ELISA.
RESULTSCompared with Group F, the xenograft weight was obviously lowered in Group B-E (P < 0.05, P < 0.01). The q value was 0.92 in Group C and 0.96 in Group D, which was obtained by simple adding of the two drugs. Compared with Group F, EG- FR mRNA expression in Group D and E, COX-2 mRNA expression in Group A-E; Bcl-2 mRNA expression in Group B-D; COX-2 protein expression in Group B-E; Bcl-2 protein expression in Group C and D were obviously lowered with statistical difference (P < 0.05, P < 0.01).
CONCLUSIONSAETC combined low dose and standard dose Erlotinib had synergistic effect on tumor inhibition. Its mechanism might be associated with down-regulating mRNA and protein expression levels of COX-2 and Bcl-2.
