2.Comparison of two screening scales in screening postpartum depression

Objective To compare the value of the Postpartum Depression Screening Scale (PDSS)and the Edinburgh Postnatal Depression Scale(EPDS) in the screening of postpartum depression.Methods A total of 445 women within 6 weeks postpartum completed PDSS,EPDS and Structured Clinical Interview for DSM-IV-TR Axis I Disorders-Patient Edition (SCID-I/P).The SCID-I/P was regarded as the gold standard for postpartum depression diagnosis.Results The cut-off score for PDSS and EPDS was 74 and 10,respectively.PDSS achieved the higher combination of sensitivity ( 93.33% ) and specifity ( 94.75% ).The area under the curve for PDSS and EPDS was 0.978 and 0.872,respectively ( P < 0.05 ).Conclusions Compared with EPDS,PDSS has a higher screening value.It is a simple,fast and accurate screening tool for postpartum depression.
3.Study on the status of cell differentiation in nasal NK/T-cell lymphomas.

OBJECTIVETo evaluate the status of cell differentiation in nasal NK/T cell lymphomas.
METHODSThe clinical data of 88 cases of NK/T cell lymphomas were collected. Antibodies to the following antigens were used in the immunohistochemical study: T cell differentiation antigens (CD3epsilon, CD5 and CD1a); NK cell associated antigens (CD56, CD57) and antibodies of CD34 and CD38.
RESULTS(1) Clinicopathology: clinically, frequently involved sites were the nasal cavity and the pharynx. Ulceration and erosion of the mucosa were common signs. Pathologically, diffuse infiltration of the tumor cells was observed in 68 of 88 (70.45%) cases of nasal NK/T cell lymphomas. In 71 (80.68%) cases infiltrated cells were predominantly medium to large sized; (2) Differentiation status of tumor cells: the tumor cells expressed CD3epsilon in 78/88 (88.64%); CD5 in 56/88 (63.63%), CD56 in 25/88 (28.41%) and no positivity for CD1a, CD57, CD34 and CD38.
CONCLUSIONStatus of tumor cell differentiation in nasal NK/T cell lymphoma may have passed the stage of progenitor cell differentiation but not yet to the stage of mature T or NK cells.

4.Research of sequencing weaning in the patients with chronic obstructive pulmonary disease

Objective To research the sequencing weaning in the patients with chronic obstructive pulmonary disease (COPD). Methods The COPD patients were divided into two groups in this prospective case-control study: protocol-directed group vs none- protocol-directed group. The patients in the former group received programmed ventilator weaning strategy, detaily patients were selected through the shallow breathing with fast exponential screening, then were conducted by spontaneous breathing trial SBT , finally were pulled out the endotracheal intubation. The patients in the latter group were determined the timing of ending the ventilator according to doctors subjective judgment and clinical experience. Results The ventilation time of protocoldirected and on-protocol-directed group was ( 8.01 ± 7. 22 ) and ( 10.74 ± 5.58 ) days ( P = 0. 034 ) respectively. ICU stay time was shorter in protocol-directed group than in non-protocol-directed group ( 8.86 ±6.92 vs 12 ±6.10 days, P =0. 026). The success rate of weaning in protocol-directed group was 76.31% (29/38),which was higher than that in non-protocol ~ directed group( 12/25,48% , P = 0. 009 ). Conclusions Protocoldirected weaning and SBT may reduce the ventilation time and ICU stay time and enhance success rate of weaning.
5.Nursing of catheter incarceration in patients with indwelling balloon urethral catheter

Objective To explore the correct treatment methods after indwelling catheter incarceration in burn children.Methods Twenty-six burn children with indwelling catheter were analyzed according to their conditions and locations of incarceration,classified and treated effectively.The two cases catheterization incarcerated in the membranous urethra,twenty-two cases incarcerated in the bladder and two cases incarcerated in the internal orifice of urethra.The different catheter incarcerations were respectively handled with guide wire penetration method,acupuncture and surgical operation method.Results Twenty-six burn children could removed catheter after treatment,among them,nineteen cases with guide wire penetration method,four cases with acupuncture,three cases with surgical operation method which was removed of foreign body through perineal urethrotomy.Twenty-six burn children were better recovery after operation.Again indwelling catheter happened in one patient after operation,indwelling catheter was removed after two weeks,and the patient was discharged.Conclusions Nursing staff should skillfully master physiological and anatomical characteristics of male patients,the structure,know performance,operation and notes of balloon urethral catheter,and correctly grasp the decannulation method of the balloon catheter.
6.High expression of proteasome subunit PSMA7 in colorectal cancer is significantly correlated with liver metastasis.

OBJECTIVETo investigate the correlation between the proteasome subunit PSMA7 expression in colorectal cancer and its role in liver metastasis.
METHODSTo identify the PSMA7 protein expression in 62 primary site colorectal cancers, 34 lymph node metastatic sites and 13 liver metastatic sites by immunohistochemistry and clarify the correlation of its expression with the clinicopathological parameters.
RESULTSHigh expression of PSMA7 was detected in 38.7% (24/62) of primary site colorectal cancer, 52.9% (18/34) of lymph node metastatic sites and 100% (13/13) liver metastatic sites but not in the normal colorectal tissue. High expression of PSMA7 was significantly correlated with liver metastasis (P = 0.028). The survival rate was significantly lower in patients with high expression of PSMA7 than in those with low expression of PSMA7 (P = 0.0008). As well, in multivariate analysis, PSMA7 expression demonstrated to be an independent prognostic factor (P = 0.004, relative risk 5.057; 95% confidence interval, 1.682-15.201).
CONCLUSIONPSMA7 may play an important role in the colorectal cancer progression. Evaluation of PSMA7 expression in primary colorectal cancer at the time of surgery might be a valuable test in defining patients with a high risk of developing liver metastasis.

7.Clinical efficacy of valsartan combined with atorvastatin on patients with diabetic nephropathy

Objective To evaluate the effect and safety of valsartan combined with atorvastatin in the treatment of early stage diabetic nephropathy ( DN).Methods Sixty-three patients with early stage DN were recruited in this study and divided into two groups.The patients in experiment group ( n =30 ) were given valsartan 80 mg· d -1 combined with atorvastatin 20 mg · d -1 , and the control group ( n =33 ) were only given valsartan treatment.The treatment lasted for 4 months.After treatment, the data of serum level of serum creatinine ( Scr) , urinary albumin excretion rate ( UAER) , glomeru-lar filtration rate ( GFR ) and β2 -microglobulin (β2 -MG ) were compared between the two groups.Results After 4 -month treat-ment, the serum Scr, UAER and β2 -MG were decreased in both groups ( P<0.05 ) , but the changes in experiment group was much more significant than that in control group ( P<0.05 ).The adverse reactions such as cough, headache and dizziness were not statistically different between the two groups ( P<0.05 ).Conclusion Valsar-tan combined with atorvastatin treatment can improve the clinical efficacy in the treatment of diabetic nephropathy, without increasing adverse reactions.
8.Relationship between CO II gene of mtDNA of Lucilia sericata and latitude interval.

OBJECTIVE:
To deduce the region that the geographical species of Lucilia sericata come from and determine the scene of crime (SOC) based on the gene analysis of mtDNA CO II.
METHODS:
A 635 bp region for CO II of 4 Lucilia sericata (belong to 2 geographical species) were collected and sequenced, compared with the data of GenBank. A neighbour-joining tree with the Tamura and Nei model was constructed by MEGA2.1 package. The number of inherit intervals of inner-species were analyzes by Kimura's two-parameter model and used for construction the relationships between hereditary and latitude interval by SPSS10.5 soft.
RESULTS:
It showed that they had the relationships between inherit and latitude interval for the 8 geographical species of Lucilia sericata for CO II.
CONCLUSION
This method can be the evidence deducing the region that the geographical species of Lucilia sericata come from and further to determine the scene of crime (SOC).

10.Comparison on the performance of both temporal and temporal-spatial models for outbreak detection through China Infectious Diseases Automated-alert and Response System(CIDARS)

Objective To analyze the pilot results of both temporal and temporal-spatial models in outbreaks detection in China Infectious Diseases Automated-alert and Response System (CIDARS)to further improve the system. Methods The amount of signal, sensitivity, false alarm rate and time to detection regarding these two models of CIDARS, were analyzed from December 6,2009 to December 5,2010 in 221 pilot counties of 20 provinces. Results The sensitivity of these two models was equal(both 98.15%). However, when comparing to the temporal model, the temporal-spatial model had a 59.86% reduction on the signals(15 702)while the false alarm rate of the temporal-spatial model(0.73%)was lower than the temporal model(1.79%), and the time to detection of the temporal-spatial model(0 day)was also 1 day shorter than the temporal model.Conclusion Comparing to the temporal model, the temporal-spatial model of CIDARS seemed to be better performed on outbreak detection.
