1.Effect of postoperative pain control in trauma patients who are in painless ward with standardized management

Objective To investigate effect of postoperative pain control in trauma patients who are in painless ward with standardized management. Methods Patients were divided into two groups randomly according to their ID number. One was study group, another was control group. Study group received painless ward standardized management. Control group received routine postoperative pain control. Compare the effect of postoperative pain control of two groups. Results In the first day after operation, pain score of study group and control group were (2.51 + 1.37) and (3.79 + 1.50) respectively, the difference was statistically (t =4.682,P<0. 01) ; Night sleeping time of study group was longer than control group, the difference was statistically significant (t = 5.734,5. 074,2.711,2.194, P < 0.05), satisfaction degree was higher in study group (Z =-5.839,-6.34,-4.50,-7.23,-6.88,P<0.01). Conclusions Standardized pain management in painless ward can reduce postoperative pain and increase satisfaction and comfort of patient.
2.Cloning, purification, and antigenic characterization of three recombinant fragments derived from SARS-CoV S1 domain.

OBJECTIVEThe present study aimed to clone and express three fragments of genomic RNA derived from SARS associated coronavirus (SARS-CoV) S1 domain and to study its immunogenicity.
METHODSThe S1 domain gene was amplified by PCR with specific primers and was inserted into the prokaryotic expression vector pQE-30. Three fragments (40-751, 746-1344 and 746-2001 bp) derived from S1 domain produced after the recombinant plasmid (pQE-30/S1) was digested by restriction endonucleases. The three fragments were cloned into pQE-30 and expressed in M15 strains of Escherichia coli. The expression products, designated S1a, S1b and S1c respectively, were purified by Ni affinity chromatography. The immunogenicity was analyzed by Western Blot and ELISA using serologically confirmed sera from SARS patients and the sera from healthy donors was used as control at the same assay.
RESULTSThree recombinant plasmids (pQE-30/S1a, pQE-30/S1b, pQE-30/S1c) were constructed.Fusion proteins with relative molecular mass of 26,700, 22,500 and 46,000 dalton were successfully expressed with amounts of 35%, 35% and 30% of total cell protein and purified by Ni affinity chromatography, respectively. Western Blot and ELISA analysis showed that the S1c protein could be specifically recognized by the sera from SARS patients.
CONCLUSIONThe recombinant S1c protein was a good immunogen and has the potential to be used as a vaccine against SARS-CoV infection.

3.Effects of yiqi huoxue recipe and Coxsackie virus B type 3 on the expression of ribosomal protein S20 in rat cardiac myocytes.

OBJECTIVETo study the effects of yiqi huoxue recipe and Coxsackie B virus type 3 (CVB3) on the expression of ribosomal protein S20 in rat cardiac myocytes, to explore the pathogenesis of myocarditis induced by CVB3 and the mechanism of yiqi huoxue recipe on gene level, and to further investigate whether yiqi huoxue recipe is an effective prescription for CVB3 myocarditis.
METHODSA modified suppression subtractive hybridization (SSH) was used to isolate differentially expressed genes between the CVB3 infection group and the treatment group with yiqi huoxue recipe. The results were further verified by fluorescence RT-PCR.
RESULTSThe results of SSH showed that the gene expression of ribosomal protein S20 in the treatment group was higher than that in the CVB3 infection group (P<0.05), which agreed with the results of fluorescent RT-PCR.
CONCLUSIONDown-regulation of ribosomal protein S20 mRNA expression might be one of the mechanisms in CVB3 myocarditis, and yiqi huoxue recipe could achieve the treatment of viral myocarditis by regulating the expression of ribosomal protein S20.

4.Clinical study of heparin-induced thrombocytopenia.

OBJECTIVETo observe the incidence of heparin-induced thrombocytopenia (HIT) in patients received unfractionated heparin (UFH) treatment, and explore the feasibility of monitoring HIT by platelet counts, as well as the significance of HIT-antibody test in HIT diagnosis.
METHODS145 patients received UFH treatment in Vascular Surgery Department were studied. Before and after the UFH treatment, platelet counts, HIT-antibody ELISA test and heparin-induced platelet aggregation (HIPA) were tested.
RESULTSAmong the 145 patients, thrombocytopenia occurred in 40 (27.6%) cases, HIT-antibody ELISA test positive in 59 (40.7%) cases, HIPA test positive in 26 (17.9%) cases. The HIT was diagnosed in 24 (16.5%) cases, and heparin-induced thrombocytopenia and thrombosis (HITTS) occurred in 5 (3.4% in all cases, and 20.8% in HIT patients). In HIT patients, 15 patients (62.5%) were thrombocytopenia, HIT-antibody positive and HIPA test positive. Platelet counts in all of the 24 patients recovered to normal or level before UFH treatment in 3-6 days after heparin withdrawal therapy.
CONCLUSIONHIT can be early diagnosed by monitoring platelet counts, HIT-antibody ELISA test and HIPA test. Withdrawal of heparin therapy in time and use of alternative anticoagulant, HITTS rate might be expected to decline further.
