2.Comparison of biological characteristics of human gingival junctional epithelial cells and oral epithelial cells

Objective:To isolate P-cadherin positive and negative oral gingival epithelial cells , and to compare the biological characteristics with junctional epithelial cells .Methods:Human oral gingival epi-thelial cells and junctional epithelial cells were cultured .P-cadherin positive and negative cells were iso-lated from oral gingival epithelial cells .The cellular adhesion , proliferation and migration were measured and compared .Results:The P-cadherin positive cells accounted for 20%of oral gingival epithelial cells . Compared with juctional epithelial cells , P-cadherin positive oral gingival epithelial cells showed similar properties of adhesion and migration , and stronger proliferation ability (0.72 ±0.06 vs.0.60 ±0.05, P<0.05).P-cadherin negative oral gingival epithelial cells showed weaker ability of adhesion (48%± 6%vs.87%±11%, P<0.05), proliferation (0.36 ±0.04 vs.0.60 ±0.05, P<0.05) and migration (10.3 ±2.7 vs.23.4 ±4.8, P<0.05).Conclusion: P-cadherin positive oral gingival epithelial cells showed some similar but different biological characteristics , compared with juctional epithelial cells , which suggested that during the process of transforming oral gingival epithelial cells into juctional epithelial cells, complex gene and protein changes were involved instead of simply cellular migration .
3.Analysis of risk factors of hyperthermia in the early period of an acute cerebral infarction

Objective To investigate the risk factors of hyperthermia in early period of an acute cerebral infarction.Methods 299 patients admitted to hospital from January 2003 to December 2004 with a diagnosis of cerebral infartion by CT or MRI,were retrospectively studied.The risks of hyperthermia in the early period for stroke,such as admission conscious level,bulbar palsy,emphysema,atrial fibrillation,multi-cerebral infarction,diabetes mellitus and chronic heart failure were analyzed,Logistic regression analysis was used to determine the factors that were associated with hyperthermia in the early period of stroke.Results 71 patients had fever in early period of an acute cerebral infarction(23.7%).The most common infection was pneumonia(18.4%).The analysis showed that the admission conscious level and atrial fibrillation were the risks of early hyperthermia of an acute cerebral infarction.Conclusion Admission conscious level(subconscious or unconscious/coma) and atrial fibrillation and chronic heart failure are independent risk factors for development of early hyperthermia for acute stroke.
4.Effects of dexmedetomidine on postoperative cognitive dysfunction in elderly patients undergoing laparoscopic radical trachelectomy

ObjectiveTo investigate the effects of dexmedetomidine on postoperative cognitive dysfunction (POCD) in the elderly patients undergoing laparoscopic radical trachelectomy.MethodsSixty ASA Ⅰ or Ⅱ patients (aged 65-80 yr and weighing 55-74 kg) undergoing elective laparoscopic radical trachelectomy were randomly divided into two groups:30 patients received intravenous injection of dexmedetomidine 0.2 μg · kg - 1 ·h- 1 ( group D EX) and 30 received same volume of normal saline (control group,group NS) after induction of anesthesia.Cognitive function was assessed at 1 day before operation and on the 7th day after operation using a battery of nine cognitive dysfunction tests.ResultsCompared with group NS,on the 7th postoperative day,the incidence of POCD was decreased in group DEX ( P < 0.05 ).ConclusionDexmedetomidine can improve POCD in the elderly patients undergoing laparoscopic radicaltrachelectomy.
5.Expression of Metabolic Pattern Glutamic Acid Receptor in Hippocampus of Juvenile Rat Model of Epilepsy

Objective To investigate the changes and possible mechanisms of the expressions of metabolic pattern glutamic acid receptor 1(mGluR1) and mGluR3 in hippocampus of juvenile rats submitted to lithium chloride-pilocarpine induced model of epilepsy in 6 h,5 d,60 d after status epilepticus(SE) onset.Methods Seizures were induced in the juvenile rats with lithium and pilocarpine injected intraperito-neally,and behavioral changes and EEG were observed.Eighteen SD juvenile rats with SE were randomly divided into following groups: groupⅠ,in which the rats were killed at 6 h after SE onset(6 h SE),group Ⅱ,in which the animals were killed during the seizure-free period(5 days after SE onset),and group Ⅲ,in which the animals were killed in 60 days after SE induction(period of spontaneous recurrent seizures).And intraperitoneal injection of saline water control groups were divided into: groupⅠa,group Ⅱa and group Ⅲa.The hippocampus tissues after the rats were put to death were collected,the expressions of mGluR1 and mGluR3 mRNA were detected by reverse transcriptase polymerase chain reaction(RT-PCR) in the hippocampus of juvenile rats.Results EEG of rats in group Ⅰ were abnormal,but normal in groupⅡ,and 5(83%) cases of the juvenile rats in group Ⅲ manifested dissemination of sharp waves,spikes or spike wave.The saline control group did not spontaneously attack.There was more significant upregulation of mGluR1 mRNA expression(Pa0.05).The expressional levels of mGluR3 mRNA were upregulated in groupⅠ,group Ⅱ and group Ⅲ(Pa
6.Clinical observation of 31 patients of congenital ectopia lentis treated with different operative manipulations

AIM: To determine the optimal operation method and reduce operative complications by retrospective analysis of outcomes of different operative manipulation for patients with congenital ectopia lentis ( CEL) .
●METHODS: A retrospective study ranged from Jan. 1, 2010 to Jan. 1, 2015 was conducted and 31 CEL patients (57 eyes) treated with different operations were taken into this study. The postoperative outcome, operation related complications was analyzed in terms of different operative manipulations.
● RESULTS: The most common types of CEL are idiopathic and Marfan - related CEL. All eyes ( 98%) benefited from operation except 1 Marchesani syndrome eye concurrent with optic atrophy induced by glaucoma. A total of 3 eyes ( 5%) were treated by phacoemulsification combined with intraocular lens implantation; 8 eyes ( 14%) were treated by phacoemulsification, capsular tension ring implantation combined with intraocular lens implantation, among these 8 eyes, 1 eye was treated with capsular tension ring implantation combined with suture fixation; 39 eyes ( 68%) were treated by lens excision and anterior vitrectomy through corneal incision combined with intraocular lens implantation with suture fixation, among these 39 eyes, 2 eyes were treated with trabeculectomy , but not combined with intraocular lens implantation; 5 eyes ( 9%) were treated by intracapsular cataract extraction and anterior vitrectomy combined with intraocular lens implantation with suture fixation; 2 eyes ( 4%) were treated by lens excision and vitrectomy through pars plana combined with silicon oil injection.
● CONCLUSION: Almost all patients can obtain satisfactory outcomes through various operative manipulations. Phacoemulsification combined with intraocular lens implantation was the first choice for patients with lens dislocation range less than 90°. Phacoemulsification, capsular tension ring implantation combined with intraocular lens implantation was recommended for 90° - 180°, and lens excision and anterior vitrectomy through corneal incision combined with intraocular lens implantation with suture fixation was more suitable for lens dislocation range more than 180°. The appropriate operative methods for different types of CEL patients is the key to success.

Objective To study the changes of diastolic function of left ventricle by tissue Doppler imaging (TDI) and the serum level of endocrine factors,and analyse the relations between these factors and mass or diastolic function of left ventricle in patients with essential hypertension.Methods Sixty one patients with essential hypertension were divided into left ventricular hypertrophy(LVH) group and no left ventricular hypertrophy(NLVH) group.Twenty healthy subjects were considered as control group.The early(e) and late(a) diastolic maximal myocardial velocity and e/a of lateral wall motion of mitral valve annulus were recorded by TDI.The levels of atrial natritic peptide(ANP),brain natriuretic peptide(BNP),endothelium(ET),calcitonic gene related peptide(CGRP) and insulin like growth factor 1(IGF 1) were measured by radio immunology analysis.Results The e wave maximal velocity in LVH group was lower than that of NLVH group and control group [( 14.56 ? 7.83 ) cm/s,( 16.40 ? 0.66 ) cm/s,( 18.68 ? 3.78 ) cm/s,respectively],and a wave maximal velocity in LVH group was higher than that of NLVH group and control group [( 18.28 ? 9.60 ) cm/s,( 16.03 ? 5.88 ) cm/s ,( 14.53 ? 1.28 ) cm/s,respectively]; The e and a maximal velocity velocities in both LVH and NLVH groups had statistic differences with those of control group(P

OBJECTIVETo investigate the mechanism that the formulas for activating blood and resolving stasis can regulate hemopoietic stem cell to produce new blood.
METHODRats were established animal model of acute cerebral infarction by referencing Olivette' method. They were randomly divided into model group, the group of the high, middle, low dose of the formulas for activating blood and resolving stasis. Each group and then wasrandomly divided into subgroups by 1, 3, 7, 14, 28 d. Xuesaitong capsule was formulated into 20, 40, 60 g x L(-1) with normal saline. The rats were given gavage drugs once a day until the experient ended, and the model group was administrated by intragastrical perfusion of normal saline. ELISA was used to detect the expression of SCF in peripheral blood and bone marrow among different groups at different time points. Flow cytometry was used to observe the changes of CD117 in blood and bone marrow.
RESULTThe CD117+ HSC and SCF concentration in peripheral blood and bone marrow of model group were increasing during 1-14 d,there was a peak on the 14th day, then the expression was reducing. CD117+ HSC and SCF concentration rising trend in the group of the high, middle dose of the formulas for activating blood and resolving stasis was preceded model group (P < 0.05).
CONCLUSIONActivating blood and resolving stasis can regulate hemopoietic stem cell to produce new blood, and it is through the regulation of CD117+ HSC number to achieve the purpose.

9.Histopathological features of EB virus-related human immunodeficiency virus negative plasmablastic lymphoma

Objective To investigate the clinicopathological features of human immunodeficiency virus (HIV) negative plasmablastic lymphoma (PBL) with no-immunosuppression,so as to accelerate the understanding for this group of disease.Methods The histological features of 6 HIV-PBL patients with no-immunodeficiency were retrospectively analyzed.Epstein-Barr virus (EBV) status was detected by in situ hybridization.Then,immunohistochemistry and fluorescence in situ hybridization (FISH) method were used to determine the immunophenotype,latent status of EBV and MYC translocation in PBL,respectively.Results HIV-PBL showed monotonous proliferation of plasmablastoid or immunoblast-like cells.Giant cells and necrosis could be observed,with less reactive cells in the background and higher mitoses.All the cases had EBV infection and type Ⅰ latency status of EBV (LMP1-/EBNA2-),and expressed terminal B-cell differentiation immunophenotype (CD20-/CD3-/CD138+/Kappa or Lambda+).Six HIV-PBL patients were elderly (median age was 69.5 years old),had equal incidence of PBL between male and female and showed high frequency of involvement of extranodal and extraoral lesion sites (6 cases and 5 cases,respectively).Median sutvival was 25.5 months.In addition,3 HIV-PBL patients had IGH/MYC translocations.Conclusions HIV-PBL is a new entity with unique clinical features including no-HIV infection,elderly,EBV positivity,and more involvement in extranodal and extraoral sites.HIV-PBL should be distinguished from HIV+ PBL.
10.Thoughts on practice of medical liability insurance at the hospital

An analysis of medical liability insurance practice at the hospital in the past 6 years found that medical liability insurance failed to work as expected in medical dispute prevention and risk sharing.The analysis also found that such an insurance will take a greater role in risk sharing,exposure to conflicts,cause discovery,and responsibility,with enhanced government role in guidance and intervention.Thus medical liability insurance is highly advocated by such means as compulsory execution,coordinating mechanism of insurance funding,prevention“medical dispute profiteers”,and adaptation to new approaches in medical dispute settlement.
