1.Clinical Analysis of Drug Fever Induced by Piperacillin Sodium and Tazobactam Sodium for Injection

OBJECTIVE:To provide reference for the timely and accurate judgment of drug fever induced by Piperacillin sodi-um and tazobactam sodium for injection and rational drug use in the clinic. METHODS:The medication,clinical manifestations, laboratory examination results and the physician treatment of 30 inpatients with drug fever induced by Piperacillin sodium and tazo-bactam sodium for injection from Sept. 2013 to Sept. 2014 were analyzed retrospectively and statistically. RESULTS:Most of the drug fever induced by Piperacillin sodium and tazobactam sodium for injection occurred in continuous 7 to 14 d medication,and the cumulative dosages were between 1.3 to 2.7 g/kg;73.3% of the 30 patients had fever during the intravenous drip,with body temperature mainly≥38.5 ℃;the elevation of eosinophil and slightly increase of serum C-reactive protein (CRP) and blood cell sedimentation rate (ESR) could be used as observation indexes of drug fever,but the leucocyte reduce could’t be suitable;the body temperature dropped to normal within 24 to 48 hours after stopped using it. CONCLUSIONS:The drug fever induced by Piperacillin sodium and tazobactam sodium for injection has no correlation with the patients’gender or age,and no special diagnos-tic criteria. But it has certain correlation with duration of medication,cumulative days and dosages and it can be used as reference of judgment with the combination of hematological examination index. Clinicians should improve the understanding and attention about drug fever to stop using suspicious drug in time.
2. Immunosuppressive and regulatory mechanism of artesunate

OBJECTIVE: To determine the immunoregulatory mechanism of artesunate both in vitro and in vivo. METHODS: The immunosuppressive action of artesunate was investigated through lymphocyte proliferation and cytotoxicity was also detected in vitro. The delayed-type hypersensitivity (DTH) model in mice was used for the investigation of artesunate in vivo. The mRINA expression of T-bet, GATA-3, ROR-γt and IL-17 gene was determined by RT-PCR. ELISA assay was used to detect the contents of IL-17, and Western blot assay was applied to investigate the p38 mitogen-activated protein kinase (MAPK) activation. RESULTS: Artesunate could inhibit ConA-induced T lymphocyte and LPS-induced B lymphocyte proliferation in vitro significantly, which also showed lower toxicity than dexamethasone. Meanwhile, topical application of artesunate could both suppress the increase on ear thickness in DTH mice and inhibit the thymus index and spleen index. Furthermore, artesunate was found to regulate the expression level of transcription factor (T-bet in the GATA-3), and down-regulate ROR-γt and IL-17 expression. What's more, the production of IL-17 was decreased in ear tissue. Finally, artesunate was observed to decrease p38 mitogen-activated protein kinase (MAPK) activation. CONCLUSION: Artesunate, as a new immunomodulatory agents, might contribute to performing immunosuppression through modulating the balance of treg/Th17 and anti-inflammatory.
3.Research progress of intraoperative floppy iris syndrome during phacoemulsification

? Intraoperative floppy iris syndrome is a particular symptom in phacoemulsification, and it is often occurred in patients with the history of using alpha-1 adrenergic antagonist before cataract surgery. Although the pupil be fully mydriatic before the operation, the progressive constricting of the pupil, the relaxing of iris even the iridoptosis and the unstable anterior chamber still can be found during the phacoemulsification. If treated inappropriately, severe complications and terrible outcomes may happen, which would influenced the surgery effectives. This paper reviewed the epidemiological characteristics, pathogenesis, risk factors, clinical manifestations, complications, and coping strategies of the intraoperative floppy iris syndrome.

5. Hypoglycemic effect and mechanism of raspberry ketone on diabetic model mice

OBJECTIVE: To observe the hypoglycemic effect of raspberry ketone and its mechanism on diabetic model mice induced by alloxan. METHODS: Healthy male KM mice were used to establish diabetic models by injecting alloxan via tail vein, and then randomly divided into model control group, metformin group (90 mg · kg-1), and raspberry ketone low, middle and high (200, 400, 800 mg · kg-1) dose groups. Normal control group was set. All groups had been treated for 20 d. Fasting blood glucose (FBG) was measured on 0, 7 and 14 d. After 20 d, glucose tolerance test was carried out. Fast insulin (FINS) of the mice were measured, and the insulin sensitivity index(ISI) was calculated by determining the contents of FBG and FINS. Superoxide dismutase (SOD), glutathione peroxidase (GSH-Px), and malonaldehyde (MDA) in serum were measured. Pathological changes of pancreas and insulin expression were examined. RESULTS: Raspberry ketone could effectively control the increase of fasting plasma glucose (P < 0.05) and reduce the role of the area under the glucose curve. Compared with the model group, the levels of FINS, SOD and GSH-Px activity were significantly improved (P < 0.05), and MDA content was decreased (P < 0.05) in the high dose raspberry ketone group. The pathological symptoms of pancreas were relieved in the high dose group. CONCLUSION: Raspberry ketone can effectively control blood glucose, protect pancreatic islet β cells, and improve insulin secretion in diabetic mice by effectively inhibiting the oxidative stress.

8.Clinical analysis of posterior capsular rupture in small incision non -phacoemulsification cataract surgery

AIM:To analyze the cause of posterior capsular rupture in small incision non - phacoemulsification cataract surgery and investigate the treatment methods.
METHODS: The clinical data of 108 cases ( 121 eyes ) undergone small incision non - phacoemulsification cataract surgery combined with intraocular lenses ( IOL) implantation were retrospectively analyzed. The causes of posterior capsular rupture and treatment of 6 patients (6 eyes) after surgery were analyzed and summarized.
RESULTS: Six cases ( 6 eyes, 4. 96%) occurred posterior capsular rupture during the operation, of which 2 cases ( 2 eyes ) underwent posterior chamber IOL implantation intraoperative, 2 cases (2 eyes) receivedIIstage IOL implantation in the ciliary sulcus at 1wk postoperative, 2 cases ( 2 eyes ) were given anterior chamber IOL implantation. One case ( 1 eye ) whose vision acuity <0. 1 was macular degeneration; One case ( 1 eye ) whose vision acuity = 0. 3 was diabetic retinopathy;0. 4~0. 6 in 2 cases (2 eyes), 0. 6~0. 8 in 2 cases (2 eyes) after 1 ~3mo postoperatively. No severe syndrome was found after surgery.
CONCLUSION:Posterior capsular rupture which occurs in different stages of the operation is the commonest intraoperative complication in small lincision non -phacoemulsification cataract surgery. With improving operative techniques, and continuously accumulating experience, incidence of posterior capsular rupture can be effectively reduced. Even posterior capsular rupture occurs intraoperative, patients can still get a good outcome after timely and proper treatment.

Objective To develop fluorescence quantitative reverse transcription polymerase chain reaction (FQ-RT-PCR) for detection and quantitation of transforming growth factor ?1 (TGF-?1) mRNA in peripheral blood mononuclear cell (PBMC) of patients with chronic hepatic disease.Methods The TGF-?1 recombined plasmid was constructed by conventional molecular biological techniques,and strand-specific cRNA standard was synthesized by T7 RNA polymerase in vitro transcription.The cRNA standard and a TaqMan-MGB probe were used for quantitation of the TGF-?1 mRNA,and the assay was evaluated.Moreover,the plasma TGF-?1 was detected by ELISA.Results Sequence analysis indicated that the recombined plasmid contains the specific 102bp fragment of TGF-?1. The FQ-RT-PCR could detect as low as 6.81 copies of standard cRNA,the linear range was from 6.81 to 6.81?10~9 copies,and the coefficient variation was 1.28%-2.27% and 2.56%-2.61% respectively in intra and inter-assay.The levels of TGF-?1 mRNA in PBMC and plasma TGF-?1 of patients with chronic hepatic disease were significantly higher than that of healthy controls(P

Objective To observe the changes of pulmonary oxidative stress after cigarette smoking exposure,its re-lationship with inflammatory cytokines,and the effects of smoking cessation. Methods Fifty male BALB / c mice were randomly divided into the smoke exposure group,smoke cessation group,and the controls. Mice in smoke cessation group were exposed to cigarette smoking for 16 weeks. On 4,8,and 12 week after smoking cessation mice were executed and the bronchoalveolar lavage fluid(BALF)and lung tissue were collected. The morphologi-cal alternations of lung tissue were observed. Mean length of interval and mean alveolar number were measured. Total cell numbers in BALF were counted. Superoxide dismutase(SOD)activity was measured with hydroxylamine method,malondialdehyde(MDA)level was measured with TBA method. The levels of pulmonary interleukin-8 (IL-8)and tumor necrosis factor-α(TNF-α)in BALF and lung tissue homogenate were measured with ELISA. Re-sults Compared with the mice in the controls,emphysematous changes were remarkable in the lung of cigarette ex-posed mice,the total cell numbers in BALF were increased significantly(P < 0. 05)and reduced gradually after smoking cessation(P < 0. 05). SOD and MDA levels increased remarkably in the cigarette exposure group(P <0. 05),and declined gradually after smoking cessation. The levels of IL-8 and TNF-α in BALF and lung tissue ho-mogenate in the smoke exposure group increased significantly( P < 0. 05),and lowered time-dependently after smoking cessation,but not reached to normal level even 12 weeks after smoking cessation. SOD and MDA levels were positively correlated with the cytokine changes. Conclusion Abnormal oxidative stress in the airways caused by cigarette smoking exposure was merely partially reversed after smoking cessation. And the inflammation remains persistent concomitantly.
