Chinese Journal of Endemiology 2012;31(5):556-558
Objective In order to understand the Sanya of iodine deficiency disorders(IDD) prevention and cure, and to provide a scientific basis for formulating corresponding control measures. Methods According to the requirements of Iodine Deficiency Disorders Monitoring Program, one townships(districts) in Sanya of Hainan in 2011 were selected based on their positions of east, south, west, north and center, respectively.Four administrative villages in each township(district) were selected in the same way.Fifteen salt samples,8 urine samples of women of childbearing age and 1 drinking water sample from each adninistrative village were collected. In the primary school of each township(district),40 students aged 8 to 10 were selected to check up their thyroid, test their intelligence quotient (IQ) and urinary iodine.Thirty three fifth-grade students in each of the primary schools were selected to launch a questionnaire survey of health education knowledge and test(semiquantitative test) their household edible salt.Salt iodine was detected by direct titrimetry; urinary iodine and water iodine were detected by As-Ce catalytic spectrophotometry; and children's IQ values were measured using the Combined Raven Test (CRT) in rural edition. Results Totally 300 salt samples were tested, and the median salt iodine was 31.0 mg/kg.The iodized salt coverage rate was 96.67% (290/300),and the consumption rate of qualified iodized salt was 96.00% (288/300).Totally 200 urine samples were tested, and the median urinary iodine was 194.6 μg/L;and the range of urinary iodine was between 18.50 μg/L and 655.10 μg/L,the proportions of less than 50 μg/L and between 50 μg/L and < 100 μg/L were 4.00% (8/200) and 13.50% (27/200),respectively.A total of 200 children were examined by palpation, and the goiter rate was 1.00%(2/200).Mean IQ value of the students was 97.66 and there were 8 students whose IQ values were lower than 69 and 20 students whose IQ values were between 70 and 79.The average score was 3.24 of the 165 students who took part in the questionnaire survey of health education knowledge. There were 43 students who failed in the exam, and the failed rate was 26.06%.The coverage rate of iodized salt in household was 96.36% (159/165).The median of urinary iodine of the 160 women of childbearing age was 162.95 μg/L,the range of urinary iodine were between 9.50 μg/L and 908.80 μg/L,and the proportion of less than 100 μg/L was 19.38% (31/160).The consumption rate of eligible iodized salt was 96.90% (155/160).The levels of water iodine of the 20 water samples from the 5 townships(districts) were all less than 10.0 μg/L.Conclusions Sanya is still in the area lack of iodine in the environment. In general Sanya has reached the goals of elimination of iodine deficiency disorders, but there are still some of the children and women of childbearing age who are iodine deficient.

Objective To clarify whether the intravenous administration of embryonic stem cells(ESCs)could home in on injured myocardium caused by acute myocardial infarction(MI)in rats and to improve the cardiac function afterwards, and to explore the possible effect of inflammatory factor TNF-?on it.Methods ESCs were cultured in vitro and transfected with green fluorescence protein(GFP).The model of heart failure post MI in rats was established by permanent ligation of left coronary anterior descend artery. After operation the rats were administered with 10~7 ESC via tail vein every other day for 6 days. The sham and MI without ESCs infusion groups served as control.Six weeks later, the hemodynamic measurement was employed to evaluate the cardiac function.Immunohistochemistry assay was used to detect the GFP expression in heart and other organs, and the cardiac specific protein Tropinin I(Tn I)expression in GFP positive spot of heart.Meanwhile, the co-culture in vitro of ESC and neonatal rat cardiomyocytes was used to observe the effect of overexpression of TNF-?in cardiomyocytes on ESC migration. Results Six weeks after MI,cardiac function was significantly improved in rats administered with ESC in comparison with those of MI group.Histological changes demonstrated that infused ESCs formed GFP-positive grafts in infracted myocardium.Alternatively, the positive immunostaining for Tn I was found in the area corresponding to GRP positive staining. In regard to other organs, only in spleen a few GFP positive cells were found. This would indicate that circulating ESC could translocate to infracted myocardium and to form cardiac tissue.Migration assay of ESC in vitro indicated that cultured cardiomyocytes with overexpression of TNF-aobviously enhanced the migration of ESCs in comparison with cardiomyocytes without transfection. Conclusion Intravenously infused ESC could home in on infarcted myocurdium and futher differentiate into cardiomyocytes which led to the improvement of cardiac function. In the setting of acute MI the homing mechanism Could be associated with locally released inflammatory factors such as TNF-?which may play as a chemotactic agent on circulating ESC.

Objective To investigate the safety, efficacy and effects of emergent percutaneous coronary intervention (PCI) in patients from Second Affiliated Hospital,Medical College,Zhejiang University with cardiogenic shock (CS) complicating acute myocardial infarction (AMI).Method Twenty-seven patients with CS complicating with AMI were treated by PCI with intraaortic balloon counterpulsation (IABP) support. The change of hemodynamics before and after IABP and PCI,the characteristics of PCI,the mortality during hospitalization, the major adverse cardiac events (MACE) and left ventricular ejection fraction at 30-day follow-up were observed. Results The hemodynamics were significantly improved after IABP.No patients died during PCI.Two patients died after PCI and the total mortality was 7.4% in hospital.During the period of 30-day follow-up, one patient died of heart failure. The left ventricular ejection fraction greatly improved at 30 days after PCI. Conclusions The data suggested that the use of IABP during PCI in patients with CS complicating AMI was safe, decreased mortality and improved prognosis.

9.Clinical observation of sitagliptin combined with insulin aspart 30 with in the treatment of secondary failure of sulphonylurea in type 2 diabetes mellitus
Chinese Journal of Postgraduates of Medicine 2016;39(3):258-260
Objective To observe the effect of sitagliptin combined with insulin aspart 30 in the treatment of secondary failure of sulphonylurea in type 2 diabetes mellitus. Methods Fifty-six cases were divided into group A and group B in random block design, with 28 cases of each group. The patients in group A was treated with sitagliptin combined with insulin aspart 30, while the patients in group B was given subcutaneous injection of insulin aspart 30R. All patients were treated for 12 weeks. Fasting plasma glucose(FPG), 2-hour postprandial plasma glucose(2 hPG), glycosylated hemeglobin(HbA1c), insulin secretion index (HOMA-β), body mass index (BMI), and incidence of low blood glucose before and after treatment were compared. Results Compared with that in group B, FPG [(5.61 ± 1.14) mmol/L vs. (7.8 ± 1.22) mmol/L], 2 hPG [(7.62 ± 1.35) mmol/L vs(9.72 ± 1.41) mmol/L] and HbA1c [(7.11 ± 0.83)%vs.(8.32 ± 1.04)%] in group A had a significant decrease;HOMA-β[(50.31 ± 5.12) vs. (41.86 ± 4.53)] of group A was higher than that of group B (P

ve To explore the mutagenicity of selenium and lead, the antagonistic effects of selenium on lead. Methods The ICR mice were orally perfused with sodium selenite (Na2SeO3) at different concentrations of 2.5,5.0 and 10 mg/kg and lead acetate [Pb(Ac)2] at different concentrations of 5,10 and 25 mg/kg jointly and con-tinuously for 3 days, and then the numbers of bone marrow polychromative erythrocytes were counted. Results The frequencies of micronucleus of mice only exposed to 10 mg/kg Na2SeO3, 10, 25 mg/kg Pb(Ac)2 respectively showed higher levels compared with those of control (P
