3.Investigation on compliance of anti-platelet therapy after coronary stent implantation

Objective To explore compliance of anti-platelet therapy of patients after coronary stent implantation and relative factors. Methods 75 patients after coronary stent implantation were investigated by APGAR and drug compliance evaluation scale. Results 44.0 % of patients (n =33) failed to take anti-platelet drugs exactly following the prescription, the main form is omitting to take anti-platelet drugs (28.0%). The total score of understanding about antiplatelet medication knowledge in 75 patients was (26.36 ± 2.85 ), they took not much knowledge about antiplatelet druys' side effects and adverse consequences. The median adherence score of compliamce of antiplatelet drugs was 15.61. Fully accounted for 56.0%. The main non-complicance behavior was missed 28.0%. Different time of stent placement, antiplatelet druy type, duration of patient understanding of medication and medication compliance in patients with family functioning were statistically significant differences ( P < 0. 05 ) ; Medication compliance and stent placement time was negatively correlated (r =-0. 304,P < 0. 01 ), and duration of antiplatelet drug use to understand the situation, family functioning was positively correlated (r =0. 259,0. 266 ; P < 0. 05 ). Conclusions The rate of good compliance of antiplatelet therapy of patients after stent implantation is much lower. Nurses should emphasize the education about anti-platelet therapy and start family support system in order to improve the compliance of anti-platelet therapy.
6.Primary bone lymphom:report of 7 cases and literature review

To explore the clinic characters,treatments and prognosis of patients with primary bone lym-phoma( PLB ) .The clinical symptoms, signs, X -ray features, pathological morphology, immunophenotype and treatment of 7 patients with PLB were analyzed retrospectively and the pertinent literatures were reviewed.The re-sults showed that the main complains of 7 cases of PBL were local pain.The CT showed osteolytic bone destruc-tion and soft tissue swelling.There were 3 cases of diffuse large B cell lymphoma,1 case of Burkitt-type lympho-ma,1 case of periferal T-cell lymphoma,1 case of anaplastic large cell lymphoma,and 1 case of Hodgkins lym-phoma.2 patients presented with stageⅠ,4 with stageⅡ,and 1 with stage 3.The therapeutic procedure includes local radiotherapy combined with chemotherapy and targeted therapy.The clinical presentation of PLB is not spe-cial.The diagnosis and identification of histological type of PLB should be established by histopathological and im-munohistochemistry examinations.Early diagnosis and active therapy could improve the prognosis of PLB.Combi-nation therapy including radiotherapy and chemotherapy is the optimal treatment for PLB.
7.Clinical study of cardiovascular abnormalities with Down′s syndrome in 96 children

Objective To investigate the spectrum,diagnosis,time of therapy and management of the congeni-tal heart disease(CHD)in patients with Down′s syndrome(DS).Methods A retrospective report was undertaken of 96 cases in children with DS accompanied by CHD in Department of Pediatric Cardiology,Beijing Anzhen Hospital Af-filiated to Capital Medical University.Data were collected and analyzed about their clinical characteristics,and types of cardiovascular abnormalities,and the important laboratory examinations such as echocardiography and catheterization as well as the procedures of diagnosis and treatments were summarized.Then the interventions,complications and prognosis of different patients were estimated.Results (1)Single congenital heart disease was found in 33 cases (34.38%),a-mong which ventricular septal defect was the most common (14 cases,14.58%),followed by atrioventricular septal de-fect and atrial septal defect (equally,7 cases,7.29%).Multi -cardiovascular abnormalities were discovered in 63 ca-ses,and patent ductus arteriosus turned out to be the most common (42 cases,66.67%).(2)Cat-heterization was car-ried out in 18 cases of serious pulmonary arterial hypertension,and 8 cases were proved resistant pulmonary arterial hy-pertension without operation opportunity.The other 8 cases were estimated as high pulmonary arterial hypertension and medical therapy was suggested before reassessment to reduce surgical risks.(3)Operations were undertaken in 61 ca-ses,among which percutaneous interventional occlusion was performed in 7 cases and surgical interventions were per-formed in 54 patients,in which perioperation complications and death were found in 5 cases and 4 cases,respectively. Conclusions Operation interventions are practicable and most cases recovered well with systematic examinations and assessment in patients with DS and cardiovascular malformations.Early diagnosis and timely interventions are highly suggested.Also close attentions should be paid to follow -up and re -estimation after medical therapy.
8.Effect of central retinal artery occlusion on retinal microstructure of macula in vivo

AIM: To investigate the effect of ischemia induced by central retinal artery occlusion on retinal microstructure of macula using optical coherence tomography (OCT). METHODS: Fourteen eyes of 14 patients with unilateral central retinal artery occlusion (CRAO) in two to three days without fully recovery of retinal circulation underwent OCT examination with 4.5 mm length horizontal and vertical line scans through foveola to measure the retinal neurosensory layer (RNL) thickness on foveola, 175 ?m (fovea), 750 ?m (macula) to foveola, respectively. The other normal eyes of patients as control group underwent the same examination and measurement. RESULTS: The mean RNL thickness(?m) on foveola, fovea, macula were 169 91?10 96, 176 36?11 74 and 256 45?16 95 respectively in normal control eyes, and 235 64?47 02 , 241 84?49 36 and 401 57?54 53 respectively in CRAO eyes with retinal ischemia. Aim To ascertain whether defibrase has the protective effect against reperfusion injury after cerebral ischemia.Methods 70 renovascular hypertensive rats(RHR) were randomly divided into defibrase group, control group and sham-operated group.Reversible middle cerebral artery occlusion(MCAO) models were produced by the modified. Longa's method,and reperfusion was begun 2 hours after occlusion.Rats in the defibrase group were given defibrase 10 U?kg-1 body weight via femonal intraveneous injection, and in the control group with the same amount of saline. The brain pieces were processed by TTC and HE staining and the infarct size,brain microvessels damage and secondary bleeding were compared between the two groups. Results The volume of infarction in the defibrase group was obviously smaller than in the control group, the damage of brain microvessels was less severe, and the bleeding lesions under optical microscope were less than in the control group. Conclusion Defibrase has protective effect against reperfusion injury post cerebral ischemia.
