1.Reversal effect of delavirdine on multidrug resistance-associated protein 2-mediated multidrug resistance

Objective To study the effect and the possible mechanism of delavirdine on muhidrng resist-ance-aasociated protein 2(MRAP2)-mediated multidrug resistance in LLC/cMOAT cells. Methods MTT assay was used to determine the effects of delavirdine on proliferation of LLC/CMV and LLC/cMOAT cells. The inhibitory effects of vincristine (VCR),cisplatin (DDP),adriamycin (ADM) and etoposide (VP-16) used alone or in combi-nation with delavirdine on the proliferation of LLC/CMV and LLC/cMOAT cells were evaluated by MTT assay. The cell cycle distribution, the apoptosis rate of the cells treated with different concentration of delavirdine and the intra-cellular concentration of ADM were determined by flow cytometry (FCM). Results Delavirdine at the concentration of 4 μmol/L and below showed no significant cytotoxicity to LLC/CMV and LLC/cMOAT cells. The resistance of LLC/cMOAT cells to VCB, VP-16, ADM and DDP were 9.58,1.11,2.98 and 3.96 folds of that of LLC/CMV cells. When 2 μmol/L delavirdine was added, the resistance was 5.21 and 2.55 folds respectively;when 4 μmol/L dela-virdine was edded,the resistance was 7.56 and 3.03 ,while 2,4 μmol/L delavirdine made no significant changes to the chemosensitivity of LLC/CMV cells to VCR and DDP(P>0.05). Cellular cycle analysis demonstrated that 0,6, 12,24 hours after co-cultured with delavirdine the amount of cells at G1 phase increased from(38.92±0.15)% to (56.87±2.23)%,(65.36±2.76)% and (74.77±5.06)%. The cell apoptosis rate increased from 1.77% to 17.45% and 28.52% when treated with delavirdine at 2,4 μmol/L and VCR for 24 h. When treated with 2,4 μmol/L delavirdine, ADM accumulation in LLC/cMOAT cells was enhanced significantly(P<0.05). Conclusion DLV can partially reverse the multidrng resistance of LLC/cMOAT cells, and the reverse effect correlates to the concentration. The possible mechanism may involve the growth arrest at G1, increasing of intracellular drug concen-tration and promoting apoptosis.
2.Effects and mechanisms of Rapamycin on renal interstitial fibrosis in rats

Objective To discuss the intervention effects and mechanisms for Rapamycin on renal interstitial fibrosis in rats.Methods A total of 72 Wistar male rats were randomly divided into 3 groups,normal group(n =24) ,model group(n =24), treatment group (n =24).The model group and treatment group received adenine 200 mg/kg daily,and the treatment group was also given Rapamycin 5 mg/(kg·d) at the 8th day,the normal group was just given the same amount of normal saline for 6 weeks.In the end of the 2nd,4th and 6th week,8 rats in each group were sacrificed respectively.The expression of hepatocyte growth factor(HGF), transforming growth factor-β (TGF-β) and neutrophils gelatinases related apolipoprotein (NGAL) in each group were observed.The software of image analysis system was used for semi-quantitative analysis.Results HE and Masson staining results showed that the renal tubular were progressive swelling, and changed with interstitial fibrosis,atrophy and even necrosis in model group from 2 weeks to 6 weeks.The pathological changes of kidney were more ease in the treatment group compared with those in model group.Immunohistochemical staining results showed that HGF expression levels of renal interstitial tissue in model group and treatment group at the 2nd week were significantly higher than those of normal group(P <0.05), and were significantly decreased at the 4th week and 6th week (P < 0.05);HGF expression levels of renal interstitial tissue in treatment group were significantly higher than those in model group (P < 0.05).NGAL expression levels of renal interstitial tissue in model group and treatment group at the 2nd week were significantly higher than those in normal group(P <0.05) ,and were significantly decreased at the 4th week and 6th week(P < 0.05);NGAL expression levels of renal interstitial tissue in treatment group were significantly higher than those in model group (P < 0.05).TGF-β expression levels of renal interstitial tissue in model group and treatment group at the 2nd,4th,and 6th week were significantly higher than those in normal group (P <0.05) ,while TGF-β expression levels of renal interstitial tissue in treatment group were significantly lower than those in model group (P < 0.05).Conclusion Rapamycin could improve the rat kidney tissue pathology, relieve renal tubular expansion, and slow progression of renal interstitial fibrosis, and has certain protective effect to the kidney.
3.Application of tunnel and penetrating ureteroneocystomy in kidney transplantation

Objective To investigate the new method of ureter bladder anastomosis in renal transplantation.Methods The ureter was pulled into bladder for 1.2 cm through tunnel at the lateral- top of bladder wall,and the ureter fixed on the bladder wall by 2-3 acus with catgut suture.Results Forty of 42 patients had no complications,and recovered very well,except for 1 cases of necrosis caused by acute rejection and 1 case on urine leakage caused by catheter obstruction from blood clot.Conclu- sion This method is simple,easy to operate,safe and reliable with less complications.
4.Optimizing extraction process of Chonglian oral solution by orthogonal test

ObjectiveTo optimize Chonglian oral solution extracting craft. MethodsWith the obtaining rate of extract and the total content of the Pariphyllin Ⅰ and Phillyrin presented in the extract as the indexes for the water extraction process,and with the total content of the Pariphyllin Ⅰ and Phillyrin presented in the extract as the indexes for the alcohol deposition process,orthogonal design was used to optimize the conditions for the extraction process respectively. ResultsThe optimal conditions for the water extraction of Chonglian oral solution was as following:to add water 10 times,decocting 3 times,1 hour each time.The optimal conditions for the alcohol deposition of Chonglian oral solution was as following:concentrated for the relative density to 1.10(80C hot test),cold,add ethanol to the solution for the ethanol content of the solution reached 80%,static settlement for 24 hours. ConclusionThe extracting method is reasonable,stable,and suitable to industrialized producting.
5.Study on Quality Control Methods of Yuling Tea

Objective To establish the quality standard of Yuling Tea for its quality control. Methods Astragalus, Fructus cnidii L, Schisandra chinensis and Lycii Fructus in Yuling Tea were identified by TLC. The content of Schisandrin was determined by HPLC. Welch Materials C18 column (250 mm×4.6 mm, 5 μm) with mobile phase of methanol-water (70∶30) was used. The detective wavelength was 250 nm. Results The TLC for identification was simple and special. Schisandrin showed a good linear relationship in 0.222-0.222 0 μg, r=0.999 9. The average recovery was 98.96%, and RSD was 0.74%. Conclusion The method can accurately determine the content of Schisandrin in Yuling Tea. It can be used for quality control of the preparation.
6.Clinical observation of low molecular weight heparin combined with astragalus injection in the treatment of diabetic nephropathy

Objective To observe the effect of low molecular weight heparin calcium combined with astraga-lus injection in the treatment of diabetic nephropathy.Methods From January 2013 to December 2014,78 patients with stage III diabetic nephropathy in our hospital were enrolled into the study.They were randomly divided into con-trol group(41 cases) and treatment group(37 cases).The control group received general therapy,such as low protein diets,compound alpha keto acid,insulin,irbesartan,simvastatin.The treatment group was treated with low molecular weight heparin calcium and astragalus injection on the basis of general therapy.The changes of urinary albumin and 24 hours urinary protein after treatment in the two groups were observed.Results After treatment,the negative rate of urinary micro albumin in the treatment group ( 75.68%) was significantly higher than that of the control group (29.27%)(χ2 =16.765,P<0.01).24 hour urinary protein of the treatment group[(176.2 ±82.3)mg/24h]was significantly lower than that of the control group[(223.8 ±87.8)mg/24h](t=2.471,P<0.05).Conclusion Low molecular weight heparin calcium combined with astragalus injection can reduce the proteinuria of diabetic nephropa-thy,it is an effective intervention therapy for early diabetic nephropathy.
7.Simultaneous Determination of Three Marker Components in Fufang Buwu Syrup by TLCS

Objective To establish an accurate method for determining the content of three components in Fufang Buwu Syrup. Methods TLC scanner was selected to detect three components with silica gel G thin layer plate. The sample was separated by using cyclohexane-ethyl acetate-methylenechloride-formic acid (3∶1∶1∶0.2),λS=300 nm. Results The linearity between peak area and ferulic acid was achieved in the range of 0.36-0.84μg, psoralen was achieved in the range of 0.12-0.28 μg, emodin was achieved in the range of 0.01-0.05 μg. The average recovery was 100.7%, 100.8%, 101.0%, and RSD was 1.26%, 1.44%, 1.86%, respectively. Conclusion The method is simple and accurate, which can be used for quality control of Fufang Buwu Syrup.

10.Risk Factors of Diabetic Foot Patients with Type 2 Diabetic Mellitus:62 Case Report

Objective To investigate the clinical characteristics and risk factors of diabetic foot (DF) patients with type 2 diabetic mellitus.Methods The data of 62 DF patients with type 2 diabetic mellitus included age, sex, the duration of disease, the concentrations of HbA1C, hemoglobin (Hb), blood-lipids, electromyogram, CDFI of lower extremities artery were analyzed retrospectively and compared with 60 patients with type 2 diabetic mellitus, but without DF.Results The factors of age, sex, Hb, albumin, electromyogram, CDFI of lower extremities artery were significantly different between two groups ( P<0.05), but no significant difference in the duration of disease, fasting blood-glucose, HBA1C, glomerular filtration rate, blood-lipids ( P>0.05).Conclusion The senility, male, peripheral neuropathy and peripheral vessel disease are the risk factors of DF with type 2 diabetic mellitus, and anemia and hypoproteinemia are often occurred in such patients.
