Objective To evaluate the effects of applying clinical pathway in clinic teaching of kidney diseases. Methods Data of average in-hospital days, costs, percentage of antibiotic drugs, improvement rate and satisfaction of patients who received the clinic pathway in nephrology depart-ment from January 2010 to December 2012 were used to evaluate the quality of clinical pathway and were compared with those of patients not received clinic pathway in 2009. Teaching effects of eight-year clinic medical students whose scores were provided by internal medical teaching section were al-so analyzed. Questionnaire of hospital quality was adopted to assess satisfaction of patients, nurses and doctors and the results were compared with those of 2009 students. All data were analyzed by SPSS 13.0 software. Chi-square test was used for enumeration data and results were expressed as percentage while t test was used for measurement data and results were expressed as (x±s),with P<0.05 stand-ing for statistically significant differences. Results After applying clinical pathway, average in-hos-pital days were reduced from 10.7 d in 2009 to 8.7 d in 2012,percentage of medical drugs were de-creased from 86.6% in 2009 to 33.4% in 2012 and percentage of antibiotic drug were reduced from 79.1%to 23.0%. Average in-hospital days and costs of IgA nephropathy and lupus nephritis were de-creased. Satisfaction of patients was increased from 94.1% to 99.0% and scores of students were in-creased from (81.0 ±2.5)points to (89.5 ±2.8)points. Conclusions Application of clinical path-way in clinic medical teaching should be encouraged. It can enhance the teaching quality and is help-ful for culturing inno-vative clinic thinking of medical students and cultivate practical medical talents.
3.The measurement of transforming growth factor ?1 mRNA in peripheral blood mononuclear cell of patients with chronic hepatic disease by fluorescence quantitative reverse transcription polymerase chain reaction
Chinese Journal of Laboratory Medicine 2003;0(08):-
Objective To develop fluorescence quantitative reverse transcription polymerase chain reaction (FQ-RT-PCR) for detection and quantitation of transforming growth factor ?1 (TGF-?1) mRNA in peripheral blood mononuclear cell (PBMC) of patients with chronic hepatic disease. Methods The TGF-?1 recombined plasmid was constructed by conventional molecular biological techniques, and strand-specific cRNA standard was synthesized by T7 RNA polymerase in vitro transcription. The cRNA standard and a TaqMan-MGB probe were used for quantitation of the TGF-?1 mRNA,and the assay was evaluated. Moreover, the plasma TGF-?1 was detected by ELISA.Results Sequence analysis indicated that the recombined plasmid contains the specific 102bp fragment of TGF-?1. The FQ-RT-PCR could detect as low as 6.81 copies of standard cRNA,the linear range was from 6.81 to 6.81?10~9 copies, and the coefficient variation was 1.28%-2.27% and 2.56%-2.61% respectively in intra and inter-assay.The levels of TGF-?1 mRNA in PBMC and plasma TGF-?1 of patients with chronic hepatic disease were significantly higher than that of healthy controls(P

Objective To seak a available way of evaluating morphology of filtering blebs after trabeculectomy is very important for forecasting the successful rate of surgery. At the present time, filtering blebs were commonly evaluated by using the slim lamp microscope, but it did not to exactly reflect the filtering passage. This study aimed to evaluate the morphology of filtering blebs after trabeculectomy with anterior segment optical coherence tomography (AS-OCT).MethodsThis was a prospective study.Sixty-nine eyes of 53 patients who had previously undergone trabeculectomy were selected in this study, including 25 eyes with open-angle glaucoma,38 eyes with close-angle glaucoma and 6 eyes with secondary glaucoma. These filtering blebs were classified into functional type (typeⅠandⅡ) and nonfunction type (type Ⅲ and Ⅳ) under the slit lamp microscope based on the van Buskirk grading scales.Intra-bleb morphology and structure were characterized as diffuse-like,cystic-like,encapsulating-like and flatten-like by AS-OCT in reference to the Leung method. The consistency between the two methods was evaluated by the Chi-Square test.Written informed consent was obtained from all the patients before surgery.ResultsThe average follow-up time was 10.78±11.0 months. All the patients finished the examination during the follow-up duration. The average intraocular pressure was(14.9±4.5)mmHg during the observation period. There were 38/69 (55.1%) functioning blebs and 31/69 (44.9%) non-function blebs under the slim lamp microscope.AS-OCT imaging showed diffuse-like blebs in 26 eyes (15.9%),cystic-like blebs in 11 eyes (27.5%),encapsulating-like blebs in 19 eyes (18.8%) and flatten-like in 13 eyes with the intraocular pressure of(13.16±3.77)mmHg,(15.36±2.92)mmHg,(15.77±5.07)mmHg and (16.62±5.33)mmHg,respectively,showing a significant difference among them (F=3.32,P<0.05).These patterns of different OCT presented a good consistency with the clinical outcome (χ~2=0.03,P=0.86).ConclusionAS-OCT allows the observation of filtering blebs after glaucoma surgery. It is probably to visualize the internal structure of filtering bleb and deep sclerectomy.Functional and dysfunctional filtering blebs delivery gives different OCT patterns. This could be a new way to assess the postoperative healing process.

Migraine is one of the common and frequently encountered diseases. The study proves that 5-hydroxytryptamine (5-HT) receptor, plays an important role in the occurrence of migraine. Rat striatum was used for preparation of the cell membrane stationary phase (CMSP) in our experiments. The cell membrane chromatography (CMC)-offline-HPLC system was applied to specifically recognize the components from the drug pair of Chuanxiong Rhizoma and Angelicae Dahuricae Radix, which interact with the receptors on CMSP. The dissociation equilibrium constant (KD) was measured in a rat striatum/CMC system, performed by continuously pumping sumatriptan, a 5-HT1D agonist, ranging from 2.42 x 10(-8) to 4.84 x 10(-7) mol · L(-1) through a CMC column, and the capacity factors (k') were recorded. The KD value obtained from the model was (4.59 ± 0.33) x 10(-6) mol · L(-1) for imperatorin, and the rat model of migraine induced by nitroglycerin was applied to validate the pharmacological effects of the drug pair. The results indicated that the CMC method could be a quick and efficient way for characterizing the drug-receptor interactions in vitro.
