3.Historical Overview of Taenia asiatica in Taiwan.

An overview of the epidemiological, biological, and clinical studies of Taenia and taeniasis in Taiwan for the past century is presented. The phenomenal observations that led to the discovery of Taenia asiatica as a new species, which differ from Taenia solium and Taenia saginata, are described. Parasitological surveys of the aborigines in Taiwan revealed a high prevalence of taeniasis, which might be due to the culture of eating raw liver of hunted wild boars. Chemotherapeutic deworming trials involving many patients with taeniasis were discussed. Praziquantel was found to be very effective, but sometimes complete worms could not be recovered from the feces after treatment, probably due to the dissolution of the proglottids. Atabrine, despite some side effects, can still be used, in properly controlled dosages, as the drug of choice for human T. asiatica infection if we need to recover the expelled worms for morphological examinations. Research results on the infection of T. asiatica eggs from Taiwan aborigines in experimental animals were also noted. Since the pig serve as the natural intermediate host of T. asiatica and the predilection site is the liver, a differential comparison of other parasitic pathogens that might cause apparently similar lesions is also presented.

4.Protective Factors of Demoralization among Cancer Patients in Taiwan: An Age-matched and Gender-matched Study.

PURPOSE: This study aimed to explore the protective factors of demoralization in cancer patients via investigation of cancer patients' demographic and disease characteristics. METHODS: This was a cross-sectional descriptive study. We used a structured questionnaire, which contained items on demographic and disease characteristics, as well as the Demoralization Scale Mandarin Version (DS-MV), with a cutoff of 30 or more indicating high demoralization. Data were analyzed with age-matched and gender-matched conditional logistic regression analysis. For the study, 428 questionnaires were delivered and 411 were recovered. After being age-matched and gender-matched, 182 participants of high demoralization (DS-MV > 30) and low demoralization (DS-MV ≤ 30) were obtained respectively, for a total of 364 participants. RESULTS: Cancer patients' demoralization was significantly related to family support (p = .019), education (p = .049), and monthly income (p = .001). Family support [odds ratio = 0.38; p = .028; 95% confidence interval (0.16, 0.91)] and monthly income [odds ratio = 0.49; p = .009; 95% confidence interval (0.29, 0.84)] were protective factors of demoralization in cancer patients. CONCLUSION: Early and appropriate demoralization assessment of cancer patients' demographic and disease characteristics is very important in clinical settings. Healthcare providers might regularly monitor demoralization in cancer patients, and develop related nursing care guidelines or treatment for demoralization in cancer patients. The study results can be a reference for healthcare providers who work with cancer patients.

5.Low-power laser irradiation inhibits arecoline-induced fibrosis:an in vitro study

Oral submucous fibrosis (OSF) is a potentially malignant disorder that is characterized by a progressive fibrosis in the oral submucosa. Arecoline, an alkaloid compound of the areca nut, is reported to be a major aetiological factor in the development of OSF. Low-power laser irradiation (LPLI) has been reported to be beneficial in fibrosis prevention in different damaged organs. The aim of this study was to investigate the potential therapeutic effects of LPLI on arecoline-induced fibrosis. Arecoline-stimulated human gingival fibroblasts (HGFs) were treated with or without LPLI. The expression levels of the fibrotic marker genes alpha-smooth muscle actin (α-SMA) and connective tissue growth factor (CTGF/CCN2) were analysed by quantitative real-time reverse transcription polymerase chain reaction (RT-PCR) and western blots. In addition, the transcriptional activity of CCN2 was further determined by a reporter assay. The results indicated that arecoline increased the messenger RNA and protein expression of CCN2 and α-SMA in HGF. Interestingly, both LPLI and forskolin, an adenylyl cyclase activator, reduced the expression of arecoline-mediated fibrotic marker genes and inhibited the transcriptional activity of CCN2. Moreover, pretreatment with SQ22536, an adenylyl cyclase inhibitor, blocked LPLI's inhibition of the expression of arecoline-mediated fibrotic marker genes. Our data suggest that LPLI may inhibit the expression of arecoline-mediated fibrotic marker genes via the cAMP signalling pathway.
6.The Influence of Pituitary Adenoma Size on Vision and Visual Outcomes after Trans-Sphenoidal Adenectomy: A Report of 78 Cases.

OBJECTIVE: The aims of this study were to investigate the quantitative relationship between pituitary macroadenoma size and degree of visual impairment, and assess visual improvement after surgical resection of the tumor. METHODS: The medical records of patients with pituitary adenoma, who had undergone trans-sphenoidal adenectomy between January 2009 and January 2011, were reviewed. Patients underwent an ocular examination and brain MRI before and after surgery. The visual impairment score (VIS) was derived by combining the scores of best-corrected visual acuity and visual field. The relationship between VIS and tumor size/tumor type/position of the optic chiasm was assessed. RESULTS: Seventy-eight patients were included (41 male, 37 female). Thirty-two (41%) patients experienced blurred vision or visual field defect as an initial symptom. Receiver operating characteristic curve analysis showed that tumors <2.2 cm tended to cause minimal or no visual impairment. Statistical analysis showed that 1) poor preoperative vision is related to tumor size, displacement of the optic chiasm in the sagittal view on MRI and optic atrophy, and 2) poorer visual prognosis is associated with greater preoperative VIS. In multivariate analysis the only factor significantly related to VIS improvement was increasing pituitary adenoma size, which predicted decreased improvement. CONCLUSION: Results from this study show that pituitary adenomas larger than 2 cm cause defects in vision while adenomas 2 cm or smaller do not cause significant visual impairment. Patients with a large macroadenoma or giant adenoma should undergo surgical resection as soon as possible to prevent permanent visual loss.

7.Effects of occupation, life style and genetic polymorphism of CYP1A1, GSTM1, and GSTT1 on urinary 1-hydroxypyrene and 2-naphthol concentration.

OBJECTIVES: This study was performed to describe the distribution patterns of urinary 1-hydroxypyrene (1-OHP) and 2naphthol concentration in coke oven workers and workers not occupationally exposed to polycyclic aromatic hydrocarbons (PAH), and to determine the effects of occupation life style, and genetic polymorphism of cytochrome P450 1A1 (CYP1A1), glutathione Stransferase mu 1 (GSTM1) and theta 1 (GSTT1) on urinary 1- OHP and 2-naphthol concentration. METHODS: The study subjects were 19 coke oven workers and 156 shipyard workers. A questionnaire was used to obtain data about detailed smoking and food intake history. Urinary 1OHP and 2naphthol concentration and genetic polymorphism of CYPIA1, GSTM1, and GSTT1 were analyzed. RESULTS: The urinary 1-OHP and 2-naphthol concentration was higher in the coke oven workers and in smokers. Urinary 1-OHP concentration was significantly correlated with time after last intake of roasted meat in non-smoking coke oven workers, whereas urinary 2-naphthol concentration was with amount of cigarette smoking at the sampling day in smoking shipyard workers. Urinary 1-OHP, but not 2-naphthol, concentration of the shipyard workers with Ile/Ile type of CYP1A1 vas significantly lower than that of the shipyard workers with other CYP1A1 genotype. CONCLUSIONS: Urinary 1-OHP would be a better marker for occupational exposure to PAH in coke oven workers, and urinary 2naphthol might be better for non-occupational inhalation exposure to PAH. CYP1A1 would not play an important role in the metabolism of naphthalene but in the metabolism of pyrene.

8.Neurogenic heterotopic ossification after a stroke: diagnostic and radiological challenges.

Heterotopic ossification (HO) is the aberrant formation of ectopic bone within the soft tissues, of which the aetiology is usually either traumatic or neurogenic. Neurogenic HO is a known but uncommon complication that occurs after a cerebral or spinal insult. The condition may present with a spectrum of symptoms and is often difficult to diagnose clinically. Although different imaging modalities have been used to diagnose HO, clinicians and radiologists may occasionally encounter radiological features of HO that may mimic other disease conditions. We herein report a rare case of neurogenic HO occurring in the non-paretic limb of a patient, and the diagnostic and radiological challenges encountered.

9.Wide range of normality in deep tendon reflexes in the normal population

Background and objective: Deep tendon reflexes are important physical signs in neurological
examination. Despite being an established technique clinically, there is lack of normality data in the
healthy population, especially among the elderly. This study aims to determine the range of normality
in deep tendon reflexes among the adults. Methods: The study subjects consisted of 176 healthy
volunteers. They were examined by trained assessors using standardized protocol. Results: Among the
commonly elicited deep tendon reflexes, isolated absent reflexes were found in up to 34.4% (supinator)
in the older group, and 12.5% (triceps) in the young adults, significantly higher in the older group.
Symmetrical absent reflexes was seen in up to 26.3% of supinator in the older group. Absent reflexes
of the entire limbs both sides were however, much less common particularly among young adults,
with 6.3% of the older group having absent reflexes in the entire lower limbs, and 2.5% in the entire
upper limbs. Isolated asymmetry in reflex was present in up to 17.0% (triceps). However, only 6.3%
had asymmetrical reflexes in the contiguous anatomical region.
Conclusion: Isolated absent or asymmetry deep tendon reflexes were common particularly in the
elderly.
10.Status of group B streptococcal vaccine development

Streptococcus agalactiae (group B streptococcus, GBS) is a leading causal organism of neonatal invasive diseases and severe infections in the elderly. Despite significant advances in the diagnosis and treatment of GBS infections and improvement in personal hygiene standards, this pathogen is still a global health concern. Thus, an effective vaccine against GBS would augment existing strategies to substantially decrease GBS infection. In 2014, World Health Organization convened the first meeting for consultation on GBS vaccine development, focusing on the GBS maternal immunization program, which was aimed at reducing infections in neonates and young infants worldwide. Here, we review the history of GBS infections, the current vaccine candidates, and the current status of immunogenicity assays used to evaluate the clinical efficacy of GBS vaccines.
