2.Unusual case of lipoma arborescens in the subacromialsubdeltoid bursa

A 38-year-old female presented with a 10-month history of
right shoulder pain with impingement symptoms. She was
diagnosed on magnetic resonance (MR) imaging to have
supraspinatus tendon tear and degenerative changes
contributing to subacromial impingement. She also had
lipoma arborescens of the subacromial-subdeltoid bursa, an
uncommon condition in a particularly rare location. Lipoma
arborescens is a benign intra-articular condition
characterized by lipomatous proliferation of synovium with
replacement of subsynovial tissue by mature adipocytes. It
is typically a monoarticular process affecting the knee. Due
to the presence of pathognomonic fat, diagnosis is usually
straightforward with MR as the preferred imaging modality

Background and objective: Deep tendon reflexes are important physical signs in neurological
examination. Despite being an established technique clinically, there is lack of normality data in the
healthy population, especially among the elderly. This study aims to determine the range of normality
in deep tendon reflexes among the adults. Methods: The study subjects consisted of 176 healthy
volunteers. They were examined by trained assessors using standardized protocol. Results: Among the
commonly elicited deep tendon reflexes, isolated absent reflexes were found in up to 34.4% (supinator)
in the older group, and 12.5% (triceps) in the young adults, significantly higher in the older group.
Symmetrical absent reflexes was seen in up to 26.3% of supinator in the older group. Absent reflexes
of the entire limbs both sides were however, much less common particularly among young adults,
with 6.3% of the older group having absent reflexes in the entire lower limbs, and 2.5% in the entire
upper limbs. Isolated asymmetry in reflex was present in up to 17.0% (triceps). However, only 6.3%
had asymmetrical reflexes in the contiguous anatomical region.
Conclusion: Isolated absent or asymmetry deep tendon reflexes were common particularly in the
elderly.
4.Neurogenic heterotopic ossification after a stroke: diagnostic and radiological challenges.

Heterotopic ossification (HO) is the aberrant formation of ectopic bone within the soft tissues, of which the aetiology is usually either traumatic or neurogenic. Neurogenic HO is a known but uncommon complication that occurs after a cerebral or spinal insult. The condition may present with a spectrum of symptoms and is often difficult to diagnose clinically. Although different imaging modalities have been used to diagnose HO, clinicians and radiologists may occasionally encounter radiological features of HO that may mimic other disease conditions. We herein report a rare case of neurogenic HO occurring in the non-paretic limb of a patient, and the diagnostic and radiological challenges encountered.

5.Dislocation of the penis: a rare complication after traumatic pelvic injury.

Traumatic injury to the male external genitalia is frequently encountered, but acute traumatic dislocation of the penile structure is extremely rare, with only a few reports found in the literature. We herein report the case of a 21-year-old man who sustained blunt trauma to the pelvis following a motor vehicle accident, and had features suspicious of penile dislocation. With the use of computed tomography and bedside ultrasonography, a diagnosis of penile dislocation was made, which was subsequently confirmed intraoperatively. Immediate surgical intervention via gentle manipulation of the penile tissue back to its native position was performed in order to restore normal anatomy. The exact mechanism of penile dislocation is not known. However, circumferential laceration around the foreskin causing degloving injury of the penis is suggested in our patient.

6.A stepwise approach in the management of chronic spontaneous urticaria in children

BACKGROUND: There is limited literature in the management of chronic urticaria in children. Treatment algorithms are generally extrapolated from adult studies. OBJECTIVE: Utility of a weight and age-based algorithm for antihistamines in management of chronic spontaneous urticaria (CSU) in childhood. To document associated factors that predict for step of control of CSU and time taken to attain control of symptoms in children. METHODS: A workgroup comprising of allergists, nurses, and pharmacists convened to develop a stepwise treatment algorithm in management of children with CSU. Sequential patients presenting to the paediatric allergy service with CSU were included in this observational, prospective study. RESULTS: Ninety-eight patients were recruited from September 2012 to September 2013. Majority were male, Chinese with median age 4 years 7 months. A third of patients with CSU had a family history of acute urticaria. Ten point two percent had previously resolved CSU, 25.5% had associated angioedema, and 53.1% had a history of atopy. A total of 96.9% of patients achieved control of symptoms, of which 91.8% achieved control with cetirizine. Fifty percent of all the patients were controlled on step 2 or higher. Forty-seven point eight percent of those on step 2 or higher were between 2 to 6 years of age compared to 32.6% and 19.6% who were 6 years and older and lesser than 2 years of age respectively. Eighty percent of those with previously resolved CSU required an increase to step 2 and above to achieve chronic urticaria control. CONCLUSION: We propose a weight- and age-based titration algorithm for different antihistamines for CSU in children using a stepwise approach to achieve control. This algorithm may improve the management and safety profile for paediatric CSU patients and allow for review in a more systematic manner for physicians dealing with CSU in children.

9.Complementary and alternative medicine for migraine and tension-type headache among the Malaysians

Background: The literature on complementary and alternative (CAM) therapies in South East Asia
is limited. The objective of the study was to evaluate the frequencies of CAM therapies in the
Malaysian patients with migraine and tension-type headache (TTH). We also assessed the reasons for
CAM use. Methods: This was a cross-sectional study. The study was conducted in a tertiary hospital
in Malaysia. Consecutive patients presenting with migraine and TTH to the neurology clinic were
recruited. Demographic characteristics were documented. Data on CAM use, including frequency and
the reasons was collected. Results: Six hundred and eighty five patients (365 TTH and 320 migraine)
were recruited. They consisted of 305 (44.5%) Malay, 174 (25.4%) Chinese, 169 (24.7%) Indian
and 37 (5.4%) patients from other ethnic groups. A total of 478 (69.8%) patients, comprising 266
(55.6%) patients with TTH and 212 (44.4%) patients with migraine used CAM therapies. The most
commonly used CAM therapies were medicated oil (355 patients, 51.8%) and massage (246 patients,
35.9%). The most common reasons for CAM use were reduction of pain (47.7 %), stress reduction
(34.9%), and “cooling” effect (27.4%). Thirteen (4.1%) patients with migraine practiced reflexology,
whereas 5 (1.4%) patients with TTH practiced reflexology (p=0.032). Twenty three (7.2%) patients
with migraine took vitamin supplements compared to 13 (3.6%) patients with TTH (p=0.039). On
univariate analysis, CAM use was significantly associated with ethnic groups (p=0.014), gender
(p<0.0001), age (p=0.004), salary (p=0.014), educational level (p=0.003), and headache subtypes
(p= 0.067). On logistic regression, married women were more likely to use CAM (OR=1.864, 95%
CI 1.216-2.858, p=0.004).
Conclusion: A large number of Malaysian headache patients used CAM. The most common reasons
were reduction of pain, stress and “heatiness”. Medicated oil was most commonly used, followed by
massage. Married women were more likely to use CAM, and this was the only determinant in this study.
