1.Carcinosarcoma and Multiple Early Gastric Carcinomas of the Stomach.

We report a rare case of carcinosarcoma arising in the stomach. The tumor is presented in the posterior wall of the gastric lower body and antrum of a 56-year-old man. Grossly the tumor has polypoid appearance with diffuse surface ulceration and measures 5.5 cm in diameter. The tumor is accompanied with three separate well differentiated early gastric adenocarcinomas (two EGC type IIc & IIa). Microscopically, the tumor consists of moderately well differentiated adenocarcinoma in the periphery, and lobulated sarcomatous areas in the center, which shows chondroid differentiation. Transitional areas between adenocarcinoma and chondrosarcoma are evident. Immunohistochemical studies show positivity for cytokeratin and carcinoembryonic antigen in the epithelial component, and for vimentin and S-100 in the sarcomatous component. The transitional areas are positive in carcinoembryonic antigen, vimentin, S-100, and cytokeratin. The tumor extended to the subserosa and showed metastasis of only adenocarcinomatous component in six out of 47 dissected perigastric lymph nodes.

2.Composite Adenocarcinoma and Choriocarcinoma of the Sigmoid Colon with Hepatic Metastasis of the Choriocarcinomatous Component.

A rare case of hepatic metastasis with a choriocarcinomatous component from a composite adenocarcinoma and choriocarcinoma of the sigmoid colon in a 60-year-old man is reported. The hepatic metastasis displayed choriocarcinoma with extensive hemorrhagic necrosis. The tumor cells were poorly differentiated with scattered foci of bizzare syncytiotrophoblastic cells. Retrospective examination of the previous colonic carcinoma proved that the tumor was composed of two distinctive elements. One was a moderately well differentiated adenocarcinoma located in mucosa and submucosa. The other was a deep seated and undifferentiated carcinoma which was made up of hyperchromatic bizzare cells with syncytiotrophoblastic cells. There were transitional foci from adenocarcinoma to undifferentiated carcinoma with trophoblastic cells. Immunohistochemical staining showed beta-hCG expression in the undifferentiated cells of both the primary and the metastatic tumors. Implications for the possible origin and cause of tumor cell heterogeneity are briefly discussed.

4.Metastatic Angiosarcoma of the Lung: HRCT Findings.

We describe a case of cavitary metastasis to the lungs from a small angiosarcoma of the scalp, in which themetastatic lesions were complicated by pneumothorax and pulmonary hemorrhage. On high-resolution CT, the lesionssimulated the findings of Langerhans cell histiocytosis. Thin-walled cavitary metastatic lesions were similar tothose of thin walled air cysts in Langerhans cell histiocytosis. Ground-glass opacity simulated the findings ofsmoke r's respiratory bronchiolitis in Langerhans cell histio-cytosis but histologically represented hemorrhageduring metastasis of the angiosarcoma.

6.A case of fulminant community-acquired Acinetobacter baumannii pneumonia in Korea.

Acinetobacter baumannii (AB) is a common pathogen found in patients with hospital-acquired pneumonia all over the world. Community-acquired AB pneumonia, however, is very rare and has seldom been reported in Asia-Pacific countries. Community-acquired AB pneumonia has a fulminant course and is associated with a higher mortality than hospital-acquired AB pneumonia. In Korea, no case of fatal community-acquired AB pneumonia has been reported to date. Here, we describe the first fatal case of fulminant community-acquired AB pneumonia in Korea.

7.Percutaneous Abscess Drainage of Multiloculated Liver Abscess.

PURPOSE: Recently there have been some reports that percutaneous absces drainage(PAD) was ineffective in treating multiloculated liver abscess. We therefae, reviewed our results of catheter drainage in jultiloculated liver absces. MATERIALS AND METHODS: PADs in 10 cases of multiloculated liver abscesses were performed with 8.5F Pig tail, 12 & 14F Sump cahteters, under ultrasonic & fluoroscopic guidance. RESULTS: All the 10 cases were successfully drained without major complications. Mean drainage duration was 16.3 day and the result was not significantly different from those obtained by draining unilocular pyogenic liver abscess. The success was the result of using large caliber catheter and repetition in insertion of guidewire deeply into abscess cavity to make communications between the Iocules which was proven by abscessogram. CONCLUSION: PAD was safe and effective method for multiloculated abscess as unilocular liver abscess, and it is recommended that the multiloculated liver abscess be draincd.

Intracystic (encysted) papillary carcinoma of the breast (IPC) is a rare tumor with predilection for elderly postmenopausal women and has distinctive pathological features that should be distinguished from those of ductal carcinoma in situ (DCIS) of the papillary type and invasive papillary carcinoma. IPC can appear in a pure form or can be associated with DCIS or invasive carcinoma. It is usually a low-grade carcinoma with overall good prognosis, presenting low or intermediate nuclear grade, no necrosis, high positivity to estrogen receptors, and the negativity for c-erbB-2. We report here a case of IPC managed by lumpectomy with ipsilateral axillary lymph node dissection

10.Comparative Study of Postoperative Complications of High Ligation and Stripping with Endovenous Laser Treatment for Varicose Veins.

PURPOSE: Endovenous laser treatment (EVLT) has been performed as a minimally invasive alternative to conventional surgery for saphenous vein incompetence. It is thought to minimize postoperative complications and improve the quality of life compared with high ligation and stripping. The aim of this retrospective study was to compare the postoperative complications and quality of life of EVLT with high ligation and stripping (HS). METHODS: A total of 294 patients with 390 saphenous veins were included in this study. Patients with saphenous vein incompetence were treated with either HS (n=98; HS group) or EVLT (n=196; EVLT group). Patients were evaluated preoperatively and at 1, 3 months, postoperatively. Postoperative complications, the Venous Clinical Severity Score (VCSS) and the Aberdeen Varicose Vein Symptom Severity Score (AVSS) were compared between the two groups. RESULTS: The total postoperative complication rate was 31.62%, in HS and 27.47% in the EVLT groups. Most of the complications were minor and self-limited. There were no statistical differences in the total complication rate and the occurrence of complications such as hematoma, paresthesia and phlebitis. Pain was less severe following EVLT compared to HS. The EVLT patients were discharged and resumed their normal activity earlier than the HS patients. The two groups had similar improvement of the VCSS and AVSS at 1 and 3 months, postoperatively. CONCLUSION: With the similar, comparable short-term safety outcome, both HS and EVLT may be considered as safe procedures for the treatment of varicose veins. Long-term, large population-based randomized studies with promising data for the improvement of quality of life scores will be needed for the evaluation of EVLT before it becomes a standard treatment for saphenous vein incompetence.
