Objective:
The effectiveness of music therapy programs for the elderly with dementia has been verified through several studies, but there is a limitation in that most of them are compared with a single group, no treatment group or waiting group. In this study, to overcome this problem, the two groups that performed similar interventions were selected and compared retrospectively.
Methods:
For 37 people who participated in the treatment programs for the elderly with dementia at Dongdaemun Center for De-mentia and its branch center, we retrospectively analyzed the changes in Korean version of Geriatric Depression Scale and GeriatricQuality of Life Scale-Dementia scores measured before and after treatment.
Results:
Prior to treatment, there is no significant difference between the groups in the demographic variables and the depression and quality of life scores. The difference between pre- and post-scores was not significant in the group at the branch center without a music therapy, but depression was significantly reduced and quality of life had improved in the group at Dongdaemun center with the music therapy program.
Conclusion
It was confirmed that participation in the structured music therapy program had a positive effect on the improve-ment of depression and quality of life in the elderly with dementia compared to other similar interventions.

5.A Case of Congenital Self-Healing Reticulohistiocytosis.

Congenital self-healing reticulohistiocytosis (CSHR) is a rare variant of cutaneous reticulohistiocytosis. We report a newborn who had about 25 reddish brown papulonodular lesions on whole body at birth. No extracutaneous involvement was found. All skin lesions involuted within 4 months spontaneously. Biopsy of a skin lesion showed marked aggregated histiocytic infiltration with some eosinophils. S-100 protein and lysozyme were positive. Electron microscopy revealed numerous dense bodies, often with a myelin-like core and some Birbeck granules. We described a detailed clinical features of this case and reviewed the literatures.

Lymphoma is the third most common childhood malignancy after leukemia and brain tumor. In contrast to adult non-Hodgkin's lymphoma, that occurring in children is usually found extranodally. The most common site is the abdomen, including the gastrointestinal tract, kidney and pancreas, and the next most common is the extranodal head and neck. Gastrointestinal non-Hodgkin's lymphoma in adults is usually considered to be MALT lymphoma, a distinct B-cell type, but the occurrence of this variety in the small bowel of children is relatively rare. We report a case of high-grade MALT lymphoma occurring in a nine-year-old boy who presented with ileoileal intussusception.

7.Effect of Nebulized Steroid (Budesonide) and Parenteral Steroid (Dexamethasone) on Children with Croup.

PURPOSE: This prospective study was conducted to evaulate the effect of nebulized steroid (budesonide) and parenteral steroid (dexamethasone) on chidren with croup. METHODS: Twenty nine infants and children (6months-5years of age) admitted to hospital with croup were randomly assigned to receive either 1mg (4ml) of nebulized budesonide (17 patients) or 0.6mg/kg intramuscular injection of dexamethasone sodium phosphate (12 patients). Total croup score, heart rate and respiratory rate of patients were assessed on admission, 30 minutes, 2 hours, 6 hours, 12 hours and 24 hours after treatment, respectively. RESULTS: 1) In total cases, total croup score were significantly decreased at 30 minutes after treatment in the group treated with nebulized budesonide (p<0.01) but at 2 hours after treatmetn in the group treated with dexamethasone (p<0.05). 2) In mild cases (total croup score<5), total croup score were significantly decreased at 2 hours after treatment in the both group (p<0.05). 3) In moderate to severe cases (total croup score> or =5), total croup score were significantly decreased at 30 minutes after treatment in the group treated with nebulized budesonide (p<0.05). But in group treated with dexamethasone, there were no statistical significance because of small cases. 4) Respiratory rate was decreased after treatment in the both group, but significantly decreased at 2 hours after treatment in the group treated with dexamethasone (p<0.05). 5) Heart rate was decreased after treatment in the both group, but significantly decreased at 30 minutes after treatment in the group treated with dexamethasone (p<0.05). 6) There were no significant side effects in the both group. CONCLUSIONS: We conclude that both nebulized budesonide and parenteral dexamethasone were effective in treatment of croup, and that nebulized budesonide leads to more rapid clinical improvement in children with croup, especially moderate to severe one.

8.Assessment of the Synergistic Effect of Seven Antimicrobial Combinations on Extensively DrugResistant Acinetobacter baumannii Strains

Background:
To solve the difficulty in determining the appropriate treatment regimen for patients infected with extensively drug-resistant Acinetobacter baumannii (XDRAB), it is necessary to develop various strategies to increase the therapeutic effect of antimicrobial agents. The purpose of this study was to select the treatment combination showing the greatest antimicrobial effect among seven candidate antimicrobial substances.
Methods:
Seven strains of XDRAB were used in this study. The composition of the treatment consisted of colistin as the base and one of the seven antimicrobial substances, doripenem, minocycline, tigecycline, linezolid, fusidic acid, vancomycin, or alyteserin E4K peptide. The interaction between the drugs in each combination was evaluated by measuring the synergy rates using time-kill analysis.
Results:
The synergy rates of the seven combinations tested in the time-kill assay in this study were as follows, in descending order from the combination with the highest synergy rate: colistin + minocycline (57.1%), colistin + alyteserin E4K (50.0%), colistin + tigecycline (42.9%), colistin + vancomycin (28.6%), colistin + doripenem (14.3%), colistin + fusidic acid (14.3%), and colistin + linzolid (0%). None of the combinations showed antagonism. The three combinations showing bactericidal activity and the rates of their bactericidal activity were colistin + alyteserin E4K combination (33.3%), colistin + minocycline (14.3%), and colistin + vancomycin (14.3%).
Conclusion
The colistin + minocycline and colistin + alyteserin E4K treatment combinations, which showed high synergy rates, can be considered as promising candidates for future in vivo experiments evaluating combination therapies.
9.Update on the Proposal for Creating a Guideline for Cancer Registration of the Gastrointestinal Tumors (I-2).

BACKGROUND: Cancer registries play a fundamental role in cancer control and multicenter collaborative research. Recently, the need for reassessment of cancer registry criteria has arisen due to the newly released 2010 World Health Organization (WHO) classification. Accordingly, development of new coding guidelines for cancer is necessary to improve the quality of cancer registries, as well as to prevent conflicts that may arise when seeking medical insurance compensation. METHODS: With funding from the Management Center for Health Promotion, 35 members of the Gastrointestinal Pathology Study Group and the Cancer Registration Committee of the Korean Society of Pathologists (KSP) participated in a second workshop for gastrointestinal tumor registration in Korea. RESULTS: The topics of gastric epithelial tumor, colonic intramucosal carcinoma, neuroendocrine tumor (NET), gastrointestinal stromal tumor (GIST) and appendiceal mucinous tumor were discussed for new coding guidelines. A survey was then conducted among 208 members of the KSP for a consensus of the guidelines proposed in the workshop. CONCLUSIONS: Although a few issues were set aside for further discussion, such as coding for non-gastric GIST and some types of NET, the members agreed upon most of the proposed guidelines. Therefore, we suggest using the newly revised International Classification of Diseases for Oncology, 3rd edition (ICD-O-3) coding guidelines for registering gastrointestinal tumors in Korea.

Adenoma malignum is a well differentiated form of adenocarcinoma. Despite of benign histologic appearance, this tumor has malignant clinical course. Because of its rarity and subtle histologic changes, it may be missed. Therefore it has poor prognosis. Early diagnosis and early treatment is required by thorough examination. We report one case of adenoma malignum of uterine cervix with a brief review of the literature.
