1.Involvement of Chemokine Receptor 4/Stromal Cell-derived Factor 1 System in Human Salivary Gland Carcinoma Motility

Salivary gland carcinoma such as adenoid cystic carcinoma (AdCC) is characterized by slow growth, diffuse invasion and lung metastasis, which determine the patient's prognosis. It is important to clarify an attractant molecule leading tumor cells to migrate. We examined the effects of stromal cell-derived factor (SDF) -1, a chemokine, on salivary gland carcinoma cell clone HSG and its subclone HSG-AZA3. SDF-1 promoted the invasion and migration of HSG and HSG-AZA3 cells dose-dependently. Immunocytostaining and RT-PCR indicated that HSG and HSG-AZA3 cells expressed SDF-1 receptor, CXCR4, both in protein and mRNA level, respectively. CXCR4 was present on the cell surface of HSG cells, and was downregulated by SDF-1 addition. Finally, we confirmed that CXCR4 was expressed in the tissue of AdCC. Our study suggests that SDF-1 and CXCR4 play a role in the migration of carcinoma of salivary gland origin.
2.Study of Health-related Quality of Life of Women and Fruits Granola

The purpose of this study was to examine the health-related Quality of Life (QOL) of women and the effects of “fruits granola” on women’s health. It was based on an Internet questionnaire survey on QOL (using a QOL questionnaire for women that was prepared by LSTT Co., Ltd.) of 3,460 subjects who answered questions concerning physical and mental matters. It also had 165 subjects, 108 of whom had mild constipation and 57 of whom did not, consume fruits granola freely for a period of time and then compared their QOL before and after consumption.The results of the screening survey showed significantly higher scores in all categories among women who did not have constipation than women who did. Additionally, the mental QOL score was significantly higher among women who had the habit of eating fruits granola.Moreover, the overall QOL score significantly increased after consumption of fruits granola. These results suggest that consumption of fruits granola may be effective in improving women’s QOL.
3.Prognosis of Stanford Type B Acute Aortic Dissection and Availability of Early Rehabilitation Program in Medical Treatment.

Stanford type B acute aortic dissection without complications has been considered to be an indication for medical rather than surgical treatment. To investigate the availability of medical treatment and early rehabilitation, we evaluated 90 cases treated between 1986 and 1999 with type B acute aortic dissection. These consisted of 79 nonruptured cases and 11 ruptured cases at the beginning of treatment in our medical center. No surgery was performed in any of the nonruptured cases but surgery was performed in 8 of 11 ruptured cases. Surgical mortality in the rupture type was 12.5% (1/8). During medical treatment of the nonrupture type, 3 patients died of sudden rupture (1 case) and bowel ischemia (2 cases). An early rehabilitation program in which the goal was for the patient to walk around the ward within 2 weeks was performed for 31 consecutive cases of nonrupture type without vascular complications. Mortality was not significantly different between the early and conventional rehabilitation groups. The incidence of pneumonia and ICU syndrome during medical treatment was 13.0% (6/46) and 37% (17/46), respectively in the conventional group and 0% and 12.9% (4/31), respectively in the early group. The incidence of ICU syndrome was significantly lower in the early group than in the conventional group. Despite the limitations of this study, medical treatment and early rehabilitation showed good results in cases of uncomplicated type B acute aortic dissection.
4.Cooperation between Hospital and Nursing Home by E-mail

After 1985, the decrease in domestic nursing power progressed gradually. The care for the elderly shifted from domestic private nursing to public nursing-care services when the long-term nursing-care insurance law was enacted in 2000. Because many of the elderly receiving nursing care suffer from chronic ailments of hypertension or the consequences of cerebrovascular disease, etc., cooperation between hospitals and nursing homes is necessary and indispensable. We began exchanging information by E-mail on a trial basis with a special elderly nursing home in April 2009. We aimed to integrate medical care and nursing by sharing medical and nursing information. We concluded that the use of E-mail could serve our purpose. The reason why we reached this conclusion is as follows:The mechanism of the information transmission by E-mail, that also relates to semiotics and narratology, is related to the essence of the description. This mechanism functions as a tool for mutual understanding among hospitals, nursing homes, and families. This also functions as a device to make the medical and nursing experience join. As a result, this mechanism enables the elderly to escape death as dying of sickness in the hospital and to die a natural death in the course of nursing. It is an easy method that can be introduced at a low cost for the purpose of establishing cooperation in medical and nursing care among hospitals, clinics, nursing homes, and nursing support centers, etc. especially in medically underserved remote areas.
5.Effects of the Intake of Fruits Granola on Defecation and Quality of Life

In order to investigate the effects of fruits granola intake on defecation and quality of life (QOL), 20 females (mean age: 20.0 ± 1.1 y.o.) with frequency of defecation of less than 5 times per week were selected for this open study. Daily intake of 50 g of fruits granola was used as a substitute staple food for 2 weeks, and changes in defecation and QOL were compared before and after the procedure. The results showed that, compared with the pre-intake period, the frequency of defecation and the amount of defecation increased after fruits granola intake (p = 0.014, p = 0.024), and improvement of QOL (p = 0.011) was also observed. We conclude that, a continuous intake of fruits granola can improve defecation, and also improve quality of life.

8.Analysis of Topophilia in the Elderly Living in Mountainous Area

The aim of this study was to examine topophilia in the elderly living in mountainous areas. Topophilia, which is the geographical concept invented by Yi-Fu Tuan in 1999, is defined as the affective bond between people and place or environmental setting. A total of 120 elderly subjects living in a mountainous area responded to a standardized, validated 9-item Life Satisfaction Index K (subjective well-being) developed by Wataru Koyano and a new structured 6-item questionnaire on topophilia. Factor analyses revealed two domains of topophilia (public emotion and private emotion toward the living place). Public emotion was the emotion of being hard to leave the living place. Private emotion was the emotion of not loving the living place. There were areas where the elderly had a lesser degree of attachment toward the present state of the elderly could hardly have a sense of well-being and attachments to the current domicile. It is considered that not only physical but also mental approach is necessary to support the daily life of the elderly living in the mountainous area. Also, we examined the influences of aging and the living place on a subjective well-being and topophilia. There was no significant correlation between age and subjective well-being, but there was significant positive correlation between age and the degree of topophilia (r=0.234, p‹0.01). On the other hand, the degree of subjective well-being by the place of residence was significantly different (p‹0.001; ANOVA), and the degree of topophilia by the place of residence was not significantly different. These findings suggest that subjective well-being is not influenced by age but influenced by the place of residence, and topophillia is not influenced by the place of residence but influenced by age.
9.Therapeutic effect of the cordless mini TENS on shoulder stiffness and painful diseases.

The therapeutic effect of a cordless mini TENS on shoulder stiffness and painful diseases and patients' favorite intensity of stimulation were studied. Effect of this apparatus on peripheral circulation was also examind using a laser Doppler measuring instrument. The 114 cases with shoulder stiffness used the TENS only once (SS group), and the 53 cases continuasly (SC group). The 65 cases with painful diseases used the TENS only once (PS group) and the 53 cases continuasly (PC group). A great majority of the patients prefered stimulation with moderate frequency and high intensity. Effectiveness was seen in 76% of SS group and 87% of SC group, and in 75% of PS group and 91% of PC group. Peripheral circulation was significantly increased after the stimulation of the apparatus.
10.Problems in School Life and Countermeasures for Patients With Orthostatic Dysregulation During the COVID-19 Pandemic

Orthostatic dysregulation (OD) is mainly caused by circulatory dysregulation by the autonomic nervous system, but it can be psychosomatic and may be associated with school refusal. Here, we investigated the causes of school refusal and problems specific to the COVID-19 pandemic in pediatric patients meeting the diagnostic criteria for psychosomatic OD. This study involved 67 patients with OD (20 boys, 47 girls) who visited the pediatric outpatient department from April 1, 2020 to October 31, 2021. Mean age was 13.7 ± 2.4 years. The subtypes of OD included postural tachycardia syndrome (POTS) in 43 cases, delayed orthostatic hypotension (delayed OH) in 15 cases, instantaneous orthostatic hypotension (INOH) and vasovagal syncope (VVS) with INOH in 1 case each , and 7 other cases that met the conventional diagnostic criteria. Of the 67 patients, 37 met the diagnostic criteria for psychosomatic OD and 22 had a history of long-term school refusal. The COVID-19 pandemic, school closures, and suspected school infections have completely changed the living environment for children. As a result, these children could not rest at school and were forced to return home early. They had worsening of symptoms that was thought to be due to disruptions of daily rhythms caused by staying at home and getting insufficient exercise. At this hospital, we explain the pathophysiology of the autonomic nervous system, provide lifestyle guidance to patients and their families, cooperate with schools with consideration of school refusal and each child’s mental and physical condition, and cooperate with psychiatry. This paper discusses problems and countermeasures specific to the COVID-19 pandemic.
