The present study aimed to investigate whether daily steps in male college students have actually decreased in the last decade. If the decline in steps is found, we additionally aimed to examine whether the decline in steps is attributed to a reduction of exercise or, rather, to a reduction of non-exercise activity. A serial cross-sectional study was performed from 2003 to 2012 at the Osaka Institute of Technology. A total of 1,215 male college freshmen were analyzed. Every year, scores of the physical activity levels were evaluated by a questionnaire, and we measured daily steps of participants using a pedometer during usual 1 week. Daily behavior record was also analyzed (n = 69). There were no significant trends or differences in characteristics of participants and the scores of physical activity levels throughout the 10 years. However, interestingly, significant decreasing trends were observed in the steps of average and weekend. In addition, daily behavior record analyses indicate that a cell-phone or a PC usage increased significantly in both weekday and weekend and that playing video games increased significantly in weekday. Therefore, these data support the view that daily steps have actually reduced in the last decade. Our findings also raise the possibility that the decline in steps may be related to the decline in non-exercise activity associated with the increase in a cell-phone or a PC usage and playing the video games.

The present study aimed to examine the effects of Internet addiction on daily steps. A cross-sectional study was performed at the Osaka Institute of Technology. A total of 334 male college freshmen were analyzed. The participants were assigned to five groups according to Internet addiction levels. Internet addiction levels were assessed by Diagnostic Questionnaire, which has been reported by Young (1998), and we measured daily steps of the participants using a pedometer during usual 1 week. There were no significant trends or differences among five groups in physical characteristics of the participants, sleep duration, frequency of eating breakfast, and walking during commute time. However, with the level of Internet addiction increasing, daily steps showed a statistically significant decreasing trend. Therefore, Internet addiction levels might be related to reductions in steps in a dose-response manner. To clarify this point, further investigations will be required after considering confounding factors.
3.Long-term Results after Surgical Repair of Partial Atrioventricular Septal Defect in Children. Semiquantitative Assessment of Mitral and Tricuspid Regurgitation by Doppler Color Flow Imaging.

The severity of mitral regurgitation (MR) and tricuspid regurgitation (TR) was evaluated semiquantitatively by Doppler color flow imaging. The maximum MR area/body surface area (MRA/BSA) correlated significantly to the severity of angiographyic changes (tau=0.897). The maximum TR area/body surface area (TRA/BSA) also correlated significantly to the severity in angiography (tau=0.874). The cutoff values were 0.5, 2, 4, and 8cm2/m2 for MRA/BSA and 1, 2.5, 5, and 10cm2/m2 for TRA/BSA. Fourteen children (mean age 4.2 years) underwent repair of partial atrioventricular septal defects (P-AVSD) from 1985 to 1992. The cleft in the anterior leaflet was closed in the mitral valve; other procedures such as annuloplasty were not performed. They have been followed for periods from 7 months to 7 years and 5 months (mean 4 years); they were examined by echo cardiography and the Holter electrical cardiogram at the end of the period. MR had reduced to grade 0-II in all cases. No patients were given any medication, and all remained in NYHA Functional Class I. Paroxysmal supraventricular tachycardia developed in only one patient. We concluded that no annuloplasty in mitral valve is needed in children suffering from P-AVSD.

5.Clinical Application of Angioscopy in the Field of Cardiovascular Surgery.

Direct observation of the intravascular atherosclerotic change provides many diagnostic information and supports successful vascular surgery. Angioscopic inspections were performed in the 23 patients who underwent laser angioplasty for the peripheral arteries and 7 patients with aorto-coronary bypass surgery in this study. Useful images could be obtained in 19 (83%) of 23 observations in the peripheral arteries, and the successful rates of angioscopic observation were influenced by the diameter and the degree of kinking of each vessel. On the other hand, intraoperative observation of coronary artery with angioscope could be performed easily in all cases. From these findings, intraoperative application of angioscopy was considered to be useful procedure for the cardiovascular surgery.
6.An immediately accessible database for information regarding ingestion timing and maximum dose setting: Development and applications to address inquiries to clarify doubts concerning prescription

Objective: Optimizing the time of ingestion and avoiding overdose are important aspects of medication therapy. However, seeking explanations for selecting the time of ingestion and maximum dose for a certain drug is time consuming. The aim of this study is to develop a database (DB) that enables a rapid search of the basis for the time of ingestion and maximum dose setting.
Methods: The basis for the time of ingestion and maximum dose setting were surveyed for 38 and 184 drugs, respectively. Package inserts, interview forms, and other documents preserved in our department were surveyed, and this was followed by an inquiry of the staff of pharmaceutical companies. Standard responses to a prescription with incorrect timing or dosage were determined and included into a DB together with information whose quality was dissected. The efficiency of DB was evaluated: the time taken to obtain information and consistency of inquiries to clarify doubts concerning prescription with the use of DB and without the use DB were compared.
Results: The information of newer drugs, but not of the older drugs, were easily obtained without the need to inquire the staff of the pharmaceutical companies. Operation of the DB was convenient and was acceptable for most pharmacists working in our department. The DB markedly reduced the time taken to obtain information. Further, with the aid of DB, the consistency in the responses to inquiries to clarify doubts concerning a pharmacist’s recommendation was remarkably increased.
Conclusion: The DB developed in the present study may contribute to the improvement of not only the efficiency but also the quality of dispensation.
7.The Effects of Cognitive Rehabilitation on Patients with Post-stroke Depression in Long-term Care Facilities: A Before-and-after Comparison Trial

Introduction: The purpose of this study was to examine the effects of cognitive rehabilitation for depression, cognitive function, ADL, and QOL in patients with post-stroke depression (PSD). In addition, we evaluated the effects of cognitive rehabilitation on customer satisfaction with long-term care facilities and rehabilitation.Methods: This study included 8 participants with PSD who were residents of a long-term care facility located in Ibaraki Prefecture, Japan. In addition to their ordinary rehabilitation (2 sessions per week, 20 minutes per session), participants underwent 36 sessions of cognitive rehabilitation (3 sessions per week, 20 minutes per session, for 12 weeks). Primary outcome measures: Depression severity was measured using the Geriatric Depression Scale (GDS), Zung Self Depression Scale (SDS), and the Depression Scale of the Japan Stroke Scale (JSS-D). Frontal lobe function was assessed using the Frontal Assessment Battery (FAB). Secondary outcome measures: QOL was assessed using SF-8. Customer Satisfaction with the rehabilitation was measured using the Customer Satisfaction Scale based on Need Satisfaction (CSSNS). Assessments were performed before and after the rehabilitation.Results: The severity scores of depression, frontal lobe function, and customer satisfaction with the rehabilitation all significantly improved after the cognitive rehabilitation sessions.Conclusions: The results obtained in this trial indicate that, for patients with PSD depressive symptoms, frontal lobe function, QOL, and rehabilitation customer satisfaction may be improved by the inclusion of cognitive rehabilitation sessions alongside regular rehabilitation.
8.Perioperative Anticoagulation Therapy for Patient with Abdominal Aortic Aneurysm after Heart Valve Surgery.

Under scheduled anticoagulation therapy, surgery for abdominal aortic aneurysm was performed in 4 patients who had undergone heart valve surgery and implantation of a mechanical prosthesis. Warfarin and antiplatelet agents were prescribed in all cases preoperatively. Antiplatelet agents were discontinued from seven to 10 days before operation. Warfarin was stopped from two to three days before operation and heparin (200IU/kg/day) was administered by continuous intravenous infusion to produce an activated clotting time of around 150 seconds. Bolus intravenous heparin of 3, 000 IU was added before aortic crossclamp. Oral anticoagulants were resumed from the beginning of oral intake, and heparin was stopped when the prothrombin time reached therapeutic levels (% PT=40%). In three patients perioperative courses were uneventful. Intraperitoneal hemorrhage occurred in one patient who simultaneously underwent cholecystectomy and aneurysmectomy with Y-grafting. He required blood transfusion and interruption of anticoagulation. Brain thromboembolism occurred in this patient 26 days after the operation. We believe that scheduled anticoagulation for the operation of abdominal aortic aneurysm is safe and useful in patients with prior prosthetic heart valve surgery. However, the coexistence of coagulopathy requires more intensive anticoagulation therapy.
9.Postoperative Meningitis in Patients with Cervical Cord Tumor: A Case Report.

Postoperative meningitis after spinal surgery is a rare complication that can result in a life-threatening condition. Linezolid (LZD) is an oxazolidinone which has been approved in Japan for infections caused by methicillin-resistant Staphylococcus aureus. The authors encountered a case of postoperative meningitis with cerebrospinal fluid leakage (liquorrhoea) that occurred after resection of a cervical cord tumor. The infection was caused by methicillin-resistant Staphylococcus epidermidis(MRSE). Debridement and suture of the dura matter was carried out. LZD was given intravenously. The infection was cured without any sequelae. Based on this result, we concluded that LZD might be considered as one of the first choices for the treatment of postsurgical meningitis caused by MRSE.
