1.Functional Assessment of Plant Extracts by Application of Novel Neutrophil Activity Measurement System

Objective: The overproduction of reactive oxygen species leads to oxidative stress, which is related to lifestyle-related disease and cancer. Although antioxidants are considered as one of the countermeasures to oxidative stress, it is necessary to develop the assessment methodology for the antioxidant capacity which is closer to the inner body conditions. In this study, we examined antioxidant actions of plant extracts by using newly-developed neutrophil activity measurement system.
Method: Lemon verbena, Green tea, Camellia japonica and Antiallerge® were used as plant extracts, and were diluted to medium in wide-range concentrations. Each solution was added on modified Mebiol Gel® (hydrogel), and the mixture of blood and luminol were set onto the hydrogel in each tube. The amount of reactive oxygen species were measured by luminol-dependent chemiluminescence, whereas the cell count in the hydrogel was quantified as migratory activity of neutrophils.
Result: Lemon verbena and Green tea significantly inhibited reactive oxygen species in a concentration-dependent manner, whereas Camellia japonica and Antiallerge® did not. The migratory activity of neutrophils was not affected by those plant extracts. Also, damaged cells were not detected.
Conclusion: It was suggested that Lemon verbena and Green tea scavenged reactive oxygen species without causing cell death of neutrophils. This new method for measuring neutrophil activities can be applied not only for assessing the status of inflammation and oxidative stress but also as the screening system for predicting the effectiveness of antioxidant and anti-inflammatory substances.
2.Groin lymph node detection and sentinel lymph node biopsy in vulvar cancer.

OBJECTIVE: To identify suitable diagnostic tools and evaluate the efficacy of sentinel lymph node (SLN) biopsy for inguinal lymph node metastasis in vulvar cancer. METHODS: Data from 41 patients with vulvar cancer were evaluated retrospectively, including magnetic resonance imaging (MRI) measurements, SLN biopsy status, groin lymph node metastasis, and prognosis. RESULTS: SLN biopsy was conducted in 12 patients who had stage I to III disease. Groin lymphadenectomy was omitted in five of the nine patients with negative SLNs. All SLN-negative patients who did not undergo groin lymphadenectomy showed no evidence of disease after treatment. On MRI, the long and short diameters of the inguinal node were significantly longer in metastasis-positive cases, compared with negative cases, in 25 patients whose nodes were evaluated pathologically (long diameter, 12.8 mm vs. 8.8 mm, p=0.025; short diameter, 9.2 mm vs. 6.7 mm, p=0.041). The threshold of >10.0 mm for the long axis gave a sensitivity, specificity, positive predictive value, and negative predictive value of 87.5%, 70.6%, 58.3%, and 92.3%, respectively, using a binary classification test. Decision tree analysis revealed a sensitivity, specificity, and accuracy of 87.5%, 70.6%, and 76.0%, respectively, with the threshold of >10.0 mm for the long axis on MRI. The criteria of >10.0 mm for the long axis on MRI predicted an advanced stage and poorer prognosis using a validation set of 15 cases (p=0.028). CONCLUSION: Minimally invasive surgery after preoperative evaluation on MRI and SLN biopsy is a feasible strategy for patients with vulvar cancer.

3.Relationship between mood and sleep status with performance accuracy in university kyudo athletes

The mood and sleep status influence subsequent physical performance. However, the relationship between these factors and the accuracy of hitting the mark in kyudo remains unexplored. This study aimed to compare the mood and sleep status of university kyudo athletes on days with higher and lower accuracy. A total of 15 university kyudo athletes (age, 20 ± 2 years; 11 men) participated in the study. Participants were instructed to shoot 20 arrows in the morning for two to three days, with the days of higher and lower accuracy subsequently determined and compared for mood and sleep status. Sleep status on the preceding day of the accuracy assessments was measured using the Nemuri SCAN, while mood status immediately before the accuracy assessments were measured using the Profile of Mood States 2nd Edition (POMS2). None of the study participants demonstrated consistent accuracy across all accuracy assessments. When a three-day accuracy assessment was administered, and consistent accuracy was demonstrated on two days, the average mood and sleep status from those two days were incorporated into the analysis. Results indicated that total sleep time and sleep quality were significantly lower on the day before the higher accuracy day than on the day before the lower accuracy day (P < 0.05). Additionally, the Vigor-Activity score of POMS2 was trending lower on the higher accuracy day than on the lower accuracy day (P = 0.09). These results suggest that kyudo athletes may perform better under sub-optimal mood and sleep conditions rather than optimal conditions.
4.Cross-sectional associations of practicing YOSAKOI Soran and physical functions of middle-aged and older women

Insufficient physical function is strongly associated with decreased activity of daily living and health-related quality of life in middle-aged and older women. YOSAKOI Soran is one of the traditional Japanese dances to have naruko (wooden hand clappers) in their hands as they dance and incorporate the phrase from the Soran Bushi in their song. YOSAKOI Soran festivals can be seen being held in various places across the country, proving its growing popularity with residents as a celebration regardless of age or occupation. The purpose of this study was to examine the cross-sectional associations between YOSAKOI Soran and the physical functions of middle-aged and older women. A total of 30 middle-aged and older women practicing YOSAKOI Soran (YOSAKOI group) and 163 middle-aged and older women who had not practiced YOSAKOI Soran (control group) participated in this cross-sectional study. Handgrip strength, 30-second chair stand performance, and flexibility were used to measure physical functions. Handgrip strength was not significantly differed between the YOSAKOI group and the control group (P = 0.79). 30-second chair stand performance and flexibility were significantly higher in the YOSAKOI group than in the control group (both P < 0.05). These results remained significant after adjusting for age and body mass index (both P < 0.05). The present results suggest that practicing YOSAKOI Soran may increase lower extremity function and flexibility in middle-aged and older women.
5.Arterial stiffness in young male athletes

Arterial stiffness in endurance athletes is low, whereas arterial stiffness in strength athletes is high. The adaptation of the arterial stiffness may be different depending on the training type. On the other hand, there are mixed-trained athletes that can’t be classified as endurance- or strength- trained athletes. The aim of this study was to investigate the arterial stiffness among mixed-trained athletes. The total of 51 young male athletes (15 long-distance runners, 10 handball players and 26 kendo players) and 16 young healthy sedentary individuals (control group) participated in this study. Carotid-femoral pulse wave velocity (cfPWV), carotid-brachial PWV (cbPWV) and femoral-ankle PWV (faPWV), were measured as indices of central and peripheral (upper and lower limbs) arterial stiffness, respectively. cfPWV showed significantly lower in long-distance runners (high endurance capacity) and handball players (strength and high endurance capacities) compared with kendo players (strength and low endurance capacities) and control groups (P < 0.05 for both). cbPWV showed significantly lower in handball players and kendo players compared with the control group (P < 0.05 for both). There was no difference in faPWV among the groups. These data suggest that the competitive characteristics of athletes (i.e., endurance capacity or target muscle groups of sport) influence the adaptation of arterial stiffness. It can be speculated that endurance capacity in mixed-trained athletes can affect central arterial stiffness and similar to endurance trained athletes. In addition, the adaptation in upper limb arterial stiffness will be included in the training effects associated with the sports-specific target muscle groups, while lower limb arterial stiffness may not be unaffected by any type of exercise.
6.Serial Changes in Score on the Japanese Version of the Trail Making Test (TMT-J) After Minor Ischemic Stroke

The Trail Making Test (TMT) is a widely used measure of attention impairment. The time needed to complete the TMT (TMT score) is longer with greater impairment of attention in patients with brain diseases. TMT score becomes large in a proportion of patients with minor ischemic stroke. The Japanese version of the TMT- (TMT-J) was published in 2019. The purpose of this study was to clarify serial changes in TMT-J scores in patients with minor ischemic stroke. We retrospectively reviewed the TMT-J scores in those patients who completed the test both 8-14 days and 29-35 days after stroke onset. On initial evaluation, 1 of 21 patients could not complete TMT-J Part A. TMT-J Part A scores had a mean of 67 s and were abnormally large in 45% of the 20 patients who completed this part. Two of these 20 patients could not complete TMT-J Part B. TMT-J Part B scores had a mean of 135 s and were abnormally large in 61% of the 18 patients who completed this part. On second evaluation, scores on Part A and Part B improved in 76% and 73% of patients, respectively. This study demonstrated that abnormal TMT-J scores 8-14 days after onset of minor ischemic stroke improved over time in most patients.
7.Activity Report of the Kyoto Hospice and Palliative Care Unit Liaison Committee: Regional Palliative Care Cooperation Developed from Face-to-Face Relationships

To improve the quality of palliative care in the Kyoto region, we thought that closely connecting hospice and palliative care units (PCU) is necessary. Subsequently, we established the Kyoto PCU Liaison Committee in September 2017. This committee was created as a place to casually discuss the problems that individual PCU facilities have, deliberate on their worries together, grow and develop, and support newly launched facilities. Furthermore, discussions were held on current topics (emergency hospitalization, blood transfusion, smoking, bereaved family meetings, etc.) at the liaison meetings. While meetings were adjourned in 2020 due to the COVID-19 pandemic, we continued to exchange opinions on infection control, PCU management, etc., using the email network at first. Later, these meetings resumed via web conference systems. Thus, by having face-to-face relationships on a daily basis, we were able to maintain cooperation between PCUs even during the pandemic, and collaborate with cancer treatment hospitals. Overall, by forming a team of PCUs in Kyoto Prefecture, we aim to enable patients and their families to live with peace of mind wherever they are.
