1.The Effect of Care Guidance on the Caregivers of Patients with Unilateral Spatial Neglect

Objective:The purpose of this study was to investigate whether guidance related to hard-pressed care burden in caregivers of patients with Unilateral Spatial Neglect(USN)improves the caregiver's care burden, quality of life(QOL), depression scores, and the patient's degree of USN, cognitive function, and activities of daily living(ADL). Methods:We provided guidance to 4 caregivers based on the results of a questionnaire filled out at baseline. Next, we compared the caregiver's scores on the Zarit caregiver burden interview(J-ZBI), MOS 36-item short-form health survey(SF-36), and the geriatric depression scale(GDS)before and after guidance. We also checked their scores on the behavioral inattention test(BIT), Catherine Bergego scale(CBS), mini-mental state examination(MMSE), and the functional independence measure(FIM)one month later. Results:Despite individual differences, the caregiver's scores on J-ZBI, SF-36 and the GDS were seen to improve after guidance. Moreover, it also improved the awareness for USN, as seen in the CBS anosognosia score. Further, the MMSE score revealed an improvement in cognitive function. Conclusion:We suggest improving the caregiver's care burden, QOL, depression symptoms, the awareness for USN, and the cognitive function of patients with USN, by providing guidance twice to each caregiver of patients with USN patients.
2.Use of CAPD on Articular Rheumatism Patient Unable to Exchange Dialysis Bags Because of Joint Deformity of Fingers: A Case Report

Along with hemodialysis, continuous ambulatory peritoneal dialysis (CAPD), which performs the work of the kidneys, is used in treating chronic renal failure. CAPD (hereafter referred to as PD) can be conveniently done at the patient's home.Other advantages of it include less adveres effects on the cardiovascular system as homeostasis of body fluid is maintained;less dietary restriction;reduction in the frequency of outpatient treatment;availability of a system suited to the patient's lifestyle;and being comparatively easy to return to work. Nonetheless, home PD patients remain at risk of peritonitis, exit-site infection, and undernutrition due to protein-losing enteropathy. Moreover, patients may feel that they have gotten wary of taking care of themselve. Recently, we have come upon a case of end-stage renal failure in a 59-year-old articular rheumatic who was unable to exchange dialysis bags because her finger joints were deformed. So, we applied a PD system using the SMAP method to the patient with the result that she became able to manage to operate the PD by herself.Our patient wished to take it upon herself to perform PD using a UV-F twin-bag system. Ordinarily, bags have to be exchanged four times a day and every time bags are exchanged, several clamping procedures are required. It was impossible for her to exchange bags because of deformed finger joints. To make the impossible possible, a supplementary device for exchanging bags had to be fixed. It was also necessary for her to learn how to manipulate the newly attached device in addition to the existing device. With the SMAP method, there is plenty of time before PD gets started. So it was possible to examine the self-management method thoroughly and learn the trick of operating the new device. Our experience showed that PD by the SMAP method is useful to articular rheumatic patients with finger joint deformities.

3.Review of the results of cervical cancer mass screening during the last 10 years.

From April 1983 through March 1993, 10, 767 women underwent health examinations at the Health Care Center in Obihiro Kosei Hospital. Cervical smears were taken from theuterine cervix for cervical cancer screening. One hundred and six women had abnormal results, greater than class III. Those patients who were diagnosed as having carcinoma numbered 10 (0.09%). Of the cervical carcinomas found, 1 was frankly invasive (adenocarcinoma Ib); 3, microinvasive (2; squamous cell carcinoma and 1; co-existence of adenocarcinoma and squamous cell carcinoma); and 6, carcinomas in situ (squamous cell carcinoma).
4.Physical Therapy for Extrapersonal Neglect during Walking:Attempt Intervention Using Transcutaneous Electrical Nerve Stimulation and a Smartphone

Objective:The purpose of this study was to determine whether combination therapy has an effect on extrapersonal neglect (EP).Methods:The subject, who was capable of walking without the help of a cane or an orthosis, had right brain damage with left unilateral spatial neglect. We used a single-case study (BAB) design, consisting of a 2-week intervention phase, a 2-week baseline phase, and another 2-week intervention phase. The subject was trained seven times a week. Period A (baseline) included normal visual scanning training during walking, while Period B (intervention) included both normal visual scanning training with transcutaneous electrical nerve stimulation (TENS) for neck muscles and video feedback (VF) with a smartphone during walking. The primary outcomes used Behavioral Inattention Test (BIT), and the Catherine Bergego Scale (CBS).Results:Both BIT and CBS improved in Period B. In particular, the CBS sub-items “Collides with people or objects on the left side” and “Experiences difficulty in finding his way towards the left when traveling in familiar places or in the rehabilitation unit” during walking improved, showing improvement in the results of CBS observation scores and anosognosia scores.Conclusion:In this study, we suggested that combination therapy has a beneficial effect on EP and awareness. We also reported the safety and feasibility of the combination therapy.

8.Studies on asymptomatic microhematuria observed in health care center of Obihiro Kosei hospital.

From January 1981 through December 1986, 27, 513 individuals consisting of 17, 918 males and 9, 595 females underwent health examinations at Health Care Center in Obihiro Kosei Hospital. 14.2% of healthy and asymptomatic men and 29.1% of healthy and asymptomatic women had microscopic hematuria (one or more RBCs per HPF).
To evaluate the clinical significance of microhematuria, about one third of patients with asymptomatic microhematuria who had undergone complete urological examination were reviewed.
Genitourinary neoplasms were found in 0.21%(8 bladder cancers, 2 prostatic cancers and 1 renal cell cancer). The incidence of cancers increases proportionate to increase in age, furthermore, lesions were found more commonly in men than in women. We could find no relationship between the degree of hematuria and the cause. The results suggest that patients with asymptomatic microhematuria should undergo urological examinations.
