1.Value of susceptibility weighted imaging in glioma classification

Objective To study the characteristics of susceptibility weighted imaging (SWI) and it's role in glioma classfication. Methods 45 glioma patients with pathologically confirmion, underwent preopera-tive plain MRI, enhanced MRI and SWI examination. By using double blind method, the score of T1 WI, T2WI, enhanced T1 WI, SWI and enhanced SWI were acquired to display enhancing tumor, peritumoral edema, tumor hemorrhage and tumor vein. The tumor volume of low signal areas was measured by using measurement software to acquire the bleeding of tumor. Results In 26 cases of high grade gliomas,18 patients with intratu-moral, multiple patchy low signal cords in varying degrees, which were proved by pathology as tumor hemorrhage and tumor blood vessels; in 19 case of low grade gliomas, 10 cases occured a few spots, linear low signal in tumor, which were proved by pathology as tumor hemorrhage; the bleeding of high grade gliomas was higher than that of the low level group ( P < 0.05 ). SWI was superior to T, WI, T2 WI on displaying tumor hemorrhage and tumor vein(P<0. 05). Conclusion Different grades of glioma show obvious different display on SWI, and SWI probably be helpful for evaluation of glioma grading preoperatively.
2.CT Guided Interventional Treatment of Intracranial Hematoma

Objective To investigate the value of CT guided interventional treatment in intracranial hematoma.Methods There were 108 cases with intracranial hematoma(30~100 ml,average of 50 ml) in our hospital from 2000~2003.Among them,58 cases were located in putamen,6 cases in subcortial,4 cases in thalamus,4 cases in cerebella,12 cases were traumatic eprdural hematoma and 12 cases were subdural hematoma.The table distance of the aspirated slice was the sum of table distance at the slice of the maximum hematoma and half slice thickness.It was considered successful for CT guided aspiration if the distance between the front of aspiration needle and the center ofthe biggest hematoma was not more than 1mm and it was regarded effective,when the patients' lives were saved and their life quality was improved.Results The success rate of CT guided aspiration was 99.8%.The effective rate of hypertensive intracerebralhemorrhage located in putamen,subcortial and thalamus was 94.3%.The effective rate of CT guided interventional treatment for cerebellar hypertensive intracerebral hemorrhage above 30ml was 0,the cure rate of traumatic epidural and subdural hematoma was 97.2%.Conclusion The effect of CT guided interventional treatment of intracranial hematoma is well except for cerebellar hematoma.
3.Effects of TCM Periodic Therapy on Sex Hormone, Glucose and Lipid Metabolism of Patients with Polycystic Ovary Syndrome

Objective To observe the clinical efficacy of TCM periodic therapy combined with estradiol progesterone tablets (Diane-35) in the treatment for polycystic ovary syndrome (PCOS), and its effects on serum sex hormone, glucose and lipid metabolism.Methods Totally 74 patients with PCOS were randomly divided into Western medicine (WM) group and Chinese and Western medicine (CWM) group, 37 cases in each group. WM group was given Diane-35, while CWM group was treated with TCM periodic therapy additionally, for three courses, 28 d of one course. Clinical symptoms, sex hormones and glucose and lipid metabolism of two groups were observed before and after treatment and withdrawal treatment for 3 months.Results Two patients in each group were lost to follow-up. Markedly effective rate and total efficiency rate of CWM group were better than WM group (P<0.05). After treatment, symptom integrals of CWM group decreased significantly (P<0.01). Serum E2 increased (P<0.01), while LH, T and LH/FSH decreased in two groups (P<0.01). After treatment and withdrawal treatment for 3 months, improvement of clinical symptoms and sex hormone levels in CWM group was superior to CM group (P<0.01), FPG, FINS, TG, TC and BMI of CWM group decreased in CWM group (P<0.05), and lower than that of CM group (P<0.05,P<0.01). Adverse reaction rates of WM group and CWM group were 17.1% and 5.71% (P<0.05).Conclusion Efficacy of TCM periodic therapy combined with Diane-35 in treatment of PCOS is significant, which can obviously improve patients’ clinical symptoms, and regulate hormone and lipid metabolism disorders, with fewer adverse reactions.
4.Plate fixation versus intramedullary nailing for midshaft clavicular fractures: a prospective rndomized controlled trial

Objective To compare the clinical effects of closed reduction and intramedullary nailing versus open reduction and plate fixation in the treatment of displaced midshaft clavicular fractures.Methods A prospective,randomized,controlled trial was performed between July 2012 and May 2014 in 194 patients with acute displaced midshaft clavicular fracture.They were 136 males and 58 females,from 16 to 65 years of age.They were randomly divided into 2 groups to received either closed reduction and intramedullary nailing (n =98) or open reduction and plate fixation (n =96).We recorded operation time,intraoperative blood loss,hospitalization expenses,fracture healing time and complication rate.Functional assessments were conducted at 3,6 and 12 months using the Disabilities of the Arm,Shoulder and Hand (DASH) and Constant-Murley scores.The preoperative general data showed no statistical significance between the 2 groups (P > O.05).Results All the 194 patients were followed up for 12 to 27 months (average,15.6 ±3.1 months).The operation time (27.7 ± 16.3 min),intraoperative blood loss (18.6 ± 14.4 mL),hospitalization expenses (12,462.0 ±3,263.4 yuan),and fracture healing time (12.1 ± 3.0 weeks) in the intramedullary nailing group were significantly better than those(62.3 ± 19.4 min,40.3 ± 17.4 mL,24,760.0 ± 3,320.4 yuan,and 16.9 ± 2.8 weeks) in the plate fixation group (P < 0.05).At 3 months after surgery,the DASH (13.4 ± 3.5) and Constant-Murley (88.5 ±5.9) scores in the plate fixation group were significantly better than those (24.8 ± 6.7 and 69.1 ± 6.2,respectively) in the intramedullary nailing group (P < 0.05).At 6 and 12 months after surgery,there were no significant differences in the Constant-Murley scores or DASH scores between the 2 groups (P > 0.05).The complication rate in the intramedullary nailing group (25.5%,25/98) was significantly higher than in the plate fixation group (9.4%,9/96) (P < 0.05).Conclusions In the treatment of displaced midshaft clavicular fractures,compared with open reduction and plate fixation,closed reduction and intramedullary nailing has advantages of less invasion,lower cost and faster fracture healing,but a disadvantage of higher complication rate.The 2 treatments may lead to similar functional recovery of the shoulder.
5.Influence of new baicalin-metal complexes on Kv1.4 and Cav3.2 ion channels

OBJECTIVE To investigate the effect of new baicalin(BC) metal ions(Co2+,Cu2+, and Ni2+)complexes(BMCs)on ion channels Kv1.4 and Cav3.2. METHODS HEK293 or CHO cells loaded with various ion channels(hERG,Kv1.2,Kv1.3,Kv1.4,Kv1.5,Kv1.6,Kv1.7,Kv1.8,Kir1.1, Kir2.1,KCNQ and Cav3.2)were obtained by stable transfection method. Whole-cell patch-clamp tech?nique was used to record current changes of each ion channel induced by BC and BMC in 10μmoL · L-1. The effect of different concentrations(0.3,1,3,10 and 30μmoL · L-1)of BC-Co and BC-Cu on Kv1.4 and Cav3.2 current was detected by whole-cell patch-clamp technique. RESULTS A model of HEK293 cells or CHO cells that stably expressed various ion channels was obtained. BMCs (BC-Co,BC-Cu and BC-Ni)had some impact on various ion channels,especially on Kv1.4 and Cav3.2. The inhibitory rate induced by BC-Co,BC-Cu and BC-Ni(10 μmol · L-1)was 91%,76% and-10%,respectively,for Kv1.4;and 43%,57%and-14%,respectively,for Cav3.2. IC50 of BC-Co was 1.69 and 0.81μmoL·L-1 for Kv1.4 and Cav3.2. IC50 of BC-Cu was 1.66 and 0.58μmoL · L-1 for Kv1.4 and Cav3.2. CONCLUSION BC-Cu and BC-Co concentration-dependently inhibit Kv1.4 and Cav3.2 ion channels.
6.Diffusion-weighted magnetic resonance imaging in glioma classification

Objective To evaluate the usefulness of diffusion-weighted magnetic resonance imaging (DWI) and apparent diffusion coefficients (ADC) in differentiating tumor, edema, and normal brain tissue, and in classification of cerebral gliomas. Methods Using Philips Achieva 1.5T super conduct MR scanner, 46 patients with gliomas underwent conventional MR imaging and DWI. The ADC of regions of interest ( ROI ) were measured with two different b values, 0s/mm2 and 1000s/mm2. ROIs were manually placed over areas of tumor,edema, and normal brain tissue. Results The signals of DWI and ADC map in tumor tissue were respectively higher and lower than those in normal brain tissue. The ADC values in tumor were 1.68 ± 0.18,1.65 ± 0.20,1.29 ±0.16,1.21 ±0.21 for grade Ⅰ、Ⅱ、Ⅲ、Ⅳ gliomas, respectively; the corresponding figures in edema were 1.74 ±0.22,1.72 ±0.19,1.35 ±0.20,1.28 ±0.19, respective. The ADC values in tumor were not significantly different from the values in edema (P >0.05); however, both values were significantly different from those in normal brain ( P <0.05 ). The ADC value of low grade ( grade1-2 ) gliomas was significantly higher than that of high grade (grade3-4) glioma (P<0.05). Conclusion ADC values can aid in distinguishing tumors from normal tissues, but can not distinguish tumors from adjacent edema. Individually, ADC values overlapped considerahly. ADC value is associated with cellularity of gliomas. DWI can help characterizing the malignancy of gliomas.
7.Measuring the electricity frequency properties of blood.

In order to understand the electricity frequency specialties of blood, we have developed a wide frequency electricity characteristic testing system and used it to test the amplitude frequency property and phase frequency property of the blood in different states and constituents at 1 Hz to 20 MHz. Further analysis on the results of tests helped us know some important properties of blood at even more microcosmic levels from a new angle. Meanwhile, some problems and considerations on the improvement of the electricity model of biotic tissue and blood were pointed out. (1) From 1 Hz to 5 KHz, the impedance of blood descended 99%. (2) Simple equivalent circuit of resistance and capacitance must be used in series equivalent but not in usual parallel connection equivalent. (3) Experiment indicated, equivalent circuits of blood need more analysis, because simple equivalent circuit of resistance and capacitance is liable to gross error. (4) When the three element model is used for measuring the resistance of inside liquid, capacitance of cell membrane and the resistance of outside liquid of blood, the three testing frequencies must be very similar.

8.Research on electricity frequency property of blood.

On the basis of our previous work, the electric frequency property of human blood in different components, in physiological state and in pathological state (diabetes) are tested and analyzed in the range of 1Hz-20MHz progressively. Among the different components of blood; the lowest electrical impedance is serum; the plasma and the whole blood gradually become larger, the blood corpuscle is the largest one. Otherwise, the negative phase of serum is the largest, the plasma and the whole blood are lower, and the blood corpuscle is the lowest. Here, the question is why the effect of the electric capacity of serum and plasma is the biggest in the condition of no cell and cell membrane; diabetes mellitus is an endocrine disorder in which blood changes obviously, the electric frequency property of the blood of diabetic patients changes markedly; the electrical impedance of blood decreases (more obviously with low frequency), the negative phase increases (more obviously with high frequency). These indicate that the increase of electric conductivity in diabetic patients' blood is due to electric capacitance conductivity that is related to the changes of cell membrane, deformation abilities and aggregation of RBC. Related experiments demonstrate again that with the progressing of research in the electric frequency property of blood, we may use the theory and method of electricity to examine some important characters of blood in a different way, and so to corroborate other tests and analyses.

9.Correlation between the immunophenotypes, treatment strategies and prognosis of gastrointestinal diffuse large B-cell lymphoma

Objective To explore the differences in the prognosis of patients with different immunophenotypes gastrointestinal diffuse large B-cell lymphoma (GI-DLBCL) who received different treatment strategies.Methods From March 2006 to January 2016,at Nanfang Hospital,Southern Medical University in Guangzhou,the clinical data of 99 patients with pathologically confirmed GI-DLBCL were retrospectively analyzed.According to treatment strategies,patients were divided into chemotherapy alone group and combination of surgery and chemotherapy group.According to immunophenotypes,patients were divided into germinal center B-cell-like (GCB) type and non-GCB type.The two-year progression-free survival (PFS) rate and overall survival (OS) rate were evaluated.Kaplan-Meier analysis,log-rank test and Cox regression were performed for statistical analysis.Results Among the 99 patients with GI-DLBCL,51 patients were treated with chemotherapy alone,and 48 patients were treated with combination of surgery and chemotherapy.Forty-one cases were GCB phenotype and 40 cases were non-GCB phenotype.The median follow-up time was 25 months.The two-year PFS and OS rates were 70.9% and 89.5%,respectively.The two-year PFS and OS rates of chemotherapy alone group were 63.6% and 85.0%,respectively;both were lower than those of combination of surgery and chemotherapy group (79.4% and 94.7%),and the differences were statistically significant (x2 =4.232,P =0.040 and x2 =4.260,P =0.039).The two-year PFS and OS rates of GCB group were 68.8% and 93.9%,respectively.And the two-year PFS and OS rates of non-GCB group were 73.2% and 85.6%,respectively.There were no statistically significant differences between these two groups (both P > 0.05).Among 41 patients with GCB type,25 were treated with combination of surgery and chemotherapy and 16 were treated with chemotherapy alone.The two-year PFS rate of patients treated with combination of surgery and chemotherapy (83.1%) was higher than that of patients treated with chemotherapy alone (49.2%),and the difference was statistically significant (x2 =5.627,P =0.018).The results of multivariate analysis indicated that treatment strategy was not an independent prognostic factor for all the enrolled patients and in patients with GCB type (all P > 0.05).Conclusions Immunophenotypes may lack evaluation value of prognosis in patients with GI-DLBCL.Although among all the enrolled patients and patients with GCB type,the prognosis of patients treated with combination of surgery and chemotherapy is better than that of patients treated with chemotherapy alone,treatment strategy is not an independent prognostic factor.Multi-factors should be evaluated before selection of treatment strategy.
10.Prognostic efficiency of interim 18F-FDG PET/CT in patients with diffuse large B-cell lymphoma

Objective To explore whether the prognostic efficiency of international prognostic index (IPI) can be further improved by interim 18F-fluorodeoxyglucose (FDG) PET/CT.Methods A total of 185 patients (116 males,69 females;average age 49 years) with pathologically confirmed diffuse large Bcell lymphoma (DLBCL) from January 2004 to January 2014 were enrolled in this retrospective study.The risk was classified by IPI (0-2 was considered as low risk,3-5 was considered as high risk) and all patients underwent 18F-FDG PET/CT scan before and after 4 cycles of immunochemotherapy.Based on interim 18 F-FDG PET/CT imaging,5-point Deauville score was used to reclassify DLBCL patient into negative group and positive group,and the interpretations were evaluated for 2-year progression-free survival (PFS) and overall survival (OS) rates.Kaplan-Meier analysis,log-rank test and Cox regression were used for data analysis.Results With a median follow-up of 27 (2-146) months,the 2-year PFS and OS rates were 60%(111/185) and 81%(150/185),respectively.A total of 114 patients were included in the low-risk group and 71 patients were in the high-risk group.Both 2-year PFS rates and OS rates between the 2 groups were statistically different:72%(82/114) vs 37%(26/71),x2=20.86,P<0.01;90%(103/114) vs 63%(45/71),x2=13.39,P<0.01.The interim PET/CT showed 113 patients with negative results and 72 patients with positive results,whose 2-year PFS rates and OS rates were also statistically different:82% (93/113) vs 24%(17/72),x2 =66.66,P<0.01;90%(102/113) vs 51%(37/72),x2 =33.11,P<0.01.In the low-risk group,85 patients were PET-negative and 29 patients were PET-positive.The 2-year PFS rates were 88% (75/85) and 31%(9/29),respectively (x2 =35.52,P<0.01).The 2-year OS rates were 96% (82/85) and 66%(19/29),respectively (x2 =11.88,P<0.01).In the high-risk group,28 patients were PET-negative and 43 patients were PET-positive.The 2-year PFS and OS rates were 64%(18/28) vs 19%(8/43;x2 =17.33,P<0.01) and 86%(24/28) vs 49%(21/43;x2=9.95,P<0.01),respectively.Conclusions Both IPI and interim 18F-FDG PET/CT have the prognostic value for patients with DLBCL.Interim 18F-FDG PET/CT can improve the prognostic efficiency of IPI.
