Objective: To evaluate sensory recovery in noninnervated radial forearm free flap after tongue reconstruction. Methods: Sixty-five cases of tongue reconstruction by using free radial forearm flaps were analyzed. Flap sensations to touch, sharp vs dull, two-point discrimination and warmth vs cold was evaluated in each of these patients at 6 months and 12 months after treatment. Results:Twenty-nine flaps (44.6%) showed good sensory recovery, thirty-two flaps (50%) recovered partly, four flaps (6.1%) was anesthetic. Conclusion: Spontaneous recovery of flap sensation can be re-established after reconstruction. Delay or failure of sensory recovery in flap reconstruction could be caused by radiation therapy.

Objective To investigate the features of resting energy expenditure (REE) in patients with well controlled type 2 diabetes mellitus (T2DM). Methods Totally 45 T2DM patients with stable blood glucose were enrolled. The general conditions, biochemical indicators, measurements of REE ( MREE), and basal energy expenditure (BEE) calculated with Harris-Benedict formula (HBEE) and Owen formula (OBEE) were recorded and compared. Results MREE had no significant difference with HBEE or OBEE in T2DM patients with stable blood glucose (P > 0. 05). Correlation analysis showed that REE was significantly correlated with gender, age, body weight, body height, body surface area, and fat-free mass ( all P < 0.05 ), but was not correlated with body mass index, fat mass, fasting plasma glucose, postprandial plasma glucose, haemoglobin Alc, total cholesterol, triglyceride, total protein, albumin, and haemoglobin (P > 0. 05 ). Multiple regression analysis showed that fat free mass and age had the closest correlation with REE. Conclusions REE does not increase in T2DM patients with well controlled blood glucose. Factors that influence their REE are similar with healthy individuals. Determi nation of REE can provide useful information for the nutrition treatment of T2DM.

OBJECTIVE: To evaluate the efficacy and safety of Mahuang Zhisou Capsule (MHZSC), a compound traditional Chinese herbal medicine, in treatment of acute upper respiratory tract infection of wind-cold syndrome. METHODS: A multi-center, double-blind, double dummy, randomized controlled trial was conducted. A total of 240 patients were randomized into treatment group (120 cases, treated with MHZSC) and control group (120 cases, treated with Mahuang Zhisou Pill). The therapeutic course was 3 days. RESULTS: Seven cases were excluded and three cases lost to follow-up. Two hundred and thirty-three cases entered intention-to-treat (ITT) analysis and 230 cases fitted in per-protocol population set (PPS) analysis. After treatment, the therapeutic effect of upper respiratory tract infection was calculated by ITT and PPS analysis respectively. The significant response rate and total response rate in the treatment group were 78.07% and 98.25% (ITT and PPS), while the significant response rates in the control group were 70.59% (ITT) and 70.69%(PPS), and the total response rates were 95.80% (ITT) and 95.69% (PPS) respectively. There were no significant differences in clinical efficacy between the two groups (P>0.05). CONCLUSION: MHZSC is effective and safe in treatment of acute upper respiratory tract infection of external wind-cold syndrome.

Chinese Medical Journal 2009;122(24):2961-2966
BACKGROUNDSympathetic ophthalmia (SO), a rare, bilateral, diffuse granulomatous uveitis, usually occurs after open globe injury or intraocular surgery. We sought to identify the risk factors for the development of SO after open globe injury and describe their demographic and clinical features and outcomes of treatments.
METHODSA retrospective study of inpatients with globe injury in 15 tertiary referral hospitals of China from January 2001 to December 2005 was conducted. The information of demography, nature and mechanism of injury, time and ways of treatments and outcomes was reviewed. Diagnosis of SO was made based on a history of ocular trauma or surgery and subsequent development of bilateral or contralateral uveitis consistent with SO. Any association between related parameters and development of SO was analyzed.
RESULTSAmong 9103 patients (9776 eyes) of globe injury, SO occurred after open globe injury in 18 cases with an occurrence rate of 0.37%, vitrectomy of closed globe injury in 2 (0.37%) and perforation of burned eyes in another 2. For open globe injury, the median age ((36.72 +/- 13.59) years, P = 0.01) was higher in patients with SO; there were no significant effects of sexes, injury type, uvea proplaps, once or multi-intraocular surgery, once or multi-vitrectomy and endophthalmitis on incidence of SO; 0.70% endophthalmitis concurred with SO; 83.33% of SO occurred within 1 year after injury or last ocular surgery. SO developed in a fellow eye one week after evisceration of the perforating burned eye. Good final visual acuity was obtained in sympathizing eyes with prompt treatment.
CONCLUSIONSFor open globe injuries, SO sufferers were relatively older and any injury type could induce SO with equal possibility. The initial open globe injury was more likely to be the trigger of SO than subsequent intraocular surgeries including vitrectomy. Prophylactic enucleation after injury is not recommended.

Objective To observe the therapeutic effect of DENG Tie-tao's Foot-bath Recipe (mainly composed of Radix Achyranthis Bidentatae,Rhizoma Chuanxiong,Spica Prunellae,Fructus Evodiae,Ramulus Uncariae Cum Uncis, etc.)for the treatment of essential hypertension with hyperactivity of liver Yang syndrome. Methods Eighty essential hypertension patients with hyperactivity of liver Yang syndrome were randomly divided into treatment group and control group,40 cases in each group. Both groups were given basic antihypertensive western medicine treatment, and additionally the treatment group was given external application of DENG Tie-Tao's Foot-bath Recipe and the control group was given foot-bath with warm water. Seven days constituted one treatment course. Before and after treatment , the scores of traditional Chinese medicine(TCM)symptoms and Athens Insomnia Scale were observed , blood pressure variability (BPV)was detected with ambulatory blood pressure monitoring instrument , and plasma norepinephrine(NE) and epinephrine(E) levels were examined by enzyme-linked immunosorbent assay (ELISA). Results (1)The total effective rate for improving TCM symptom scores of the treatment group was 87.5%, and that of the control group was 75.0% , the difference being significant (P < 0.05). (2)The total effective rate for lowering blood pressure of the treatment group was 85.0%, and that of the control group was 72.5% , the difference being significant (P < 0.05). (3)After treatment, scores of Athens Insomnia Scale in the two groups were obviously decreased(P<0.01 compared with those before treatment),and the decrease in the treatment group was superior to that in the control group (P <0.01). (4)After treatment,24h BPV,daytime BPV and night BPV in the two groups were decreased to certain degrees (P < 0.05 compared with those before treatment),and the decrease of 24h BPV and night BPV in the treatment group was superior to that in the control group(P<0.05).(5)After treatment, plasma NE and E levels in the two groups were decreased to certain degree (P < 0 . 05 compared with those before treatment), and the decrease in the treatment group was superior to that in the control group (P < 0.05). Conclusion DENG Tie-Tao's Foot-bath Recipe exerts synergistic action on lowering blood pressure steadily by relieving TCM symptoms, improving the quality of sleep, decreasing 24hBPV of essential hypertension with hyperactivity of liver yang syndrome. And the regulation of plasma NE and E levels may be one of its therapeutic mechanisms.
