2.Changes of P wave dispersion on ECG in patients with paroxysmal atrial fibrillation after radiofrequen-cy ablation

Objective:To explore the influence of radiofrequency ablation (RFA)on P-wave dispersion (difference between maximum and minimum P-wave interval)in patients with paroxysmal atrial fibrillation (PAF).Methods:A total of 95 PAF patients underwent circumferential pulmonary vein isolation.The preoperative and postoperative 12-lead ECG were recorded for 24h,maximum P-wave interval (Pmax)and minimum P-wave interval (Pmin)were measured,and P-wave dispersion was calculated.Since three months after operation,patients received outpatient follow up and dynamic ECG every month for six months.The operation was judged for successful or not according to symptoms of atrial fibrillation and dynamic ECG results,and patients were divided into success group (n=70)and recurrence group (n=25).Results:Compared with before operation,there was significant reduction in P-wave dis-persion [(48.84±4.08)ms vs.(42.22±4.02)ms,P <0.01]in all PAF patients after RFA.Compared with be-fore operation,there was significant reduction in P-wave dispersion [(47.63±3.58)ms vs.(40.67±4.82)ms,P<0.01]in success group;and reduction in P-wave dispersion [(49.51±1.64)ms vs.(48.26±3.48)ms]in recur-rence group,but no significant difference (P =0.346).After circumferential pulmonary vein isolation,P-wave dis-persion in success group was significantly lower than that of recurrence group (P <0.01).Conclusion:Circumferen-tial pulmonary vein ablation can improve P-wave dispersion in patients with atrial fibrillation,and it's more signifi-cant for success group.P-wave dispersion can predict the recurrence of atrial fibrillation after circumferential pulmo-nary vein ablation.
3.Transnasal endoscopic removal of lacrimal sac and intraorbital foreign body.

We reported a case of a work-related accident involving periorbital and intraorbital metal foreign bodies. The patient was a 43-year-old woman whose nasal dorsum was struck while she was sawing wood with nails. X-ray plain films of skull bone and computed tomographic scan of nasal sinus revealed two 5-mm-long metal objects lodged in right lacrimal sac and medial bulbar space of the right orbit. No deficits were noted in her visual acuities, visual fields, and ocular movements. Based on accurate positioning by imaging examination, transnasal endoscopic removal of metal foreign bodies was successfully accomplished. The patient recovered well without any complications after surgery.

4.A comparative trial of a combined therapy (Yangyin-Qinggan decoction combined with paroxetine)versus mono-therapy on depressive patients with somatic symptoms

Objective To evaluate the effectiveness of treating depressive patients with somatic symptoms with combined TCM and western medicine.Methods 69 patients were randomly divided into 2 groups:a combined therapy group (treated with Yangyin-Qinggan decoction and paroxetine) and mono-therapy (treated with paroxetine alone as an active control),The depressive and somatic symptoms were assessed before (0 week),during (2 weeks and 4 weeks) and at the end point (8 weeks) of the treatment.Results ①Both therapies alleviated the depressive symptoms:HAMD assessed among patients receiving combined therapy are listed as:(0 week:19.29±2.38),(2 weeks:17.38 ± 2.37),(4 weeks:15.27 ± 2.15),(8 weeks:13.35 ± 2.09) ;combined therapy started to ease the depressive symptoms after 2 weeks of treatment (2 week compared with 0weeks),similar improvements could also be noticed after 4 weeks of treatment 4 weeks with 2 weeks:P<0.05)and at the end of this research (after 8 weeks of treatment:8 weeks with 4 weeks:P<0.05).HAMD for mono-therapy are as:(0 week:18.69±3.03),(2 weeks:16.63±3.09),(4 weeks:15.20±2.95),(8 weeks:14.60±2.72) ; mono-therapy started to alleviate the depressive symptoms also,after 2 weeks of treatment (2 weeks compared with 0 week:P<0.05),yet only slight improvements could be seen after 4 weeks (4 weeks with 2 weeks:P>0.05) and 8 weeks of the treatment (8 weeks with 4 weeks:P>0.05).The combined therapy turned to be more effective in alleviating depressive symptoms at the end point of the treatment (P<0.05).②In terms of improving the somatic symptoms,the Somatization Symptom Scale (SSS) among patients with combined therapy were as follows:(0 week:48.74±4.07),(2 weeks:46.74±4.16),(4 weeks:43.74±3.77),(8 weeks:41.18 ± 3.50) ; Combined therapy was witnessed to start to ease those symptoms after 2 weeks o f treatment (2week compared with 0 weeks,P<0.05),similar patterns were found again,after 4 weeks (4 weeks with 2 weeks:P<0.05) and 8 weeks of the treatment (8 weeks with 4 weeks:P<0.05).Mono-therapy was found to ease the somatic symptoms in a less-effective way,yet no difference was found between any two SSS after 2 weeks,4weeks or 8 weeks of treatment (P>0.05,respectively).Combined therapy was better at improving the somatic symptoms (P< 0.01).Conclusion Combined therapy proved to be more effective in both relieving depression and somatic symptoms.
5.Simple virilizing with 21-hydroxylase deficiency: a report of 5 cases and literature review

The clinical manifestations, laboratory tests and treatment of 5 cases of simple virilizing with 211-hydroxylase deficiency were analyzed. The average age was 17 at the first hospital visit. The manifestation included abnormal external genitalia at birth, no secondary sexual characteristics developed during adolescence and no menstrual cycles, and their near-adult height (NAH) was affected. The chromosome karyotype of the patients was 46 XX. The CT scans showed bilateral adrenal hyperplasia. The testosterone levels were (665 ± 334 ) μg/L, which were reduced to (81 ± 25 ) μg/L after dexamethasone suppression. The levels of adrenocorticotropic hormone were 123 (57 - 563) ng/L and < 5 ng/L respectively before and after dexamethasone suppression test. Early diagnosis and early treatment is particularly important for improvement of the disease outcomes.

Objective To explore the TCM syndrome characteristics of cardiac syndrome X(CSX).Methods The signs and symptoms of 51 patients with CSX were analyzed according to the diagnosis of TCM syndromes to summarize their syndrome character- istics.Results of the 51 CSX cases,the following signs and symptoms took dominance:chest pain,fullness in chest,epigastric and abdominal distention,emotional distress,dark purple tongue with petechia,greasy coating,string-taut pulse.The syndromes were mainly of Biao-Superficial excess,including qi stagnation,phlegm retention and blood stasis,occupying 66. 7%,accompanied with Benroot deficiency,including qi deficiency,yin deficiency,qi and yin both deficiency,occupying 33.3%.Conclusion Qi stagnation, phlegm retention and blood stasis are the primary syndromes of CSX.
7.The efficacy study of chitosan in combination with far infrared treatment on the blood flow of arteriovenous fistula

Objective To investigate the role of chitosan in combination with far infrared treatment on vascular access flow of arteriovenous fistula .Methods A total of 172 hemodialysis patients were recruited in this study .The patients were admitted to the blood purification center in Department of Nephrology in Daping Hospital from 2012 to 2013 .The 172 hemodialysis patients ,were randomized into 4 groups ,43 patients in each group ,including control group ,chitosan group ,far infrared group and combined treat‐ment group .The combined treatment group were treated with chitosan and far infrared in vessel puncture for 40 min at the begin‐ning of each dialysis ;chitosan group accepted the chitosan treatment only ;far infrared group just used far infrared therapy device ;control group were treated with conventional nurses .Arteriovenous fistula blood flow was detected by doppler ultrasound equip‐ment ,the maximum blood flow Qa0 (at the beginning of the study) ,Qa1 (1 month later in the study) ,Qa2(3 months later in the study) ,Qa3(6 months later in the study) ,were analyzed and compared .Results There were no significant difference at different times between the blood flow of arteriovenous fistula in the chitosan group (P>0 .05) .Qa2 and Qa3 of the far infrared group were significantly higher than Qa0 (P<0 .05) .Qa1 ,Qa2 ,Qa3 were significantly higher than Qa0 in the combined treatment group (P<0 .05) .Qa2/Qa0 and Qa3/Qa0 in the far infrared group were significantly higher than control group and chitosan group (P<0 .05) . Compared with the other three group ,Qa1/Qa0 ,Qa2/Qa0 and Qa3/Qa0 were significantly increased in the combined treatment group (P<0 .05) .Conclusion The chitosan and far infrared combined treatment can effectively protect the blood flow of arteriove‐nous fistula in the hemodialysis patients .The combined treatment is one effective method for nursing arteriovenous fistula .

10.Detections of brain biochemical changes in prefrontal lobes of the adolescents with depression using magnetic resonance spectroscopy

Objective To explore the brain biochemical changes in the frontal lobe of adolescents with depression using proton magnetic resonance spectroscopy (1H-MRS). Methods Twenty-four patients and twenty-three healthy subjects matched for age, sex and education level were enrolled in the study. All the subjects underwent multivoxel 1H MRS to measure the bilateral metabolic levels of N-acetylaspartate (NAA), choline (Cho), creatine (Cr) in the prefrontal lobes. Results The NAA/Cr and Cho/Cr ratios in the left dorsolateral prefrontal white matter of the depressive adolescents were significantly lower than those of the healthy subjects [NAA/Cr: 1.67 ± 0.32, t = 3.126, P = 0.004; Cho/Cr: 1.28 ± 0.30, t = 2.362, P = 0.024], and the ratios of NAA/Cr in the right dorsolateral prefrontal white matter of the depressive adolescents was also significantly lower than that of the healthy subjects [NAA/Cr:1.65 ± 0.26, t=2.969, P=0.006]. There was no significant difference in the metabolic ratios in the bilateral anterior cingulate gray matter between the depressive adolescents and the healthy controls. Conclusions Biochemical abnormalities in prefrontal white matter are involved in the pathophysiology of depression. Importantly , these abnormalities are already present early in the course of the disorder.
