1.Absorbability and bone-conductivity of nano-hydroxyapatite/collagen composite implanted under the calvarial periosteum

Objective:To study the bone-conductivity and absorbability of nano-hydroxyapatite/collagen(nHAC) composite implanted under the calvarial periosteum in rabbits.Methods:24 nHAC samples and 24 HA samples were prepaired in the shape of round disk with the diameter of 8 mm and thickness of 3 mm.nHAC samples were implanted under calvarial periosteum on the left side and HA samples on the right of 24 rabbits.The bone-conductivity and absorbability of the samples were examined by new bone height measuring and residual implant materia measuring 2,4,8 and 12 weeks after operation. Results:2,4,8 and 12 weeks after implantation new bone height(mm) in nHAC group was 0.54?0.09,0.72?0.12,1.83?0.14 and 2.63?0.07,that in HA group 0.13?0.11,0.31?0.12,1.23?0.05 and 1.75?0.14,respectively(P
2.Association between therapeutic effect of speech disorders and age on treatment of children with cerebral palsy

Objective To observe the association between therapeutic effect of speech disorder and the age on treatment of children with cerebral palsy,and to explore the best treatment period of speech disorder children with cerebral palsy.Methods One hundred and twenty-one cases of speech disorder children with cerebral palsy aged 1 to 6 years in Maternal and Child Health Center of Hebei Province,Hebei Children's Hospital,were selected as subjects,and the patients were randomly divided into training group,which included 64 cases who received scalp acupuncture combined with language and 57 cases treated with speech training group.After 6 courses of treatment,the therapeutic effect for speech disorder of children between 1 to 3 years old and > 3 to 6 years old were compared,the association between children's age for treatment and treatment effects was explored.Results The total treatment efficiency of 1 to 3 years and > 3 to 6 years old children in scalp acupuncture group combined with language training group were 95.45% and 83.33% respectively,the difference was significant (x2 =6.859 8,P < 0.05) ; while in 1 to 3 years and > 3 to 6 year-old children in speech training group the total treatment efficiency were 84.62% and 77.42%,and the difference was significant(x2 =6.625 7,P < 0.05).In both groups,total efficiency of 1 to 3 years and > 3 to 6 years old children were 89.58% and 80.82%,respectively,and the difference was statistically significant (x2 =9.617 5,P < 0.01).Conclusions The best treatment period of speech disorder children with cerebral palsy is less than 3 years old,as age grows,the effect is gradually decreasing.

Fluorogenic quantitative polymerase chain reaction(FQ-PCR) is an emerging technique with high specificity,susceptibility,automatism and has been widely used for the detection and quantification of microorganism,some genetic diseases and cancer.The design and establishment of standard FQ-PCR laboratory are required not only by gene expansion and diagnosis but also by biosafty and veracity.The aim of the paper is to introduce the structural characteristics,instruments requirement and comfort parameters of FQ-PCR laboratory.
4.A study of the effect of mannequin training on management of unanticipated difficult airway

Objective This study on a medium-fidelity simulator (SimMan , Laerdal Medical Corpo-ration,Wappingers Falls, NY, USA) examined the management of unanticipated difficult airway by residents of anesthesiology and the effect of training in this context.Methods 30 residents of anesthesiology were devided into two groups (T and E). There were two scenarios investigated:'can't intubate, can oxygenate'(CI) and'can't intubate, can't oxygenate'(CICO). The E group was trained by scene simulation in the second, fourth, sixth month respectively before and after the training, and the T group received the same situation simulation training on the same day after the theoretical training and in the second, sixth month after the training. In con-trast to standard operating procedures, the performance of two groups of trainees was recorded on the basis of pre established evaluation criteria. Data differences between the two groups were analyzed using SPSS 23.0,t test, M-W test andx2 test.ResultIn CI, success rate of placement of astandard and intubating laryngeal mask air-way were high in T group (87% vs. 55% ,P=0.037). This was sustained over time. There was no difference in duration and incidence of desaturation between two groups. In CICO, there was a more structured approach following training in T group (P<0.05), which wasn't sustained over time. But this was sustained over 6 months in E group.ConclusionSituational simulation training can significantly improve the anesthesiology residents' ability to take proper response measures to the unexpected difficult airway, and significantly shorten the training time for emergency airway treatment. Repeated situational simulation training should be conducted at intervals of 2 months or less, allowing residents to maintain emergency response to the emergency airway.
5.Discussion of anti-inflammatory mechanism of cyclooxygenase (COX-2) inhibitor in improving cardiovascular safety.

The new generation cyclooxygenase (COX-2) inhibitor could reduce the gastrointestinal side effect of NSAID drugs, but eventually increase the cardiovascular risk, because its selective inhibition of COX-2 induces the imbalance between PGI2 and TXA2 and the reduction of vasodilatory NO. Under pathological conditions, active oxygen species (O2-*2, etc) were used to induce endo- thelial dysfunction, activate NF-κB to induce expressions of pro-inflammatory cytokines IL-1β and TNF-α, increase ET-1, TXA2 with vasoconstrictor effect, reduce PGI2 and NO with vasodilatory effect, generate further oxidative damage together with NO, and reduce the bioavailability of NO. NO-NSAIDs and NO-Coxibs drugs raised the level of NO by introducing NO-donor (ONO2). NSAIDs drugs enhanced the anti-inflammatory activity of COX-2 and reduced gastrointestinal side effects by inhibiting selectively COX-2. If antioxidant structures with active ingredients of traditional Chinese medicines were introduced to improve the antioxidant activity of NSAIDs, they could scavenge the active oxygen species to protect the normal function of vascular endothelia and enhance the bioavailability of NO, which is conducive to enhance the cardiovascular safety of cyclooxygenase (COX-2) inhibitor.

6.Role of remote ischemic preconditioning in prevention of contrast induced -nephropathy in elderly patients undergoing coronary artery angiography

Objective To explore the role of remote ischemic preconditioning(RIPC)in prevention of contrast -induced nephropathy(CIN)in elderly patients undergoing coronary artery angiography(CAA).Methods 106 elderly patients were enrolled in this randomized control trial.According to random number table,the patients were randomized into control group (n =53)and RIPC group(n =53).All of the patients received 1 000mL of 0.9% sodium chloride injection before CAA.The RIPC group patients underwent RIPC in their right arms with sphygmomanometer cuff infla-tion for 5 minutes prior to the CAA,three cycles were repeated.Serum creatinine was detected before and 48 hours after CAA.Results CIN was reported in 10 cases in the control group and 3 cases in the RIPC group(χ2 =4.30, P =0.04).The levels of serum creatinine were increased[(96.38 ±9.50)μmol/L vs (88.87 ±10.24)μmol/L] after CAA in the control group(t =2.28,P =0.03),and there was no difference in the RIPC group(t =1.17,P =0.24).Conclusion RIPC has a protective effect on CIN in elderly patients in our study.Since this method is harm-less and cost effective,further studies is required to popularize PIPC to our clinical practice for prevention of CIN.
7.The reliability of a functioning assessment tool based on the International Classification of Functioning, Disability and Health

Objective:To investigate the reliability of a tool based on the International Classification of Functioning, Disability and Health (ICF) in the assessment of functioning despite disability.Methods:A total of 371 patients were assessed using a tool based on the ICF rehabilitation set combined with a numerical rating scale ranging from 0 to 10. The internal consistency, inter-rater reliability and intra-rater reliability were analyzed.Results:The scale′s Cronbach′s alpha coefficient was 0.89. The inter-rater correlation coefficient was 0.85, and the correlations among the items ranged from 0.78 to 0.94. The intra-rater correlation coefficient with the scale was 0.95, with the item correlations ranging from 0.72 to 0.97. Only item b230 Hearing function did not correlate well.Conclusions:The functioning assessment tool based on the ICF rehabilitation set when combined with a numerical rating scale has internal consistency, inter-rater reliability and intra-rater reliability sufficient for use in clinical practice.
8.Quality evaluation and stability investigation of asarone submicro emulsion injection.

The content of the asarone submicro emulsion injection was determind by HPLC method, and thereby a quality evaluation method was established based on indexes of pH value, particle size, peroxide value, methoxy aniline values, free fatty acid, lysophosphatidylcholine, visible foreign substances, insoluble particle, sterility, bacterial endotoxin and impurities, etc. The results showed that the injection exhibited uniform physical appearance and all the products were in milkwhite liquid. The content of the three batches products were respectively 102.9%, 100.8%, 97.70% of the labeled amount, with mean particle size of 210-250 nm, and other indexes all met with the standards. The reserved samples showed no obvious change in terms of detection indexes and indicated good stability after the accelerated stability test and long-term stability for 12 months. The quality evaluation method established in this study could be applied to quality control and stability investigation of asarone submicron emulsion injection, which laid a basis for further clinical research and application.

9.Effect of Yangxue Qingnao Granule on the Expression of CD11b in CA1 Region of Hippocampus of Vascular Dementia Rats.

OBJECTIVETo observe the effect of Yangxue Qingnao Granule (YQG) on the expression of CD11b in CA1 region of hippocampus of vascular dementia rats, and to explore its regulation on microglias.
METHODSTotally 144 SD rats were randomly divided into the sham-operation group, the vascular dementia model group (model), and the YQG treated group (treated). The vascular dementia rat model was prepared by modified Pulsinelli's four-vessel occlusion. Rats in the sham-operation group and the model group were administered with normal saline -(at the daily dose of 10 mL/kg) by gastrogavage, while those in the treated group were administered with YQG (0.32 g/mL, at the daily dose of 10 mL/kg) by gastrogavage. All administration was performed once per day for 8 successive weeks. The expression of CD11b in CA1 region of hippocampus of vascular dementia rats was detected at week 1, 2, 4, and 8, respectively.
RESULTSCompared with the sham-operation group, the expression of CD11b in CA1 region of hippocampus of vascular dementia rats were significantly enhanced in the model group at each time point (P < 0.01). Compared with the model group, the expression of CD11b in CA1 region of hippocampus of vascular dementia rats significantly decreased in the treated group at each time point (P < 0.01), especially at week 2.
CONCLUSIONObvious activation and proliferation of microglias could be seen in CA1 region of hippocampus of vascular dementia rats, and YQG could inhibit activation and proliferation of microglias.

10.Influence of parental compliance on therapeutic effect of children with epilepsy

Objective To investigate the influence of parental compliance on the therapeutic effect of children with epilepsy.Methods Two hundred and sixty children with epilepsy and their parents admitted to the First People's Hospital of Wenling from December 2013 to June 2016 were enrolled,and the classical Morisky medication adherence questionnaire (MMAS-8) was applied to evaluate the compliance of parents for treatment of their children with epilepsy;after the patient taking drug for 3 days,fasting venous blood was collected in the morning,the concentration of the blood drug was tested and the influence of parent compliance on the blood drug concentration of the child with epilepsy was evaluated.Results In 260 patients,122 cases took karma form,and 138 cases took valproate orally.There were parents with good medication compliance in 130 cases (50%),medium medication compliance 80 cases (30.76%) and poor adherence to the doctor order in 50 cases (19.23%).In cases using medication irregularly,there were 26 cases sometimes without taking any drug (10.0%),17 patients' medication being interrupted (6.54%) and 10 cases having excessive medication (3.85%);no relationships were found between parental compliance and each of the following items,family role,occupation and age (all P > 0.05);and the compliance was related to gender,indicating that women's good compliance level was higher than that of males';the education level was positively proportional to the compliance,and the compliance of parents with senior high school or above degree was higher than those with primary school and junior secondary school levels (83 cases vs.9 cases,38 cases,both P < 0.01).Under situation of parents with poor compliance,their children had blood drug concentration higher or lower than proper range of drug level (high in 22 cases,low in 41 cases,higher than the result in good compliance 0 cases and 17 cases respectively),thus seriously affected the safety and efficacy of the treatment;the patients' frequency of irregular medication in parents' good compliance group was significantly lower than that in parents' poor compliance group [3.08% (4/130) vs.72.0% (36/50),P < 0.05].Conclusion To improve the therapeutic effect of epileptic children,their parental good cooperation is necessary.
