3.Analyses for risk factors of sub-health status in Tibetan people with logistic regression model and negative binomial model

BACKGROUND:Researches about sub-health status in general community population are rare,especially in minor nationalities.OBJECTIVE:To explore the sub-health status and its risk factors with logistic regression model and negative binomial regression model in Tibetan people.DESIGN,TIME AND SETTING:A cross-sectional study was performed among subjects selected from Songpan county in the Aba Tibetan and Qiang Autonomous Prefecture of Sichuan Province on September 2007.PARTICIPANTS:A total of 1791 Tibetan subjects from were recruited from Songpan county in the Aba Tibetan and Qiang Autonomous Prefecture of Sichuan Province and were involved in this study,among which 1763 subjects completed the sub-health questionnaire.Their average age was 27 years,ranging from 10 to 80.There were 731 male subjects (41.5%)and 1032 female ones (58.5%).All subjects were informed of the survey regimen.METHODS:The sub-health rating scale framed by Chen Qingshan et al was used to assess the sub-health status of subjects with stratified and cluster sampling,including the prevalence rate of sub-health status,the number of sub-health symptoms and various dimensional prevalence.MAIN OUTCOME MEASURES:Risk factors of sub-health status with logistic regression model and negative binomial regression model.RESULTS:There were 1038 subjects suffering from one or more sub-health symptoms,and they were considered as being in the sub-health status with the prevalence of sub-health 58.9%.The split-half reliability and internal consistency of the scale were separately 0.7658 and 0.8154.Logistic regression results indicated that the seniors (P=0.0004),female (P<0.0001),marriage status (P=0.0008),mental labor (P=0.0021)and smoking (P<0.0001)were main contributing factors of sub-health status.However,marriage status (P=0.0689)and occupation (P=0.1398)weren't statistically significant in negative binomial regression analyses.CONCLUSION:Tibetan populations living in high altitude are also suffering from sub-health status and should be given more attention.Further more negative binomial regression model could be popularized to analyze sub-health and its influencing factors.

5.Application of anterior intervertebral fusion with SOLIS for the treatment of traumatic cervical disc protru-sion

Objective To evaluate the application of SOLIS in the surgery on anterior intervertebral fusion for the treatment of traumatic cervical disc protrnsion. Methods 53 traumatic single segment cervical disc protrusion were divided into three groups randomly. The A group (SOLIS) :n=18, SOLIS filled with autocancellous bone were implanted between the cervical vertebrae. The B group(Syncage) :n =16, as the first group Syncage filled with auto-cancellous bone were implanted. The C group:n=19,anterior cervical decompression and fusion with bone autograft and internal fixation with ACPS. The operations of the surgery on anterior cervical decompression in three groups were the same. Recovery of nerve function was observed by JOA score and the fusion state, stability and changes of interver-tebra height of fusion segment were evaluated by X ray. Results The average duration of the follow-up was 16.5 months in 53 cases. Bony fusion could be found 3~9months after operation in all cases. In all A (SOLIS) group cases,the movement of the implanted $OLIS Cage were not found. The heights of the cervical intervertebral spaces and the physiological curvature were restored without loss in the duration of follow-up. The average JOA was(10.4±2.4) be-fore operation and(14.9±2.0)after operation. There was highly significant variation between the former and the latter (P<0.01). In the B (Syncage)group,5 cases had cage subsidence, therefore the height of intervertebra and the phys-iological curvature were lost. The average JOA was (10.6±2.5) before operation and (13.2±2.3) after operation.There was significant variation between the former and the latter(P<0.05). In the C group the third group, the move-ment of the fixed titanium plate were not found in the duration of follow-up in 18 cases. The titanium plate fixation was loose in 1 case. The average JOA was(10.5±2.4) bofore operation and(15.1±1.9) after operation. There was high-ly significant variation between the former and the latter(P<0.01). After operation,there was significant difference in the JOA score between the A group and the B group,between the C group and the B group(P<0.05) ,and there was no difference between the A group and the C group(P>0.05). Conclusion Treatment of single segment prolapse of the cervical intervertebral disc with SOLIS cage has some advantages such as higher fusion rate, higher recovery rate of nerve function and low rate of loss of intervertebra] height.
6.Acinetobacter baumannii: Five Year Infection Surveillance

OBJECTIVE To study the isolation rate of Acinetobacter baumannii, characteristics of infection distribution and antimicrobial resistance rates. METHODS Surveillance data of A. baumannii infection of wards, its distribution and resistance rates to 20 kinds of antibiotics in our hospital for 5 years were analyzed . RESULTS During the 5-year infection surveillance, 523 strains of A. baumannii were isolated in our hospital mostly from sputum. The commonest site was lower respiratory tract. The ward with the highest incidence of A. baumannii infection was ICU. The isolation rate was 11.1% in 2004, though 3.9% in 2000. A. baumannii was resistant to 20 kinds of antibiotics, especially to imipenem, cefoperazone /sulbactam and ampicillin/sulbactam. CONCLUSIONS The isolation rate of A. baumannii and antimicrobial resistance rate are increasing year by year. The increased rate of multidrug-resistance should be payed more attention .
7.Development Strategy of Clinical Pharmacy under the Condition of New Medical Reform in China

OBJECTIVE: To study the development strategy of clinical pharmacy under the condition of New Medical Reform in China. METHODS: The status quo and obstacle factors of clinical pharmacy were analyzed to probe into the chance for clinical pharmacy in China brought out by New Medical Reform. RESULTS: The reimbursement mechanism reform of state hospitals, National Essential Drug System, medical insurance system, charge for pharmaceutical care system and the construction of primary hospitals and public health service provide development opportunities of clinical pharmacy. CONCLUSION: The development strategy of clinical pharmacy should be associated with New Medical Reform and relevant policy, laws and regulations of clinical pharmacy should be improved. Great importance should be attached to exploring the training system of clinical pharmacists and approaches of clinical pharmacy.
8.Role of heterogeneity of cancer-associated fibroblasts in targeted therapy of pancreatic cancer

The incidence of pancreatic cancer is increasing year by year, but the clinical diagnosis and treatment progress is limited and the prognosis is poor. Tumor microenvironment (TME) is closely related to the invasion, metastasis and chemotherapy resistance of pancreatic cancer. Cancer-associated fibroblasts (CAFs) are fibroblasts in a state of continuous activation, which are the most prominent components in TME. CAFs can promote the malignant biological behavior of pancreatic cancer through a variety of molecule-mediated mechanisms. Moreover, several attempts targeting CAFs for the treatment of pancreatic cancer have been largely unsuccessful. It may be related to the heterogeneity of CAFs in pancreatic cancer. Therefore, in-depth study of its heterogeneity and accurate targeting of some specific phenotypes and functional CAFs subtypes in the matrix on this basis may be more promising in the clinical treatment of pancreatic cancer.
9.Mediating effect of insomnia in mobile phone addiction, aggressive behaviors and self-perceived identity among university students

Objective:
To investigate the mediating effect of insomnia among mobile phone addiction, aggressive behaviors and self-perceived identity among university students.
Methods:
A total of 740 university students were sampled from five universities in Binjiang District, Hangzhou City using a cluster random sampling method. The mobile phone addiction, aggressive behaviors and self-perceived identity were assessed using the Mobile Phone Addiction Index Scale, the Athens Insomnia Scale, the Chinese college students' version of the Buss-Perry Aggression Questionnaire, and the Self-Perceived Identity Scale, and the mediating effect of insomnia among mobile phone addiction, aggressive behaviors and self-perceived identity was examined using Process macro program and Bootstrap method.
Results:
A total of 740 questionnaires were allocated, and 700 valid questionnaires were recovered, with an effective recovery rate of 94.59%. The respondents included 221 men (31.57%) and 479 women (68.43%), and there were 607 respondents with a specialty of medicine (86.71%). There were 331 participants detected with mobile phone addiction (47.29%), 90 with aggressive behaviors (12.86%) and 289 with low-level self-perceived identity (41.29%), and the prevalence rates of mild, moderate, severe and extremely severe insomnia were 28.00%, 26.14%, 26.43% and 19.43% among respondents, respectively. Mobile phone addiction had a direct predictive effect on aggressive behaviors (β=0.301, P<0.001) and self-perceived identity (β=-0.129, P<0.001), and presented an indirect predictive effect on aggressive behaviors (effect of mobile phone addiction on insomnia: β=0.083, P<0.001; effect of insomnia on aggressive behaviors: β=0.999, P<0.001; effect of insomnia on self-perceived identity: β=-0.698, P<0.001). The contributions of mediating effects caused by insomnia were 21.61% and 31.02% to total effects.
Conclusions
Insomnia presents partial mediating effects among mobile phone addiction, aggressive behaviors and self-perceived identity among university students. Mobile phone addiction may directly affect aggressive behaviors and self-perceived identity, and indirectly affect aggressive behaviors and self-perceived identity via insomnia.
