1.Role of JAK2/STAT3-regulated prohibitin in cardioprotection of H2 S postconditioning in hypoxia/reoxygenation-treated cardiomyocytes

Aim ToinvestigatewhethertheJAK2/STAT3 signaling pathway regulates prohibitin expres-sion to protect cardiomyocytes against hypoxia/reoxy-genation injury in hydrogen sulfide postconditioning. Methods Primaryculturedcardiomyocytesfromneo-natal rats were divided into 6 groups: control group ( Normal) , hypoxia/reoxygenation group ( H/R ) , hy-drogen sulfide postconditioning group ( NP) , hydrogen sulfide with AG490 group ( N + A ) , AG490 group ( AG) , DMSO group ( DMSO) . The survival percent-age of cardiomyocytes and the release of LDH were tested at pre-hypoxia and reoxygenation 2h. After reox-ygenation, cell apoptosis was detected by flow cytome-try. The expression of t-STAT3, p-SATAT3 and PHB were determined with Western blot analysis. Results No obvious changes were observed among the groups before hypoxia (P <0. 05). After reoxygenation 2h, compared with H/R group, NP group significantly im-proved the survival rate of cardiomyocytes ( P <0. 05 ) , inhibited the release of LDH and the myocardi-al apoptosis ( P <0. 05 ) , meanwhile up-regulated the p-STAT3 and PHB expression. However, AG490 abol-ished the cardioprotection offered by hydrogen sulfide postconditioning and the increase in p-STAT3 and PHB expression.Conclusion Hydrogensulfidepostcondi-tioning may protect cardiomyocytes against hypoxia/reoxygenation injury through the JAK2/STAT3 pathway upregulating the expression of prohibitin.
2.A comparative trial of a combined therapy (Yangyin-Qinggan decoction combined with paroxetine)versus mono-therapy on depressive patients with somatic symptoms

Objective To evaluate the effectiveness of treating depressive patients with somatic symptoms with combined TCM and western medicine.Methods 69 patients were randomly divided into 2 groups:a combined therapy group (treated with Yangyin-Qinggan decoction and paroxetine) and mono-therapy (treated with paroxetine alone as an active control),The depressive and somatic symptoms were assessed before (0 week),during (2 weeks and 4 weeks) and at the end point (8 weeks) of the treatment.Results ①Both therapies alleviated the depressive symptoms:HAMD assessed among patients receiving combined therapy are listed as:(0 week:19.29±2.38),(2 weeks:17.38 ± 2.37),(4 weeks:15.27 ± 2.15),(8 weeks:13.35 ± 2.09) ;combined therapy started to ease the depressive symptoms after 2 weeks of treatment (2 week compared with 0weeks),similar improvements could also be noticed after 4 weeks of treatment 4 weeks with 2 weeks:P<0.05)and at the end of this research (after 8 weeks of treatment:8 weeks with 4 weeks:P<0.05).HAMD for mono-therapy are as:(0 week:18.69±3.03),(2 weeks:16.63±3.09),(4 weeks:15.20±2.95),(8 weeks:14.60±2.72) ; mono-therapy started to alleviate the depressive symptoms also,after 2 weeks of treatment (2 weeks compared with 0 week:P<0.05),yet only slight improvements could be seen after 4 weeks (4 weeks with 2 weeks:P>0.05) and 8 weeks of the treatment (8 weeks with 4 weeks:P>0.05).The combined therapy turned to be more effective in alleviating depressive symptoms at the end point of the treatment (P<0.05).②In terms of improving the somatic symptoms,the Somatization Symptom Scale (SSS) among patients with combined therapy were as follows:(0 week:48.74±4.07),(2 weeks:46.74±4.16),(4 weeks:43.74±3.77),(8 weeks:41.18 ± 3.50) ; Combined therapy was witnessed to start to ease those symptoms after 2 weeks o f treatment (2week compared with 0 weeks,P<0.05),similar patterns were found again,after 4 weeks (4 weeks with 2 weeks:P<0.05) and 8 weeks of the treatment (8 weeks with 4 weeks:P<0.05).Mono-therapy was found to ease the somatic symptoms in a less-effective way,yet no difference was found between any two SSS after 2 weeks,4weeks or 8 weeks of treatment (P>0.05,respectively).Combined therapy was better at improving the somatic symptoms (P< 0.01).Conclusion Combined therapy proved to be more effective in both relieving depression and somatic symptoms.
3.Genotyping of SCCmec and Detection of Antibiotic-resistant Genes in Meticillin-resistant Staphylococcus aureus

OBJECTIVE To investigate the staphylococcal cassette chromosome mec(SCCmec) genotype characteristics and antibiotic-resistant genes in meticillin-resistant Staphylococcus aureus(MRSA) isolated from Lianyungang.METHODS The SCCmec of clinically isolated MRSA strains were genotyped with a novel multiplex PCR strategy reported by Zhangetal.Antibiotic-resistant genes of aac(6′)/aph(2″),aph(3′)Ⅲ,tetM,erm,TEM,and ant(4′,4″) were analyzed by traditional PCR.RESULTS The isolates were almost SCCmec Ⅲ positve,only one isolate couldn′t be typed.The positive rates of aac(6′)/aph(2″),aph(3′)Ⅲ,tetM,and erm were 98%,46%,72% and 86%,respectively.TEM and ant(4′,4″) tested were all negative.CONCLUSIONS Almost all genotypes of MRSA prevailing in Lianyungang carry the SCCmec Ⅲ gene.There are high positive percentages of antibiotic-resistant genes of aac(6′)/aph(2″),aph(3′)Ⅲ,tetM and erm in the isolates.The novel multiplex PCR strategy recommended by Zhang et al can be applied into genotyping study of MRSA SCCmec effectively.
4.Dynamic Expression of Nogo Receptor after Spinal Cord Injury in Rats

Objective To observe the dynamic expression of Nogo receptor (NgR) in spinal cord of rats after spinal cord injury. Methods 108 Sprague-Dawley rats were randomly assigned into normal group, sham operated group and model group, with 36 rats in each group. The model of spinal cord injury was established with the modified Allen's method. The rats were killed 24 h, 3 days, 7 days and 14 days respectively after intervention (9 rats from each group), and expression of NgR in the spinal cord tissue of the rats was detected with immunohistochemistry and Western blotting, and expression of NgR mRNA was detected with fluorescence quantitative PCR. Results There was no significant change in the expression of NgR in the normal group and the sham operated group (P>0.05). The expression of protein and mRNA of NgR was less in the model group 24 h after modeling, dropped to the lowest on the 3rd day, then rapidly peaked on the 7th day, and gradually declined on the 14th day after spinal cord injury. Compared with the normal group, there were significant differences in expression of NgR in immunohistochemistry and Western blotting in the model group at each time point after spinal cord injury (P<0.05). Compared with the sham operated group, there were significant differences in expression of NgR mRNA in the model group at each time point after spinal cord injury (P<0.01). Conclusion The expression of NgR and mRNA peaks on the 7th day after spinal cord injury in the rats, and maintains at high level for a long time, which may associated with the difficulty of axonal regeneration after spinal cord injury.
5.A comparative study on the absorption kinetics parameters of rhubarb free anthraquinones between normal dogs and dogs with severe acute pancreatitis.

OBJECTIVETo study the effects of severe acute pancreatitis (SAP) on absorption kinetic parameters of rhubarb free anthraquinones.
METHODSEleven healthy Beagle dogs were randomly divided into the normal group (n = 6) and the SAP group (n = 5). The SAP animal model was prepared by surgery through portal vein blood channel building to collect blood from normal dogs and dogs with SAP. The free anthraquinones (20 mg/kg) was given by gastrogavage. The concentrations of five anthraquinones (aloe emodin, rhein, emodin, chrysophanol, and physcion) in the blood plasma of the portal vein and the femoral artery were determined using high performance liquid chromatography (HPLC). The kinetic parameters were calculated using MATLAB2007B Software. The half life (t(1/2Ka)), the absorption peak time (Tmax), the peak concentration (Cmax), the area under the curve [AUC(0-infinity)], and the mean residence time (MRT) were calculated using the statistical moment method. The transport velocity of corresponding medicines from the gastrointestinal tract to the blood (ka) was calculated.
RESULTSThere was no difference in the chemical composition absorption type of the portal vein and the femoral artery between the two groups. Aloe emodin could be detected in the portal vein of each animal at each time point, and they were in the quantitative range. Rhein could be detected in the portal vein of each animal at each time point, and they were lower than the quantitative limit at few time points. Emodin and chrysophanol could be detected in the portal vein of partial animals at each time point, and most of them were higher than the quantitative limit. Physcion could be detected only in the portal vein of less animals at few time points. Rhein could be detected in the femoral artery at most time points, but the rhein plasma concentration at most time points were lower than the quantitative limit. Lower concentration of aloe emodin, emodin, and chrysophanol could be detected in the femoral artery at only few time points. Physcion was not detected in the femoral artery. The rhein plasma concentration of the femoral artery and the chrysophanol Cmax of the portal vein at 45 min were higher in the SAP group than in the normal group with statistical difference (P<0.05). There was no statistical difference in the rest indices. The AUC of rhein in the two groups were 59.32% and 66.07% of the total free anthraquinones respectively.
CONCLUSIONSSAP could not obviously affect the absorption kinetics parameters of rhubarb free anthraquinones. The intestinal tract and the liver might possibly play important roles in metabolizing or transforming rhubarb free anthraquinones.

6.Short-term efficacy of short stem ceramic-on-ceramic total hip arthroplasty for young and middle-aged patients of femoral head necrosis

Objective To evaluate the clinical effects and experiences of short stem ceramic-on-ceramic total hip arthroplasty in the treatment of young and middle-aged patients with femoral head necrosis.Methods From December 2009 to March 2011,40 patients with femoral head necrosis were treated primarily with short stem ceramic-on-ceramic hip arthroplasty (43 hips) through posterior-lateral or anterior-lateral approach.There were 23 males,17 females; with the mean age of 40.5 years (range,27-55 years).All cases were stage Ⅳ or more according Steinberg stage.The patients were followed up at the 3,6,12 months after operation,and then once each year.Standard X-ray films were taken at the follow-ups.Femoral prosthesis subsidence was assessed through X-ray films.Harris scores system and WOMAC Osteoarthritis Index were recorded before and after operation to evaluate clinical effects of surgery.Results Thirty-eight patients (41 hips) were followed up for 21-36 months,with an average of 31.3 months.The femoral stem subsided 0.68±2.90 mm at the last follow-up.After operation,hip pain of patients was relieved obviously.The Harris score improved from 38.7±12.1 (15-66) preoperatively to 95.2±4.49 (83-100) postoperatively,WOMAC Index from 66.7±12.0 (49-94) to 12.8±5.6 (0-24).During follow-up period,there was no serious complication such as femoral neck fracture,infection of wound,pulmonary embolism,et al.No dislocation,implant prosthesis failure and infection were observed.Conclusion The short stem hip arthroplasty preserves bone stock to a great extent.The short stem implant showed a good stability and osseous integration.The wear rate of ceramic-on-ceramic bearing surface is extremely low.Short stem hip arthroplasty plus ceramic-on-ceramic bearing surface might be a good option for the young and middle-aged patients with femoral head necrosis.
7.Profile and prevalence of aspirin resistance in patients with metabolic syndrome

Objective Aspirin has been used extensively in primary and secondary prevention of cardiovascular disease,particularly for subjects at high risk such as metabolic syndrome.However,the responsiveness to aspirin treatment may vary among individuals.The present study was conducted to investigate the profile and prevalence of aspirin resistance in patients with metabolic syndrome.Methods In 221 consecutive patients,platelet aggregation induced by arachidonic acid (0.5mg/ml) was assessed after 10 days of aspirin treatment (200mg/d).Aspirin resistance was defined as mean optical platelet aggregation =20%.Results Aspirin resistance occurred in 39 patients (17.6%).Serum fibrinogen level was higher in patients with than in those without aspirin resistance (2.6_+0.4g/l vs 2.4±0.4g/L,P=0.017).The 2 groups,aspirin resistance group and no aspirin resistance group,did not differ significantly,with regard to gender,age,body mass index,waist-hip ratio,blood pressure level,serum cholesterol level and history of myocardial or cerebral infarction.Multivariate logistic regression analysis revealed that only serum fibrinogen level entered the model (odds ratio 2.973,p=0.023).Subgroup analysis further showed that aspirin resistance occurred more in male patients with myocardial infarction (50% vs14.5%,P=0.02) and in female patients with diastolic blood pressure=85mmHg (34% vs 15.5%,P=0.043).But after multifactor logistic regression,in women blood pressure=85mmHg was not a predictor any more.Conclusions In patients with metabolic syndrome,aspirin resistance is not uncommon,especially for men with history of myocardial infarction.Patients with aspirin resistance have an increased serum fibrinogen level.(J Geriatr Cardio12008;5:7-10)

The pertinent literature on application of traditional Chinese medicine (TCM) in hemopoietic stem cell transplantation was summarized, it indicated that the intervention of TCM could raise the mobilization effect, speed up the hemopoiesis and immunologic reconstruction after transplantation, decrease the incidence of complications, and prolong the life span of patients, showing a preliminary achievement.

9.The impact of ossified anterior longitudinal ligaments surrounding osteotomy vertebra on the lordosing capability of pedicle subtraction osteotomy in thoracolumbar kyphotic deformity secondary to ankylosing spondylitis

Objective To investigate anterior longitudinal ligaments (ALL) ossified surrounding osteotomy vertebra impact the lordosing effect of pedicle subtraction osteotomy (PSO) in patients with thoracolumbar kyphosis secondary to ankylosing spondylitis (AS).Methods We retrospectively reviewed 102 AS patients with thoracolumbar kyphosis treated with single-level PSO at our institution from September 2007 to August 2015.There were 92 male and 10 female.The average age was (35.6±11.8)years old (range from 17 to 65 years old).Patients were stratified into ossified group (54 cases) and non-ossified group (48 cases)based on the presence of ALL ossification adjacent to osteotomy vertebra.Compared the contribution of adjacent disc wedging to total correction of each PSO segment between the ossified and non-ossified groups.The long-term correction loss of spine and pelvic sagittal morphology were also evaluated and compared between the 2 groups.Results Patients in the ossified group accomplished significantly lower amount of correction in single level segment of PSO (36.3°±6.9° vs.41.5°±6.9°),and there was significant difference between the two groups.The contribution of adjacent disc wedging to total correction of PSO was significantly larger in the non-ossified group (22.9% vs.7.8%,P<0.001).For subgroups with a minimum 2 year follow-up,loss of corrections concerned sagittal vertical axis (SVA),which was (1.7±4.5) cm vs.(-0.2±4.0) cm in ossified group and non-ossified group,and there was significant difference between the two groups.Pelvic tilt (PT) was 3.5°±8.2° vs.2.0°± 10.4°,lumbar lordosis (LL) was-7.9°±11.9° vs.-0.1°± 11.9° and sacral slope (SS) was 4.5°±9.3° vs.1.6°±7.9°,and there were all significant differences between the two groups.The change of adjacent disc wedging angle was marginally higher in the unossified group (-2.1°±6.2° vs.-0.1°±3.7°,P=0.09),but there was no significant difference between the two groups.No significant correction loss of osteotomy angle was observed in both groups.Conclusion Osteotomy vertebrae accompanied by unossified adjacent ALL in PSO of AS were prone to create more disc-originated lordosing effect immediately after surgery.However,a correction loss might occur more commonly during a long term follow-up.
10.Survey of protection level of radiation equipment room in thirty-three medical units in East China

Objective The rapid development of radiation equipment has put forward higher requirements for protective facil -ities.The aim of this study was to research the radiation protection status of treatment equipment room in thirty -three medical units from East China area , and to put forward corresponding countermeasures to improve the level of radiation protection . Methods The li-cense and archival of X-ray machine, CT, gamma knife, accelerator, after loading therapy machine were surveyed in the thirty-three medical units .The engine room size , wall thickness , lead equivalent of protection doors , and leakage level of equipment were detec-ted.All the data were evaluated and analyzed on the basis of the corresponding national standards . Results ①The inspection pass rates of radiation treatment equipments in the first class hospitals were afterloader (100%), X-ray apparatus(99%), gastrointestinal machine(98%), CT(98%), medical processing accelerator (98%), gamma knife(97%), DSA(94%), PET-CT(94%), ECT (90%), dental engine(84%)and molybdenum target drone (84%).The inspection pass rates of radiation treatment equipments in other class hospitals were medical processing accelerator (100%), X-ray apparatus(91%), CT(89%), gastrointestinal machine (80%), DSA(80%) and (80%) gamma knife.②The qualified rates of the leakage radiation levels of radiation therapy , diagnostic radiology and nuclear medicine were 100%, 99% and 97%. Conclusion The radiation protection of treatment equipments and facilities in thirty-three medical units from East China area needs to be strengthened . We should strengthen supervision , implement the rules and regulations , strictly perform the regulation of radiation protection evalua-tion, and strengthen the training of radiation protection regulations and skills .
