1.Influence of diabetes educators in diabetic patients' health education

Objective To investigate the important role of diabetes educators in diabetic patients'health education process.Methods A total of 73 diabetic patients were evaluated by the same self-designed questionnaire before and after health education by diabetes educators,and the results were compared.Results After health education,the rate of 73 diabetic patients' diet knowledge including total calorie,amount of staple food,non-staple food,oil,vegetable and fruit were respectively 90.4%,97.3%,98.6%,98.6%,98.6% and 95.9%,all of which were higher than those before health education (26.0%,78.1%,68.5%,42.5%,58.9% and 71.2%),and the differences were statistically significant (x2 =43.31,12.25,20.17,39.09,29.12,14.73,respectively; P < 0.01).After the diabetes health education,the rate of 73 diabetic patients' insulin types knowledge was 64.4%,was higher than this before health education (45.2%),the difference was statistically significant (x2 =10.29,P < 0.01).The rate of patients' self-monitoring behaviors including keeping a diary,recording special events,measuring blood pressure weekly,testing HbA1 c every three months,doing fundus examination at regular intervals and inspecting blood fat,liver and kidney function regularly after health education were respectively 91.8%,98.6%,98.6%,98.6%,98.6% and 98.6%,obviously higher than those before education (32.9%,24.7%,46.6%,39.7%,50.7% and 54.8%),and the differences were statistically significant (x2 =39.34,52.07,36.10,41.09,33.11,30.11,respectively; P <0.01).Conclusions The diabetes educators play an important role in improving diabetes patients' level of healthy knowledge.
2.Change of neurocytes in acute intoxicated encephalopathy induced by 1, 2-dichloroethane after intervention with antagonists.

OBJECTIVETo study the role of N-methyl-D-aspartate receptor and Ca(2+) in acute intoxicated encephalopathy induced by 1, 2-dichloroethane (1, 2-DCE) in vitro.
METHODSNeurocytes of new born rats were cultured in vitro, which were administered with different doses of 1, 2-DCE, and NMDAR and Ca(2+) antagonists including Ketamine and Nimodiping respectively. The cell morphologic structures were observed under light microscope, and its proliferation was detected by Cell Counting Kit-VIII.
RESULTS1, 2-DCE could damage the normal morphological structure of neurocytes: the cell body swelled and broke down, the karyon slurred or disappeared, the axone became shorten and thick, connection of neurocytes was reduced, the cell membrane was half-baked, injury of neurocytes became severer with the increase of the dose of 1, 2-DCE. There was no statistical difference in the proliferation of neurocytes between every 1, 2-DCE groups (P > 0.05), but there was significantly statistical difference between 1, 2-DCE groups, the control group, and the retarder groups (P < 0.01).
CONCLUSION1, 2-DCE can damage the normal morphological structure of neurocytes, and the damage will become severer with the increase of the dose of 1, 2-DCE. However, the cell morphologic structures and proliferation of antagonist groups are much better than those in the 1, 2-DCE groups.

3.A mutation 1633-26(C-->A) in EXT1 gene causes multiple exostoses.

OBJECTIVETo study the gene mutation in a patient with multiple exostoses, identify the disease-causing gene mutation.
METHODSPolymerase chain reaction and DNA sequencing were used to screen the EXT1 or EXT2 gene mutation, while mismatch primer amplification and restriction endonuclease digestion were performed to confirm the mutation.
RESULTSBy DNA sequencing, a mutation in the seventh intron was detected and located at 26 bp of 3' splice site upstream in EXT1 gene, which was unreported before. Mismatch primer amplification and restriction fragment length polymorphism analysis suggested that this mutation was not detected in the normal control.
CONCLUSIONThe mutation 1633-26(C-->A) may be the disease-causing mutation in this patient with multiple exostoses.

4.Mutation analysis of ganglioside-induced differentiation associated protein-1 gene in Chinese Charcot-Marie-Tooth disease.

OBJECTIVETo study the mutation feature of ganglioside-induced differentiation associated protein-1 (GDAP1) gene in Chinese Charcot-Marie-Tooth disease(CMT) patients.
METHODSMutation analysis was carried out by use of polymerase chain reaction-single strand conformation polymorphism(PCR-SSCP) combined with DNA direct sequencing of the six exons and their flanking regions of GDAP1 gene in twenty-three CMT patients, including 8 probands of autosomal recessive CMT families and 15 sporadic patients.
RESULTSA compound heterozygous mutation A533G and A767G were unveiled in one autosomal recessive CMT kindred. The homozygous and heterozygous T507G were common SNPs in Chinese population.
CONCLUSIONA533G and A767G of GDAP1 gene were new mutations firstly reported.

6.Correlation between 320 row volume CT perfusion parameters of primary liver cancer and liver tumor volume and liver function Child-Pugh classification

Objective To explore the relationship between the values of total tumor perfusion parameters in primary hepatocellular carcinoma and tumor volume and peritumoral perfusion parameters,and analyze its correlation with liver ChildPugh classification.Methods Forty-seven patients with primary liver cancer in the First Affiliated Hospital of Zhengzhou University from January 2013 to January 2015 were selected to perform 320 row volume CT perfusion imaging.The parameters of hepatic artery perfusion(HAP),portal vein perfusion(PVP) and hepatic perfusion index(HAPI) in tumor and peritumoral liver tissues were calculated based on total tumor measurement.The relationship between tumor perfusion parameters and ChildPugh classification of liver function,tumor volume and total tumor perfusion parameters,total tumor perfusion parameters and peritumoral perfusion parameters were analyzed.Results There was no correlation between tumor volume and total tumor perfusion parameters,peritumoral perfusion parameters (P > 0.05),and there was no correlation between total tumor perfusion parameters and peritumoral perfusion parameters(P >0.05).There were significant differences between HAP,PVP and HAP of total tumor perfusion in different liver Child-Pugh classifications(P < 0.05).With the increase of Child-Pugh classification of liver function,the HAP and HAPI of tumor gradually increased while PVP gradually decreased (P < 0.05).Conclusions There is no correlation between tumor volume,total tumor perfusion parameters and peritumoral perfusion parameters.There is also no correlation between total tumor perfusion parameters and peritumoral perfusion parameters.There are significant differences in perfusion parameters between different Child-Pugh classifications of liver function,and the perfusion parameters obtained by the total tumor measurement can be used as the imaging indexes to reflect liver reserve function.
7.Water-related diseases along the eastern route of the South-to-North Water Diversion Project

The aim of this study is to investigate the epidemic condition of water-related diseases in the eastern route of South-to-North Water Diversion Project (SNWDP).All data were extracted from published literatures in Chinese about water-related diseases in the eastern route of SNWDP.Pooled analysis was used to explore geographical distribution and epidemiology of the disease.A total of 325 articles about water-related diseases were retrieved during 1953 to 2013,and 209 articles were included in this study.Pooling analysis showed that Shandong Province had the largest number of cases for water-related diseases,following by Jiangsu,Hebei,Tianjin,and Anhui.The numbers of cases were relative small before 1960s according to epidemic curve,and the curve peaked in the 1970s,and decreased after the 1980s.A total of 1 383 834 cases of bacillary dysentery was reported,accounting for 84% of all water-related diseases on these regions of SNWDP,and followed by hepatitis A,hepatitis E,Japanese encephalitis,typhoid and paratyphoid fever and hemorrhagic fever with renal syndrome.Other reported diseases displayed scatter condition and a small numbers of cases.The prevalence of water-related diseases is sporadic and a trend of decline along the regions of the eastern route of SNWDP.
8.Efficacy comparison of laparoscopic versus open tension-free hernia repair using biologic mesh for inguinal strangulated hernia.

OBJECTIVETo compare the efficacy of laparoscopic versus open tension-free mesh repair using biologic mesh for inguinal strangulated hernia.
METHODSClinical data of 27 patients with inguinal strangulated hernia in the Shanxi Provincial People's Hospital between January 2012 and April 2014 were analyzed retrospectively. All the patients underwent one-stage tension-free repair using biological mesh, including laparoscopic(n=13) and open procedures(n=14).
RESULTSAs compared with the open group, the laparoscopic group had shorter operative time [(90.8±11.6) min vs. (130.8±32.5) min, P<0.01], lower rates of hematoma/seroma and wound infection[(7.7% vs. 42.9%) and (0 vs. 28.6%) respectively, both P<0.05], faster recovery of bowel function [(2.5±0.3) d vs. (3.8±1.4) d, P<0.01], and shorter hospital stay [(6.3±1.8) d vs. (9.8±3.2) d, P<0.01]. The mean follow-up was 5.7 months (ranged from 2 to 12 months), and no recurrence or serious complications occurred.
CONCLUSIONLaparoscopic tension-free hernia repair using biological mesh for inguinal strangulated hernia has significant advantage versus open operation.
9.Genetic linkage analysis in localizing a gene of autosomal dominant familial dilated cardiomyopathy with conduction defect.

OBJECTIVE:
To localize the gene of autosomal dominant familial dilated cardiomyopathy with conduction defect.
METHODS:
A Chinese family which was diagnosed as dilated cardiomyopathy with conduction defect was studied. Venous blood (3 - 5 mL) from some family members was collected, and genomic DNA was extracted from the blood. Then whole genome wide scan was performed after excluding the known markers on the candidate loci (CMD1A, CMD1 E, CMD1F, and CMD1H) by two-point linkage analysis.
RESULTS:
No significant evidence for linkage was found in the two point linkage analyses to the known markers in the analyzed family. And the whole genome wide scan showed the maximum LOD score reached 2.68 at marker D3S1614 ( at recombination fraction theta = 0).
CONCLUSION
The related gene in this kindred is located on 3q26 other than on CMD1A, CMD1H, CMD1E, and CMD1F.

10.Effect of Interval Between Neoadjuvant Chemotherapy and Surgery on Oncological Outcomes in Poor Responders With Locally Advanced Breast Cancer

Purpose:
The interval between neoadjuvant chemotherapy (NAC) and surgery for locally advanced breast cancer (LABC) remains controversial. At the same time, the prognostic effect of delayed surgery in patients with poor responses is currently unclear.
Methods:
Data was collected from patients who had poor responses to NAC and underwent modified radical surgery from January 2013 to December 2018. The interval from completion of NAC to surgery was divided into two groups: a longer (greater than four weeks) or shorter (four weeks or less) interval. The associations of these interval groups with overall survival (OS) and recurrence-free survival (RFS) were evaluated by multivariable Cox models adjusting for the existing prognostic factors. Propensity score matching (PSM) was used to minimize election bias.
Results:
A total of 1,229 patients (mean age, 47.2 ± 8.9 years; median follow-up duration, 32.67 [6.57–52.63] months) were included. The 5-year OS rates were 73.2% and 60.8% in the shorter (n = 171) and longer interval group (n = 1,058), respectively, while the 3-year RFS rates were 80.8% and 71.7%, respectively. In multivariate Cox analysis, the longer interval was associated with an increased risk of mortality (hazard ratio [HR], 1.43; 95% confidence interval [CI], 1.01–2.02; p = 0.046) and recurrence (HR, 1.50; 95% CI, 1.12–1.99; p = 0.006).There was an interaction between the molecular subtype and the surgery interval for OS (pinteraction = 0.014) and RFS (pinteraction = 0.027). After PSM, no significant difference in OS (p = 0.180) and RFS (p = 0.069) was observed between the two groups.
Conclusion
Among LABC patients with a poor response, those with a longer interval between NAC and surgery had worse OS and RFS. The results indicate that these patients should receive modified radical surgery timely, which may in turn improve their prognosis.
