3.Clinical Observation of Amlodipine Besylate Combined with Valsartan in the Treatment of Hypertension

OBJECTIVE:To observe therapeutic efficacy and safety of amlodipine besylate combined with valsartan in the treatment of hypertension. METHODS:158 patients with hypertension were randomly divided into control group (n=78) and treatment group (n=80). Control group was treated with amlodipine besylate,2.5 mg/time,3 times/d;treatment group was addi-tionally treated with valsartan,80 mg/time,once a day. A treatment course lasted for 4 weeks,and both received 5 courses. Clini-cal efficacy,blood pressure,plasma concentrations of ET-1,NT-proBNP and Ang Ⅱ were compared between 2 groups. RE-SULTS:The compliance rates of treatment group after 4,8,12,16,20 weeks were higher than that of the control group(P<0.05). Compared with control group,SBP and DBP of treatment group were decreased significantly after 4,8,12 and 20 weeks,with sta-tistical significance(P<0.05). After 20 weeks,the serum levels of ET-1,NT-proBNP and Ang Ⅱwere decreased significantly,es-pecially treatment group,with statistical significance (P<0.05). There was no statistical significance in the incidence of ADR be-tween 2 groups (P>0.05). CONCLUSIONS:Amlodipine besylate combined with valsartan can effectively control the blood pres-sure in patients with hypertension,which may be related to the inhibition of ET-1,NT-proBNP and AngⅡ.

Mutations in isocitrate dehydrogenase are among the most common genetic alterations in intrahepatic cholangiocarcinoma (ICC).Mutant IDH proteins in ICC and other malignancies acquire an abnormal enzymatic activity, allowing the conversion of alpha-ketoglutarate (alphaKG) to 2-hydroxyglutarate (2HG), which inhibits the activity of multiple alphaKG-dependent dioxygenases, and results in alterations in cell differentiation and tumorigenesis.This review will focus on recent advances, which may help understand the function of IDH mutation in intrahepatic cholangiocarcinoma.

6.Fetal Echocardiography for Total Anomalous Pulmonary Venous Connection and Misdiagnosis Analysis

Purpose To investigate the diagnostic value of fetal echocardiography in total anomalous pulmonary venous connection (TAPVC),and to analyze the reasons ofmisdiagnosis so as to improve accuracy of prenatal diagnosis.Materials and Methods The prenatal echocardiographic data of 24 cases of TAPVC confirmed by postnatal echocardiography and prenatal MRI from October 2001 to December 2015 in Children's Hospital Affiliated to Nanjing Medical University and Shanghai Children's Medical Center Affiliated to Shanghai Jiaotong University School of Medicine were retrospectively analyzed.Results In 24 cases,there were 14 fetuses of supracardiac type,7 cases of intracardiac type,2 cases of infracardiac type,and 1 case of mixed type.15 fetuses had TAPVC only,whereas 9 cases were associated with other cardiovascular anomalies,most commonly including bilateral right atria,single atrium,single ventricle,common atrioventricular valve and pulmonary stenosis.On 4-chamber views,the common sonographic sign was absent of pulmonary venous return to the left atrium in 24 cases (100.0%),and the presence of venous confluence between left atrium and descending aorta was observed in 22 cases (91.7%).In all 24 cases,1 case was missed diagnosis,and 3 cases were misdiagnosed as persistent left superior vena cava,common pulmonary vein atresia and patent left atrial cardinal vein.Conclusion TAPVC can be accurately diagnosed by fetal echocardiography,but it is prone to be misdiagnosed.
7.Application of three-dimensional reconstruction technique of MSCT in examination of colonic tumours

Objective To discuss the clinical value of application of three-dimensional reconstruction technique of MSCT in examination of colonic tumours.Methods Thirty patients underwent scanning using helical CT and post-processing on the workstation.All CTC images were reconstructed using shaded surface display (SSD) on workstation.Perspestive images such as the ones from DCBE were generated via raysum.The images could clearly demonstrate the extent and detail of the disorder by adjusting the CT value threshold.These results and that of barium enema and colon endoscopy were compared.Results Thirty cases had completely clinical and pathological data.3D images could correctly reveal the macroscopic morphology of colon tract and location of lesions.The correctly diagnostic rate was 100% by MSCT,while the diagnostic rate by CTVE and colon endoscopy was basicly identical.Conclusion With the utilization of multiple methods,3D reconstruction can effectively demonstrate colonic tumours,and can be applied routinely in examination of colonic tumours.
8.Effects of midazolam on macroscopic voltage-gated potassium currents in rat sympathetic neurons

AIM: To explore the influence of different concentration midazolam on the macroscopic voltage gated potassium currents and to discuss the relationship between potassium currents and inhibitory effect of clinical relevant concentration midazolam on sympathetic nervous system. METHODS: Superior sympathetic ganglion neurons were dissociated enzymatically from 7 to10 day old rat. Experiments were performed about 5 h after plating at room temperature (20- 24 ℃ ). Appropriate solution was chosen to separate the K + current from the other transmembrane currents. 1 ?mol?L -1 TTX was applied to the extracelluar solution to block the Na + current. Midazolam was also resolved in extracelluar solution to get various concentration ( 0.1 , 0.3 ,3,10,50,100 ?mol?L -1 ). Currents were recorded with the patch clamp technique in whole cell configuration using glass electrodes with a tip resistance of 2- 4 M . Potassium currents were evoked by test pulse from -100 mV to +30 mV with holding potential -80mV. Data were analyzed using Clampfit 6.0 and Oringih 5.0 software. Whole cell current records were corrected for leakage and capacitance by using the P/5 protocol. RESULTS: Midazolam dose dependently inhibited the whole cell potassium currents. Clinical relevant concentration midazolam ( 0.3 ?mol?L -1 ) only reduced the peak currents by 3.89 %(P= 0.88 ). The concentration required to produce 50% current inhibition(IC 50 ) was 76.065 ?mol?L -1 . CONCLUSION: Midazolam inhibits the whole cell potassium current significantly and dose dependently, but clinical relevant concentration midazolam has minor effect on the potassium currents, indicating that the inhibitory effect of midazolam on potassium current is not related to the suppression of activity of sympathetic system.
9.Comparison of P63 Protein Expression in Non-small Cell Lung Cancer between Xuanwei and Other Regions

Objective The purpose of the study was to compare the P63 expression in non-small cell lung cancer (NSCLC) between Xuanwei and other regions, and to investigate the relationship of P63 expression and biological behavior. Methods Immunohistochemical method was used. Results The results indicated that the expression of P63 in lung squamous cell carcinoma was extraordinarily high. P63 was related to the TNM staging system,tissue differentiation degree and lymph node metastasis,but not related to gender. In NSCLC,there was no significant difference of the P63 positive expression rate in the same pathological types, staging, tissue differentiation degree, lymph node metastasis and gender between Xuanwei and other regions. It indicated that the expression of P63 was not the reason why it was high incidence of lung cancer in Xuanwei region.
10.Optimization of bone removal ratio for dual-energy cerebral subtraction angiography with Flash dual-source CT

Objective To evaluate the clinical application of regulating bone removal ratio in dual-energy cerebral subtraction angiography.Methods A total of 45 patients who underwent two phases dual-energy CT angiography (CTA) were recruited and divided into several groups by the value of bone removal ratio,as follows: group A (Ratio=1.6), group B (Ratio=1.7), group C (Ratio=1.8) and group D (Ratio=1.9,default), as well as group E (100kV Neuro-DSA).The image quality of dual-energy CTA was evaluated by double blind method according to bone suppression and integrity of the vessel lumen (1-5 points, 1=poor, 5=excellent, 3-5=satisfactory).The bone removal ratio dependence of the image quality was analyzed using the self-control method.Moreover,the signal-to-noise ratio (SNR),contrast-to-noise ratio (CNR) and image scores were compared with the Neuro-DSA subtraction among these groups.Results The overall satisfaction of basis cranii images that were executed by dual-energy CTA with different bone removal ratios was listed as below: group A 73.33%, group B 91.11%, group C 78.52%, group D 70.37%.The satisfaction of siphon segment and petrous segment reached the maximum when the value of ratio was 1.7,and with significantly difference (P<0.05).The SNR and CNR were not significantly different among group B, group D, and group E (P>0.05).Conclusion With optimal bone removal ratio,the single enhanced dual-energy subtraction image quality with flash dual-source CT is consistent well with that of conventional 100 kV Neuro-DSA subtraction.Thus,this method is recommended in clinical emergency cerebrovascular examination for the improved image quality,which can compensate the shortage of vascular defect in basis cranill.
